We report a case of natural killer(NK)/T-cell lymphoma with concomitant syndrome of inappropriate antidiuretic hormone secretion(SIADH).The patient was a 64-yearold woman with a history of nasopharyngeal carcinoma of over 30 *** was admitted with a chief complaint of intermittent fever for 2 *** after admission indicated a swollen lymph node below the left *** imaging examinations on admission indicated multiple enlarged lymph nodes throughout the *** performed a left submandibular lymph node biopsy,and the results revealed NK/T-cell lymphoma.A biochemical examination indicated Epstein-Barr virus *** the same time,the patient developed *** on her laboratory examination and clinical manifestation,decreased plasma osmolality,urine osmolality greater than plasma osmolality,lack of skin swelling,normal blood pressure,normal renal function,no adrenal function detected on serology,and no abnormalities in imaging examination of the adrenal glands,the likelihood of SIADH in the patient was *** fluid restriction and administration of sodium chloride,the patient’s blood sodium level gradually ***,the immune function of the patient declined,there were severe symptoms of infection,and she died of respiratory ***/T-cell lymphoma associated with SIADH has not,to our knowledge,been previously reported in *** case emphasizes the importance of monitoring serum ion levels,especially serum sodium,in patients with NK/T-cell lymphoma.

Background Continuous positive airway pressure (CPAP) treatment has been proven to be effective in improving the symptoms of coexisting coronary heart disease (CHD) in patients with obstructive sleep apnea hypopnea syndrome (OSAHS). However,it is still unclear whether such improvements are linked to changes in vascular endothelial function. This research was carried out to investigate the effects of CPAP treatment on vascular endothelial function in patients with OSAHS and *** Thirty-six patients with moderate or severe OSAHS and CHD undergoing three months of CPAP treatment were recruited for this study. The changes in their morning plasma nitric oxide (NO) and endothelin (ET) levels,NO/ET ratio,total ischemic burden (TIB) of the myocardium,apnea hypopnea index (AHI),and minimal and mean pulse oxygen saturation (SpO_2) were compared and analyzed before and during CPAP treatment. Results Compared with the plasma levels of ET [(51.39±11.69) ng/L] and NO [(36.67±11.86) μmol/L],NO/ET (0.71±0.14),AHI (32.4±7.9),minimal SpO_2 [(68.9±11.4)%],and myocardial TIB [(66.29±16.37) mm·min] before treatment,there were significant decreases in ET [(33.41±10.03) ng/L] ( P <0.05),increases in NO [(59.89±10.26) μmol/L] and NO/ET (1.79±0.38) ( P <0.01),decreases in AHI (1.9±0.5),and increases in minimal SpO_2 [(90.6±1.8) %] (all P <0.01) and myocardial TIB [(36.42±10.87) mm·min] ( P <0.05) after three months of CPAP *** CPAP treatment may play an important role in the improvement and protection of vascular endothelial dysfunction and myocardial ischemia in OSAHS patients with CHD.

MSCs Relieve Lung Injury of COPD Mice through Promoting Proliferation of Endogenous Lung Stem Cells

Summary： Bone marrow mesenchymal stem cells （MSCs） transplantation could repair injury tissue, but no study confirms whether MSCs can promote the proliferation of endogenous lung stem cells to repair alveolar epithelial cells of mice with chronic obstructive pulmonary disease （COPD）. This study was designed to investigate the effect of MSCs on the proliferation of endogenous lung stem cells in COPD mice to confirm the repair mechanism of MSCs. The mice were divided into control group, COPD group, and COPD＋MSCs group. The following indexes were detected： HE staining of lung tissue, the mean linear intercept （MLI） and alveolar destructive index （DI）, the total cell number in bronchoalveo- lar lavage fluid （BALF）, pulmonary function, alveolar wall apoptosis index （AI） and proliferation index （PI）, the number of CD45-/CD31-/Sca-1＋ cells by flow cytometry （FCM）, and the number of bronchoal- veolar stem cells （BASCs） in bronchoalveolar duct junction （BADJ） by immunofluorescence. As com- pared with control group, the number of inflammatory cells in lung tissue was increased, alveolar septa was destroyed and the emphysema-like changes were seen, and the changes of lung function were in line with COPD in COPD group; AI of alveolar wall was significantly increased and PI significantly decreased in COPD group. There was no significant difference in the number of CD45-/CD31/Sca-1＋ cells and BASCs between control group and COPD group. As compared with COPD group, the number of inflammatory cells in BALF was decreased, the number of CD45/CD31/Sca-l＋ cells and BASCs was increased, AI of alveolar wall was decreased and PI was increased, and emphysema-like changes were relieved in COPD＋MSCs group. These findings suggested that MSCs transplantation can relieve lung injury by promoting proliferation of endogenous lung stem cells in the cigarette smoke-induced COPD mice.

