Evaluation of the “Helicobacter pylori” Eradication Rate by Bismuth Quadritherapy or Concomitant Quadritherapy in Black Africans

Introduction: Gastric infection by “Helicobacter pylori” remains a topical issue due to the evolving scientific data concerning its pathophysiology, the diseases and pathologies concerned, which now extend beyond the gastric or digestive sphere, and the treatment methods faced with the development of antibiotic resistance. Diagnosis of infection involves two inseparable aspects: identification of the bacterium itself and identification of the endoscopic and histological lesions caused by the bacterium. Objective: To evaluate the rate of eradication of Helicobacter pylori infection by bismuth quadruple therapy and concomitant treatment. Patients and methods: This was a prospective, cross-sectional, analytical study of all outpatients in the hepato-gastroenterology department during the study period from 1 January 2022 to 30 November 2023. All patients had undergone oeso-gastroduodenal fibroscopy and the diagnosis was made by histological examination of the gastric biopsy. Results: Our 113 patients comprised 68 men and 46 women. The sex ratio was 1.48 in favour of men. The mean age of our patients was 40.28 years, with extremes of 13 and 80 years. The clinical signs that prompted oeso-gastroduodenal fibroscopy were as follows: epigastralgia (69.91%), dyspepsia (14.16%), vomiting (7.08%), gastro-oesophageal reflux (6.19%) and altered general condition with weight loss (2.65%). The endoscopic lesions were: gastropathy (antral, fundic and diffuse: antro-fundial) (69.02%), bulbar ulcer (6.19%), gastric ulcer (5.31%). Helicobacter pylori infection was associated with chronic gastritis in all our patients;it was mild in 50%, moderate in 41% and severe in 9%. The eradication rate of Helicobacter pylori according to the treatment protocol used was 92.30% for bismuth quadruple therapy and 94.12% for concomitant quadruple therapy. Introduction: The severity of Sars-Cov-2 infection is associated with the development of acute respiratory distress syndrome (ARDS). The progression to ARDS appears to be driven by a major inflammatory mechanism potentially sensitive to corticosteroids. Observation: This article describes the case of an elderly patient was admitted to emergency departments for intense asthenia, accompanied by motor diarrhoea, dyspnoea with desaturation in ambient air, in a context of strong suspicion of infection linked to COVID-19. The article also reviews the existing literature on the diagnosis and treatment of this severe form of the disease. Conclusion: Corticosteroids, and in particular dexamethasone, have been shown to be effective in the management of patients with COVID-19, an oxygen-releasing disease.

Thyroidectomy: Frequency and Indications in the General Surgery department of the Hôpital National Ignace Deen/CHU de Conakry

Introduction: Thyroidectomy is a surgical procedure to remove all or part of the thyroid gland. The aim of this study was to report the results of thyroidectomy in the general surgery department of the Hôpital national Ignace Deen/CHU de Conakry. Methodology: This was a retrospective study, of seven (07) years (January 1, 2016 - August 31, 2023), in the General Surgery department of the Ignace Deen National Hospital-CHU in Conakry. We included all records of patients admitted and operated on for thyroidectomy and with up-to-date medical records. The variables were epidemiological, clinical and therapeutic. Results: During the study period, we recorded 3221 cases of surgery, including 40 thyroidectomies (1.24% of cases). The average age was 42.4 years. Women were the most represented, with a sex ratio of 0.16. The reason for consultation was anterior cervical swelling in 86% (n = 25) of cases, followed by signs of cervical compression 21% (n = 6) and signs of thyrotoxicosis 31% (n = 9). Indications for thyroidectomy were dominated by homogeneous goitres in 69% (n = 20) of cases, basedow’s disease in 20.7% (n = 6) and nodular goitres in 6.9% (n = 2) of cases. The surgical procedures were lobo-isthmectomies in 72.4% (n = 21), subtotal thyroidectomies 13.8% (n = 4), total thyroidectomies 10.3% (n = 3). Postoperative follow-up was straightforward in 69% (n = 20). Complications included haemorrhage in 20.7% (n = 6) and recurrence in 6.9% (n = 2). The average hospital stay was 7 days. Conclusion: Thyroidectomy is a relatively frequent surgical procedure in our department. Indications are dominated by homogeneous goitres. Morbidity is related to hemorrhage. Rigorous hemostasis could improve the quality of thyroidectomy.

