Objective:In this study,we screened for therapeutic targets of the Xian-Lian-Jie-Du decoction(XLJDD)for colorectal cancer(CRC)and explored the underlying mechanisms using network pharmacology techniques.Methods:Genes associated with CRC were collected from the Gene Cards database.The names of the active compounds of XLJDD were used as keywords in the“chemical name”in the Traditional Chinese Medicine Systems Pharmacology(TCMSP)database to search the targets.The protein-protein interaction(PPI)network was constructed using Cytoscape 3.8.1.Gene Ontology functional enrichment and Kyoto Encyclopedia of Genes and Genomes(KEGG)pathway enrichment analyses were performed to identify key target proteins.Results:A total of 234 XLJDD-related targets and 250 cross-targets between XLJDD and CRC were collected based on the TCMSP and HIT 2.0 databases.A PPI network constructed based on the STRING database revealed interactions for all 250 proteins.The network results revealed TP53,MYC,CCND1,AKT1,CASP3,and STAT3 as core potential targets.KEGG pathway analysis of the 250 potential XLJDD targets for CRC in the Metascape database was performed using RStudio software.The top 12 gene ratio aggregated analysis results were visualized in bubble charts.The interleukin(IL)-17 signaling pathway had the highest correlation with the tumor necrosis factor(TNF)signaling pathway.Conclusions:XLJDD may be effective in ameliorating CRC by controlling inflammatory factors related to the IL-17 and TNF pathways and targeting proto-oncogenes and tumor suppressor genes,including MYC,CCND1,CTNNB1,and TP53.

Pregnancy outcomes in papillary thyroid cancer survivors:A retrospective cohort study

Objective:The purpose of this study was to evaluate the pregnancy outcomes among papillary thyroid cancer survivors.Methods:65 cases singleton gravidas of papillary thyroid cancer survivors treated in the Department of Obstetrics,Peking University People's Hospital between January 2011 and January 2020 were selected retrospectively as the study group,and 130 cases of singleton pregnancy during the same time were randomly selected as the control group.The differences in general conditions,maternal and fetal complications between the two groups were analyzed.Risk factors of maternal complications and fetal malformation in patients with papillary thyroid cancer survivors were analyzed.Data was analyzed with t-test,χ^(2)test and logistic regression for statistical differences.Results:Totally 65 cases of papillary thyroid cancer with 70 pregnancies were analyzed,which including 65 cases achieved live birth,5 cases underwent induced labor because of fetal malformation.The study group showed higher incidence in gestational diabetes(20%vs 10%),cesarean section(39.1%vs 24.6%),postpartum hemorrhage(350193 ml vs 290181 ml)and fetal malformation(5.7%vs 1.5%),and lower live birth rate(91.4%vs 99.2%)(P<0.05).Multivariate analysis by logistic regression found that the patient's age,thyroid function,operation mode and postoperative I-131 treatment were not the risk factors which may affect the pregnancy outcomes or fetal malformation.Conclusion:Elevated maternal and fetal complications were observed among pregnancies in papillary thyroid cancer survivors.Management of these survivors may require a careful gestation monitoring,and further investigation is needed.

Diagnostic Value of Circulating PIGF in Combination with Flt-1 in Early Cervical Cancer

The utility of placental growth factor(PIGF)and its receptor VEGFR-1(Flt-1)as biomarkers for cervical cancer has not been clarified yet.To address this issue,we investigated the levels of soluble PIGF(sPIGF)and soluble Flt-1(sFlt-1)in the serum from patients with early cervical cancer,cervical intraepithelial neoplasia(CIN)and controls in this study.sPIGF and sFlt-1 were detected in 44 preoperative patients with cervical cancer,18 cases with CIN,and 20 controls by ELISA.It was found that both sPIGF and sFlt-1 were significantly increased in the cervical cancer group as compared with those in CIN and control groups.sPIGF presented a high diagnostic ability of cervical cancer,with a sensitivity of 61.36%and a specifcity of 89.47%;and sFlt-1 with a sensitivity of 50.00%and a specifcity of 92.I 1%。Importantly,the combined use of sPIGF and sFlt-1 could increase the diagnostic rate of cervical cancer,with a sensitivity of 70.45%and a specificity of 92.11%.These results indicated that both sPIGF and sFlt-1 in circulation can serve as possible valuable diagnostic biomarkers for cervical cancer,and the combined use of them can be more valuable to diagnose the patients with early cervical cancer.

