Impact of an acute hemodynamic response-guided protocol for primary prophylaxis of variceal bleeding

AIM To evaluate the long-term outcome of an acute hemodynamic response-guided protocol in which acute responders to intravenous propranolol received traditional nonselective beta-blockers(nSBBs) and acute nonresponders received *** Retrospective review of a protocol for primary prophylaxis of variceal bleeding guided by the acute hemodynamic response to intravenous propranolol. Fifty-two acute responders treated with traditional nSBB(i.e. propranolol or nadolol) were compared with 24 acute nonresponders receiving carvedilol. A second hemodynamic study was performed in 27 and 13 patients, respectively. The primary endpoint was development of first or further decompensation. Secondary endpoints included death from any cause, association between acute and chronic hemodynamic response, and baseline clinical and laboratory variables related to the acute hemodynamic *** Acute responders and acute nonresponders presented similar 1, 2, and 3-year probabilities of first decompensation(nSBB: 0%, 13.7%, 26.1% vs carvedilol: 0%, 20%, 20%, P = 0.968) or further decompensation(21.2%, 26.1%, 40.9% vs 21.2%, 50.0%, 50.0%, P = 0.525). A previous episode of hepatic encephalopathy was the only independent predictor of decompensation [hazard ratio(95% confidence interval): 8.03(2.76-23.37)]. Mortality rates were similar in acute responders and acute nonresponders with compensated(P = 0.428) or decompensated cirrhosis(P = 0.429). no clinical, laboratory, endoscopic or hemodynamic parameter predicted the acute hemodynamic response. In patients receiving traditional nSBB, the acute and chronic changes of hepatic venous pressure gradient were correlated(r = 0.59, P = 0.001). Thin films of SnSe and SnSe2 have been deposited using the ultrasonic spray pyrolysis（USP） *** the best of our knowledge this is the first report of the deposition of SnSe and SnSe2 thin films using a single spray *** use of a single spray solution for obtaining both a p-type material,SnSe,and an n-type material,SnSe2,simplifies the deposition *** SnSe2 thin films have a bandgap of 1.1 eV and the SnSe thin films have a band gap of 0.9 *** Hall measurements were used to determine the resistivity of the thin *** SnSe2 thin films show a resistivity of 36.73 Ωcm and n-type conductivity while the SnSe thin films show a resistivity of 180 Ωcm and p-type conductivity.

Under the present changing climate conditions and the observed temperature increase,it is of high importance to understand its effects on aquatic microbial life,and organisms’adaptations at the biochemical *** adjust to temperature or salinity stress and avoid cell damage,organisms alter their degree of fatty acids(FAs)***,temperature is expected to have strong effects on both the quantity and quality of FAs in aquatic *** we review some recent findings about FAs sensitivity to climate change in contrasting ***,heat waves may induce changes in the relative abundance of polyunsaturated FAs(PUFA).However,the impact of the exposure to warming waters is different in temperate and polar *** cold marine waters,high concentration of omega-3(ω3)FAs such as eicosapentaenoic acid(EPA)is promoted due to the activation of the desaturase *** this way,cells have enough energy to produce or activate antioxidant protection mechanisms and avoid oxidative stress due to heat ***,under high irradiance and heat wave conditions in temperate environments,photosystems’protection is achieved by decreasing EPA concentration due to desaturase *** FAs are transferred in aquatic food ***,any alteration in the production of essential FAs by phytoplankton(the main source ofω3)due to climate warming can be transferred to higher trophic levels,with cascading effects for the entire aquatic ecosystem.

An overview of animal models for investigating the pathogenesis and therapeutic strategies in acute hepatic failure

Acute hepatic failure (AHF) is a severe liver injury accompanied by hepatic encephalopathy which causes multiorgan failure with an extremely high mortality rate, even if intensive care is provided. Management of severe AHF continues to be one of the most challenging problems in clinical medicine. Liver transplantation has been shown to be the most effective therapy, but the procedure is limited by shortage of donor organs. Although a number of clinical trials testing different liver assist devices are under way, these systems alone have no significant effect on patient survival and are only regarded as a useful approach to bridge patients with AHF to liver transplantation. As a result, reproducible experimental animal models resembling the clinical conditions are still needed. The three main approaches used to create an animal model for AHF are: surgical procedures, toxic liver injury and infective procedures. Most common models are based on surgical techniques (total/partial hepatectomy, complete/transient devascularization) or the use of hepatotoxic drugs (acetaminophen, galactosamine, thioacetamide, and others), and very few satisfactory viral models are available. We have recently developed a viral model of AHF by means of the inoculation of rabbits with the virus of rabbit hemorrhagic disease. This model displays biochemical and histological characteristics, and clinical features that resemble those in human AHF. In the present article an overview is given of the most widely used animal models of AHF, and their main advantages and disadvantages are reviewed.

