Effect and Safety of Acupuncture for Type 2 Diabetes Mellitus:A Systematic Review and Meta-analysis of 21 Randomised Controlled Trials

Objective:To evaluate the effects of acupuncture on hypoglycaemic outcomes in type 2 diabetes mellitus(T2 DM).Methods:Pub Med,Embase,Cochrane Library,and Clinical *** were searched from inception up to July 2020,to identify randomised controlled trials(RCTs)that enrolled patients with T2 DM and compared acupuncture combined with antidiabetic drugs to antidiabetic drugs *** primary outcomes were haemoglobin A1 c(Hb A1 c)and fasting blood glucose(FBG).The secondary outcomes included 2-h blood glucose(2 h BG),fasting insulin(FINS),homeostatic model assessment for insulin resistance(HOMA-IR),and acupuncture-related adverse *** difference(MD)and 95%confidence interval(CI)were used as the effect measure in the *** quality of evidence was assessed using the Grading of Recommendations Assessment,Development and Evaluation ***:Twenty-one RCTs(n=1,188)were *** meta-analytic results showed that the acupuncture group had greater reductions in FBG(MD-6.46 mg/d L,95%CI-11.95 to-0.98;moderate-quality evidence)and HOMA-IR(MD-1.23,95%CI-2.16 to-0.31;low-quality evidence),but comparable changes in Hb A1 c(MD-0.39%,95%CI-0.84 to 1.61;very-low-quality evidence),2 h BG(MD-4.99 mg/d L,95%CI-20.74 to 10.76;low-quality evidence),and FINS(MD-1.32μIU/m L,95%CI-3.76 to 1.12;low-quality evidence).No data on the incidence of diabetic complications were *** acupuncture-related adverse events reported were ***:The current evidence suggests that acupuncture,as a complementary therapy to antidiabetic drugs,has a small but statistically significant effect on decreasing FBG and improving insulin *** effects of acupuncture on Hb A1 c,2 h BG,and FINS remain *** is generally safe in patients with mild *** evidence for the long-term effects of acupuncture on T2 DM is needed.(Registration ***42018115639)

Therapy to Obese Type 2 Diabetes Mellitus:How Far Will We Go Down the Wrong Road?

traditional glucose-lowering chemical agents, including various types of insulin and insulin secretagogues, insulin sensitizers, gliptins, etc., are based on diabetic pathogenesis of insulin resistance(IR) and islet insufficiency. Numerous evidence-based medical studies have shown that these traditional hypoglycemic chemical agents do not provide cardiovascular benefit to patients with type 2 diabetes mellitus(T2 DM) and may even increase the risk of all-cause mortality. Based on research evidence published to date, these studies show that overload of energy could increase the incidence and prevalence of T2 DM, and reduction in the heat load can significantly reduce the incidence of T2 DM. Therefore, the essence of T2 DM is heat overload, meaning heat overload is the etiology of obese T2 DM. At the same time, results of numerous studies show that heat overloading is the cause of IR. IR and islet dysfunction are protective factors in intervening with heat *** drugs, which are based on the mechanisms of IR and islet insufficiency, increase caloric reserve and cause or worsen obesity, which is equivalent to exacerbating the basic etiology and the cardiovascular risk factor of T2 DM. Thus, a reasonable strategy for prevention and treatment of obese T2 DM appears to promote the negative balance of calories and the elimination of caloric reserves. chinese herbal medicines can promote negative balance of heat in many aspects, which can bring new hope for prevention and treatment of T2 DM.

Effect of catgut embedding at acupoints versus non-acupoints in abdominal obesity:a randomized clinical trial

OBJECTIVE:To explore the difference of catgut embedding effect between acupoints and non-acupoints in patients with abdominal obesity(AO).METHODS:In this multicenter,double-blind,randomized controlled trial,all subjects were randomly assigned into the acupoint catgut embedding(ACE)group and control group(catgut embedding at non-acupoints).With a 12-week actual intervention period and a 4-week period of *** circumference(WC),body weight,body mass index(BMI),hip circumference(HC)and appetite were applied and assessed at baseline and after 6,12 and 16 ***:After the total intervention phase(12 weeks),the WC,body weight,BMI,HC and visual analogue scale scores of appetite,decreased significantly in the two groups as compared to the baseline(P<0.001).Meanwhile,after the 4-week follow-up,the indicators still decreased significantly in the ACE group(P<0.001).At 12 and 16 weeks,catgut embedding at acupoints showed significantly advantages to non-acupoints in WC and appetite(P<0.05).No serious adverse events were observed in ACE group and control ***:Catgut embedding at acupoints and non-acupoints are all effective and safe for *** can effectively treat AO as expected and deliver lasting results.