Clinicopathologic Characteristics and Survival of Patients with Rare Malignant Ovarian Yolk Sac Tumors:A Population-based Analysis

Yolk sac tumors (YSTs) are rare malignant germ cell tumors that usually affect youngfemales. To date, there have been few studies on YSTs. We evaluated the relationship betweenclinicopathologic characteristics of patients with ovarian YSTs and disease outcome based onSurveillance, Epidemiology, and End Results data. The K aplan-Meier method and log-rank testwere used to evaluate differences in survival rates. Data for 269 patients were analyzed. Theincidence of YSTs among ovarian germ cell tumors (OGCTs) cases was 0.4%;median patient agewas 22.0 years, and most tumors were unilateral. Patients presented with distant metastasis (37.5%),localized disease (49. 1%), and regional spread (8.9%). American Joint Committee on Cancer stagewas available for 13 patients (stage IA, n=2;stage IC, n=1;stage IA, n=1;stage IB, n=3;stageIlC, n=2;and stage IV, n=4). Survival rates at 1, 3, and 5 years were 91.0%, 84.0%, and 83.2%,respectively, for overall survival (OS) and 92.0%, 85.4%, and 84.5%, respectively, for disease-specific survival (DSS). The 5-year OS and DSS of patients with ovary tumors were 91.5% and92.9%, respectively, compared to 74.8% and 77.2%, respectively, for those with extra-ovarianspread (P50 years was associated with shorter OS and DSS(both P<0.001), whereas no associatios of OS and DSS were observed with pathologic grade (P=0.49for OS and 0.52 for DSS). In summary, YSTs are typically unilateral, of a high grade, and localizedto the ovary;extra-ovarian spread has a poor outcome, and postmenopausal women have worseprognosis than premenopausal women.

Intraoperative consultation includes gross evaluation and frozen section,whitch is an integral component of surgical oncology for many reasons.First,it allows appropriate surgical procedures to be undertaken.Second,it can potentially protect the patient from a second procedure.Thitd,it helps towards counselling of paients and their family members and lasty,it faclilitates early management planning.Udlizaion of frozen section is justified by the reported sensi dvity,specificity,positive and negativwe predictive values all of which are as high as 90-100%in some studies.It is very impor tant for gynecologists to have good communication with pathologi sts,especially for cass with an indeterminate,frozen result.Discussion with the pathologist can help the gynecologist understand the diagnostic dilemma and the differential diagnosis,which can help them make best surgical decisions.

IL-1β up-regulates expression of IL-8 in endometrial stromal cells in vitro

Objective:To investigate the effects of interleukin-1beta(IL-1β) on expression of IL-8 in endometrial stromal cells(ESC) and evaluate the relationship between IL-1β and IL-8,and the significance of IL-1β in the development of endometriosis.Methods:The endometrial stromal cells obtained from patient with and without endometriosis cultured within 3～5 passage were exposed to various concentrations of IL-1β.The amount of IL-8 protein was assessed by ELISA.The expression of IL-8 mRNA was determined by RT-PCR.Results:1.IL-8 protein was detected in culture supernatant of which the cells were not treated with IL-1β.The amount of IL-8 protein secretion increased obviously after stimulation with IL-1β at 1.0ng/ml for 4h and the peak of secretion was at 12h.2.Expression of IL-8 mRNA was positive in unstimulated endometrial stromal cells.However,after stromal cells were incubated with IL-1β,the intensity of expression of IL-8 mRNA was obviously increased and demonstrated a dose-and time-dependent manner.Increase of IL-8 mRNA was observed following stimulation with IL-1β for 4h,and the peak at 12h.Conclusions:IL-1β induces endometrial stromal cell of endometriosis to express IL-8 not only at transcription level but also at post-transcription level.This up-regulation is dose-and time-dependent.IL-1β may play an important role in the onset of endometriosis.【

