The liver has a complex cellular composition and a remarkable regenerative *** primary cell types in the liver are two parenchymal cell populations,hepatocytes and cholangiocytes,that perform most of the functions of the liver and that are helped through interactions with non-parenchymal cell types comprising stellate cells,endothelia and various hemopoietic cell *** regulation of the cells in the liver is mediated by an insoluble complex of proteins and carbohydrates,the extracellular matrix,working synergistically with soluble paracrine and systemic *** recent years,with the rapid development of genetic sequencing technologies,research on the liver’s cellular composition and its regulatory mechanisms during various conditions has been extensively *** breakthroughs in strategies for cell transplantation are enabling a future in which there can be a rescue of patients with end-stage liver diseases,offering potential solutions to the chronic shortage of livers and alternatives to liver *** review will focus on the cellular mechanisms of liver homeostasis and how to select ideal sources of cells to be transplanted to achieve liver regeneration and *** advances are summarized for promoting the treatment of end-stage liver diseases by forms of cell transplantation that now include grafting strategies.

AIM: To investigate the survival of individuals with colorectal cancer (CRC) with inflammatory bowel disease (IBD-associated CRC) compared to that of individuals without IBD diagnosed with CRC. METHODS: Epidemiologic, clinical, and follow-up data were obtained from the Colon Cancer Family Registry (Colon CFR). IBD-associated cases were identified from self-report of physician diagnosis. For a subset of participants, medical records were examined to confirm self-report of IBD. Cox proportional hazards regression was applied to estimate adjusted hazard ratios (aHR) and 95%CI of mortality, comparing IBD-associated to non-IBD-associated CRC, adjusted for age at CRC diagnosis, sex, Colon CFR phase, and number of prior endoscopies. Following imputation to complete CRC stage information, adjustment for CRC stage was examined. RESULTS: A total of 7202 CRC cases, including 250 cases of IBD-associated CRC, were analyzed. Over a twelve year follow-up period following CRC diagnosis, 2013 and 74 deaths occurred among non-IBD associated CRC and IBD-associated CRC patients, respectively. The difference in survival between IBD-associated and non-IBD CRC cases was not statistically significant (aHR = 1.08; 95%CI: 0.85-1.36). However, the assumption of proportional hazards necessary for valid inference from Cox regression was not met over the entire follow-up period, and we therefore limited analyses to within five years after CRC diagnosis when the assumption of proportional hazards was met. Over this period, there was evidence of worse prognosis for IBD-associated CRC (aHR = 1.36; 95%CI: 1.05-1.76). Results were similar when adjusted for CRC stage, or restricted to IBD confirmed in medical records. CONCLUSION: These results support the hypothesis that IBD-associated CRC has a worse prognosis than non-IBD-associated CRC.

OBJECTIVE: We sought to estimate the incidence and clinical significance of a negative therapeutic loop electrosurgical excision procedure (LEEP) and to evalu ate patient specimens for limiting histologic features associated with a negativ e LEEP. METHODS: We identified 674 patients with biopsy-confirmed high-grade c ervical dysplasia who were treated with LEEP from 1991 through 2001. The results of these LEEP procedures were reviewed for the absence of dysplasia or the pres ence of cervical intraepithelial neoplasia stages 1-3. Computerized pathology f iles of patients were then reviewed through July 2002 to determine whether dyspl asia recurred. Slides of negative LEEP specimens were reviewed to confirm the ab sence of dysplasia and to search for histologic features that may have limited o ur interpretation of the specimen. RESULTS: Ninety-three (14%)of LEEP specimen s reviewed were completely negative for dysplasia. Clinical follow-up was avail able on 75 of the 93 patients, with a median follow-up time of 2 years. Eightee n (24%) patients had subsequent positive follow-up, including carcinoma (n = 2 ), high-grade squamous intraepithelial lesions (n = 8), low-grade squamous int raepithelial lesion (n = 6), and atypical squamous cells of undetermined signifi cance (n = 2). Patients with negative LEEPs had a recurrence rate similar to pat ients with positive LEEPs (24%versus 27%). Limiting histologic features were m ore commonly identified in negative LEEPs as compared with LEEPs containing dysplasia (16% versus 5%, P < .001). CONCLUSION: A negative LEEP is not an uncommon finding, o ccurring in 14%(95%confidence interval 11-17%) of specimens at our instituti on. Negative LEEPs are more likely to contain histologic features that limit pat hology interpretation. A negative LEEP is not a reassuring finding and was assoc iated with a recurrence rate similar to those of a positive LEEP. Both negative and positive populations should be carefully followed.

