objective： To estimate the impact of parametrial infiltration and lymph node metastasis on clinical outcome in women with early-stage cervical cancer following radical hysterectomy and pelvic lymphadenectomy. Methods： Clinical records and pathologic slides of 532 patients with early-stage cervical cancer （330 Ib and 202 Ila） treated with radical hysterectomy and pelvic lymphadenectomy were reviewed. The study group comprised 520 patients with squamous call carcinoma and 12 patients with adenocarcinoma of the cervix. Median follow-up time was 67 months. The association among the various histopathologic predictors of outcome was determined with analysis. The influence of the predictors on outcome was examined with log rank survival methods and the Cox regression model. Results： FIGO stage, histologic type, tumor size, depth of invasion, parametdal infiltration, lymph node metastasis, and remote metastasis were identified as significantly biologically relevant and therefore were included as candidate predictors in multivariate analysis. In particular, parametrial infiltration and lymph node metastasis were found to be simultaneous predictors of death on multivariate analysis （P 〈 0.05）. After controlling for these two factors, the other variables considered were not statistically significant up to a two-way interaction. Conclusion： Presence of parametdal infiltration and/or lymph node metastasis in women with early-stage cervical cancer is an independent poor prognostic factor. In addition, the relatively poor survival of women with more than one lymph nodes identified with cancer cells.

Conventional methods to treat urinary stress incontinence, including the Sling, Burch, and Pereyra modification methods, are limited by several shortcomings due to disrupted nerve and vaginal wall integrity. The nerve-sparing Burch-Sling method represents a surgical advancement through the use of a nerve-sparing sling to treat genuine stress urinary incontinence. The procedure involves retropubic urethropexy using the FDA-approved Burch-Sling device. In this technique, the vagina is elevated bilaterally at the urethrovesical junction to the mid-urethra toward Cooper’s ligament above the base of the bladder. Then, the anterior vaginal wall and fascia are used as an endogenous suburethral sling without dissection. Two hundred twenty cases were included in this study;two hundred patients underwent the outpatient nerve-sparing sling method, and the other twenty underwent the novel abdominal Burch method. There were no major complications. The follow-up duration ranged from 6 months to eight years. All procedures were performed at the U.S. Women’s Institute at a 400-bed hospital in Fountain Valley, CA.

Cryopreservation is currently the only effective tool for long-term storage of semen in most species. However, it is well-recognized that, even in species that freeze well, some individuals resist cryopreservation. Work from this laboratory has demonstrated a relationship between maternal lipid content and the chemical constitution of the embryos they produce. The objective of the present study was to determine if a similar relationship might exist in paternal body chemistry and the animal’s semen sample and if such a difference could be determined with a simple weight test. Semen samples were obtained from cattle with known differences in body composition. The samples first underwent semen analysis and were then prepared as either cell-free (CF) or neat specimens (NS). Known volumes of each sample were weighed, and the remainder of the samples was analyzed for lipids, total proteins, and total carbohydrates using a series of spectrophotometric assays and blood chemistry techniques. As expected, weight differences were seen in the CF vs NS preparations of individual semen samples (p observed weights and total protein (p < 0.047) as well as triglyceride levels (p < 0.003). Together, these data suggest it might be possible to develop an algorithm to allow adjustment in cryoprotectants based on a simple weight procedure, allowing modification of cryoprotectants on an individual basis and potentially improving outcomes for valuable animals currently classified as “poor freezers”.

A c. 300-year oxygen and carbon isotope record derived from fine-grained and ostracod carbonate from Qinghai Lake testifies to dramatic interannual tointerdecadal limnological change. Fine-grained carbonates, which are mainly authigenic, are likely to have formed in the epilimnion of the lake and their isotopic composition reflects the summer temperature and, more importantly, the isotopic composition of the near-surface waters, which is mainly a function of evaporative concentration. Ostracod shells are secreted in the benthos of the lake, and their isotopic composition reflects summer bottom-water conditions, together with fractionation effects, which may differ between species. Differences betweencontemporaneous values from authigenic carbonates and ostracod shells may provide an indication of stratification within the lake and variations in effective precipitation over the northeast part of the Tibetan Plateau over the past 300 years. A period of moderate evaporative concentration, from about 300 to 100 yr BP, was interrupted by a marked wet phase from ～100 to 40 yr BP, which was in turnfollowed by a return to drier conditions in the most recent part of the *** increase in ? 18O values in the latter part of the record accords well withinstrumental records of lake-level lowering and salinity increase since about 1955 AD.

Preeclampsia complicates 3%-5% of pregnancies and is one of the major causes of maternal morbidity and mortality. The pathologic mechanisms are well described but despite decades of research, the exact etiology of preeclampsia remains poorly understood. For years it was believed that the etiology of preeclampsia was the result of maternal factors, but recent evidence suggests that preeclampsia may be a couple specific disease where the interplay between both female and male factors plays an important role. Recent studies have suggested a complex etiologic mechanism that includes genetic imprinting, immune maladaptation, placental ischemia and generalized endothelial dysfunction. The immunological hypothesis suggests exaggerated maternal response against fetal antigens. While the role of maternal exposure to new paternal antigens in the development of preeclampsia was the initial focus of research in this area, studies examining pregnancy outcomes in pregnancies from donor oocytes provide intriguingly similar findings. The pregnancies that resulted from male or female donor gametes or donor embryos bring new insight into the role of immune response to new antigens in pathogenesis ofpreeclampsia. The primary goal of the current review is the role of exposure to new gametes on the development of preeclampsia. The objective was therefore to provide a review of current literature on the role of cohabitation length, semen exposure and gamete source in development of preeclampsia.

In searching of differentially expressed genes in human uterine leiomyomas, differential display was used with twelve pairs of primers to compare human uterine leiomyomas with matched myometrium. False positives were eliminated by reverse Northern analysis. Positives were confirmed by Northern blot analysis. RESULTS: [1] Four of 69 cDNA fragments (3 up-regulated named L1, L2 and L3 and 1 down-regulated named Mi in leiomyoma) were confirmed by Northern analysis. [2] Sequence comparison and Northern analysis proved that Li is exactly the human ribosomal protein Si9. [3] It was present ubiquitously in i3 tissues tested but in various levels and even in different size. [4] Li was highly expressed in parotidean cystadenocarcinoma, pancreatic cancer and breast cancer examined. [5] No mutations have been found in human uterine leiomyomas (n=6). CONCLUSIONS: hRPSi9 overexpression might be a universal signal in rapid cell growth tissues.
