Arsenic is a carcinogenic environmental factor found in food and drinking water around the *** mechanisms in which arsenic alters homeostasis are not fully *** the past few decades,light has been shed on varying mechanisms in which arsenic induces *** mechanisms include gut microbe perturbations,genotoxic effects,and epigenetic *** microbe perturbations have been shown to increase the level of pathogen-associated molecular patterns such as lipopolysaccharide(LPS) leading to uncontained *** in inflammation is the major factor in cirrhosis leading to hepatocellular *** in gut permeability and metabolites have also been observed as a fallout of arsenic induced gut microbe *** guts proximity and interaction through portal flow make the liver susceptible to gut perturbations and ensuing inflammatory *** and epigenetic dysregulation induced by arsenic and its toxic metabolites present a more direct mechanism that works synergistically with gut microbe perturbations to induce the incidence of *** pathways combined could be some of the main causes of arsenic-induced carcinogenesis.

Microglia are present throughout the central nervous system and are vital in neural repair,nutrition,phagocytosis,immunological regulation,and maintaining neuronal *** a healthy spinal cord,microglia are accountable for immune surveillance,however,when a spinal cord injury occurs,the microenvironment drastically changes,leading to glial scars and failed axonal *** this context,microglia vary their gene and protein expression during activation,and proliferation in reaction to the injury,influencing injury responses both favorably and unfavorably.A dynamic and multifaceted injury response is mediated by microglia,which interact directly with neurons,astrocytes,oligodendrocytes,and neural stem/progenitor *** a clear understanding of their essential nature and origin,the mechanisms of action and new functions of microglia in spinal cord injury require extensive *** review summarizes current studies on microglial genesis,physiological function,and pathological state,highlights their crucial roles in spinal cord injury,and proposes microglia as a therapeutic target.

Previous reports clearly demonstrated that Helicobacter pylori(***)infection,nonsteroidal anti-inflammatory drugs(NSAID)or low dose aspirin(ASA)use significantly and independently increased the risk for the development of peptic ulcer ***,the presence of *** infection associated with low dose ASA and/or NSAID use in the same patient is becoming more frequent and therefore the potential interaction between these factors and the consequences of it has important *** NSAID intake in the presence of *** infection may further increase the risk of peptic ulcer carried by the presence of only one risk factor is still a matter of *** on the interaction between the two risk factors yielded conflicting data and no consensus has been reached in the last *** addition,the interaction between *** infection and low-dose ASA remains even more *** real clinical practice,we can find different clinical scenarios involving these three factors associated with the presence of different gastrointestinal and cardiovascular risk *** huge variety of possible combinations greatly hinder the decision making process of physicians.

Activated hepatic stellate cells (HSCs) play important roles in hepatic fibrosis. Studies on HSCs activation in vitro have shown that this process is regulated by a wide variety of growth factors and cytokines.1 Recent data indicate that AngⅡ is responsible for the mechanisms of myocardial fibrosis and kidney fibrosis; but there are only few reports on hepatic fibrosis.2-8

Metaheuristics are commonly used in various fields,including real-life problem-solving and engineering appli*** present work introduces a novel metaheuristic algorithm named the Artificial Circulatory system Algorithm(ACSA).The control of the circulatory system inspires it and mimics the behavior of hormonal and neural regulators involved in this *** work initially evaluates the effectiveness of the suggested approach on 16 two-dimensional test functions,identified as classical benchmark *** method was subsequently examined by application to 12 CEC 2022 benchmark problems of different ***,the paper evaluates ACSA in comparison to 64 metaheuristic methods that are derived from different approaches,including evolutionary,human,physics,and ***,a sequence of statistical tests was undertaken to examine the superiority of the suggested algorithm in comparison to the 7 most widely used algorithms in the existing *** results show that the ACSA strategy can quickly reach the global optimum,avoid getting trapped in local optima,and effectively maintain a balance between exploration and *** outperformed 42 algorithms statistically,according to post-hoc *** also outperformed 9 algorithms *** study concludes that ACSA offers competitive solutions in comparison to popüler methods.

