AIM To identify certain clinical factors other than the type of gastrectomy which affect the postoperative quality of life(QOL) of patients after *** The postgastrectomy syndrome assessment scale(PGSAS)-45 was designed to assess the severity of symptoms, the living status and the QOL of gastrectomized patients. It consists of 45 items, of which 22 are original items while 23 were retrieved from the SF-8 and Gastrointestinal Symptoms Rating Scale questionnaires with permission. A nationwide surveillance study to validate PGSAS was conducted and 2368 gastric cancer patients who underwent various types of gastrectomy at 52 medical institutions were enrolled. Of these, 1777 patients who underwent total gastrectomy(TG) reconstructed with Roux-Y(n = 393), distal gastrectomy(DG) reconstructed with Billroth-I(n = 909), or DG reconstructed with Roux-Y(n = 475) were evaluated in the current study. The influence of the type of gastrectomy and other clinical factors such as age, sex, duration after surgery, the symptom severity, the degree of weight loss, dietary intake, and the ability for working on the postoperative QOL(i.e., dissatisfaction for daily life subscale, physical component summary and mental component summary of the SF-8) were examined by multiple regression analysis(MRA). In addition, importance of various symptoms such as esophageal reflux, abdominal pain, meal-related distress, indigestion, diarrhea, constipation and dumping on the postoperative living status and QOL were also appraised by *** The postoperative QOL were significantly deteriorated in patients who underwent TG compared to those after DG. However, the extent of gastrectomy was not an influential factor on patients' QOL when adjusted by the MRA. Among various clinical factors, the symptom severity, ability for working, and necessity for additional meals were the most influential factorsto the postoperative QOL. Benefit of laparoscopic liver resection in high body mass index patients

AIM: To explore the impact of body mass index(BMI) on surgical outcomes in patients undergoing laparoscopic liver resection(LLR).METHODS: From January 2010 to February 2015, sixty-eight patients who underwent primary partial liver resection in our institute were retrospectively reviewed. Surgical outcomes of LLR were compared with those of open liver resection(OLR). In addition, we analyzed associations with BMI and surgical ***: Among 68 patients, thirty-nine patients underwent LLR and 29 were performed OLR. Significant difference in operation time, blood loss, and postoperative hospital stay was observed. There were no signi ficant di fferences in mortali ty and morbidity in two groups. Twenty-two patients(32.4%) were classified as obese(BMI ≥ 25). A statistically significant correlation was observed between BMI and operation time, between BMI and blood loss in OLR, but not in LLR. The operation time and blood loss of OLR were significantly higher than that of LLR in obese patients. Open liver resection and BMI were independent predictors for prolonged operation time and increased blood loss in multivariate ***: The present study demonstrated that BMI had influenced to surgical outcomes of OLR. LLR was less influenced by BMI and had great benefit in obese patients.

Primary hepatic leiomyosarcoma with liver metastasis of rectal cancer

Primary hepatic leiomyosarcoma is a particularly rare tumor with a poor prognosis. Curative resection is currently the only effective treatment, and the efficacy of chemotherapy is unclear. This represents the first case report of a patient with primary hepatic leiomyosarcoma co-existing with metastatic liver carcinoma. We present a 59-year-old man who was diagnosed preoperatively with rectal cancer with multiple liver metastases. He underwent a curative hepatectomy after a series of chemotherapy regimens with modified FOLFOX6 consisting of 5-fluorouracil, leucovorin and oxaliplatin plus bevacizumab, FOLFIRI consisting of 5-fluorouracil, leucovorin and irinotecan plus bevacizumab, and irinotecan plus cetuximab. One of the liver tumors showed a different response to chemotherapy and was diag-nosed as a leiomyosarcoma following histopathological examination. This case suggests that irinotecan has the potential to inhibit the growth of hepatic leiomyosarcomas. The possibility of comorbid different histological types of tumors should be suspected when considering the treatment of multiple liver tumors.

BAP31 is frequently overexpressed in patients with primary colorectal cancer and correlates with better prognosis

We previously showed that B cell receptor associated protein 31(BAP31) was significantly upregulated in colorectal cancer compared with normal mucosa epithelia. However, its expression pattern and pathological role in colorectal cancer are not clearly understood. In this study, we investigated whether the expression of BAP31 was associated with the clinicopathological parameters of colorectal cancer. The expression pattern of BAP31 was detected by immunohistochemistry on a tissue microarray in both primary tumor and paired distant normal mucosa samples from 120 consecutive colorectal cancer patients. Furthermore, BAP31 protein expression was also determined in twenty colorectal adenomas and eight liver metastasis samples. There was positive expression of BAP31 in 64.17% of primary tumors and 6.67% in distant normal mucosa (P=0.000). Negative expression of BAP31 was correlated with distant metastasis (P=0.036) and lower tumor differentiation grade (P=0.001). Patients with BAP31-negative expression showed significantly lower overall survival rate (P=0.003) compared to patients with BAP31-positive expression. Our results demonstrate that BAP31 may serve as a candidate prognostic marker in colorectal cancer and negative BAP31 expression may lead to more aggressive invasion of colorectal cancer.

