Background Optimising the immunogenicity of COVID-19 vaccines to improve their protection against disease is necessary.Fractional dosing by intradermal(ID)administration has been shown to be equally immunogenic as intramuscular(IM)administration for several vaccines,but the immunogenicity of ID inactivated whole severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)at the full dose is unknown.This study(NCT04800133)investigated the superiority of antibody and T-cell responses of full-dose CoronaVac by ID over IM administration in adolescents.Methods Participants aged 11–17 years received two doses of IM or ID vaccine,followed by the 3rd dose 13–42 days later.Humoral and cellular immunogenicity outcomes were measured post-dose 2(IM-CC versus ID-CC)and post-dose 3(IMCCC versus ID-CCC).Doses 2 and 3 were administered to 173 and 104 adolescents,respectively.Results Spike protein(S)immunoglobulin G(IgG),S-receptor-binding domain(RBD)IgG,S IgG Fcγreceptor IIIa(FcγRIIIa)-binding,SNM[sum of individual(S),nucleocapsid protein(N),and membrane protein(M)peptide pool]-specific interleukin-2(IL-2)+CD4+,SNM-specific IL-2+CD8+,S-specific IL-2+CD8+,N-specific IL-2+CD4+,N-specific IL-2+CD8+and M-specific IL-2+CD4+responses fulfilled the superior and non-inferior criteria for ID-CC compared to IM-CC,whereas IgG avidity was inferior.For ID-CCC,S-RBD IgG,surrogate virus neutralisation test,90%plaque reduction neutralisation titre(PRNT90),PRNT50,S IgG avidity,S IgG FcγRIIIa-binding,M-specific IL-2+CD4+,interferon-γ+CD8+and IL-2+CD8+responses were superior and non-inferior to IM-CCC.The estimated vaccine efficacies were 49%,52%,66%and 79%for IM-CC,ID-CC,IM-CCC and ID-CCC,respectively.The ID groups reported more local,mild adverse reactions.Conclusion This is the first study to demonstrate superior antibody and M-specific T-cell responses by ID inactivated SARSCoV-2 vaccination and serves as the basis for future research to improve the immunogenicity of inactivated vaccines.

Prevalence and Antimicrobial Susceptibility of Mycobacterium abscessus in a General Hospital, China

Objective To gain greater insight into the prevalence drug resistant profiles of M. abscessus from a general hospital in Beijing, China. Methods Partial gene sequencing of 16 S, hsp65, and rpo B were used to distinguish the species of NTM isolates. All strains identified as M. abscessus were further enrolled in the drug susceptibility testing by using broth microdilution method. Results We found that M. avium complex was the most frequent NTM organism, accounting for 54.1%（33/61） of all isolates. Behind MAC, the second most common organisms were M. abscessus（22 out of 61, 36.1%）. Average rates of resistance were 4.5% for AMK, 9.1% for LZD, and 13.6% for CLA, respectively. In contrast, resistance to LEV（17/22, 77.3%）, IMI（9/22, 40.9%）, and SMX（10/22, 45.5%） was noted in more than 40% of M. abscessus isolates. DNA sequencing revealed that all the CLA-resistant isolates harbored nucleotide substitutions in position 2058（1/3, 33.3%） or 2059（2/3, 66.7%） of 23 S r RNA. Conclusion In conclusion, our data demonstrated that M. intracellulare and M. abscessus were the most common NTM species in the general hospital of Beijing. CLA, AMK, LZD showed promising activity, where as LEV, IMI, and SMX exhibited poor activity against M. abscessus in vitro.

Intestinal detoxification time of hand-foot-and-mouth disease in children with EV71 infection and the related factors

Background:Hand-foot-and-mouth disease(HFMD)is a common pediatric infectious disease caused by a variety of intestinal viruses.Enterovirus 71(EV71)is the primary pathogen that might cause severe symptoms and even death in children with HFMD.This study aimed to investigate the intestinal detoxification time of HFMD children with EV71 infection and its related factors.Methods:Sixty-five HFMD children with EV71 infection were followed up.Their stool samples were collected once every 4 to 7 days.Viral nucleic acids were detected byfl uorescent polymerase chain reaction until the results became negative.The positive rates of viral nucleic acids were analyzed by the Kaplan-Meier method.The Log-rank test and Cox-Mantel test were used to analyze factors affecting the HFMD children with EV71 infection.Results:On the 2nd,4th,6th and 10th week,the positive rates of viral nucleic acids in stool samples of the 65 children were 94.6%,48.1%,17.2%and 0,respectively.Univariate analysis showed that the intestinal detoxification time of the children were related to gender,pre-admission disease course,severity of disease,and use of steroids or gamma globulin(Pinfection was 10 weeks.The severity of disease was an important factor affecting the intestinal detoxification time of HFMD children with EV71 infection.Severe HFMD children with EV71 infection had a longer intestinal detoxification time.