BACKGROUND Multisystemic smooth muscle dysfunction syndrome(MSMDS) is a rare genetic disease worldwide. The main mutation is the actin alpha 2(ACTA2) gene p.R179 H. In this paper, we report a Chinese MSMDS patient and systematically review the previous *** SUMMARY Here, we report a 9.6-month-old Chinese girl who was diagnosed with MSMDS based on her history and symptoms, such as recurrent cough, wheezing, and complications with congenital fixed dilated pupils. Chest high-resolution computed tomography revealed inhomogeneous lung transparency, obvious exudative lesions, and some lung fissures that were markedly thickened. Cranial magnetic resonance imaging excluded bleeding and infarction but showed abnormal signals in the centrum ovale majus and bilateral periventricular regions. Echocardiography only showed patent foramen ovale, and no patent ductus arteriosus, pulmonary artery dilatation, or pulmonary hypertension was found. Bronchoscopy indicated moderate bronchial malacia. These examinations in conjunction with the typical eye abnormality suggested a diagnosis of MSMDS, and sequencing of exon 6 of the ACTA2 gene demonstrated the heterozygous mutation c.536 G>A, p.R179 H. BACKGROUND In clinical practice, community-acquired pneumonia(CAP) can be complicated by rhabdomyolysis(RM), and RM symptoms are mild and easily missed during diagnosis. Moreover, available data on RM induced by CAP are mainly from case reports. Due to the relatively low incidence of CAP-induced RM, more systematic studies are required to understand the characteristics of CAP-induced RM to improve its diagnosis and *** To investigate the clinical characteristics of patients with CAP-induced *** This was a retrospective study of 11 patients with CAP-induced RM. Baseline characteristics, diagnostic work-up, and laboratory test results were summarized and compared with those of 48 patients with exercise-induced RM admitted during the same *** CAP-induced RM was more common in men, and affected older patients compared to those with exercise-induced RM. However, the average age of the patients in this study was lower than the age of peak incidence of CAP in adults in China. The major clinical manifestations were high fever and respiratory symptoms. RM symptoms were mild and often overlooked. Patients with CAP-induced RM had elevated inflammatory parameters, respiratory alkalosis,relatively low serum potassium levels and often had abnormalities in hepatic and renal function and cardiac enzymes. OBJECTIVE： To observe the effect of Xiaochuanping powder （XP）, a traditional Chinese prescrip- tion for the treatment of cough and asthma, on serum concentrations of eosinophil cationic protein （ECP）, tumor necrosis factor （TNF）-a, and interleu- kin （IL）-4, eosinophil counts, as well as expression of matrix metalloproteinase （MMP）-9, tissue inhibi- tor of metalloproteinase （TIMP）-I in the lung tissues of asthmatic rats. METHODS： Sixty clean-grade Sprague-Dawley rats were divided randomly into six groups： normal control （NC）, asthma model, Guilong Kechuanning （GK） group, as well as high, intermediate-, and lowdose XP groups. Rats were sensitized with ovalbu- min （OVA） to trigger asthma. Serum concentrations of ECP, TNF-a and IL-4, eosinophil counts, as well as expression of MMP-9 and TIMP-1 in lung tissues were evaluated using an immunofluorescence method, mRNA expression of MMP-9 and TIMP-1 was determined using real-time quantitative poly- merase chain reaction. RESULTS： Compared with the asthma-model group, serum concentrations of ECP, TNF-a, and IL-4, and eosinophil counts decreased significantly in the high- and intermediate-dose XP groups and GK group （all P 〈 0.01）. Protein expression of MMP-9 and TIMP-1 decreased significantly in the high- and intermediate-dose XP groups and GK group （all P 〈 0.01）. Transcription of MMP-9 and TIMP-1 mRNA and the ratio of expression of MMP-9：TIMP-1 in lung tissue were significantly lower （P 〈 0.01）. CONCLUSION： XP can reduce TNF-a secretion, sup- press the infiltration/activation of eosinophils, re- duce serum concentrations of ECP and IL-4, reduce the protein and mRNA expression of MMP-9 and TIMP-1 in lung tissue, and regulate the balance between expression of MMP-9 and TIMP-1. In these ways, XP alleviated the inflammation and remodel- ing of the airways.

Interleukin-17 inhibits development of malignant pleural effusion via interleukin-9-dependent mechanism

Th17 and Th9 cells have been demonstrated to possess immune regulatory functions in malignant pleural effusion （MPE）. However, whether IL-17 can affect differentiation and function of Th9 cells in MPE remains unknown. The objective of the present study was to explore the impact of IL-17 on the in vivo differentiation of Th9 cells in relation to Th2 cells in a murine model of MPE, and to explore whether IL-17 inhibits MPE formation via IL-9-dependent mechanism. It was found that Th9 and Th2 cells were decreased in MPE from 1L-17-/- mice as compared with wild type mice. IL-17 deficiency inhibited Th9 and Th2 cell differentiation via suppressing transcription faclLors IRF4 and GATA-3, respectively. IL-17 deficiency enhanced MPE formation by promoting angiogenesis and proliferation of pleurai tumors, and thus accelerated the death of mice bearing MPE. The in vivo administration of anti-IL-9 neutralizing mAb accelerated the death of WT mice; whereas administration of exoge- nous IL-9 improved the survival of IL-17-/- mice. Our data provide the first definitive evidence that IL-17 promotes the differ- entiation of Th9 and Th2 cells in MPE. Our findings also demonstrate that IL-17 inhibits the formation of MPE and improves the survival of mice bearing MPE via an IL-9-dependent mechanism.