Effect of Vacuum Sealed Drainage on Recovery of Gastrointestinal Function in Gastric Cancer Patients after Radical Gastrectomy

Objective This study aims to explore the effect of vacuum sealed drainage on the recovery of gastrointestinal function in gastric patients after radical gastrectomy. Methods One hundred and twenty patients who received radical gastrectomy for gastric cancer were randomly divided into two *** in the control group received continuous gastrointestinal decompression to drain the gastric juices after radical gastrectomy,whereas patients in the treatment group received vacuum sealed *** postoperative variables between the two groups were compared,including time of bowel sound reoccurrence,time of the first flatus,indwelling time of gastric tube,days of hospitalization,and complications,such as anastomotic leakage,intestinal obstruction,wound infection,pulmonary infection,fever, and *** 13.0 was used to analyze the data. Results Significant differences in the following variables were observed in patients between the two groups:time of bowel sound reoccurrence,time of the first flatus,indwelling time of gastric tube,and length of hospitalization of the *** value of each of these variables was much smaller in the treatment group than in the control group(P0.05). However,a significant differences were observed in the incidence of pulmonary infection,fever,and pharyngitis among the patients between the two groups(P<0.05),with much lower incidence of the variables in the treatment group than in the control group. Conclusions Vacuum sealed drainage used in gastric cancer patients after radical gastrectomy can accelerate the recovery of gastrointestinal function and reduce postoperative ***,it shortens the indwelling time of the gastric tube, thereby making the patients feel comfortable without the disturbance from the gastric tube.

Hilar cholangiocarcinoma with intratumoral calcification: A case report

This report describes a rare case of hilar cholangiocarcinoma with intratumoral calcification that mimicked hepatolithiasis. A 73-year-old man presented to a local hospital with a calcified lesion in the hepatic hilum. At first,hepatolithiasis was diagnosed,and he underwent endoscopic stone extraction via the transpapillary route. This treatment strategy failed due to biliary stricture. He was referred to our hospital,and further examination suggested the existence of cholangiocarcinoma. He underwent left hepatectomy with caudate lobectomy and extrahepatic bile duct resection. Pathological examination revealed hilar cholangiocarcinoma with intratumoral calcification,while no stones were found. To the best of our knowledge,only one case of calcified hilar cholangiocarcinoma has been previously reported in the literature. Here,we report a rare case of calcified hilar cholangiocarcinoma and reveal its clinicopathologic features.

Portal vein stenosis after pancreatectomy following neoadjuvant chemoradiation therapy for pancreatic cancer

Extrahepatic portal vein (PV) stenosis has various causes, such as tumor encasement, pancreatitis and as a postsurgical complication. With regard to post-pancreaticoduodenectomy, intraoperative radiation therapy with/ without PV resection is reported to be associated with PV stenosis. However, there has been no report of PV stenosis after pancreatectomy following neoadjuvant chemoradiation therapy (NACRT). Here we report the cases of three patients with PV stenosis after pancreatectomy and PV resection following gemcitabine-based NACRT for pancreatic cancer and their successful treatment with stent placement. We have performed NACRT in 18 patients with borderline resectable pancreatic cancer since 2005. Of the 15 patients who completed NACRT, nine had undergone pancreatectomy. Combined portal resection was performed in eight of the nine patients. We report here three patients with PV stenosis, and thus the ratio of post-operative PV stenosis in patients with PV resection following NACRT is 37.5% in this series. We encountered no case of PV stenosis among 22 patients operated with PV resection for pancreatobiliary cancer without NACRT during the same period. A relationship between PV stenosis and NACRT is suspected, but further investigation is required to determine whether NACRT has relevance to PV stenosis.

A Prognostic Nomogram with High Accuracy Based on 2D-SWE in Patients with Acute-on-chronic Liver Failure

Background and Aims:Acute-on-chronic liver failure(ACLF)is associated with very high *** prediction of prognosis is critical in navigating optimal treatment decisions to improve patient *** study was aimed to develop a new nomogram integrating two-dimensional shear wave elastography(2D-SWE)values with other independent prognostic factors to improve the precision of predicting ACLF patient ***:A total of 449 consecutive patients with ACLF were recruited and randomly allocated to a training cohort(n=315)or a test cohort(n=134).2D-SWE values,conventional ultrasound features,laboratory tests,and other clinical characteristics were included in univariate and multivariate *** with prognostic value were then used to construct a novel prognostic *** operating curves(ROCs)were generated to evaluate and compare the performance of the novel and published models including the Model for EndStage Liver Disease(MELD),MELD combined with sodium(MELD-Na),and Jin’s *** model was validated in a prospective cohort(n=102).Results:A ACLF prognostic nomogram was developed with independent prognostic factors,including 2D-SWE,age,total bilirubin(TB),neutrophils(Neu),and the international normalized ratio(INR).The area under the ROC curve(AUC)was 0.849 for the new model in the training cohort and 0.861 in the prospective validation cohort,which were significantly greater than those for MELD(0.758),MELD-Na(0.750),and Jin’s model(0.777,all p<0.05).Calibration curve analysis revealed good agreement between the predicted and observed *** new nomogram had superior overall net benefit and clinical ***:We established and validated a 2D-SWE-based noninvasive nomogram to predict the prognosis of ACLF patients that was more accurate than other prognostic models.