Characteristics of Transient Vortices in the Boundary Layer on a Rotating Disk under Orbital Motion

The objective of this study is to experimentally examine the characteristics of transient vortices in the boundary layer on a disk undergoing both rotation and orbital motion. The velocity fluctuations on a rotating, orbiting disk （disk radius equal to orbital radius） are measured by the hot-wire method, and the effects of orbital motion on the transient vortices in the boundary layer are examined. When the ratio of the orbital speed to the speed of rotation is i-0.025, the interval of transient vortices depends on only the orbital radius, regardless of the directions of rota- tion and orbital motion. The rate of low-frequency disturbances increases as the orbital speed increases, and the vortices induced by these low-frequency disturbances travel over the disk and then develop in the region of in- creased velocity. Consequently, no vortices generated on a rotating disk under orbital motion are stationary rela- tive to the disk.

Predictive factors of survival in patients treated with definitive chemoradiotherapy for squamous cell esophageal carcinoma

AIM： The aim of the study was to evaluate the predictive factors of survival in patients with locally advanced squamous cell esophageal carcinoma （LASCOC） treated with definitive chemoradiotherapy （CRT） regimen based on the 5FU/CDDP combination. METHODS： All patients with LASCOC treated with a definitive CRT using the 5FU/CDDP combination between 1994 and 2000 were retrospectively included. Clinical complete response （CCR） to CRT was assessed by esophageal endoscopy and C-F-scan 2 mo after CRT completion. Prognostic factors of survival were assessed using univariate and multivariate analysis by the Cox regression model. RESULTS： A total of 116 patients were included in the study. A CCR to CRT was observed in 86/116 （74.1%）. The median survival was 20 mo （range 2-114） and the 5-year survival was 9.4%. Median survival of responder patients to CRT was 25 mo （range 3-114） as compared to 9 mo （range 2-81） in non-responder patients （P 〈 0.001）. In univariate analysis, survival was associated with CCR （P 〈 0.001）, WHO performance status 〈 2 （P = 0.01）, tumour length 〈 6 cm （P = 0.045） and weight loss 〈 10% was in limit of significance （P = 0.053）. In multivariate analysis, survival was dependant to CCR （P 〈 0.0001）, weight loss 〈 10% （P = 0.034） and WHO performance 〈 2 （P = 0.046）. CONCLUSION： Our results suggest that survival in patients with LASCOC b＇eated with definitive CRT was correlated to CCR, weight loss and WHO performance status.

Structure learning on Bayesian networks by finding the optimal ordering with and without priors

Ordering based search methods have advantages over graph based search methods for structure learning of Bayesian networks in terms on the efficiency. With the aim of further increasing the accuracy of ordering based search methods, we first propose to increase the search space, which can facilitate escaping from the local optima. We present our search operators with majorizations, which are easy to implement. Experiments show that the proposed algorithm can obtain significantly more accurate results. With regard to the problem of the decrease on efficiency due to the increase of the search space, we then propose to add path priors as constraints into the swap process. We analyze the coefficient which may influence the performance of the proposed algorithm, the experiments show that the constraints can enhance the efficiency greatly, while has little effect on the accuracy. The final experiments show that, compared to other competitive methods, the proposed algorithm can find better solutions while holding high efficiency at the same time on both synthetic and real data sets.

Azathioprine is currently the key drug in the maintenance treatment of inflammatory bowel diseases. However, there are still some practical issues to be resolved: one is how long we must maintain the drug. Given that inflammatory bowel diseases are to date chronic, non-curable conditions, treatment should be indefinite and only the loss of efficacy or the appearance of serious side effects may cause withdrawal. As regards to efficacy and their maintenance over time, evidence supports the continuous usefulness of the drug in the long term: in fact its withdrawal very substantially increases the risk of relapse. About side effects, azathioprine is a relatively well tolerated drug and even indefinite use seems safe. The main theoretical risks of prolonged use would be the myelotoxicity, hepatotoxicity, and the development of cancer. In fact, serious bone marrow suppression or serious liver damage are uncommon, and can be minimized with proper use of the drug. Recent metanalysis suggests that the risk of lymphoma is real, but the individual risk is rather low, and decision analysis suggests a favorable benefit/risk ratio in the long term. Therefore, in patients with inflammatory bowel diseases in whom azathioprine is effective and well tolerated, the drug should not be stopped. This recommendation concerns the use of azathioprine as a single maintenance drug, and is not necessarily applicable to patients receiving concomitant biological therapy.
