The prognostic value of lymphovascular invasion in radical prostatectomy, a systematic review and meta-analysis

To systematically evaluate the prognostic value of lymphovascular invasion （LVI） in radical prostatectomy （RP） by a meta-analysis based on the published literature. To identify relevant studies, PubMed, Cochrane Library, and Web of science database were searched from 1966 to May 2014. Finally, 25 studies （9503 patients） were included. LVI was found in 12.2% （1156/9503） of the RP specimens. LVI was found to be correlated with higher pathological tumor stages （greater than pT3 stage）（risk ratio [RR] 1.90, 95% confidence interval [CI] 1.73-2.08, P〈 0.00001）, higher Gleason scores （greater than Gs = 7）（RR 1.30, 95% CI 1.23-1.38, P 〈 0.00001）, positive pathological node （pN） status （RR 5.67, 95% CI 3.14-10.24, P 〈 0.00001）, extracapsular extension （RR 1.72, 95% CI 1.46-2.02, P 〈 0.00001）, and seminal vesicle involvement （RR 3.36, 95% CI 2.41-4.70, P 〈 0.00001）. The pooled hazard ratio （HR） was statistically significant for Biochemical Recurrence-Free （BCR-free） probability （HR 2.05, 95% ／ CI 1.64-2.56; Z = 6.30, P〈 0.00001）. sensitivity analysis showed that the pooled HR and 95% CI were not significantly altered by the omission of any single study. Begg＇s Funnel plots showed no significant publication bias （P = 0.112）. In conclusion, LVI exhibited a detrimental effect on the BCR-Free probability and clinicopathological features in RP specimens, and may prove to be an independent prognostic factor of BCR.

Postoperative intussusception in infants and children:a report of seven cases

Postoperative intussusception is an uncommon but serious condition in infants and children. Here, we report seven cases of postoperative intussusception in infants and children who were seen at our institution over the last 13 y. The patients showed increased nasogastfic drainage, vomiting, lack of stool, and/or growing abdominal dis- tension 2 to 9 d following abdominal surgery. Manual reduction was successful in five cases. In two cases, necro- sis was found and intestinal resection and anastomosis were carded out, No recurrence was observed at six months of follow-up. Postoperative intussusception should be suspected in pediatric surgical patients who showed signs of intestinal obstruction in the early postoperative period.

NEONATAL CONGENITAL CARDIOVAsCULAR ANOMALIEs: ANALYsIs OF 66 AUTOPsY CAsEs

1,330 autopsies performed at the Pathology Department of this hospital during 1958-1981, including 415 newborns, are studied. Among the 371 cases of congenital anomalies (110 newborns), 173 were congenital cardiovascular anomalies (66 newborns), a 13% morbidity forming 46.5% of all congenital anomalies seen. The 66 newborns were 60% of all neonatal congenital anomalies and 38.2% of all cardiovascular anomalies. Fetal stage diagnosis and early termination of pregnancy when necessary is the treatment of choice.

Background spleen in Traditional Chinese Medicine (TCM) is not actually the spleen in the anatomic sense designated in western medicine because its functions basically belong to the physiological category of digestive system in modern medicine, and it represents a macroscopic concept of digestion, absorption and nutrition metabolism spleen deficiency syndrome refers to the clinical phenomena such as hypofunction of digestion, absorption and nutrition metabolism By integrating TCM with modern medicine , this paper is intended to explore the pathological basis of classification of spleen deficiency in chronic gastritis Method By means of optical microscope, scanning electron microscope (sEM), transmission electron microscope (TEM) and histochemical staining, we conducted histopathological and subcellular ultrastructural (nuclei and mitochondrial) analysis of gastric mucosa of 188 patients of spleen deficiency, and that of 42 voluntary blood donors without clinical symptoms Results The gastric mucosa of patients with spleen Qi deficiency (sQD) and spleen yang deficiency (syangD) could either be affected by organic lesion (type G-occurring on the basis of chronic superficial gastritis (CsG), chronic atrophic gastritis (CAG)) or unaffected (type F-chiefly belonging to functional indigestion); spleen yin deficiency (syinD) and spleen deficiency with Qi stagnation (sDQs) both occurred on the basis of CsG and CAG; and the degree of mucosa inflammatory cells infiltration, the degree of decrease in glands propria, and the incidence of IMⅡ b in CsG and CAG were more serious than those of G-sQD and G-syangD, P sion spleen deficiency syndrome is likely to occur on the basis of organic lesion of gastric mucosa (disease with symptoms of both CsG or CAG and spleen deficiency symptoms), as well as on the basis of inorganic lesion of gastric mucosa (nondisease with symptoms, which is, despite spleen deficiency symptoms, there is no CsG or CAG) Bes