目的呼吸防护在医疗卫生工作中至关重要,应使用适当的颗粒过滤呼吸器和等效的呼吸防护用品,以确保良好的佩戴气密性,最大限度地降低医务人员的空气传播感染风险。该文旨在对呼吸防护用品适合性检验和密合性检查有效性研究进行综述和分析。方法系统检索了2003年1月至2022年4月发表的文献,检索范围包括CINAHL、Cochrane图书馆、EMBASE、PubMed、Ovid数据库Medline子库和灰色文献。研究方案已在PROSPERO注册(CRD42020213968)。结果共检索出561篇文献,25篇被纳入该综述,包括22项量性研究和3篇指南文献。在评估呼吸防护用品的有效性方面,适合性检验是更有效且被广泛采用的可靠方法,但呼吸防护程序应该同时包括适合性检验和密合性检查,以最大限度地提高呼吸防护用品的有效性。新型冠状病毒疫情凸显了医务人员相关知识的缺乏,相关教育培训可有效提高呼吸防护用品使用的有效性。结论为确保医务工作者的安全,适合性测试和密合性检查均应采用。建议由专业人员实施全面的呼吸防护项目,包括教育培训、适合性测试和密合性检查,还需要探索适用于实际使用环境的适合性检验方法。

Proactive infection control measures to prevent nosocomial transmission of carbapenem-resistant Enterobacteriaceae in a non-endemic area

Background Identification of hospitalized carbapenem-resistant Enterobacteriaceae （CRE）-positive patient is important in preventing nosocomial transmission.The objective of this study was to illustrate the implementation of proactive infection control measures in preventing nosocomial transmission of CRE in a healthcare region of over 3200 beds in Hong Kong between October 1,2010 and December 31,2011.Methods The program included active surveillance culture in patients with history of medical tourism with hospitalization and surgical operation outside Hong Kong within 12 months before admission,and ＂added test＂ as an opportunistic CRE screening in all fecal specimens submitted to the laboratory.Outbreak investigation and contact tracing were conducted for CRE-positive patients.Serial quantitative culture was performed on CRE-positive patients and the duration of fecal carriage of CRE was analyzed.Results During the study period,a total of 6533 patients were screened for CRE,of which 76 patients were positive （10 from active surveillance culture,65 from ＂added test＂,and 1 secondary case from contact tracing of 223 patients with no nosocomial outbreak）,resulting in an overall rate of CRE fecal carriage of 1.2％.The median time of fecal carriage of CRE was 43 days （range,13-119 days）.Beta-lactam-beta-lactamase-inhibitors,cephalosporins,and fluoroquinolones were associated significantly with high fecal bacterial load when used 90 days before CRE detection,while use of cephalosporins,carbapenems,and fiuoroquinolones after CRE detection are significantly associated with longer duration of carriage.The duration of fecal carriage of CRE also correlates significantly with the initial fecal bacterial load （Pearson correlation：0.53; P=0.02）.Conclusion Proactive infection control measures by enhanced surveillance program identify CRE-positive patients and data obtained are useful for the planning of and resource allocation for CRE control.

The interaction between immune cells and hepatic stellate cells(HSCs)can modulate the development of hepatic fibrosis.It can also regulate hepatic fibrosis and liver cirrhosis caused by excessive deposition of extracellular matrix(ECM).This article reviews the action mechanism of immune cells on liver fibrosis and the effect of Astragalus membranaeus and its active components on immune cells.In-depth study of interaction between immune cells and HSCs on the pathogenesis of liver fibrosis,and the regulatory effect of Astragalus membranaeus and its active components on immune mechanism will provide new insights in the treatment of liver fibrosis.

