Synchronous coexistence of liver metastases from cecal leiomyosarcoma and rectal adenocarcinoma: A case report

Multiple liver tumors represent a challenging condition for abdominal surgeons both in the selection of technique and the rarity of diagnosis. There are no case reports on co-existence of liver metastases from both intestinal leiomyosarcoma and adenocarcinoma. The patient described in this report successfully underwent resection of both primary lesions and liver metastases in combination with chemotherapy. As for the leiomyosarcoma, the primary cecal lesion was revealed more than three years after the patient's first visit. Peritoneal, lymph-node, and lung recurrences were observed afterward, and thus surgeries on those regions were performed. Pathologically, the peritoneal and lung recurrences comprised leiomyosarcoma and the lymphnode recurrence was diagnosed as adenocarcinoma. Despite newly discovered multiple lung recurrences and regional lymph-node metastases, the patient lived a normal life for 73 mo after the initial operation based on multidisciplinary therapy. He ultimately died of liver failure due to invasive lymph-node recurrence from the rectal adenocarcinoma, in addition to multiple lung recurrences from the leiomyosarcoma. Hepatic recurrence did not occur in this patient's case, which appears to be one reason for his long-term survival.

AIM To evaluate risk factors for primary sclerosing cholangitis(PSC) recurrence(rPSC) after orthotopic liver transplantation(OLT) in patients with well-preserved colons. METHODS We retrospectively evaluated the medical records of all patients transplanted for PSC in our center between July 1994 and May 2015 and selected 47 with followup of at least 60 mo for further analysis based on strict inclusion and exclusion criteria. rPSC was confirmed by magnetic resonance or endoscopic retrograde cholangiopancreatography and liver biopsy. All patients were evaluated by protocolary pre-OLT colonoscopy with randomized mucosal biopsies. Colonoscopy was repeated annually after OLT. Both organ donors and recipients were human leukocyte antigen(HLA) typed by serological and/or DNA methods. All input data were thoroughly analyzed employing relevant statistical methods.RESULTS Altogether, 31 men and 16 women with a median(range) age of 36(15-68) years at the time of OLT and a median follow-up of 122(60-249) mo were included. rPSC was confirmed in 21/47(44.7%) of patients, a median 63(12-180) mo after transplantation. De novo colitis [rPSC in 11/12, P ≤ 0.05, hazard ratio(HR): 4.02, 95% confidence interval(CI): 1.58-10.98] and history of acute cellular rejection(rPSC in 14/25, P ≤ 0.05; HR: 2.66, 95%CI: 1.03-7.86) showed strong positive associations with rPSC. According to the univariate analysis, overlapping features of autoimmune hepatitis(r PSC in 5/5, P ≤ 0.05) and HLA-DRB1*07 in the donor(r PSC in 10/15, P ≤ 0.05) represent other potential risk factors for rPSC, while the HLA-DRB1*04(rPSC in 0/6, P ≤ 0.05), HLA-DQB1*03(rPSC in 1/11, P ≤ 0.05), and HLA-DQB1*07(rPSC in 0/7, P ≤ 0.05) recipient alleles may have protective roles.CONCLUSION De novo colitis and acute cellular rejection are clinical conditions significantly predisposed towards recurrence of PSC after liver transplantation.

COVID-19 has spread surprisingly fast worldwide, and new variants continue to emerge. Recently, the World Health Organization acknowledged a new mutant strain “Omicron”, with children were accounting for a growing share of COVID-19 cases compared with other mutant strains. However, the clinical and immunological characteristics of convalescent pediatric patients after Omicron infection were lacking. In this study, we comparatively analyzed the clinical data from pediatric patients with adult patients or healthy children and the effects of SARSCoV-2 vaccine on the clinical and immune characteristics in convalescent pediatric patients. Our results indicated that convalescent pediatric patients had unique clinical and immune characteristics different from those of adult patients or healthy children, and SARS-CoV-2 vaccination significantly affected on the clinical and immune characteristics and the prevention of nucleic acid re-detectable positive(RP) in convalescent patients. Our study further deepens the understanding of the impact of Omicron on the long-term health of pediatric patients and provides a valuable reference for the prevention and treatment of children infected with Omicron.

The embolization of gastric varices is an established technique for acute bleeding in patients with portal hypertension. Here, we report an attempt to embolize a gastrorenal shunt to facilitate esophagectomy in a patient with an esophageal malignancy. To our knowledge, this is the first case in the literature to highlight the role of interventional medicine in the treatment of patients with esophageal malignancy.

Objective The individual cascades of the insulin-like growth factor-1(IGF-1)signaling pathway and the molecular mechanism of aging have not been fully clarified.In the current study,we explored the effect of DNA polymerase delta 1(POLD1)on the IGF-1 signaling pathway in cell aging.Methods First,we analyzed the relationship between IGF-1 and POLD1 expression in aging.To investigate the effect of IGF-1 on POLD1 expression and aging,the 2BS cells were incubated with youngage or old-age human serum,IGF-1 protein,or linsitinib.Next,the effect of IGF-1 on aging was examined in the 2BS cells with increased or decreased POLD1 expression to clarify the molecular mechanism.Results In this study,we found that IGF-1 expression increased and POLD1 expression decreased with aging in human serum and hippocampal tissues of SAMP8 mice,and a negative relationship between IGF-1 and POLD1 expression was observed.Furthermore,the cells cultured with old-age human serum or IGF-1 showed lower POLD1 expression and more pronounced senescence characteristics,and the effect could be reversed by treatment with linsitinib or overexpression of POLD1,while the effect of linsitinib on cell aging could be reversed with the knockdown of POLD1.Conclusion Taken collectively,our findings demonstrate that IGF-1 promotes aging by binding to IGF-1R and inhibiting the expression of POLD1.These findings offer a new target for anti-aging strategies.

