Efficacy and safety of triple therapy containing berberine,amoxicillin,and vonoprazan for Helicobacter pylori initial treatment:A randomized controlled trial

Background:With the development of traditional Chinese medicine research,berberine has shown good efficacy and safety in the eradication of Helicobacter pylori(***).The present study aimed to evaluate the efficacy and safety of triple therapy containing berberine,amoxicillin,and vonoprazan for the initial treatment of ***:This study was a single-center,open-label,parallel,randomized controlled clinical *** with *** infection were randomly(1:1:1)assigned to receive berberine triple therapy(berberine 500 mg,amoxicillin 1000 mg,vonoprazan 20 mg,A group),vonoprazan quadruple therapy(vonoprazan 20 mg,amoxicillin 1000 mg,clarithromycin 500 mg,colloidal bismuth tartrate 220 mg,B group),or rabeprazole quadruple therapy(rabeprazole 10 mg,amoxicillin 1000 mg,clarithromycin 500 mg,colloidal bismuth tartrate 220 mg,C group).The drugs were taken twice daily for 14 *** main outcome was the *** eradication *** secondary outcomes were symptom improvement rate,patient compliance,and incidence of adverse ***,factors affecting the eradication rate of *** were further ***:A total of 300 ***-infected patients were included in this study,and 263 patients completed the *** intention-to-treat(ITT)analysis showed that the eradication rates of *** in berberine triple therapy,vonoprazan quadruple therapy,and rabeprazole quadruple therapy were 70.0%(70/100),77.0%(77/100),and 69.0%(69/100),*** per-protocol(PP)analysis showed that the eradication rates of *** in these three groups were 81.4%(70/86),86.5%(77/89),and 78.4%(69/88),*** ITT analysis and PP analysis showed that the *** eradication rate did not significantly differ among the three groups(P>0.05).In addition,the symptom improvement rate,overall adverse reaction rate,and patient compliance were similar among the three groups(P>0.05).Conclusions:The efficacy of berberine triple therapy for *** initial

AIM:To conduct a meta-analysis to determine the safety and efficacy of laparoscopic liver resection(LLR) and open liver resection(OLR) for hepatocellular carcinoma(HCC).METHODS:PubMed(Medline),EMBASE and Science Citation Index Expanded and Cochrane Central Register of Controlled Trials in the Cochrane Library were searched systematically to identify relevant comparative studies reporting outcomes for both LLR and OLR for HCC between January 1992 and February *** authors independently assessed the trials for inclusion and extracted the ***-analysis was performed using Review Manager Version 5.0 software(The Cochrane Collaboration,Oxford,United Kingdom).Pooled odds ratios(OR) or weighted mean differences(WMD) with 95%CI were calculated using either fixed effects(Mantel-Haenszel method) or random effects models(DerSimonian and Laird method).Evaluated endpoints were operative outcomes(operation time,intraoperative blood loss,blood transfusion requirement),postoperative outcomes(liver failure,cirrhotic decompensation/ascites,bile leakage,postoperative bleeding,pulmonary complications,intraabdominal abscess,mortality,hospital stay and oncologic outcomes(positive resection margins and tumor recurrence).RESULTS:Fifteen eligible non-randomized studies were identified,out of which,9 high-quality studies involving 550 patients were included,with 234 patients in the LLR group and 316 patients in the OLR *** was associated with significantly lower intraoperative blood loss,based on six studies with 333 patients [WMD:-129.48 mL;95%CI:-224.76-(-34.21) mL;P = 0.008].Seven studies involving 416 patients were included to assess blood transfusion requirement between the two *** LLR group had lower blood transfusion requirement(OR:0.49;95%CI:0.26-0.91;P = 0.02).While analyzing hospital stay,six studies with 333 patients were *** in the LLR group were found to have shorter hospital stay [WMD:-3.19 d;95%CI:-4.09-(-2.28) d;P < 0.00001] than their OLR

The traditionally used oriental herbal medicine Moutan Cortex Radicis [MCR; Paeonia Suffruticosa Andrews(Paeoniaceae)] exerts anti-inflammatory, anti-spasmodic, and analgesic effects. In the present study, we investigated the therapeutic effects of differently fractioned MCR extracts in a 6-hydroxydopamine(OHDA)-induced Parkinson's disease model and neuro-blastoma B65 cells. Ethanol-extracted MCR was fractionated by n-hexane, butanol, and distilled water. Adult Sprague-Dawley rats were treated first with 20 μg of 6-OHDA, followed by three MCR extract fractions(100 or 200 mg·kg^(-1)) for 14 consecutive days. In the behavioral rotation experiment, the MCR extract-treated groups showed significantly decreased number of net turns compared with the 6-OHDA control group. The three fractions also significantly inhibited the reduction in tyrosine hydroxylase-positive cells in the substantia nigra pars compacta following 6-OHDA neurotoxicity. Western blotting analysis revealed significantly reduced tyrosine hydroxylase expression in the substantia nigra pars compacta in the 6-OHDA-treated group, which was significantly inhibited by the n-hexane or distilled water fractions of MCR. B65 cells were exposed to the extract fractions for 24 h prior to addition of 6-OHDA for 30 min; treatment with n-hexane or distilled water fractions of MCR reduced apoptotic cell death induced by 6-OHDA neurotoxicity and inhibited nitric oxide production and neuronal nitric oxide synthase expression. These results showed that n-hexane-and distilled water-fractioned MCR extracts inhibited 6-OHDA-induced neurotoxicity by suppressing nitric oxide production and neuronal nitric oxide synthase activity, suggesting that MCR extracts could serve as a novel candidate treatment for the patients with Parkinson's disease.