Role of inactivated SARS-CoV-2 vaccine induced T cell responses in ameliorating COVID-19 severity

The severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)vaccine is one of the most effective weapons to fight the coronavirus disease 2019(COVID-19)pandemic(Lazarus et al.,2022).Among all SARS-CoV-2 vaccines generated on different platforms,those consisting of the inactivated virus,such as BBIBP-CorV and CoronaVac,have been frequently used(WHO,2023).By January 30,2023,92.9%of the entire,all-ages population had initiated vaccination and 90.6%had completed their primary series based on the whole population size reported in the seventh census of Chinese mainland(China CDC,2023).However,compared to SARS-CoV-2 mRNA vaccines,which can elicit extremely high quantities of anti-spike(S)antibodies,the levels of neutralizing antibodies induced by inactivated SARS-CoV-2 vaccines are quite lower and wane quickly(Peng et al.,2022;Lim et al.,2021).

Indole-3-propionic Acid-aggravated CCl_(4)-induced Liver Fibrosis via the TGF-β1/Smads Signaling Pathway

Background and Aims:The pathogenesis of liver fibrosis involves liver damage,inflammation,oxidative stress,and intestinal dysfunction.Indole-3-propionic acid(IPA)has been demonstrated to have antioxidant,anti-inflammatory and anticancer activities,and a role in maintaining gut homeostasis.The current study aimed to investigate the role of IPA in carbon tetrachloride(CCl4)-induced liver fibrosis and explore the underlying mechanisms.Methods:The liver fibrosis model was established in male C57BL/6 mice by intraperitoneal injection of CCl4 twice weekly.IPA intervention was made orally(20 mg/kg daily).The degree of liver injury and fibrosis were assessed by serum alanine aminotransferase(ALT),aspartate aminotransferase(AST),and histopathology.Enzyme-linked immunosorbent assay and quantitative real-time polymerase chain reaction(qPCR)were used to detect the inflammatory cytokines.The malondialdehyde(MDA),glutathione,glutathione peroxidase,superoxide dismutase,and catalase were determined via commercial kits.Hepatocyte apoptosis was detected by terminal deoxynucleotidyl transferase-mediated dUTP nick end labeling assay.The expression of mRNA and protein was assayed by qPCR,Western blotting,or immunohistochemical staining.Results:After IPA treatment,the ALT and AST,apoptotic cells,and pro-inflammatory factor levels were enhanced significantly.Moreover,IPA intervention up-regulated the expression of collagen I,α-smooth muscle actin,tissue inhibitor of matrix metalloproteinase-1,matrix metalloproteinase-2,transforming growth factor-β1(TGF-β1),Smad3,and phosphorylated-Smad2/3.Additionally,IPA intervention did not affect the MDA level.Attractively,the administration of IPA remodeled the gut flora structure.Conclusions:IPA aggravated CCl_(4)-induced liver damage and fibrosis by activating HSCs via the TGF-β1/Smads signaling pathway.

3-N-Butylphthalide mitigates high glucose-induced injury to Schwann cells:association with nitrosation and apoptosis

A high glucose state readily causes peripheral axon atrophy, demyelination, loss of nerve fiber function, and delayed regeneration. However, few studies have examined whether nitration is also critical for diabetic peripheral neuropathy. Therefore, this study investigated the effects of high glucose on proliferation, apoptosis, and 3-nitrotyrosine levels of Schwann cells treated with butylphthalide. In addition, we explored potential protective mechanisms of butylphthalide on peripheral nerves. Schwann cells were cultured in vitro with high glucose then stimulated with the peroxynitrite anion inhibitors uric acid and 3-n-butylphthalide for 48 hours. Cell Counting Kit-8 and flow cytometry were used to investigate the effects of uric acid and 3-n-butylphthalide on proliferation and apoptosis of Schwann cells exposed to a high glucose environment. Effects of uric acid and 3-n-butylphthalide on levels of 3-nitrotyrosine in Schwann cells were detected by enzyme-linked immunosorbent assay. The results indicated that Schwann cells cultured in high glucose showed decreased proliferation, but increased apoptosis and intracellular 3-nitrotyrosine levels. However, intervention with uric acid or 3-n-butylphthalide could increase proliferation of Schwann cells cultured in high glucose, and inhibited apoptosis and intracellular 3-nitrotyrosine levels. According to our data, 3-n-butylphthalide may inhibit cell nitrification and apoptosis, and promote cell proliferation, thereby reducing damage to Schwann cells caused by high glucose.