目的确定急诊科(ED)无外伤病史病人的头CT影像中重要异常发现的临床预测因子。材料与方法已获得研究机构伦理委员会批准,免除病人知情同意书。本研究为回顾性研究,研究对象为2004年1月—2006年6月在急诊科接受平扫头颅CT检查的连续病人,要求年龄≥18岁,且没有创伤或已知的颅内病变。推导组(2004年1月1日—2005年8月15日间行CT检查者)采用多变量logistic回归分析来确定临床重要异常CT发现的预测因子,在验证组(所有随后的至2006年6月30日行CT检查者)对得到的结果进行验证。每个变量与临床上重要的的异常CT发现的关联强度用优势比(OR)和95%CI表示。结果在2004年1月1日—2006年6月30日间由一个单一的机构29469例连续进行的头CT影像中,3967例符合研究的条件。在这些病人的CT影像中,有548例(13.8%)显示临床上重要的异常。6个独立的头CT重要异常发现的临床预测因子被确认为年龄(每10年增加的校正OR:1.17;95%CI:1.08～1.28),局灶性神经功能缺损(校正OR:5.39;95%CI:3.90～7.47),意识状态改变(校正OR:2.32;95%CI:1.66～3.25),恶性肿瘤病史(校正OR:4.11;95%CI:2.28～7.42),恶心和(或)呕吐(校正OR:2.22;95%CI:1.14～4.33),以及凝血系统紊乱(校正OR:1.91;95%CI:1.07～3.41)。结论本研究确定了几个潜在的无外伤ED病人头颅CT异常发现的临床预测因子。在这个人群中的临床预测规则的前瞻性验证是必要的。

Simultaneous occurrence of autoimmune pancreatitis and pancreatic cancer in patients resected for focal pancreatic mass

AIM To assess the occurrence of autoimmune pancreatitis(AIP) in pancreatic resections performed for focal pancreatic enlargement.METHODS We performed a retrospective analysis of medical records of all patients who underwent pancreatic resection for a focal pancreatic enlargement at our tertiary center from January 2000 to July 2013. The indication for surgery was suspicion of a tumor based on clinical presentation, imaging findings and laboratory evaluations. The diagnosis of AIP was based on histology findings. An experienced pathologist specialized in pancreatic disease reviewed all the cases and confirmed the diagnosis in pancreatic resection specimens suggestive of AIP. The histological diagnosis of AIP was set according to the international consensus diagnostic criteria.RESULTS Two hundred ninety-five pancreatic resections were performed in 201 men and 94 women. AIP was diagnosed in 15 patients(5.1%, 12 men and 3 women) based on histology of the resected specimen. Six of them had AIP type 1, nine were diagnosed with AIP type 2. Pancreatic adenocarcinoma(PC) was also present in six patients with AIP(40%), all six were men. Patients with AIP + PC were significantly older(60.5 vs 49 years of age, P = 0.045), more likely to have been recently diagnosed with diabetes(67% vs 11%, P = 0.09), and had experienced greater weight loss(15.5 kg vs 8.5 kg, P = 0.03) than AIP patients without PC. AIP was not diagnosed in any patients prior to surgery; however, the diagnostic algorithm was not fully completed in every case. CONCLUSION The possible co-occurrence of PC and AIP suggests that preoperative diagnosis of AIP does not rule out simultaneous presence of PC.

Development and validation of an individualized nomogram for early prediction of the duration of SARS-CoV-2 shedding in COVID-19 patients with non-severe disease

With the number of cases of coronavirus disease-2019(COVID-19)increasing rapidly,the World Health Organization(WHO)has recommended that patients with mild or moderate symptoms could be released from quarantine without nucleic acid retesting,and self-isolate in the community.This may pose a potential virus transmission risk.We aimed to develop a nomogram to predict the duration of viral shedding for individual COVID-19 patients.This retrospective multicentric study enrolled 135 patients as a training cohort and 102 patients as a validation cohort.Significant factors associated with the duration of viral shedding were identified by multivariate Cox modeling in the training cohort and combined to develop a nomogram to predict the probability of viral shedding at 9,13,17,and 21 d after admission.The nomogram was validated in the validation cohort and evaluated by concordance index(C-index),area under the curve(AUC),and calibration curve.A higher absolute lymphocyte count(P=0.001)and lymphocyte-to-monocyte ratio(P=0.013)were correlated with a shorter duration of viral shedding,while a longer activated partial thromboplastin time(P=0.007)prolonged the viral shedding duration.The C-indices of the nomogram were 0.732(95%confidence interval(CI):0.685-0.777)in the training cohort and 0.703(95%CI:0.642-0.764)in the validation cohort.The AUC showed a good discriminative ability(training cohort:0.879,0.762,0.738,and 0.715 for 9,13,17,and 21 d;validation cohort:0.855,0.758,0.728,and 0.706 for 9,13,17,and 21 d),and calibration curves were consistent between outcomes and predictions in both cohorts.A predictive nomogram for viral shedding duration based on three easily accessible factors was developed to help estimate appropriate self-isolation time for patients with mild or moderate symptoms,and to control virus transmission.
