AIM: To evaluate the safety and efficacy of intravitreal etanercept in the inhibiting of proliferative vitreoretinopathy(PVR) in a model of penetrating ocular injury. METHODS: Penetrating ocular injury on the retina of rabbit was induced, which was subsequently treated using 0.1 mL of sterile water or 0.1 m L of 12.5 mg/mL etanercept. The development of PVR was evaluated by fundus images, the B-scan, and the histopathology. The mRNA and protein expressions of tumor necrosis factor-α(TNF-α), transforming growth factor β(TGF-β) as well as connective tissue growth factor(CTGF) were examined at various time points after the etanercept injection with the reverse transcriptase-polymerase chain reaction(RT-PCR) and Western blotting, respectively. The safety of etanercept was evaluated by injection of 12.5 mg/mL etanercept into a normal rabbit eye without penetrating trauma. RESULTS: Clinical assessment and grading clearly demonstrated that the PVR formation was prevented in etanercept-treated animals, which was confirmed via fundus images, B-scan and histopathology. The RT-PCR and Western blotting showed increased mRNA and protein expression of TNF-α, TGF-β as well as CTGF in the retina of rabbits following penetrating ocular injury, and these factors were dramatically mitigated by ocular etanercept treatment. In addition, there was no adverse effect of etanercept intravitreal injection in normal eyes without penetrating trauma, it showed normal structure and histology. CONCLUSION: The etanercept is a potential therapy for inhibiting PVR development. To assess the clinic application of the etanercept in preventing PVR, further clinical studies are required.

AIM: To investigate the hepatoprotective effect of improved prescription of Taohechengqi-tang (IPTT) against acute liver failure (ALF) in ***: Seventy specific pathogen free male Wistar rats were randomly divided into four groups: control group (normal rats, n = 10), ALF group (ALF model, n = 20), Stronger Neo-Minophagen C (SNMC) group (ALF model + SNMC, n = 20), and IPTT group (ALF model + IPTT, n = 20). The ALF model group was administered an intraperitoneal injection of D-galactosamine (1.4 g/kg), and the control group received normal saline intraperitoneally. The SNMC and IPTT groups were treated with SMMC (15.6 mg/kg) or IPTT (28.6 g/kg) by gavage at 24 h intervals, and the ALF and control groups were treated with normal saline. At 36 h after injection, serum alanine aminotransferase, aspartate aminotransferase, total bilirubin, albumin, and cholinesterase and prothrombin time were determined, and liver histopathological scores were observed by microscopy after hematoxylin and eosin staining. mRNA expression of high mobility group box (HMGB) 1, toll-like receptor (TLR) 4, nuclear factor kappa B (NF-κB) and caspase-3 were analyzed via fluorescence quantitative reverse transcriptase polymerase chain reaction. Proliferating cell nuclear antigen (PCNA) immunohistochemistry in liver tissue was also ***: D-galactosamine notably decreased the biochemical and coagulation profiles in serum. IPTT not only improved liver function and histopathology but also normalized the gene expression levels in liver tissue. Compared with the model group, in the IPTT and SNMC groups, HMGB1 mRNA/β-actin (0.06 ± 0.03, 0.11 ± 0.04 vs 0.25 ± 0.04, P < 0.05); TLR4 mRNA/β-actin (0.07 ± 0.02, 0.22 ± 0.08 vs 0.41 ± 0.22, P < 0.05); NF-κB mRNA/β-actin (0.74 ± 0.41, 1.78 ± 0.64 vs 2.68 ± 1.35, P < 0.05); and caspase-3 mRNA/β-actin

