Background. The clinical history of a given pigmented lesion could in fluence the therapeutic decision. Teledermatology and automated image analysis also hold great potential for revolutionizing dermatology services. Aim. The aim of this retrospective study was to evaluate the diagnostic accuracy of users with differ ent experiences in dermoscopy with and without information about patients and th eir history compared with classification by an automated analysing system. Setti ng. One hundred and fifty-seven dermoscopic images of pigmented lesions, taken and proved by histopathology at the Pigmented Lesions Clinic of thedepartment o f dermatology of the University Tuebingen,Germany, were included. Methods. All i mages were viewed by three investigators with different experience: excellent (A ), average (B) and beginner (C). In the first dermoscopic classification, no inf ormation was available. After 3 months the same images were once more classified by the three investigators, now with the information about the patients and the ir history. The melanocytic lesions were tested by the Tuebinger Mole Analyser. Results. For user A the sensitivity, specificity and diagnostic accuracy reveale d no improvement on including the history (81.3% to 84.4% , 94.6% to 92.3% and 92.0% to 90.7% ), whereas user B clearly improved his results (75.0% to 87.5% , 76.9% to 88.5% and 76.5% to 88.3% ). No change in the sensiti vity was seen by user C (84.4% ), but there was a clear improvement in the spec ificity (69.2% to 87.7% )-and diagnostic accuracy (72.2% to 87.0% ). Usi ng the computer algorithm, a sensitivity of 100% , a specificity of 76.9% and a diagnostic accuracy of 81.9% were achieved. Conclusions. The study revealed results relevant to the use of dermoscopy: (1) continuing dermoscopic education influences the diagnostic accuracy; (2) the history is helpful for averaged use rs and beginners in dermoscopy; (3) digital image analysis has the highest sensi tivity, but a lower spec

PURPOSE: To describe the clinical characteristics and course of optic disk edema (OdE) associated with isolated sudden-onset anterior uveitis. dESIGN: Interventional, noncomparative retrospective case series. METHOdS: Review of patients with sudden-onset anterior uveitis and concomitant OdE seen in an academic uveitis referral center or a general ophthalmology clinic between August 2001 and November 2002. RESULTS: A total of eleven eyes of seven patients were included. Optic nerve function was intact in all. Cranial magnetic resonance imaging was normal in four of five and revealed an abnormality not felt to be responsible for the OdE in one. Resolution of anterior uveitis occurred over a mean of 36 days while OdE resolution lagged by a mean of 20 days. CONCLUSION: Resolution of OdE trailed that of the anterior uveitis by up to 6 weeks. No abnormalities responsible for OdE were found on cranial imaging.

AIM:To evaluate the diagnosis value of MRI for neurovascular compression(NVC)in trigeminal neuralgia(TN)and facial spasm(FS).METHOdS:15patients with TN and 2patient with FS underwent SE T1WI and T2WI first,then 3d-TOF-SPGR ***:NVC was found in all the pa-tients,MRI findings were consisten t with the surgical operative findin gs ***:It is considered to be of important to use MRI to find out the causes of TN and FS,esp ecially to utilize the 3d-TOF-SPGR sequences,which may possess high se nsitivity in determining NVC.

To the Editor:The coronavirus disease2019(COVId-19)outbreak developedinto a *** hemodialysis(Hd)are more susceptible to COVId-19,and isolating patients undergoing Hd after exposure are extremely challenging since they frequently travel between their homes and dialysis centers,during which they share a hall for dialysis sessions that may last for *** successfully quarantined one infected Hd patient and other potentially exposed *** experience might serve as an important reference for dialysis centers or for people in similar situations across the country to reduce the spread of COVId-19.

Purpose To report a rare case of lymphocutaneous Nocardia brasiliensis origina ting in the eyelid. design Observational case report. Methods The clinical prese ntation, workup, and treatment of a case of lymphocutaneous Nocardia brasiliensi s originating in the eyelid are presented. Results The patient presentedwith a p reseptal cellulitis froman abrasion of the eyelid that progressed to submandibul ar lymph node suppuration. Culture was performed, and a diagnosis of lymphocutan eous Nocardia brasiliensis was made. Conclusion Nocardia brasiliensis may cause a lymphocutaneous infection of the face and must be considered in the differenti al diagnosis of preseptal cellulitis.