Background： Preterm premature rapture of membrane （PPROM） can lead to serious consequences such as intrauterine infection, prolapse of the umbilical cord, and neonatal respiratory distress syndrome. Genital infection is a very important risk which closely related with PPROM. The preliminary study only made qualitative research on genital infection, but there was no deep and clear judgment about the effects of pathogenic bacteria. This study was to analyze the association of in fections with PPROM in pregnant women in Shaanxi, China, and to establish Bayesian stepwise discriminant analysis to predict the incidence of PPROM. Methods： In training group, the 112 pregnant women with PPROM were enrolled in the case subgroup, and 108 normal pregnant women in the control subgroup using an unmatched case-control method. The sociodemographic characteristics of these participants were collected by face-to-face interviews. Vaginal excretions fiom each participant were sampled at 28 36-6 weeks of pregnancy using a sterile swab. DNA corresponding to Chlamrdia trachomalix （CT）, Ureaplasma urealyticwn （UU）, Candida albicans, group B streptococci （GBS）, herpes simplex virus- 1 （HSV-1）, and HSV-2 were detected in each participant by real-time polymerase chain reaction. A model of Bayesian discriminant analysis was established and then verified by a mull）center validation group that included 500 participants in the case subgroup and 5（10 participants in the control subgroup from five different hospitals in the Shaanxi province, respectively. Results： The sociological characteristics were not significantly different between the case and control subgroups in both training and validation groups （all P 〉 0.05）. In training group, the infection rates of UU （11.6% vs. 3.7%）, CT （17.0% vs. 5.6%）, and GBS （22.3% vs. 6.5%） showed statistically different between the case and control subgroups （all P 〈 0.05）, Iog-transfomacd quantification of UU, CE GBS, and HSV-2 showed statistically different between the case and control subgroups （P 〈 0.05

Based on the New Diagnosis and Treatment Scheme for Novel Coronavirus Infected Pneumonia(Trial Edition 5),combined with our current clinical treatment experience,we recently proposed a revision of the first edition of“Guidance for maternal and fctal management during pneumonia epidemics of novel coronavirus infection in the Wuhan Tongji Hospital".This article focused on the issues of greatest concern of pregnant women including severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)infection diagnostic criteria,inspection precautions,drug treatment options,indications and methods of termination of pregnancy,postpartum fever,breastfeeding considerations,mode of mother-to-child transmission,neonatal isolation and advice on nconatal nursing,to provide valuable experience for better management of SARS-CoV-2 infection in pregnant women and newborns.

Expression of epidermal growth factor and its receptor in gestational trophoblastic diseases

Objective: To determine whether epidermal growth factor (EGF) and its receptor have any possible correlation with etiology of gestational trophoblastic diseases. Methods: Avidin-biotin immunoperoxidase techniques with polyclonal antibodies against EGF, EGFR were used to examine 53 cases of GTD, including complete hydatidiform mole(16),invasive mole(20),gestational choriocarcinoma(17).Results:EGF was mainly localized on syncytiotrophoblasts (ST), and was found less on cytotrophoblasts. Cytologic localization of EGFR showed the similar results. The positive rate of EGF and EGFR were 0.625, 0.813 in hydatidiform mole, 0.405, 0.450 in invasive mole and 0.118, 0.235 in gestational choriocarcinoma. There was significant difference of EGF or EGFR among hydatidiform mole group and other groups, respectively (P<0.05). Conclusion:The cellular levels of EGF and EGFR decreased gradually in the development of GTD. It implied that the autocrine and paracrine mechanism may play an important role on the proliferation and differentiation of trophoblast cells and the disorder of the system may lead to GTD malignant transformation.