Background： Microvascular complications in type 2 diabetes （T2DM）, including diabatic retinopathy （DR）, diabetic kidney disease （DKD）, diabetic peripheral neuropathy （DPN） are the leading causes of visual loss, end-stage renal disease or amputation, while the current therapies are still unsatisfactory. chinese medicine （CM） has been widely used for treating diabetic mettitus. However, most of the previous studies focused on the single complication. The role of CM treatment in T2DM patients with 2 or multiple microvascular complications is not clear. Objective： To appraise the curative effect of CM in T2DM patients with 2 or multiple microvascular complications, and to compare the effects of stationary treatment and individualized treatment in T2DM patients with microvascular complications. Methods： This trial will be an 8-center, randomized, controlled study with 8 parallel groups. A total of 432 patients will be randomized to 8 groups： DR study group （32 cases） and a corresponding control group （32 cases）, DR＋DKD study group （64 cases） and a corresponding control group （64 cases）, DR＋DPN study group （64 cases） and a corresponding control group （64 cases）, DR＋DKD＋DPN study group （56 cases） and a corresponding control group （56 cases）. The control group will receive stationary treatment, and the study group will receive individualized treatment based on CM syndrome differentiation in addition to stationary treatment. The study duration will be 50 weeks, comprising a 2-week run-in period, 24 weeks of intervention, and 24 weeks of follow-up. The outcomes will assess efficacy of treatment, improvement in CM symptoms, safety assessments, adherence to the treatment, and adverse events. Conclusion： This study will provide evidence of evidence-based medicine for CM treatment in two or multiple microvascular complications caused by T2DM. （Registration No. ChiCTR-IPR-15007072）

Chronic respiratory diseases(CRDs)are among the most common noncommunicable diseases globally,with high morbidity and mortality ***,a treatment method derived from traditional chinese medicine,has been shown to be effective at treating CRDs,with little risk of adverse *** research on the mechanisms underlying the effects of acupuncture,especially,its immune regulatory function,has rapidly advanced in recent ***,the diverse immune regulatory mechanisms underlying the beneficial effects of acupuncture are summarized from the perspectives of innate immunity,adaptive immunity,and neuroimmunity.A better understanding of these mechanisms will ultimately provide a scientific basis for the clinical use of acupuncture for the treatment of CRDs.

OBJECTIVE:To evaluate the effectiveness and safety of acupuncture that reinforces the spleen to strengthen the lung in patients with stable chronic obstructive pulmonary disease(COPD).METHODS:This was a randomized,open-controlled trial in which the acupuncturist and the participants were not blinded,but the outcome evalu-ators and data analysts were ***-hundred-and-two patients with stable COPD were randomly divided into two groups in a 1∶1 *** acupuncture group received 30-minute sessions of acupuncture therapy at the same acupoints three times weekly for 6 weeks in addition to routine conventional Western medicine treatment;the control group received routine conventional Western medicine treatment *** primary outcome was the Borg scale score,which was assessed immediately after the 6-minute walk *** secondary outcomes were the 6-minute walk distance,lung function,and oxygen *** were obtained at baseline and after 6 weeks of ***:After 6 weeks of treatment,the Borg scale score in the acupuncture group was significantly better than that in the control group(2.02±0.71 versus 5.01±0.34,P<0.05).Furthermore,the post-treatment improvements in the 6-minute walk distance,lung function,and oxygen saturation were significantly greater in the acupuncture group than in the control group,showing that the acupuncture group had better exercise ***:The findings suggest that acupuncture that aims to reinforce the spleen to strengthen the lung is a safe and effective adjuvant therapy that effectively improves the exercise capacity of patients with stable COPD.

OBJECTIVE: To evaluate the efficacy of integrated traditional chinese medicine-Western medicine(TCM-WM) in the treatment of acute onset pulmonary heart disease(PHD).METHODS: A total of 240 patients met the inclusion criteria and were enrolled. These inpatients were divided into group A(treatment group) and B(control group) in order of admission according to the principles of randomization and control. The research was performed simultaneously in three hospitals. Two groups were given basic treatment that included: controlled oxygen therapy, active and effective anti-infection, maintaining airway patency,correcting O_2 deficiency and CO_2 retention, correcting acid-base imbalance and electrolyte disturbance, reducing pulmonary hypertension and treating right heart failure, nutritional support and treatment of complications. Group A was given basic treatment and integrated traditional chinese Medi-cine(TCM) differentiating therapy; group B was given basic therapy and a placebo that was similar in appearance and taste to TCM medicinal broth of pharmaceutical preparations, provided by Yibin Pharmaceutical Company(Yibin, China, Wuliangye Group).RESULTS: The mortality in the treatment group decreased by 4.98% compared with the control group. The treatment group reported improved ventilation, corrected hypoxemia, improved nutritional status and promoted digestive functions. It also significantly improved the patient's self-life skills, improved the patient's quality of life and could shorten the length of hospital ***: Comprehensive integrated TCMWM treatment showed good clinical efficacy toward the acute onset period of PHD patients.