Introduction: Head and neck cancers represent a significant burden of cancer, collectively ranking fourth for cancer incidence and second for cancer mortality worldwide. Objective: To study the epidemiological and histopathological aspects of head and neck cancers in the ENT-CCF department of Ignace Deen National Hospital. Materials and Methods: This was a descriptive retrospective study covering a period of 5 years from September 1, 2016, to September 30, 2021. Results: We collected 43 cases of head and neck cancers, representing a hospital frequency of 1.85%. The mean age was 45.21 ± 13.23 years. Males predominated, accounting for 65.12%, compared to 34.88% for females, with a male-to-female ratio of 1.87. Housewives accounted for 27.90%, followed by farmers at 23.26% and traders/merchants at 18.60%, with 41.86% being uneducated. The main risk factors were tobacco (37.21%) and alcohol (34.88%). Pain (53.49%), dysphonia (51.16%), and dyspnea (39.53%) were the main presenting symptoms. Squamous cell carcinoma (74.42%) and large cell lymphoma (18.60%) were the most commonly cited histological types. Conclusion: Head and neck cancers remain relatively common in our country. Improving prognosis depends on early diagnosis of the condition, resuscitation resources, and prompt management.

The objective of this paper is to evaluate the reliability of a system in its different states (absence of failures, partial failure and total failure) and to propose actions to improve this reliability by an approach based on Monte Carlo simulation. It consists of a probabilistic evaluation based on Markov Chains. In order to achieve this goal, the functionalities of Markov Chains and Monte Carlo simulation steps are deployed. The application is made on a production system. .

First wetland mapping at 10-m spatial resolution in South America using multi-source and multi-feature remote sensing data

Wetland degradation has been accelerating in recent years globally. Accurate information on the geographic distribution and categories of wetlands is essential for their conservation and management. Despite being the world′s fourth largest continent, South America has limited research on wetland mapping, and there is currently no available map that provides comprehensive information on wetland distribution and categories in the region. To address this issue, we used Sentinel-1, Sentinel-2 and SRTM data, developed a sample collection method and a wetland mapping method with a collection of multi-source features such as optical features, polarization features and shape features for South American wetlands. We produced a 10-m resolution wetland map based on the Google Earth Engine（GEE） platform. Our Level-1 wetland cover map accurately captured six wetland sub-categories with an overall accuracy of 96.24% and a kappa coefficient of 0.8649, while our Level-2 water cover map included five sub-categories with an overall accuracy of 97.23% and a kappa coefficient of0.9368. The results show that the total area of existing wetlands in South America is approximately 1,737,000 km2, which is6.8% of the total land area. Among the ten wetland categories, shallow sea had the largest area（960,527.4 km2）, while aquaculture ponds had the smallest area 1513.6 km2. Swamp had the second largest area（306,240.1 km2）. Brazil, Argentina,Venezuela, Bolivia, and Colombia were found to have the largest wetland areas, with Brazil and Colombia having the most diverse wetland categories. This product can serve as baseline data for subsequent monitoring, management, and conservation of South American wetlands.

This work consisted of assessing the toxicity of a recipe of three plants used in traditional Togolese medicine. Acute and sub-acute toxicity was assessed according to OECD chemical test guidelines n˚423 dated December 17, 2001 and n˚407 dated October 3, 2008 respectively. Cytotoxicity was assessed using the 3-(4,5-dimethylthiazolyl-2-yl-2,5-diphenyltetrazolium bromide (MTT) test. No significant toxicity was observed after 14 and 28 days, although a dose-dependent decrease in creatinemia was observed in male rats (the recipe to be used to moderate creatinine levels). Cytotoxicity was without effect on NCM 365 cells. The results obtained justify the use of the recipe in traditional medicine.

The role of tropical forests in the global carbon budget remains controversial,as carbon emissions from deforestation are highly *** high uncertainty arises from the use of either fixed forest carbon stock density or maps generated from satellite-based optical reflectance with limited sensitivity to biomass to generate accurate estimates of emissions from *** space missions aiming to accurately map the carbon stock density rely on direct measurements of the spatial structures of forests using lidar and *** found that lost forests are special cases,and their spatial structures can be directly measured by combining archived data acquired before and after deforestation by space missions principally aimed at measuring ***,using biomass mapping,we obtained new estimates of carbon loss from deforestation ahead of forthcoming space ***,using a high-resolution map of forest loss and the synergy of radar and lidar to estimate the aboveground biomass density of forests,we found that deforestation in the 2000s in Latin America,one of the severely deforested regions,mainly occurred in forests with a significantly lower carbon stock density than typical mature forests.