Purpose:The aim of this study was to review,systematically,evidence concerning the link between the ACTN3 R577X polymorphism and the rates and severity of non-contact injuries and exercise-induced muscle damage in athletes and individuals enrolled in exercise training ***:A computerized literature search was performed in the electronic databases PubMed,Web of Science,and SPORTDiscus,from inception until November *** included studies compared the epidemiological characteristics of non-contact injury between the different genotypes of the ACTN3 R577X ***:Our search identified 492 *** the screening of titles,abstracts,and full texts,13 studies examining the association between the ACTN3 genotypes and the rate and severity of non-contact injury were included in the *** studies were performed in 6 different countries(Spain,Japan,Brazil,China,the Republic of Korea,and Italy)and involved a total participant pool of 1093 *** the studies,2 studies involved only women,5 studies involved only men,and 6 studies involved both men and *** the studies included were classified as highquality studies(≥6 points in the Physiotherapy Evidence Database(PEDro)scale score).Overall,evidence suggests there is an association between the ACTN3 R577X genotype and non-contact injury in 12 *** studies observed a significant association between A CTN3 R577X polymorphism and exercise induced muscle damage:2 with non-contact ankle injury,3 with non-contact muscle injury,and 1 with overall non-contact ***:The present findings support the premise that possessing the ACTN3 XX genotype may predispose athletes to a higher probability of some non-contact injuries,such as muscle injury,ankle sprains,and higher levels of exercise-induced muscle damage.

BACKGROUND The Rome IV criteria eliminated abdominal discomfort for irritable bowel syndrome(IBS), which was previously included in Rome III. There are questions as to whether IBS patients with abdominal discomfort(seen in Rome III but not Rome IV) are different from those with abdominal pain(Rome IV).AIM To compare bowel symptoms and psychosocial features in IBS patients diagnosed with Rome III criteria with abdominal discomfort, abdominal pain, and pain &*** We studied IBS patients meeting Rome III criteria. We administered the IBS symptom questionnaire, psychological status, and IBS quality of life. Patients were classified according to the predominant abdominal symptom associated with defecation into an only pain group, only discomfort group, and pain & discomfort group. We compared bowel symptoms, extraintestinal symptoms, IBS quality of life, psychological status and healthcare-seeking behaviors, and efficacy among the three groups. Finally, we tested risk factors for symptom reporting in IBS *** Of the 367 Rome III IBS patients enrolled, 33.8%(124 cases) failed to meet Rome IV criteria for an IBS diagnosis. There were no meaningful differences between the pain group(n = 233) and the discomfort group(n = 83) for the following:(1) Frequency of defecatory abdominal pain or discomfort;(2) Bowel habits;(3) Coexisting extragastrointestinal pain;(4) Comorbid anxiety and depression;and(5) IBS quality of life scores except more patients in the discomfort group reported mild symptom than the pain group(22.9% vs 9.0%). There is a significant tendency for patients to report their defecatory and non-defecatory abdominal symptom as pain alone, or discomfort alone, or pain & discomfort(all P < 0.001).CONCLUSION IBS patients with abdominal discomfort have similar bowel symptoms and psychosocial features to those with abdominal pain. 目的　本研究旨在明确TGF β1诱导鼠肝细胞凋亡与Caspase 3及Caspase 8的关系。方法　通过 5 0 %CCl4腹腔注射 ,用 8周时间诱发BALB/c小鼠形成肝硬化模型。应用改良的胶原酶原位灌注法分离小鼠的肝细胞。为检测TGF β1(5ng/ml)诱导凋亡 ,利用 1.5 %的琼脂糖凝胶电泳观察DNA梯形条带 ,应用DNA荧光染料Hoechst 33342对正常肝细胞进行染色 ,并在荧光显微镜下观察比较Caspase 3和Caspase 8在TGF β1诱导的凋亡中的作用。结果　胶原酶原位二步灌流法分离肝细胞活率为 95 .2 %。正常肝细胞予以TGF β1处理后 ,应用 1.5 %的琼脂糖凝胶电泳可以发现具有凋亡特征的梯形条带。硬化肝细胞则很少出现梯形条带。经TGF β1处理的鼠正常肝细胞应用Hoechst染色发现 ,其凋亡率明显高于未处理组 ,分别为 5 8.76 %和 18.0 3% ,两组具有明显的差别。但凋亡可以被Caspase 3和Caspase 8抑制剂阻止。结论　硬化肝细胞不能象正常肝细胞那样发生TGF β1诱导的凋亡 ,提示硬化肝细胞的抑制性生长调控机制已受损。TGF β1通过Caspase 3和Cas pase 8的活化诱导鼠肝细细胞产生凋亡。

复发性腹痛:决定医疗咨询行为的原因

Recurrent abdominal pain (RAP) is associated with increased health care visits and school absences. In adults suffering from functional pain, psychosocial factors determine illness behavior and we aimed to investigate its role among children. A community sample of 40 RAP consulters, 41 RAP nonconsulters, and 36 pain-free controls and their mothers completed questionnaires on GI and non-GI symptoms, school absences, psychological symptoms, coping, self-esteem, and behavioral and cognitive responses to RAP. T-tests showed significant differences between RAP and controls in (1) GI and non-GI symptoms and school absences, (2) child distress and passive coping, and (3) mother’ s IBS severity, somatization, and fears about RAP. RAP consulters reported the same levels of GI symptoms and psychological distress as nonconsulters but missed significantly more school and their mothers reported more fears about RAP. Severity of symptoms and psychological distress did not predict consulting behavior. Only maternal fears about abdominal symptoms differentiated consulters from nonconsulters.