A Multicenter,Randomized,Double-blind Clinical Study on Wufuxinnaoqing Soft Capsule(五福心脑清胶囊) in Treatment of Chronic Stable Angina Patients with Blood Stasis Syndrome

Objective： To confirm the efficacy and safety of Wufuxinnaoqing Soft Capsule （五福心脑清胶囊, WSC） in the treatment of chronic stable angina （blood stasis syndrome）. Methods： A multicenter, randomized, double-blind, placebo-controlled trial with superiority test was designed. A total of 240 patients with chronic stable angina （blood stasis syndrome） from multiple centers were randomly and equally assigned to the treatment group and the control group. Based on standard treatment of Western medicine, the treatment group was given WSC, while the control group was given WSC mimetic, both for 12 weeks. Observed indicators included the efficacy in angina, the efficacy in chinese medicine syndrome, the withdrawal or reduce rate of nitroglycerin and routine safety indices. Results： After 12-week treatment, the significant effective rate and total effective rate of the treatment group were significantly better than those of the control group （23.5% vs. 9.2%, 64.7% vs. 30.8%）, respectively, with statistically significant difference （P〈0.01）. After 12-week treatment, the decreased points and the decreased rate of angina symptom score in the treatment group were better than in the control group （5.1±4.2 points vs. 2.8± 3.5 points, 44.9% ±37.2% vs. 25.4%±30.7%） respectively, with significant difference （P〈0.01）. After 12-week treatment, the significant effective rate and total effective rate of the treatment group were better than the control group （respectively, 30.3% vs. 15.0%, 67.2% vs. 45.0%, P〈0.01）. After 8- or 12-week treatment, the decreased points and the decreased rate of chinese medicine syndrome score in the treatment group were better than the control group （P〈0.05 or P〈0.01）. After 12-week treatment, nitroglycerin withdrawal rate and the withdrawal or reduce rate in treatment group were better than the control group （P〈0.01）. Effect of Taichong(LR 3) acupuncture in spontaneously hypertensive rats

OBJECTIVE: To evaluate the effects of Taichong(LR3) acupuncture points(acupoints) on the expression of glucose transporter protein 1(GLUT1) in the hypothalamus of spontaneously hypertensive rats(SHRs) as measured by combined positron emission tomography and computed tomography(PETCT).METHODS: Spontaneously hypertensive rats(SHR)were divided into model, Taichong(LR 3) acupuncture, and sham groups. Additionally, Tokyo Wistar rats were used as the control group. Changes in blood pressure were recorded in different groups of rats before and after the corresponding treatment. Hematoxylin and eosin(HE) staining was used to study basic morphological changes, and immunohistochemistry was used to determine GLUT1 expression in the hypothalamus. Further,PET-CT was utilized to elucidate the antihypertensive mechanism after acupuncture at the Taichong(LR 3) ***: PET-CT indicated activation of the hypothalamus. Measurement of blood pressure showed that acupuncture at the Taichong(LR 3) acupoints lowered blood pressure. HE staining did not show any significant pathological changes, although differences in cell number were observed. Immunohistochemical analysis indicated a GLUT1 downregulation in the SHRs of the Taichong(LR 3) acupuncture group after the ***: Acupuncture at Taichong(LR 3) acupoints lowered blood pressure in SHRs, with possible mechanisms being changes in cell number and GLUT1 expression in the hypothalamus.

According to folk usage of Aconitum carmichaelii Debx., the present study was designed to determine the feasibility of the stems and leaves of Aconitum carmichaelii Debx. as a new medicinal resource. Fourteen alkaloids in mother roots, fibrous roots, stems, and leaves of Aconitum carmichaelii Debx. were measured by HPLC-MS/MS. And multivariate analysis methods, such as clustering analysis and principal component analysis, were applied to analyze the difference among various parts. In addition, the acute toxicity, analgesia, and anti-inflammatory tests were carried out. The results suggested that the contents of alkaloids in mother roots and fibrous roots were approximate, but those of leaves and stems were different from mother roots and fibrous roots. The results of the acute toxicity testing demonstrated the toxicity of fibrous root was strongest, and mother roots were slightly less toxic than fibrous roots. The stems and leaves were far less toxic than mother and fibrous roots. In addition, the analgesia and inflammatory tests showed the effects of the various tissues had no difference each other. These results provided a basis for developing new complementary and alternative treatments for rheumatoid arthritis patients. Simultaneously, the approach may also turn wastes into treasure and promote the development of circular economy.
