Gemcitabine Based Combination Regimens for Treatment of Refractory Advanced Breast Cancer

Objective： Anthracycline and taxane are the standard agents in combined chemotherapy of advanced breast cancer. However, when these agents based chemotherapy is failure, the selection of salvage regimen is still of problem. Gemcitabine, an active agent in both lung cancer and pancreas cancer, is demonstrated effective in breast caner. But there have been relatively less data of gemcitabine in anthracycline and/or taxane-resistant breast cancer. Therefore we employe this study to explore the efficacy and safety of gemcitabine based combination regimen in this population. Methods： From May 2002 to March 2006, 28 patients with measurable lesion of advanced metastatic breast cancer who were resistant to prior anthracycline and taxane based chemotherapy were enrolled. Patients were treated with gemcitabine based combination chemotherapy with a median cycles of 3 （range 2-6）. Results： The overall response rate was 28.6% （8/28）, with 1 CR （Complete response 3.5%） and 7 PRs （Partial response 25%）. Stable disease was seen in 8 patients （28.6%） while disease progressed in 12 patiens （42.8%）. The median time to progression was 4.5 m （range, 2-23 m）. The main toxicity included bone marrow depression, alopecia, mucositis and peripheral neurotoxicity. The grade 3 to 4 clinical adverse effect was leukopenia in 5 cases （17.9%） and thrombocytopenia in 8 cases （30%）. Conclusion： Gemcitabine based combination regimens is feasible in anthracycline and taxane-resistant advanced breast cancer. The clinical response and TTP is acceptable with limited toxicity pattern.

In vitro amplified human leukocyte antigen(HLA)-haploidentical donor immune cell infusion(HDICI)is not commonly used in ***,our study sought to evaluate its safety for treating childhood *** September 2011 and September 2012,12 patients with childhood malignancies underwent HDICI in Sun Yat-sen University Cancer *** median patient age was 5.1 years(range,1.7-8.4 years).Of the 12 patients,9 had high-risk neuroblastoma(NB)[7 showed complete response(CR),1 showed partial response(PR),and 1 had progressive disease(PD)after multi-modal therapies],and 3 had Epstein-Barr virus(EBV)-positive lymphoproliferative disease(EBV-LPD).The 12 patients underwent a total of 92 HDICIs at a mean dose of 1.6×108immune cells/kg body weight:71 infusions with natural killer(NK)cells,8 with cytokine-induced killer(CIK)cells,and 13 with cascade primed immune cells(CAPRIs);83 infusions with immune cells from the mothers,whereas 9 with cells from the *** cases(21.7%)of fever,including 6 cases(6.5%)accompanied with chills and 1(1.1%)with febrile convulsion,occurred during infusions and were alleviated after symptomatic *** cases(5.4%)of mild emotion changes were *** other adverse events occurred during and after the completion of *** acute nor chronic graft versus host disease(GVHD)was observed following *** a median of 5.0 months(range,1.0-11.5 months)of follow-up,the 2 NB patients with PR and PD developed PD during *** the other 7 NB patients in CR,2 relapsed in the sixth month of HDICIs,and 5 maintained CR with disease-free survival(DFS)ranging from 4.5 to 11.5 months(median,7.2months).One EBV-LPD patient achieved PR,whereas 2 had stable disease(SD).Our results show that HDICI is a safe immunotherapy for childhood malignancies,thus warranting further studies.

Objective: To explore the clinical characteristics,diagnosis and treatment of pulmonary sarcoma andblastoma. Methods: Seven cases of pulmonary sarcoma(including 1 case of pulmonary carcinosarcoma, 1 case ofpulmonary malignant fibroneuroma, 1 case of pulmonarymalignant fibrous tissue tumor and 1 case of pulmonaryblastoma from August 1995 to June 2002 in our hospital) were retrospectively summarized the histologicalcharacteristics, clinical features, diagnosis and treatment. Results: Pulmonary sarcoma and blastoma both are raremalignant tumors and can be easily misdiagnosed ormistreated. The main symptoms of both diseases are cough and expectoration. X ray or CT shows node or focus in *** both are diagnosed by histological pathology. Because the symptoms of these diseases are not typical, it is difficultfor patients to be diagnosed without pathologyConclusion: These two kinds of diseases are often easy to be misdiagnosed. Early diagnosis and treatment are importantto improve the efficiency of treatment. Surgical resection is the best method of treatment, and it is important to combine with chemotherapy and radiation treatment.