determination of the negative ion number density of O2 and O in a DC discharge of oxygen plasma was made employing Langmuir probe in conjunction with eclipse laser photo- detachment technique. The temporal evolution of the extra electrons resulting from the photo- detachment of O2- and O- were used to evaluate the negative ion number density. The ratio of O2 number density to O varied from 0.03 to 0.22. number density of both O~ and O increased with increasing power and decreased as the pressure was increased. Electron number density was evaluated from the electron energy distribution function （EEDF） using the I-V recorded characteristic curves. Electron temperature between 2 and 2.7 eV were obtained. Influence of the 02（al△g） metastable state is discussed.

Faecal immunochemical tests(FITs)are the most widely colorectal cancer(CRC)diagnostic biomarker *** population screening programmes are based on this biomarker,with the goal of reducing CRC ***,in recent years,a large amount of evidence has been produced on the use of FIT to detect CRC in patients with abdominal symptoms in primary healthcare as well as in surveillance after adenoma *** aim of this review is to highlight the available evidence on these two *** will summarize the evidence on diagnostic yield in symptomatic patients with CRC and significant colonic lesion and the different options to use this(thresholds,brands,number of determinations,prediction models and combinations).We will include recommendations on FIT strategies in primary healthcare proposed by regulatory bodies and scientific societies and their potential effects on healthcare resources and CRC ***,we will show information regarding FIT-based surveillance as an alternative to endoscopic surveillance after high-risk polyp *** conclude,due to the coronavirus disease 2019 pandemic,FIT-based strategies have become extremely relevant since they enable a reduction of colonoscopy demand and access to the healthcare system by selecting individuals with the highest risk of CRC.

Spinal cord injury is a very common pathological event that has devastating functional consequences in patients. In recent years, several research groups are trying to find an effective therapy that could be applied in clinical practice. In this study, we analyzed the combination of different strategies as a potential therapy for spinal cord injury. Immunization with neural derived peptides(InDP), inhibition of glial scar formation(dipyridyl: DPY), as well as the use of biocompatible matrix(fibrin glue: FG) impregnated with bone marrow mesenchymal stem cells(MSCs) were combined and then its beneficial effects were evaluated in the induction of neuroprotection and neuroregeneration after acute SCI. Sprague-Dawley female rats were subjected to a moderate spinal cord injury and then randomly allocated into five groups: 1) phosphate buffered saline; 2) DPY; 3) InDP + DPY; 4) DPY+ FG; 5) InDP + DPY + FG + MSCs. In all rats, intervention was performed 72 hours after spinal cord injury. Locomotor and sensibility recovery was assessed in all rats. At 60 days after treatment, histological examinations of the spinal cord(hematoxylin-eosin and Bielschowsky staining) were performed. Our results showed that the combination therapy(DPY+ InDP + FG+ MSCs) was the best strategy to promote motor and sensibility recovery. In addition, significant increases in tissue preservation and axonal density were observed in the combination therapy group. Findings from this study suggest that the combination theapy(DPY+ InDP + FG + MSCs) exhibits potential effects on the protection and regeneration of neural tissue after acute spinal cord injury. All procedures were approved by the Animal Bioethics and Welfare Committee(approval no. 178544; CSnBTBIBAJ 090812960)on August 15, 2016.

Melatonin is a pleiotropic molecule that,after a short-term sleep deprivation,promotes the proliferation of neural stem cells in the adult ***,this effect has not been observed in long-term sleep *** precise mechanism exerted by melatonin on the modulation of neural stem cells is not entirely elucidated,but evidence indicates that epigenetic regulators may be involved in this *** this study,we investigated the effect of melatonin treatment during a 96-hour sleep deprivation and analyzed the expression of epigenetic modulators predicted by computational text mining and keyword *** results showed that the administration of melatonin under sleep-deprived conditions increased the MECP2 expression and reduced the SIRT1 expression in the dentate *** observed that let-7 b,mir-132,and mir-124 were highly expressed in the dentate gyrus after melatonin administration,but they were not modified by sleep *** addition,we found more Sox2^+/5-bromo-2’-deoxyuridine(BrdU)^+cells in the subgranular zone of the sleep-deprived group treated with melatonin than in the untreated *** findings may support the notion that melatonin modifies the expression of epigenetic mediators that,in turn,regulate the proliferation of neural progenitor cells in the adult dentate gyrus under long-term sleep-deprived *** procedures performed in this study were approved by the Animal Ethics Committee of the University of Guadalajara,Mexico(approval ***-16610)on January 2,2016.
