Background Brucellosis is a severe zoonotic disease that is often overlooked,particularly in impoverished countries.Timely identification of focal complications in brucellosis is crucial for improving treatment outcomes.However,there is currently a lack of established indicators or biomarkers for diagnosing these complications.Therefore,this study aimed to investigate potential warning signs of focal complications in human brucellosis,with the goal of providing practical parameters for clinicians to aid in the diagnosis and management of patients.Methods A multi-center cross-sectional study was conducted in china from December 2019 to August 2021.The study aimed to investigate the clinical characteristics and complications of patients with brucellosis using a ques-tionnaire survey and medical record system.The presence of warning signs for complications was assessed using univariate and multivariate logistic regression models.Receiver operating characteristic(ROC)curves and the area under the curve(AUC)were used for variable screening and model evaluation.Results A total of 880 participants diagnosed with human brucellosis were enrolled.The median age of the patients Was 50 years[interquartile range(IQR):41.5-58.0],and 54.8%had complications.The most common organ system affected by complications was the osteoarticular system(43.1%),with peripheral arthritis(30.0%),spondylitis(16.6%),paravertebral abscess(5.0%),and sacroilitis(2.7%)being the most prevalent.Complications in other organ systems included the genitourinary system(4.7%),respiratory system(4.7%),and hematologic system(4.6%).Several fac-tors were found to be associated with focal brucellosis.These factors included a long delay in diagnosis odds ratio(OR)=3.963,95%confidence interval(CI):1.906-8.238 for>90 daysl,the presence of underlying disease(OR=1.675,95%CI:1.176-2.384),arthralgia(OR=3.197,95%CI:1.986-5.148),eye bulging pain(OR=3.482,95%CI:1.349-8.988),C-reactive protein(CRP)>10 mg/L(OR=1.910,95%CI:1.310-2.784)and erythrocyte sedimentation rate(ESR)el

Spotlight on eltrombopag concentration in pediatric immune thrombocytopenia: A single-center observational study in china

Importance: Eltrombopag has been recommended for pediatric immune thrombocytopenia (ITP). Response and adverse drug reactions (ADRs) varied widely between individuals, even at the same dose of eltrombopag. The appropriate eltrombopag concentration in ITP has not been reported.Objective: This study aims to explore the appropriate eltrombopag concentration in pediatric ITP.Methods: This was a single-center, prospective cohort study. Children diagnosed with refractory persistent/chronic ITP and platelet count < 30×109/L were treated with eltrombopag and followed up for at least 2 months. Concentration was detected by high-performance liquid chromatography-mass spectrometry at least 2 weeks after eltrombopag. The clinical characteristics-concentration, concentration-response, and concentration-ADRs were analyzed.Results: A total of 30 patients were enrolled, comprising 13 males and 17 females, with a median age of 72 (45-94) months. The median dose and concentration were 1.39 (1.09-1.56) mg/kg and 2.70 (2.25-4.13) mg/L, respectively. Of the enrolled patients, 14 responded to treatment, whereas 16 did not. Additionally, five experienced adverse drug reactions. No linear correlation was observed between eltrombopag concentration and clinical characteristics. The concentration was lower in the response group than in the nonresponse group, but there was no significant difference (t = 0.755,P = 0.457). Patients who experienced ADRs had a higher concentration than those without ADRs (t = 2.538,P = 0.017). The area under the receiver operating characteristic curve of ADRs was 0.78 (95% confidence interval: 0.56-1.00). Youden’s index identified the cutoff point as 4.33 mg/L, with a sensitivity of 88% and a specificity of 60%. Logistic regression analysis demonstrated that a higher platelet count before eltrombopag predicted a favorable response.Interpretation: Eltrombopag proves efficacious and well-tolerated for treating pediatric ITP. However, prolonged and high-dose administration may increase the likelihood of ADRs. Thus

Background The thirteen-valent pneumococcal conjugate vaccine(PCV13)is not included in the national immu-nization program and is administered voluntarily with informed consent in china.In preparation for assessing the impact of pilot introduction in Hainan Province,we conducted a carriage study among children under 5 years of age from four locations in Hainan Province,china.Methods From March to June 2022,nasopharyngeal(NP)swabs,collected from healthy children aged younger than 59 months who lived in the 4 different locations(Haikou,Wanning,Baisha and Qiongzhong)in Hainan Province,were tested for pneumococcus using conventional culture.Pneumococcal isolates were serotyped using the Quel-lung reaction.Risk factors associated with pneumococcal colonization were assessed using univariate analysis and multivariable logistic regression adjusting for age,daycare attendance and other factors.Results Pneumococcus was isolated in 710(30.4%)of the 2333 children enrolled.Of 737 pneumococci,29 serotypes were identifed;60.9%were PCV13 serotypes;the most common vaccine serotypes were 6B(20.4%),19F(13.0%),6A(11.9%)and 23F(6.1%);and the most common nonvaccine serotypes were 23A(12.9%),34(6.1%)and nontypeable(NT)pneumococci(5.6%).Children vaccinated with PCV13 had lower carriage(17.7%vs 32.5%;P=0.0001)and fewer PCV13 serotypes(41.9%vs 62.7%;P=0.0017)compared to unimmunized children.After adjustment,NP carriage Was higher among children attending daycare(aOR=2.3,95%CI:1.7-3.2),living in rural areas(aOR=1.4,95%CI:1.1-1.8),living with siblings(aOR=1.3,95%CI:1.0-1.6)and whose mothers had completed senior high/technical sec-ondary school(aOR=1.5,95%CI:1.1-2.0).In contrast,completion of 3-4 doses of PCV13 were associated with a lower carriage rate(aOR=0.6,95%CI:0.4-0.9).Conclusions We established the baseline of pneumococcal carriage,serotype distribution and PCV13 immuni-zation rates among healthy children under 5 years of age in Hainan Province,prior to the introduction of PCV13 into the national immunization program.The high proport