Acupuncture is widely used to treat functional dyspepsia with satisfactory outcomes. Combination of the He and Mu acupoints is commonly used and has a synergistic effect on functional dyspepsia; however, its underlying mechanisms remain unclear. Therefore, a randomized controlled parallel clinical trial is currently underway at chengdu university of traditional chinese medicine, China. This trial is designed to explore the efficacy of and central responses to the He-Mu point combination in patients with functional dyspepsia using functional magnetic resonance imaging. A total of 105 patients with functional dyspepsia will be allocated into 3 groups： the low-He point group（puncturing at Zusanli（ST36））, Mu point group（puncturing at Zhongwan（CV12））, and He-Mu point combination group（puncturing at ST36 and CV12）. Every participant will receive 20 sessions of manual acupuncture for 4 weeks. The needles will be inserted perpendicularly to a depth of 1 to 2 cun. The angle of rotation and twisting will range from 90 to 180 degrees, while lifting and thrusting will range from 0.3 to 0.5 cm. The various manipulations will be performed 60 to 90 times per minute. The needles will remain in place for 30 minutes, during which manipulation will be applied every 10 minutes. Magnetic resonance imaging will be performed before and after 20 sessions of acupuncture. The primary outcome is symptom improvement according to the chinese version of the Nepean Dyspepsia Index. Secondary outcomes include the Leeds dyspepsia questionnaire, Self-Rating Anxiety Scale, Self-Rating Depression Scale, Beck Anxiety Inventory, Beck Depression Inventory, and visual analogue scale scores before and after 10 and 20 sessions of acupuncture. Needle sensation and adverse events will be used to assess the therapeutic effects. OBJECTIVE： To observe the clinical efficacy and safety of traditional chinese medicine （TCM） com- bined with Western medicine （WM） in patients with diabetic acute ischemic stroke. METHODS： Ninety patients with diabetic acute ischemic stroke were randomly divided into a treat- ment group and a control group. The 45 patients in the treatment group were given standardized treat- ment with TCM combined with WM. They received corresponding oral chinese decoctions three times daily, according to their TCM syndromes, along with basic western medical treatment. The 45 patients in the control group were given non-stan- dardized treatment with TCM combined with WM. They received an oral chinese decoction for promo- tion of blood circulation to inhibit hemostasis, regardless of their TCM syndromes, along with ba- sic western medical treatment. The treatments lasted for 4 weeks. Scores were evaluated on the National Institutes of Health Stroke Scale （NIHSS） score, activity of daily life （ADL） scores, and TCM symptoms before treatment and 2 and 4 weeks after treatment. RESULTS： Analysis of variance for repeated mea- surements showed that there were significant dif- ferences in NIHSS and ADL score before and after treatment in both groups （P〈0.05）. There were also significant differences between the scores at 2 and 4 weeks after treatment. There were signifi- cant differences in TCM syndrome scores before and after treatment in both groups （P〈0.05）. There were also significant differences between the scores at 2 and 4 weeks after treatment. The X2 test showed no statistically significant difference in the incidence of adverse reactions between the two groups （P〉0.05）. CONCLUSION： Standardized treatment was supe- rior to non-standardized treatment for clinical efficacy of TCM combined with WM in patients with diabetic acute ischemic stroke, and the superiority was more obvious in improving neural dysfunction, ADL score, and TCM symptoms.