AIM: To detect the expression of the long noncoding RNA HOTAIR in colon cancer and analyze its relationship with clinicopathological parameters of colon cancer. METHODS: Total RNA was extracted from 80 colon cancer tissues and matched tumor-adjacent normal colon tissues and reverse transcribed. Quantitative polymerase chain reaction was used to detect the expression of HOTAIR. The relationship between the expression of HOTAIR and clinicopathological parameters of colon cancer was analyzed. RESULTS: The expression of HOTAIR was significantly higher in colon cancer tissues than in matched tumoradjacent normal colon tissues(P entiated and undifferentiated cases than in highly and moderately differentiated cases; and in stages Ⅲ + Ⅳ cases than in stages?Ⅰ?+ Ⅱ cases(P ent and metastasis of colon cancer. HOTAIR may act as an oncogene and represents a new molecular target for the treatment of colon cancer.

This paper discusses the rationale for phaseⅢtesting of neoadjuvant therapy in patients affected by resectable pancreatic *** therapeutic management of patients affected by resectable pancreatic cancer is particularly troublesome due to the aggressiveness of the disease and to the limited efficacy and sometimes unfavourable risk-benefit ratio of the available therapeutic *** data on the role of adjuvant chemoradiation have been reported,while adjuvant single-agent chemotherapy significantly improved overall survival(OS)when compared to surgery alone. However,the OS figures for adjuvant chemotherapy remain *** effect,pancreatic cancer exhibits a prominent tendency to recur after a brief median time interval from surgery and extra-pancreatic dissemination represents the predominant pattern of disease *** treatment has a strong rationale in this disease but limited information on the efficacy of this approach is available from single arm trials with low levels of ***,in spite of two decades of investigation there is currently no evidence to support the routine use of pre-surgical therapy in clinical practice. To foster knowledge on the optimal management of this disease,and to produce evidence-based treatment guidelines,there is no alternative to well designed randomized *** chemotherapy is a candidate for testing because it is supported by a more robust rationale than *** chemotherapy regimens with elevated activity in advanced disease warrant *** would suggest the running of an exploratory phaseⅡrandomized trial before embarking on a large phase Ⅲ study.

BACKGROUND This manuscript describes the first known cases of sick sinus syndrome(SSS)associated with the use of anlotinib in non-small cell lung cancer patients,highlighting the need for increased vigilance and cardiac *** SUMMARY Two patients with non-small cell lung cancer developed SSS after 15 months and 5 months of anlotinib treatment,respectively,presenting with syncope and *** confirmed SSS,and different treatment approaches were taken for each *** patient received a dual-chamber permanent pacemaker,while the other discontinued the medication and experienced symptom *** Anlotinib can induce SSS,suggesting that cardiac monitoring is crucial during anlotinib *** management strategies are necessary for affected individuals.

AIM： To investigate the relationship between the -765G 〉 C COX-2 polymorphism and the development of different gastric lesions： atrophy or intestinal metaplasia and gastric adenocarcinoma. METHODS： A cross-sectional study was performed involving 320 Portuguese individuals （210 without evidence of neoplastic disease, 73 patients with gastric adenocarcinomas and 37 with atrophy or intestinal metaplasia） using a PCR-RFLP method.RESULTS： -765C allele was overrepresented in the patients with gastric adenocarcinoma （51%） when compared either with the control group （38%） or patients with atrophy or intestinal metaplasia （27%）. Callele was found to be very common in our population （0.22）, and a multivariate logistic regression analysis revealed nearly 3-fold increased risk for the progression to gastric adenocarcinoma in patients with atrophy or intestinal metaplasia carrying the -765C allele （OR = 2.67, 95% CI = 1.03-6.93; P = 0.04）.CONCLUSION： -765C carrier status should be considered as another susceptibility marker for gastric adenocarcinoma development in patients with atrophy or intestinal metaplasia.

Hypertriglyceridemia is positively correlated with the development of colorectal tubular adenoma in Japanese men

AIM： To determine the real association between serum lipid levels and colonic polyp formation. METHODS： We performed a large scale retrospective study to analyze the correlation between the incidence of colorectal adenoma or carcinoma and the fasting serum levels of total cholesterol （TC） and triglycerides （TG） in patients who underwent total colonoscopy for screening for colon cancer. RESULTS： Both levels were significantly elevated in patients with adenomas as compared with patients without any neoplastic lesion （TC 207.6 ± 29.5 vs 199.5 ±34.3, n= 4883, p〈 0.001; TG 135.0 ± 82.2 vs 108.7± 71.5, n= 4874, p〈 0.001）. The difference was significant in patients with tubular adenoma but not in those with villous or serrated adenoma. Multiple logistic regression analysis including age and sex revealed that TG was an independent correlation factor in male （p〈0.01）, but not in female patients. The level of TG in patients with invasive carcinoma did not show a significant elevation from that in patients with adenoma. These findings suggest that hypertriglyceridemia is an independent risk factor for colonic adenoma in men. CONCLUSION： Although a high level of serum triglyceride does not appear to be mechanically involved in the development of carcinoma, reduction of serum TG and intensive surveillance with total colonoscopy may have benefit in men with hypertriglyceridemia.
