Introduction Patients with stroke-like symptoms may be underutilising emergency medical services and avoiding hospitalisation during the COVID-19 *** investigated a decline in admissions for stroke and transient ischaemic attack(TIA)and emergency department(ED)stroke alert *** We retrospectively compiled total weekly hospital admissions for stroke and TIA between 31 December 2018 and 21 April 2019 versus 30 December 2019 and 19 April 2020 at five US tertiary academic comprehensive stroke centres in cities with early COVID-19 outbreaks in Boston,New York City,Providence and *** collected available data on ED stroke alerts,stroke severity using the National Institutes of Health Stroke Scale(NIHSS)and time from symptom onset to hospital *** Compared with 31 December 2018 to 21 April 2019,a decline in stroke/TIA admissions and ED stroke alerts occurred during 30 December 2019 to 19 April 2020(p trend<0.001 for each).The declines coincided with state stay-at home recommendations in late *** greatest decline in hospital admissions was observed between 23 March and 19 April 2020,with a 31%decline compared with the corresponding weeks in *** of the five centres with 2019 and 2020 stroke alert data had a 46%decline in ED stroke alerts in late March and April 2020,compared with *** baseline NIHSS during these 4 weeks was 10 in 2020 and 7 in *** was no difference in time from symptom onset to hospital *** At these five large academic US hospitals,admissions for stroke and TIA declined during the COVID-19 *** was a trend for fewer ED stroke alerts at three of the five centres with available 2019 and 2020 *** stroke therapies are time-sensitive,so decreased healthcare access or utilisation may lead to more disabling or fatal strokes,or more severe non-neurological complications related to *** findings underscore the indirect effects of this *** health offici

Summary:Dexmedetomidine(DEX),a potent and highly selective agonist for a2-adrenergic receptors(a2AR),exerts neuroprotective effects by reducing apoptosis through decreased neuronal Ca^2+***,the exact action mechanism of DEX and its effects on oxygen-glucose deprivation-reoxygenation(OGD/R)injury in vitro are *** demonstrate that DEX pretreatment reduced OGD/R injury in PC12 cells,as evidenced by decreased oxidative stress,autophagy,and neuronal ***,DEX pretreatment decreased the expression levels of stromal interaction molecule 1(STIM1)and calcium release-activated calcium channel protein 1(Orail),and reduced the concentration of intracellular calcium *** addition,variations in cytosolic calcium concentration altered apoptosis rate of PC12 cells after exposure to hypoxic conditions,which were modulated through STIM 1/Orail ***,DEX pretreatment decreased the expression levels of Beclin-1 and microtubule-associated protein 1A/1B-light chain 3(LC3),hallmark markers of autophagy,and the formation of *** conclusion,these results suggested that DEX exerts neuroprotective effects against oxidative stress,autophagy,and neuronal apoptosis afier OGD/R injury via modulation of Caf-STIM1/Orai1 *** results offer insights into the molecular mechanisms of DEX in protecting against neuronal ischemia-reperfusion injury.

Background The burden of non-communicable diseases(NCDs)is high in ***,resources and training for NCD care remain scarce,especially in rural *** care for NCDs in the developing world focuses on the WHO’s traditional 4×4 ***,we do not know the full burden of NCDs outside of that scope,like neurological disease,psychiatric illness,sickle cell disease,and *** goal of this study was to understand the burden of NCDs among inpatients in a rural district hospital in *** broadened our definition of NCDs beyond the traditional 4×4 set of NCDs,and included neurological disease,psychiatric illness,sickle cell disease,and *** We conducted a retrospective chart review of all inpatients who were admitted to the Neno District Hospital between January 2017 and October *** broke patients down by age,date of admission,type,and number of NCD diagnoses,and HIV status,and constructed multivariate regression models for length of stay and in-hospital *** Of 2239 total visits,27.5%were patients with *** with NCDs were older(37.6 vs 19.7 years,p<0.001)and made up 40.2%of total hospital *** also found two distinct populations of NCD *** first were patients 40 years and older with primary diagnoses of hypertension,heart failure,cancer,and *** second were patients under 40 years old with primary diagnoses of mental health conditions,burns,epilepsy,and *** also found significant trauma burden,accounting for 40%of all NCD *** multivariate analysis,carrying a medical NCD diagnosis was associated with longer length of stay(coefficient 5.2,p<0.001)and a higher risk of in-hospital mortality(OR 1.9,p=0.03).Burn patients also had significantly longer length of stay(coefficient 11.6,p<0.001).Conclusions There is a significant burden of NCDs in a rural hospital in Malawi,including those outside of the traditional 4×4 *** also found high rates of NCDs in the younger population(under 40

Coastal cities represented by Ningbo are directly or indirectly affected by typhoons each year. By analyzing the past three typical typhoons landing in Ningbo from 2013 to 2016, it was found that before and after the typhoon transit, reservoir and water treatment plant would be made by the destructive impact, including the increasing water level, water volume in a short time, and the deteriorating water quality. Among those, the water quality caused by typhoons increased the water treatment process load, the amount of water purification agents increased significantly,and the emergency response put a great pressure on the inventory of water plants. Based on the statistics and analysis of the basic parameters of the reservoir and water treatment plant during the typhoon season, the emergency management of the typhoon was divided into three situations, namely, pre-typhoon, typhoon period and post-typhoon. Thus, it is effective for the relevant practitioners of the reservoir and water plant to ensure the safe water supply during typhoon season.

In June 2023,the 2022 global mpox(monkeypox)outbreak began to affect Guangdong Province,one of the first regions in China's Mainland to report mpox *** July 10,2023,93 mpox cases had been reported in Guangdong *** study describes the epidemiological characteristics of these patients by collecting and analyzing data on demographics,sexual behavior,medical history,travel history,clinical symptoms,and diagnostic pathways and *** 93 mpox cases were all men aged 20-48 years,95.70%were men who have sex with men(MSM),and 48.39%were HIV-positive.A few cases were married(to women)or living with *** rash was present in almost all cases(98.91%),with the most common sites being the genital and perianal areas(70.33%).Intimate sexual contact was suspected to be the main route of infection,with a median incubation period of 8.5 days(IQR 5.0–11.8).The number of cases increased rapidly,and most patients had no history of international travel,suggesting sustained community transmission within the MSM population in Guangdong *** addition,93.55%of cases were detected by medical institutions,and more than half of these patients had two or more hospital visits before being diagnosed as an mpox case,indicating that clinicians need further training to increase their sensitivity to *** interventions should prioritize MSM while remaining vigilant for transmission to other populations,such as women and children.
