AIM: To determine the general and transplant-specific risk factors for fractures in kidney transplant ***: We conducted a cohort study of all adults who received a kidney-only transplant(n = 2723) in Ontario, Canada between 2002 and 2009. We used multivariable Cox proportional hazards regression to determine general and transplant-specific risk factors for major fractures(proximal humerus, forearm, hip, and clinical vertebral). The final model was established using the backward elimination strategy, selecting risk factors with a P-value ≤ 0.2 and forcing recipient age and sex into the model. We also assessed risk factors for other fracture locations(excluding major fractures, and fractures involving the skull, hands or feet). RESULTS: There were 132 major fractures in the follow-up(8.1 fractures per 1000 person-years). General risk factors associated with a greater risk of major fracture were older recipient age [adjusted hazard ratio(a HR) per 5-year increase 1.11, 95%CI: 1.03-1.19] and female sex(a HR = 1.81, 95%CI: 1.28-2.57). Transplant-specific risk factors associated with a greater risk of fracture included older donor age(5-year increase)(a HR = 1.09, 95%CI: 1.02-1.17) and end-stage renal disease(ESRD) caused by diabetes(a HR = 1.72, 95%CI: 1.09-2.72) or cystic kidney disease(a HR = 1.73, 95%CI: 1.08-2.78)(compared to glomerulonephritis as the reference cause). Risk factors across the two fracture locations were not consistent(major fracture locations vs other). Background: Vitamin D deficiency has been reported as a possible risk factor for Diabetic Peripheral Neuropathy(DPN). However, the results are not consistent in epidemiologic studies. In this study, we aimed to investigate the incidence of Vitamin D deficiency and the relationship between them in different age groups. Method: In this retrospective study, the 1461 patients admitted to the department of endocrinology in Xin Hua Hospital from June, 2016 to September, 2017 were divided into three age groups, including Youth group(24-44 y,n=127),Middle-aged group(45-64 y,n=779) andElderly group(≥65 y,n=555). Basic information and laboratory results were collected from the medical records. Results: The total incidence ofDiabetic Peripheral Neuropathy was 32.7%(478/1461), the three groups are 12.6%(16/127), 33.6%(262/779) and 36.0%(200/555) respectively. The 25-OH Vitamin D concentration in Middle-aged group was higher than Youth and Elderly groups(P0.05).Receiver operating characteristic analysis showed that serum 25(OH) concentration<36.30 nmol/L might predict the occurrence of DPN(P< 0.01). Conclusion: Vitamin D deficiency was common in diabetic patients especially in the elderly. High 25(OH) D concentration was firstly found to be a protective factor for the elderlybut not youth and middle-aged DPN patients, but more researches are needed for the underlying mechanisms.

目的:探讨丹蛭降糖胶囊对Goto-Kakizaki(GK)大鼠糖尿病大血管病变治疗作用机制。方法:采用高脂高糖联合内皮型一氧化氮合酶(NOS)抑制剂N-硝基-L-精氨酸甲酯(L-NAME)(0.1 g·L-1·d-1)喂养复制糖尿病大血管病变大鼠模型。按随机排列表将模型大鼠随机分为模型组、丹蛭降糖胶囊组(1 260,630,320 mg·kg-1)、阿托伐他汀组(105 mg·kg-1)、二甲双胍组(10 mg·kg-1),每组12只,连续灌胃给药8周。取Wistar大鼠12只作为正常对照组。观测大鼠体质量,饮水量,血糖及血浆TC,TG,LDL-C,HDL-C,IL-1β,IL-6,TNF-α,NO,ET-1变化;取主动脉组织,HE染色法观察病理改变;RT-PCR法检测大鼠主动脉IL-1β,IL-6,TNF-αmRNA水平,茎环RT-PCR法检测大鼠血浆与主动脉miRNA-126,miRNA-155,miRNA-146a,miRNA-21水平,对miRNAs与炎症因子之间进行典型相关性分析。结果:丹蛭降糖胶囊能增加大鼠体质量,降低饮水量,降低随机血糖,逆转大鼠主动脉组织损伤,降低血浆TC,TG,LDL-C,LDL-C,ET-1,IL-1β,IL-6,TNF-α,miRNA-155,miRNA-146a,miRNA-21水平,升高血浆HDL-C,NO含量,降低主动脉IL-1β,IL-6,TNF-αmRNA和miRNA-155,miRNA-146a,miRNA-21表达水平,升高主动脉miRNA-126表达;主动脉miRNA-126,miRNA-155,miRNA-146a,miRNA-21与炎症因子表达具有典型相关性,其中miRNA-126为负相关,miRNA-155,miRNA-146a,miRNA-21为正相关。结论:丹蛭降糖胶囊对糖尿病大血管病变具有治疗效果,作用机制可能与调节miRNA-126,miRNA-155,miRNA-146a,miRNA-21水平,减少炎症因子、减轻血管炎症反应有关。

