Retrospective Study of 23 Cases of Psoriasis Association with HIV Infection Observed in the department of dermatology-STD in the University Hospital of Donka Conakry Guinea

Introduction :The psoriasis is one of the inflammatory dermatoses with unknown etiology, with chronic evolution having episodic appearance and disappearance. Its prevalence in HIV patients varied from 2% to 5% in most of the times. Objective of this study was to describe the demographic, clinical and therapeutic characteristics of psoriasis patients infected with HIV. Methods: It is a descriptive retrospective study done from January 2003 to December 2013 based on the information from the hospital card of hospitalized patients and outpatients taken care in the department of dermatology-STD for psoriasis at the University hospital center in Donka Conakry. We included all the cases of psoriasis associated with HIV infection diagnosed from clinical and paraclinical elements. Results: We recorded 23 (24.73%) cases of psoriasis associated with HIV infection among 93 patients observed for psoriasis in which there are 4 cases of psoriasis vulgaris, 10 cases of erythrodermic psoriasis and 9 cases of arthropathic form among these numbers. We had 7 women and 16 men. The medium age of our patients was 44.5 ± 12 years [27 - 62 years]. The delayed duration time of consultation varied from 30 to more than 180 days. The psoriasis was the circumstance of the discovery of the HIV infection among 55% of cases. The pruritus was the functional sign which is the most frequent in 20 cases among 23 cases and 71.4% of cases were accompanied with pain. Family history was found in 7.10% of cases. Anxiety was the dominant factor cause in 42% and the infection 38%. The cutaneous alteration was noted in all patients;92.9% of patients had nails alteration and intertrigineous association in 78.6% of cases. The clinical forms found were psoriasis vulgaris 4/23 cases, arthropathic psoriasis 9/23 cases, erythrodermic psoriasis 10/23. More than half (13/23) cases of our patients were diagnosed stage III of the classification of WHO. The complicated forms like erythrodermic and arthropathic psoriasis

Psoriasis is a chronic,recurrent,inflammatory systemic disease that is not only characterized by skin manifestations but may also be accompanied by various comorbidities,imposing a heavy burden on patients’physical and mental health and affecting their quality of *** the continuous approval of various biological agents for clinical use has provided more effective treatment options to patients with psoriasis,no consensus on the evaluation of comprehensive treatment goals has yet been *** treat-to-target(T2T)strategy requires consideration of multiple dimensions of treatment outcomes,the development of long-term management goals,and regular assessments of treatment conditions,which are often used in the management of chronic ***,based on the latest consensuses and guidelines,research data,and clinical experience as well as the combination of survey results and expert group discussions,the present consensus focuses on 4 dimensions of short-and long-term integrated management goals for biological agents:alleviating skin lesions,improving quality of life,screening and managing psoriasis comorbidities,and ensuring drug *** implementation methods,evaluation time,treatment monitoring,and program adjustments are also herein described to achieve comprehensive management of psoriasis to the maximum *** consensus provides a reference for clinical practice.

Evaluation of Serum Soluble CD27 and CXCL10 Levels in Patients With Vitiligo:A Cross-Sectional Study

Objective:Vitiligo is a relatively common skin disfiguring disorder that exhibits a fluctuating course between activity and stability,making monitoring and management *** plays a crucial role in the pathogenesis of *** autoimmune disorders have been associated with both CD27 and chemokine(C-X-C motif)ligand 10(CXCL10).However,trials evaluating their role in vitiligo are lacking in the Egyptian *** evaluated the circulating levels of these 2 biomarkers in patients with vitiligo and the possible correlation between their levels and disease ***:This cross-sectional study included 70 patients with vitiligo and 20 healthy *** patients were clinically assessed and then divided into active and stable groups according to clinical signs of activity and Vitiligo Disease Activity *** levels of CD27 and CXCL10 in the serum were assessed using an enzyme-linked immunosorbent assay in both the patients and the controls,then the Mann-Whitney U and Kruskal-Wallis tests were used to analyze the difference between the ***:Active and stable vitiligo patients have significantly higher median serum CXCL10(385.9 and 245.2 pg/mL)and CD27(61.6 and 66.5 ng/mL)levels compared to the controls(193 pg/mL and 52.5 ng/mL,respectively,all P<0.05).In vitiligo cases,although CXCL10 levels significantly increased with disease activity(P<0.001),CD27 levels were comparable between the 2 subgroups(P=0.953).CXCL10 positively correlated with disease activity(r=0.887,P<0.001).CXCL10 had a higher sensitivity and a lower specificity(95.7%and 60.0%,respectively)compared to CD27(71.4%and 75%,respectively)for differentiating cases from ***:There is a possibility that CXCL10 and CD27 are involved in the development and course of vitiligo.

Targeted protein degradation(TPD)represented by proteolysis targeting chimeras(PROTACs)marks a significant stride in drug discovery.A plethora of innovative technologies inspired by PROTAC have not only revolutionized the landscape of TPD but have the potential to unlock functionalities beyond ***-small-molecule-based approaches play an irreplaceable role in this field.A wide variety of agents spanning a broad chemical spectrum,including peptides,nucleic acids,antibodies,and even vaccines,which not only prove instrumental in overcoming the constraints of conventional small molecule entities but also provided rapidly renewing *** we summarize the burgeoning non-small molecule technological platforms inspired by PROTACs,including three major trajectories,to provide insights for the design strategies based on novel paradigms.

To the Editor:Actinic keratosis(AK)is a precancerous skin condition caused by chronic exposure to ultraviolet *** mainly occurs on sun-exposed areas like the head and face,leading to cosmetic concerns and an increased risk of progressing to primary squamous cell carcinoma(with a 0.60%risk at 1 year and 2.57%risk at 4 years).[1]Photodynamic therapy(PDT)is a targeted,non-invasive therapy that has been approved in Europe for the treatment of AK,squamous cell carcinoma in situ,and basal cell carcinomas.5-Aminolaevulinic acid(ALA)-based PDT,which utilizes the photosensitizer ALA,has shown high cure rates,low recurrence rates,and favorable cosmetic outcomes as arst-line treatment for AK.[1]However,due to variations in skin color,ethnicity,and lifestyle habits,the incidence of AK is lower among Asians compared to Caucasians.[1]There are limited clinical studies on PDT for AK in Asia—particularly lacking large-scale multicenter *** knowledge gap hinders the establishment of treatment standards specically tailored for Asian patients with AK undergoing ***,as the aging population increases in Asia,the incidence of AK demand for medical intervention among Asians is also rising *** conducted this multicenter study aiming to comprehensively investigate the efcacy and safety of ALA-PDT in Chinese patients with AK.
