Four-year dynamic observation and study on standardized management of elderly patients with type 2 diabetes in Beijing Yongding Road community

Purpose:This study aimed to understand the joint standard of blood glucose,blood li-pids,and blood pressure in elderly patients>60 years of age with type 2 diabetes in the Beijing Yongding Road ***:One hundred ninety-two elderly patients>60 years of age who were diagnosed with type 2 diabetes in Yongding Road community served as the investigation subjects,under-went standard management,and the dynamic changes in blood glucose,blood lipids,and blood pressure were monitored for 48 ***:At the end of the observation period,the standard rates of fasting blood glucose and postprandial blood glucose were 72.55%and 80.00%,respectively,which were increased compared with 55.73%and 56.08%at baseline(P0.05).The standard rates of TG and LDL-C were 76.71%and 60.38%,respectively,which were increased compared with 54.69%and 34.74%at base-line,and the standard rate of HDL-C was 13.64%,which was decreased compared with 40.10%at baseline(P<0.01).The standard rate of BP was 58.33%,which was increased compared with 38.54%at baseline(P<0.01).The optimal control rate of blood glucose for 48 months(the standard times of the total measurement times in 48 months≥75%)was higher;the fasting blood glucose was 52.17%,the 2-h postprandial blood glucose was 60.22%,and the HbA1c was 46.45%.The optimal control rate of blood lipids was lower;LDL-C was 17.49%and HDL-C was 13.59%.The optimal control rate of BP was 9.13%.At the end of the observation period,the levels of fasting blood glucose and postprandial blood glucose were decreased by 0.7 mmol/L and 1.48 mmol/L,respectively,compared with the base-line(P<0.01).The level of HbA1c was decreased by 0.18%compared with the baseline(P<0.05).The levels of LDL-C and HDL-C were decreased by 0.4 mmol/L and 0.23 mmol/L,respectively,compared with the baseline(P<0.01).The levels of SBP and DBP were decreased by 4 mmHg compared with the bas

Impact of community public health care on treatment effect,health cognition,and self-management in patients with type 2 diabetes

BACKGROUND At present,China has become the country with the largest number of individuals with diabetes mellitus(DM)in the world,with a total of approximately 140 million patients,the majority of whom have type 2 DM(T2DM).Based on conven-tional nursing methods,community home care has important clinical significance in controlling blood sugar and disease *** To explore the impact of community public health nursing on treatment effect,health cognition,and self-management in patients with *** One hundred patients with T2DM were selected as the research *** patients were divided into either a conventional nursing group or community nursing(CN)group using the random number table *** conventional nursing group(50 cases)received routine care,while the CN group(50 cases)received community public health care in addition to routine care as that for the conventional nursing *** rate of excellent and good blood glucose control,fasting blood glucose before and after care,2-h postprandial blood glucose,health cognition,and self-management ability,and patient satisfaction were compared between the two *** The CN group had a higher rate of excellent blood sugar control than the conventional nursing group(88%vs 70%,P0.05).After nursing,fasting blood glucose and 2-h postprandial blood glucose were reduced to varying degrees in both groups,and both blood glucose levels in the CN group were lower than those of the conventional nursing group(P<0.05).Compared with the scores before care,the cognitive level score for diabetes and self-management ability score improved after care in both *** cognitive level and self-management ability of patients in the CN group were higher than those of the conventional nursing group(P<0.05).The overall satisfaction of the CN group was better than

Therapeutic Efficacy Observation on Combining Foot Bath and Massage with Auricular Point Sticking for community Senile Insomnia

Objective: To observe the therapeutic efficacy of combining foot bath and massage with auricular point sticking for community senile insomnia. Methods: A total of 210 cases treated in our community health center were randomized into an observation group and a control group by their visit sequence numbers, 105 cases in each group. Cases in the observation group were treated with foot bath and massage plus auricular point sticking, whereas cases in the control group were treated with auricular point sticking alone. After four courses of treatment, the therapeutic efficacies were evaluated. In addition, the sleep quality was also assessed using the Pittsburgh sleep quality index(PSQI). Results: The total effective rate in the observation group was 95.2%, versus 68.6% in the control group, showing a statistical significance(Pcommunity senile insomnia than auricular point sticking alone.

Contraceptive behaviors and awareness in an urban slum community of south India

Qualitative study to identify the perception and challenges faced by the faculty of community medicine in the implementation of competency-based medical education for postgraduate students

Objectives To identify perception about the key aspects of competency-based medical education(CBME)among community medicine faculty members and to ascertain the various challenges faced by them during its *** A descriptive qualitative study of 2 months’duration was conducted among the faculty members of the community medicine ***-probability purposive sampling was employed in the *** listing was done initially to elicit the views of faculty members to meet the intended *** Anthropac software was used to identify the salient variables using Smith’s Salience Score,and then pile sorting was done to identify the association between the salient *** Three faculty members participated in the free listing and pile sorting.A total of 20 responses were obtained pertaining to the key aspects of CBME,of which 12 were identified as the salient variables depending on the cut-off value of 0.125(Smith’s Salience Score)and subjected to pile ***,eight challenges were identified in the implementation of the programme during the free listing,and all were included in the second stage of pile *** maps were drawn to understand the relationship between the key aspects of CBME and involved challenges *** On employing the free listing and pile sorting methods,formulation of entrustable professional activities and their assessment using appropriate tools were the identified crucial areas in CBME,while the lack of sensitisation of stakeholders and inadequate planning were identified as the predominant challenges in the implementation of CBME.

Integration of traditional Chinese medicine and Western medicine in a Chinese community health center

Objective:Although the literature is abundant on traditional Chinese medicine(TCM)in the West,there is limited information on how TCM is integrated with Western medicine(WM).We describe how one Chinese community Health Service(CHS)system located in Beijing integrates WM and ***:Our information is based on the authors’observations,interviews with center TCM practitioners,and discussions with center ***:We summarize our observations according to the following themes:selection of type of practitioner;frequent diagnoses of patients seen by TCM clinicians;types of TCM ser-vices provided;economic factors;challenges;and future *** age,nature of the problem,and cost may determine whether or not Chinese patients initially consult TCM or WM *** of referral pathways between the WM and TCM practitioners,up to one-third of the patients receive integrated *** physicians see more patients per day than do their WM counterparts;TCM physicians also earn higher *** there are clearly close collaborative relationships between the TCM and WM practitioners,a few TCM providers report that lack of respect between the two fields may be a barrier towards further ***:Given governmental policies and the cost differentials between WM and TCM,the future for the integration of the two medical traditions within the CHS system appears to be favorable;however,issues of mutual respect and workforce issues may challenge success-ful *** impressions are limited by the fact that we observed practices in only one community in one district of Beijing.
