To the Editor:Hematopoietic cell transplantation(HCT)is a vital modality to cure severe hematological disorders.[1]Global or regional summarized data of HCT are annually reported by the Chinese Blood and Marrow Transplantation Registry Group(CBMTRG),European Society for Blood and Marrow Transplantation(EBMT)and Center for International Blood and Marrow Transplant Research(CIBMTR).It is shown that the number of transplants is continuously increasing,particularly in china in recent years.[2]Since the donor accessibility is significantly addressed by the well-established protocol of haplo-identical HCT(haplo-HCT).

To the Editor: De-escalating the treatment of ductal carcinoma in situ (DCIS) of the breast has been investigated in recent years. A 5-year adjuvant endocrine therapy (AET) regimen is the recommended treatment for hormone receptor-positive DCIS. Selective estrogen receptor modulators (such as the commonly used tamoxifen and toremifene) and aromatase inhibitors (AIs) are the currently used AET drugs. However, long-term AET is accompanied by severe side effects such as menopausal symptoms, endometrial carcinoma, cardiovascular events, venous thromboembolic events, osteoporosis, and joint pain that can significantly reduce patients’ quality of life. It was reported that only 50% of the patients completed the recommended 5-year AET mainly because of side effects, while higher adherence rates were observed with shorter-duration regimens. Therefore, de-escalation of AET should be considered to reduce the side effects of AET and improve adherence.

Objective The purpose of the present study was to observe the changes in CD4＋CD25＋Nrpl＋Treg cells after irradiation with different doses and explore the possible molecular mechanisms involved. Methods ICR mice and mouse lymphoma cell line （EL-4 cells） was used. The expressions of CD4, CD25, Nrpl, calcineurin and PKC-α were detected by flow cytometry. The expressions of TGF-131, IL-10, PKA and cAMP were estimated with ELISA. Results At 12 h after irradiation, the expression of Nrpl increased significantly in 4.0 Gy group, compared with sham-irradiation group （P〈0.05） in the spleen and thymus, respectively, when ICR mice received whole-body irradiation （WBI）. Meanwhile the synthesis of Interleukin 10 （IL-20） and transforming growth factor-β1 （TGF-β1） increased significantly after high dose irradiation （HDR）（〉 or = 1.0 Gy）. In addition, the expression of cAMP and PKA protein increased, while PKC-α, calcineurin decreased at 12h in thymus cells after 4.0 Gy X-irradiation. While TGF-β1 was clearly inhibited when the PLC-PIP2 signal pathway was stimulated or the cAMP-PKA signal pathway was blocked after 4.0 Gy X-irradiation, this did not limit the up-regulation of CD4＋CD25＋Nrpl＋Treg cells after ionizing radiation. Conclusion These results indicated that HDR might induce CD4＋CD25＋Nrpl＋Treg cells production and stimulate TGF-β1 secretion by regulating signal molecules in mice.

BACKGROUND Upper gastrointestinal bleeding(UGIB) after an acute myocardial infarction(AMI) is not an uncommon complication. Acute UGIB caused by Mallory-Weiss syndrome(MWS) is usually a dire situation with massive bleeding and hemodynamic instability. Acute UGIB caused by MWS after an AMI has not been previously *** SUMMARY A 57-year-old man with acute inferior wall ST elevation myocardial infarction underwent a primary coronary intervention of the acutely occluded right coronary artery. Six hours after the intervention, the patient had a severe UGIB,followed by vomiting. His hemoglobin level dropped from 15.3 g/d L to 9.7 g/d L. In addition to blood transfusion and a gastric acid inhibition treatment,early endoscopy was employed and MWS was diagnosed. Bleeding was stopped by endoscopic placement of titanium *** Bleeding complications after stent implantation can pose a dilemma. MWS is a rare but severe cause of acute UGIB after an AMI that requires an early endoscopic diagnosis and a hemoclip intervention to stop bleeding.

Dendritic cell(DC)tumor vaccines exert their antitumor effects through the induction of effector T *** recently identified Tc9 cells as a new potent antitumor effector T cell ***,approaches to direct DCs to preferably prime antitumor Tc9 cells should be further ***,we demonstrate that the addition of interleukin(IL)-33 potently promotes the induction of Tc9 cells by DCs in vitro and in ***-33 treatment also drives the cytotoxic activities of DC-induced Tc9 ***,IL-33 treatment enhances cell survival and proliferation of DC-primed CD8+T *** importantly,the addition of IL-33 during in vitro priming of tumor-specific Tc9 cells by DCs increases the antitumor capability of Tc9 *** studies demonstrated that IL-33 treatment inhibits exhaustive CD8+T cell differentiation by inhibiting PD-1 and 2B4 expression and increasing IL-2 and CD127(IL-7 receptor-α,IL-7Rα)expression in CD8+T ***,the addition of IL-33 further promotes the therapeutic efficacy of DC-based tumor vaccines in the OT-I mouse *** study demonstrates the important role of IL-33 in DC-induced Tc9 cell differentiation and antitumor immunity and may have important clinical implications.

This study was to evaluate effect of ^(125)I brachytherapy combined with chemotherapy on advanced non-small cell lung cancer(NSCLC). Patients with NSCLC in stages III to IV were divided into two groups: Group A(n = 27) received ^(125)I brachytherapy combined with gemcitabine and cisplatin(GP) chemotherapy, and Group B(n = 27) received GP chemotherapy only. The results showed that the overall response rate and median progression-free survival time were 78% and 11.5 months in Group A, 41% and 8 months in Group B, respectively(P 0.05). The interventional complications in Group A included 5 patients with postoperative pneumothorax and 4 patients with hemoptysis. No patients had radiation pneumonia, radiation esophagitis or esophagotracheal fistula. Chemotherapy treatment-related toxicities were not significantly different between the two groups. The relief of tumor-associated symptoms including cough, hemoptysis, chest pain, and short breath was found in both groups, without statistical difference in remission rates between Groups A and B(P > 0.05).In conclusion, ^(125)I brachytherapy combined with chemotherapy proved to be safe and effective for treating advanced NSCLC with few complications. It improves local control rate and prolongs the progression-free survival time.