Objective: To characterize the phenotype,modes of inheritance, karyotype, and molecular basis of patients with idiopathic hypogonadotropic hypogonadism(IHH).Design: Review of medical records, karyotyping, and collation of gene mutation analysis. Setting: University molecular reproductive endocrinology laboratory. Patient(s): Patients with IHH. Intervention(s): Review of medical records, laboratory studies, and molecular studies. Main Outcome Measure(s): Sense of smell, severity of IHH (complete vs. incomplete), associated anomalies, karyotype, mutation analysis, and genotype/phenotype correlations were studied. Result(s): Of 315 patients with IHH, 6.3% had one or more affected relatives. Autosomal recessive inheritance was likely in most of these familial cases, but autosomal- dominant and X- linked recessive inheritance patterns were likely in some families. Complete IHH was more commonly found in males (62% ), whereas incomplete IHH was more commonly observed in females (54.3% ). Anosmia was present in 31.3% of males and 27.9% of females. The karyotype was normal in all 19 females tested, but was abnormal in 3 of 57 (5.3% ) of males tested. Although cryptorchidism did not differ among those who were anosmic vs. normosmic, it was approximately four times more common in patients with complete IHH than incomplete IHH (15.3% vs. 3.9% ). Approximately 10% of the IHH patients tested had mutations in either the GNRHR or KAL1 gene. Conclusion(s): Idiopathic hypogonadotropic hypogonadismis a heterogeneous disorder affecting fertility, in which the number of familial cases is probably underestimated. Further study of genes that regulate hypothalamic- pituitary development and function will likely reveal important information regarding the development of normal puberty in humans.

Objective:To study the effect of Xueshuantong combined with antioxidant drugs on nerve conduction function and oxidative stress in patients with diabetic peripheral neuropathy. Methods: 138 cases of patients with diabetic peripheral neuropathy who were treated in endocrinology department of our hospital between June 2014 and October 2016 were enrolled and randomly divided into two groups. The combination group received Xueshuantong combined with antioxidant drug therapy, and the control group received antioxidant drug therapy. Before and after treatment, the nerve conduction velocity as well as serum content of oxidative stress indexes and nerve cytokines was ***: 4 weeks and 8 weeks after treatment, common peroneal nerve and median nerve MNCV and SNCV as well as serum SOD, GSH-Px, HO-1, CAT, CNTF, BDNF and SDF-1 levels of both groups were significantly higher than those before treatment while serum MDA, AOPP and 8-OHdG levels were significantly lower than those before treatment, and common peroneal nerve and median nerve MNCV and SNCV as well as serum SOD, GSH-Px, HO-1, CAT, CNTF, BDNF and SDF-1 levels of combination group were significantly higher than those of control group while serum MDA, AOPP and 8-OHdG levels were significantly lower than those of control ***:Xueshuantong combined with antioxidant drugs can improve the nerve conduction function, inhibit oxidative stress response and improve neurotrophy status in patients with diabetic peripheral neuropathy.