Previous reports have suggested that Ang-(1-7)may have a protective effect in endothelial cells against high glucose(HG)-induced cell injury thanks to a modulatory mechanism in the NF-κB signaling *** this study,we have examined whether NF-κB-IL-1βand Heme oxygenase-1(HO-1)pathways contribute to the protection of Ang-(1-7)against hyperglycemia-induced inflammation and oxidative stress in human umbilical vein endothelial cells(HUVECs).Our results indicate that time-varying exposures of HUVECs,from 1 h to 24 h,to high glucose concentrations result in an increased expression of phosphorylated(p)-p65 and HO-1 in a time-dependent *** an inhibitor of NF-κB,pyrrolidinedithiocarbamic acid(PDTC)suppressed IL-1βproduction induced by *** note,HUVECs previously treated with Ang-(1-7)(2μM)for 30 min before being exposed to HG concentrations significantly ameliorated the HG-increased in p-p65 and IL-1βexpression;whereas obviously up-regulated the level of HO-1,along with inhibition of oxidative stress,inflammation,and the HG-induced ***,when HUVECs were previously treated either with PDTC or IL-1Ra for 30 min before being exposed to HG,it significantly prevented damages caused by high glucose concentrations mentioned above,while the treatment of HO-1 inhibitor sn-protoporphyrin(snPP)before exposure to both HG and Ang-(1-7)significantly blocked the protective effect exerted by Ang-(1-7)on endothelial cells against injuries induced by HG mentioned *** conclude,the data of this study showed that activation and inhibition of the NF-κB-IL-1βpathway and HO-1 pathway may constitute an important defense mechanism against endothelial cell damage caused by HG *** additionally gave new evidence showing that exogenous Ang-(1-7)exerts a protective effect on HUVECs against the HG-induced cell injury via the inhibition and the activation of the NF-κB-IL-1βpathway and the HO-1 pathway,respectively.

Development of a novel two color tracer perfusion technique for the hydrodynamic study of aqueous outflow in bovine eyes

Background Elevation of intraocular pressure is usually associated with primary open angle glaucoma and caused by increased outflow resistance. A two-color fluorescent tracer technique was developed to investigate the hydrodynamics of aqueous humor outflow with changing intraocular pressure within the same eye, to better understand the relationship between outflow facility and effective filtration area. Methods Eighteen enucleated bovine eyes were first perfused at 30 mmHg with Dulbecco＇s phosphate-buffered saline containing 5.5 mmol/L D-glucose. After a stable baseline facility, red fluorescent microspheres （0.5 um, 0.002% v/v） were exchanged and perfused. Eyes in the one-color control group （n=6） were immediately perfused with fixative. In the experimental group （n=6）, eyes were perfused with green tracer after intraocular pressure reduced to 7 mmHg, while in the two-color control group （n=6）, eyes were perfused with green tracer with intraocular pressure remaining at 30 mmHg. All 12 eyes were then per＇fusion-fixed. Outflow facility was continuously recorded in all eyes. Confocal images were taken along the inner wall of the aqueous plexus and the percent of the effective filtration length （PEFL; length of inner wall exhibiting tracer labeling/total length of inner wall） was measured. The relationships between outflow facility and PEFL were analyzed statistically. Results No significant differences were found in baseline facilities （***-1） among the three groups （the experimental group： 0.93±0.12; the two-color control group： 0.90±0.19; the one-color control group： 0.98±0.13）. In the experimental group, the outflow facility was significantly higher at 7 mmHg （4.29±1.01） than that at 30 mmHg （1.90±0.67, P 〈0.001）, which corresponded to a significant increase in the PEFL at 7 mmHg （54.70±8.42） from that at 30 mmHg （（11.76±4.56）%, P 〈0.001）. AIM:To describe maternity and newborn charges for an economic analysis of surrogate pregnancies on the health care resource ***:A retrospective chart review of all women identified as being surrogates and the infants born from these pregnancies was performed between January 1,2012 and December 31,*** maternity diagnoses,mode of delivery,duration of hospitalization,and hospital charges were collected together with infants' birth weights,gestational age,length of hospital stay,and hospital *** associated with the in vitro fertilization cycles,artificial insemination,or embryo(s)transfer into the surrogate were not considered in the maternity charges.A ratio contrasting the maternity hospital charges for the surrogate carrier was compared as a ratio to the mean charges for 2540 infants delivered in 2013 after naturalconception and adjusted to the baseline hospital charges for both maternity and newborn ***:Analysis of sixty-nine infants delivered from both gestational and traditional surrogate women found an increased in multiple births,NICU admission,and length of stay with hospital charges several multiples beyond that of a term infant conceived naturally and provided care in our *** singletons and twins(per infant)hospital charges were increased 26 times(P s charges were increased 173 times(P sery at our ***:Maternity costs for surrogates exceed those of women who conceive naturally,and these costs are especially magnified in women with triplets and multiple births.

Objective:To observe the effectiveness and safety of one-time endodontics in the treatment of chronic apical periodontitis with sinus tract in pediatric deciduous ***:109 cases of children with chronic apical periodontitis with sinus tract in the deciduous teeth treated in our hospital from January 2022 to December 2023 were selected and grouped by the randomized numerical table method,with 54 cases in the experimental group and 55 cases in the control *** experimental group was treated with one-time endodontics and the control group was treated with conventional ***:After the treatment,the total effective rate of treatment was higher in the experimental group than in the control group(Pse events was lower in the experimental group than in the control group(Psatisfaction of the children's family members was higher in the experimental group than in the control group(Ps lower in the experimental group than in the control group(Psion:In the experimental group,children with chronic apical periodontitis with sinus tract of the deciduous teeth were given one-time endodontic treatment,and the results of its implementation were relatively good.

KCNQ2 gene encoding for voltage-gated potassium channel subunitsKv7.2 and its mutation have been shown to cause benign familial neonatal seizures(BFNs) or epileptic encephalopathy with developmental delay or intellectual disability(ID).Here We report a clinical and Chinese family in which two members were diagnosed with KCNQ2 encephalopathy presenting early onset refractory seizures in the first week of life and neurocognitive *** exome sequencing was used to detect the underlying genetic cause.A novel frame shift mutation c.2422 hetup T was identified in KCNQ2 in two affected *** patients in this family had onset of seizures but the severity of seizures was various and lead to different *** findings suggest that KCNQ2 mutation should be considered in neonates with medicine resistent seizures of unknown cause and contray to previous reports that all mutations published so far are missense mutations,frameshift mutation also can be found in KCNQ2 encephalopathy.