Background Erythropoietin （EPO） functions as a tissue-protective cytokine in addition to its crucial hormonal role in red cell production and neuron protection. This study aimed to determine the neuron protective effect of erythropoietin on experimental rats enduring spinal cord injury （SCI） by assessing thrombospondin-1 （TSP-1） level and transforming growth factor-β （TGF-β） in the development of a rat model of SCI. Methods Sixty Sprague-Dawley rats were randomly assigned to three groups： sham operation control group, SCI group and EPO treatment group. By using a weight-drop contusion SCI model, the rats in the SCI group and EPO treatment group were sacrificed at 24 hours and 7 days subsequently. The Basso, Beattie, and Bresnahan （BBB） scores were examined for locomotor function. Pathological changes were observed after HE staining. The expressions of thrombospondin-2 （TSP-1） and TGF-β were determined by immunohistochemical staining and Western blotting. Results Slighter locomotor dysfunction was discovered and it was recovered abruptly as higher BBB scores were found in the EPO treatment group than in the SCI group （P 〈0.01）. Pathologically, progressive disruption of the dorsal white matter and regeneration of a few neurons were also observed in SCI rats. TSP-1 and TGF-β expression increased at 24 hours and 7 days after SCI in the injured segment, and it was higher in the SCI group than in the EPO treatment group. Spinal cord samples from the animals demonstrated a TSP-1 optical density of 112.2±6.8 and TSP-1 positive cells of 5.7±1.3 respectively. After injury, the TSP-1 optical density and cell number increased to 287.2±14.3/mm^2 and 23.2±2.6/mm^2 at 24 hours and to 232.1±13.2/mm^2 and 15.2±2.3/mm^2 at 7 days respectively. When EPO treated rats compared with the SCI rats, the TSP-1 optical density and cell number decreased to 213.1 ±11.6/mm^2 and 11.9±1.6/mm^2 at 24 hours and to 189.9±10.5/mm^2 and 9.3±1.5/mm^2 at 7 days, respectively （P 〈0.01）. Double-composite rectangular truss bridge and its joint analysis

This paper describes a novel composite tubular truss bridge with concrete slab and concrete-filled rectangular chords. With concrete slab plus truss system and joints rein- forced with concrete and Perfobond Leiste rib, double composite truss bridge proved to be a fairly suitable solution in negative moment area. Perfobond Leiste shear connector （PBL） is widely implemented in the composite structure for its outstanding fatigue resistance. In this pilot bridge, Perfobond Leister ribs （PBR） were installed in the truss girder＇s joints, which played double roles as shear connector and stiffener. An erection method and overall bridge structural analysis were then presented. Typical joints in the pilot bridge were selected to analyze the effect of PBR. Investigation of the effect of PBR in concrete- filled tubular joints was elaborated. Comparison has revealed that concrete-filled tubular joints with PBR have much higher constraint capability than joints without PBR. For rect- angular tubular truss, the punching shear force of the concrete filled joint with PBR is approximately 43% larger than that of the joint without PBR. Fatigue performance of the joint installed with PBR was improved, which was found through analysis of the stress concentration factor of joint. The PBR installed in the joints mitigated the stress concen- tration factor in the chord face. Therefore, the advantages of this new type of bridge are demonstrated, including the convenience of construction using rectangular truss, inno- vative concept of structural design and better global and local performances.

Sensors play an important role in shaping and monitoring human *** of methods to use Fiber Bragg Grating(FBG)with enhanced sensitivity has attracted great interest in the field of medical *** this paper,a novel apodization function is proposed and performance evaluation and optimization of the same have been made.A comparison was conducted between various existing apodization functions and the proposed one based on optical characteristics and sensor *** results evince the implementation of the proposed apodization function for vital sign *** optical characteristics considered for evaluation are Peak Resonance Reflectivity level,Side Lobes Reflectivity level and FullWidth HalfMaximum(FWHM).The proposed novel apodization novel function has better FWHM,which is narrower than the FWHM of uniform *** characteristics like a quality parameter,detection accuracy and sensitivity also show *** proposed novel apodization function is demonstrated to have a better shift in wavelength in terms of temperature and pulse measurement than the existing *** sensitivity of the proposed apodized function is enhanced with a Poly-dimethylsiloxane coating of varying thickness,which is 6 times and 5.14 times greater than uniform Fiber Bragg grating and FBG with the proposed novel apodization function,respectively,enhancing its utilization in the field of medicine.

As the dominant macrofauna in many soils,earthworms play a significant role in mineral weathering and nutrient release from mineral structures into available forms for root *** is a trioctahedral micaceous mineral that can be weathered to other minerals through release of its interlayer potassium(K).In this study,we investigated the effect of earthworm activities and associated microbial communities on phlogopite weathering during the production of phlogopite-enriched vermicompost with different quantities of phlogopite *** experiment was run under laboratory conditions using three levels of phlogopite(0,20%and 40%by weight)in *** Petri dish contained 20 g of a mixture of cow manure and phlogopite,and vermicomposting process was performed by inoculation of eight Eisenia foetida *** in clay mineralogy and C/N ratio in each dish were assessed at days 0,45,90,135,and 180 during *** results indicated that phlogopite was partially weathered to vermiculite and interstratified mica-vermiculite during the vermicomposting *** rate of weathering was higher during the early stages of vermicomposting(the first 90 d).The weathering of phlogopite was influenced considerably by the activity of earthworms and associated microbial communities during the organic matter transformation *** the early stages of the process,the weathering rate for different levels of phlogopite was not significantly different,but over time it accelerated steadily,and the difference increased.