Cretaceous-Neogene Exhumation of the Daqing Shan,North china Constrained by Apatite Fission Track Thermochronology

The Daqing Shan(DQS)located in the Yinshan-Yanshan Orogenic Belt plays an important role in the Mesozoic to Cenozoic evolution of the North china Craton.However,the cooling and exhumation history since the Cretaceous is still controversial.Integrating the apatite fission track(AFT)data in both this study and previous works,a three-stage exhumation history from Cretaceous to Neogene of the DQS is proposed.(1)The first stage is composed of the early exhumation during Early Cretaceous driven by the collision between the North china and Siberia cratons(ca.148-132 Ma)and the far-field effect of the subduction of the Pacific Plate(ca.132-114 Ma).(2)Due to the subsidence of the Hetao Basin and the subsequent compensation between the DQS and the Hetao Basin,the DQS experienced the second rapid exhumation from Early Eocene to Early Oligocene(ca.54-29 Ma).(3)Since the Late Miocene(ca.13.5 Ma),the third rapid cooling and exhumation of the DQS occurred due to the far-field effect of the uplift of the Tibetan Plateau and the subduction of the Pacific Plate.

Rapid report of source parameters of 2023 M6.2 Jishishan,Gansu earthquake sequence

The M6.2 earthquake in Jishishan,Gansu Province,on December 18,2023,caused extraordinary earthquake disasters.It was located in the northern part of the north−south seismic zone,which is a key area for earthquake monitoring in china.The newly built dense strong motion stations in this area provide unprecedented conditions for high-precision earthquake relocation,especially the earthquake focal depth.This paper uses the newly built strong motion and traditional broadband seismic networks to relocate the source locations of the M3.0 and above aftershocks and to invert their focal mechanisms.The horizontal error of earthquake location is estimated to be 0.5−1 km,and the vertical error is 1−2 km.The focal depth range of aftershocks is 9.6−14.6 km,distributed in a 12-km-long strip with SSE direction.Aftershocks in the south are more concentrated horizontally and vertically,while aftershocks in the north are more scattered.The focal mechanisms of the main shock and aftershocks are relatively consistent,and the P-axis orientation is consistent with the regional strain direction.There is a seismic blank area of M3.0 and above,about 3−5 km between the main shock and aftershocks.It is suggested that the energy released by the main shock rupture is concentrated in this area.Based on the earthquake location and focal mechanism of the main shock,it is inferred that the Northern Lajishan fault zone is the seismogenic structure of the main shock,and the main shock did not occur on the main fault,but on a secondary fault.The initial rupture depth and centroid depth of the main shock were 12.8 and 14.0 km,respectively.The source rupture depth may not be the main reason for the severe earthquake disaster.

Cell Consistency Evaluation Method Based on Multiple Unsupervised Learning Algorithms

Unsupervised learning algorithms can effectively solve sample imbalance.To address battery consistency anomalies in new energy vehicles,we adopt a variety of unsupervised learning algorithms to evaluate and predict the battery consistency of three vehicles using charging fragment data from actual operating conditions.We extract battery-related features,such as the mean of maximum difference,standard deviation,and entropy of batteries and then apply principal component analysis to reduce the dimensionality and record the amount of preserved information.We then build models through a collection of unsupervised learning algorithms for the anomaly detection of cell consistency faults.We also determine whether unsupervised and supervised learning algorithms can address the battery consistency problem and document the parameter tuning process.In addition,we compare the prediction effectiveness of charging and discharging features modeled individually and in combination,determine the choice of charging and discharging features to be modeled in combination,and visualize the multidimensional data for fault detection.Experimental results show that the unsupervised learning algorithm is effective in visualizing and predicting vehicle core conformance faults,and can accurately predict faults in real time.The“distance+boxplot”algorithm shows the best performance with a prediction accuracy of 80%,a recall rate of 100%,and an F1 of 0.89.The proposed approach can be applied to monitor battery consistency faults in real time and reduce the possibility of disasters arising from consistency faults.
