Transient elastography and diffusion-weighted magnetic resonance imaging for assessment of liver fibrosis in children with chronic hepatitis C

Nephronophthisis-related ciliopathies(NPHP-RC)represent one of the most common causes of chronic kidney disease in the first three decades of life and are characterized by a broad genetic and clinical heterogeneity.1 To date,more than 90 genes have been identified that cause autosomalrecessive NPHP-RC if mutated,accounting for up to 60%of cases.1 Among these,homozygous deletions of NPHP1 are the most common *** genes localize to primary cilia,basal bodies,or the centrosome and lead to a primary ciliary disruption if mutated,thereby causing a broad phenotypical spectrum.1 Patients that suffer from NPHP-RC can either have an isolated renal phenotype,such as polyuria,polydipsia,decreased urinary concentration ability,and secondary enuresis due to loss of tubular function,or present with extrarenal symptoms including retinal degeneration,cerebellar vermis hypoplasia,or hepatic *** are often diagnosed in early adolescence,reaching end-stage renal disease before 25 years of age,but early onset and rapidly progressive forms of NPHP-RC also exist.1 Renal ultrasound indicates kidneys of normal or reduced renal length with increased echogenicity and corticomedullary cysts.

We assessed the global incidence, mortality, and disability-adjusted life years(DALYs) associated with various liver diseases, including alcohol-related liver disease(ALD), hepatitis B/C virus infections(HBV or HCV), liver cancer, metabolic dysfunction-associated steatotic liver disease(MASLD), and other chronic liver diseases, from the 2019 Global Burden of Disease study. Additionally, we analyzed the global trends in hepatology research and drug development. From 2000 to 2019, prevalence rates increased for ALD, MASLD and other liver diseases, while they decreased for HBV, HCV, and liver cancer. Countries with a high socio-demographic index(SDI) exhibited the lowest mortality rates and DALYs. The burden of liver diseases varied due to factors like sex and region. In nine representative countries, MASLD,along with hepatobiliary cancer, showed highest increase in funding in hepatology research. Globally, the major research categories in hepatology papers from 2000 to 2019 were cancer, pathobiology, and MASLD. The United States(U.S.) was at the forefront of hepatology research, with China gradually increasing its influence over time. Hepatologists worldwide are increasingly focusing on studying the communication between the liver and other organs, while underestimating the research on ALD. Cancer, HCV, and MASLD were the primary diseases targeted for therapeutic development in clinical trials. However, the proportion of new drugs approved for the treatment of liver diseases was relatively low among all newly approved drugs in the U.S., China, Japan, and the European Union. Notably, there were no approved drug for the treatment of ALD in the world.

BACKGROUND Hepatitis B virus(HBV)is categorized as one of the smallest enveloped DNA viruses and is the prototypical virus of the Hepatoviridae *** is usually transmitted through body fluids such as blood,semen,and vaginal *** majority(more than 95%)of immunocompetent adults infected with HBV spontaneously clear the *** the context of the high prevalence of HBV infection in Albania,the research gap is characterized by the lack of studies aimed at advancing the current understanding and improving the prevailing *** main objective of this study was to address the low rate of HBV diagnosis and the lack of a comprehensive national program to facilitate widespread *** To analyze the prevalence of HBV infection in Albania and elucidate the persistently high prevalence despite efforts and measures *** Using a systematic literature review,we collected existing research on the epidemiology of HBV in Albania from PubMed,Cochrane Library,Google Scholar,and Albanian Medical Journals,focusing on studies published after the 1980s and conducted solely in the Albanian *** The findings reveal a dynamic shift in HBV prevalence in Albania over several *** high,the prevalence gradually declined following the implementation of screening and vaccination ***,the prevalence rates have remained notably high,exceeding 8%in recent *** factors include vertical transmission,inadequate healthcare infrastructure,and challenges in screening and *** among Albanian refugees in neighboring countries also reported high prevalence rates,emphasizing the need for transnational *** advancements in screening,vaccination,and healthcare infrastructure,Albania continues to face a substantial burden of HBV *** The persistence of high prevalence underscores the complexity of the issue,requiring ongoing efforts to ensure a comprehensive understand

Liver transplantation serves as a life-saving intervention for patients with endstage liver disease,yet long-term survival remains a ***-liver transplant obesity seems to have a significant contribution to this challenge and it emerges as a significant risk factor for graft steatosis,metabolic syndrome and denovo malignancy *** review synthesizes current literature on prevalence,risk factors and management strategies for post-liver transplant obesity,emphasizing its impact on graft and patient *** review consultation was conducted in Medline/PubMed,SciELO and EMBASE,with the combination of the following keywords:Weight management,liver transplantation,immunosuppressive therapy,lifestyle interventions,bariatric *** therapy has a significant influence on long-term survival of liver transplant patients,yet it seems to have lesser effect on post-transplant obesity development than previously ***,it significantly contributes to the development of other components of metabolic *** predisposing factors for post-transplant obesity development encompass elevated recipient and donor body mass index,a history of alcoholic liver disease,hepatocellular carcinoma,male gender,the absence of cellular rejection and the marital status of the *** immunosuppressive regimens,pharmacotherapy,lifestyle interventions and bariatric surgery represent key components in mitigating post-transplant obesity and improving long-term survival and quality of life in this group of *** identification and intervention thus hold paramount *** research is warranted to refine optimal management strategies and enhance outcomes in this patient population.
