Background As treatment of coronary artery disease improved over the last years,management of elder patients remained a matter of debate since this age group has been underrepresented in most *** study aimed to evaluate a population of patients with≥85 years old with non-ST-segment elevation myocardial infarction(NSTeMI)and compare the prognosis according to coronary revascularization *** We retrospectively studied 324 patients included in a national multicenter registry between October 2010 and October 2018,who underwent coronary angiography and had at least one stenosis≥50%.Results In this population,73.1%of the patients underwent percutaneous coronary intervention(PCI)and 26.9%of the patients underwent optimized medical treatment(OMT).The OMT group had more past history of diabetes,stroke and *** coronary angiography,the PCI group used more often the femoral artery access and single-vessel lesions were also more ***-vessel disease was more common in the OMT *** hospitalization,there were more major bleeding events and death in the PCI *** the one-year follow-up,there were no significant differences in all-cause mortality *** Very old patients with NSTeMI submitted to OMT had more comorbidities and more three-vessel disease,factors that could have influenced the therapeutic *** undergoing PCI had more in-hospital major bleeding events and mortality,with no significant differences after one year.

Background and objectives:Lung cancer(LC)is the leading cause of cancer *** treated with chemotherapy are at risk of developing chemotherapy-induced febrile neutropenia(FN),a potentially life-threatening *** aims of this study were(1)to characterize FN admissions of patients with LC in a pulmonology department,and(2)to determine associations between patient profiles,first-line antibiotic failure(FLAF)and ***:Retrospective observational case-series,based on the analysis of medical records of LC patients that required hospitalization due to chemotherapy-induced ***:A total of 42 cases of FN were revised,corresponding to 36 patients,of which 86.1%were male,with a mean age of 66.71±9.83 *** patients had a performance status(PS)equal or less than 1,and metastatic disease was present in 40.5%(n=17).Respiratory tract infections accounted for 42.9%(n=18)of FN cases,and multidrug-resistant Staphylococcus aureus was the most isolated *** mortality rate was 16.7%(n=7),and the FLAF was 26.2%(n=11).Mortality was associated with a PS≥2(P=0.011),infection by a Gram-negative agent(P=0.001)and severe anemia(P=0.048).FLAF was associated with longer hospitalizations(P=0.020),PS≥2(P=0.049),respiratory infections(P=0.024),and infection by a Gram-negative(P=0.003)or multidrug-resistant agent(P=0.014).Conclusions:Lower PS,severe anemia,and infections by Gram-negative or multi-resistant agents seem to be associated with worse outcomes in FN patients.

The global pandemic of coronavirus disease 2019(COVID-19)changed dramatically all priorities on medical society and created several challenges for clinicians caring for patients with liver *** performed a comprehensive review about how COVID-19 can affect the liver,the influence of liver diseases on the risk of developing severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)and COVID-19 severity and also some strategies to overcome all the challenges clinicians have to face in the management of patients with liver diseases in a period of time when all the focus turned on *** analyze the relationship between COVID-19 and non-alcoholic fatty liver disease,alcoholic liver disease,viral hepatitis,autoimmune liver disease,cirrhosis,hepatocellular carcinoma and liver transplantation,as well as the approach to SARS-CoV-2 vaccination.

Introduction:MD is considered a rare *** adequate model that explains MD's pathophysiology is not well ***,the vascular theory of the disease has been ***:To characterize a MD population according to its cardiovascular risk and correlate it to the MD clinical ***:In this retrospective chart study the data of 31 MD patients observed between January 2017 and April 2018 in a tertiary university hospital were *** included in the study were diagnosed according to the Barany Society *** follow-ups,patients with autoimmune disease,atopy or allergy,major psychiatric disease and migraine were ***,gender,cardiovascular risk factors,audiometric and vestibular parameters,occurrence of MD attacks in the previous 6 months,vestibular medication in course and time course of MD were recorded and compared between groups(with and without cardiovascular risk factors).Results:31 patients(61.3%females)mean aged 60.3 years(±9.7)were studied.74%of the population had at least one risk factor and 51.6%of patients had attacks in the last 6 *** was a statistically significant difference in the occurrence of MD attacks in the last 6 months(p=0.014)between MD patients with and without risk *** PTA thresholds were higher and speech discrimination was lower in individuals with more cardiovascular risk ***:Treatment of MD focusing on vascular risk factors may allow a better control of symptoms and result in a decreased need for ablative procedures in this disorder.

Hepatobiliary manifestations are common in inflammatory bowel disease(IBD),with 30%of patients presenting abnormal liver tests and 5%developing chronic liver *** range from asymptomatic elevated liver tests to lifethreatening disease and usually follow an independent course from *** pathogenesis of liver manifestations or complications and IBD can be closely related by sharing a common auto-immune background(in primary sclerosing cholangitis,IgG4-related cholangitis,and autoimmune hepatitis),intestinal inflammation(in portal vein thrombosis and granulomatous hepatitis),metabolic impairment(in non-alcoholic fatty liver disease or cholelithiasis),or drug toxicity(in drug induced liver injury or hepatitis B virus infection reactivation).Their evaluation should prompt a full diagnostic workup to identify and readily treat all complications,improving management and outcome.

To the editor： Xie et al, analyzed warfarin doses in pulmonary embolism （Pe） and its adverse effects in patients with obstructive sleep apnea hypopnea syndrome （OSAHS）. The key findings were that OSAHS patients required a significantly higher dose of warfarin than their non-OSAHS counterparts （4.73 mg vs. 3.61 mg, P 〈 0.001） and that Pe recurrence was higher in OSAHS than in non-OSAHS groups after withdrawal ofwarfarin （21.43% vs. 6.78%, P = 0.047）.
