Descriptive epidemiology for <i>Mycoplasma pneumoniae</i>Infection Using (Nursery) School Absenteeism Surveillance System, and Proposal for Countermeasures

Objective: We present descriptive epidemiology of Mycoplasma infection using the (Nursery) School Absenteeism Surveillance System ((N)SASSy) in addition to national official sentinel surveillance for infectious diseases (NOSSID). It is expected to be helpful for early detection and response to outbreak of Mycoplasma infection and also to be useful for antimicrobial resistance measures. Method: The study period was 2010-2014 seasons. The study area was the whole of Ibaraki prefecture, Japan. NOSSID reports the number of patients with Mycoplasma pneumonia from 13 sentinel hospitals. We compare the number of patients and the incidence rate of NOSSID with the same information in (N)SASSy. Result: In NOSSID, the largest number of patients by age was 27 patients of one year old in 2012. (N)SASSy showed that the incidence rate in the elementary schools is high in 2011 and 2012. Especially, the second grade students were the highest. Discussion: Even though Mycoplasma infection is well known as a pediatric disease, we showed that second grade was the highest in incidence at first. Because (N)SASSy is timely and real-time information collection, evaluation and sharing countermeasures with (nursery) schools, public health centers, and physicians are useful for students and nursery school children, and communities. Conclusion: Because mycoplasma infection is common pediatric infectious diseases and because some patients rarely develop severe infections, we must prevent larger outbreaks. (N)SASSy can provide timely intervention at the initial phase of outbreak by monitoring situations in (nursery) schools and comparing data to baseline information.

Tuberculosis disease stands for the second leading cause of death worldwide after COVID-19, most active tuberculosis cases result from the reactivation of latent TB infection through impairment of immune response. Several factors are known to sustain that *** mansoni, a parasite of the helminth genus that possesses switching power from an immune profile type Th1 to Th2 that favors reactivation of latent TB bacteria. The aim of the study was to assess the prevalence of the co-infection between the two endemic infections. Systematic literature was contacted at the University Clinical Research center at the University of Sciences, Techniques, and Technologies of Bamako in Mali. Original articles were included, and full texts were reviewed to assess the prevalence and better understand the immunological changes that occur during the co-infection. In total, 3530 original articles were retrieved through database search, 53 were included in the qualitative analysis, and data from 10 were included in the meta-analysis. Prevalence of the co-infection ranged from 4% to 34% in the literature. Most of the articles reported that immunity against infection with helminth parasite and more specifically Schistosoma mansoni infection enhances latent TB reactivation through Th1/Th2. In sum, the impact of Schistosoma mansoni co-infection with Mycobacterium tuberculosis is under-investigated. Understanding the role of this endemic tropical parasite as a contributing factor to TB epidemiology and burden could help integrate its elimination as one of the strategies to achieve the END-TB objectives by the year 2035.

Background： Nontraumatic osteonecrosis of the femoral head （NONFH） is a debilitating disease that represents a significant financial burden for both individuals and healthcare systems. Despite its significance, however, its prevalence in the Chinese general population remains unknown. This study aimed to investigate the prevalence of NONFH and its associated risk factors in the Chinese population. Methods： A nationally representative survey of 30,030 respondents was undertaken from June 2012 to August 2013. All participants underwent a questionnaire investigation, physical examination of hip, and bilateral hip joint X-ray and/or magnetic resonance imaging exalnination. Blood samples were taken after overnight fasting to test serum total cholesterol, triglyceride, and high-density lipoprotein （HDL） and low-density lipoprotein （LDL） levels. We then used multivariate logistic regression analysis to investigate the associations between various metabolic, demographic, and lifestyle-related variables and NONFH. Results： NONFH was diagnosed in 218 subjects （0.725%） and the estimated NONFH cases were 8.12 million among Chinesepeople aged 15 years and over. The prevalence of NONFH was significantly higher in males than in females （1.02% vs. 0.51%, x^2 = 24.997, P 〈 0.001 ）. Among NONFH patients, North residents were subjected to higher prevalence of NONFH than that of South residents （0.85% vs. 0.61%,x^2= 5.847, P = 0.016）. Our multivariate regression analysis showed that high blood levels oftriglycerides, total cholesterol, LDL-cholesterol, and non-H DL-cholesterol, male, urban residence, family history of osteonecrosis of the femoral head. heavy smoking, alcohol abuse and glucocorticoid intake, overweight, and obesity were all significantly associated with an increased risk of NONFH. Invasive candidiasis(IC) bears a high risk of morbidity and mortality in the intensive care units(ICU). With the current advances in critical care and the use of widespectrum antibiotics, invasive fungal infections(IFIs) and IC in particular, have turned into a growing concern in the ICU. Further to blood cultures, some auxil-iary laboratory tests and biomarkers are developed to enable an earlier detection of infection, however these test are neither consistently available nor validated in our setting. On the other hand, patients' clinical status and local epidemiology data may justify the empiric antifungal approach using the proper antifungal option. The clinical approach to the management of IC in febrile, non-neutropenic critically ill patients has been defined in available international guidelines; nevertheless such recommendations need to be customized when applied to our local practice. Over the past three years, Iranian experts from intensive care and infectious diseases disciplines have tried to draw a consensus on the management of IFI with a particular focus on IC in the ICU. The established IFI-clinical forum(IFI-CF), comprising the scientific leaders in the field, has recently come up with and updated recommendation on the same(June 2014). The purpose of this review is to put together literature insights and Iranian experts' opinion at the IFI-CF, to propose an updated practical overview on recommended approaches for the management of IC in the ICU.

本文的研究目的是探讨中国2型糖尿病(type 2 diabetes mellitus, T2DM)患者的中等至剧烈强度身体活动(Moderate-to-vigorous-intensity physical activity, MVPA)时间和久坐(sedentary,SED)时间与心血管疾病(cardiovascular disease, CVD)风险及多种因素[即血压(blood pressure, BP)、体重指数(body mass index, BMI)、低密度脂蛋白胆固醇(low-density lipoprotein cholesterol, LDL-C)和糖化血红蛋白A1(glycated hemoglobin A1c, HbA1c)]控制状况的关系。本研究为一项基于9152名T2DM人群的横断面研究，数据来自于“Multifactorial Intervention on Type 2 Diabetes Study”，简称MIDiab研究。根据患者自我报告的MVPA时间和SED时间，将身体活动水平分为：低(<150 min·week–1)、中(150～450 min·week–1)、高(≥450min·week–1)三组；久坐行为水平也分为：低(<4 h·d–1)、中(4～8 h·d–1)、高(≥8 h·d–1)三组。本研究的主要结局指标是CVD，将有自我报告CVD疾病史的患者定义为有CVD风险。采用混合效应逻辑回归模型估计与MVPA时间和SED时间相关的CVD风险和多因素控制状态的优势比(odds ratio, OR)及95%置信区间(confidence interval, CI)，对中国地理区域特征进行调整。本研究人群年龄的平均±标准差为(60.87±8.44)岁，其中44.5%为女性，25.1%的患者自我报告有CVD疾病史。校正潜在的混杂因素后，发现高水平的MVPA时间与CVD风险之间存在负相关关系，且独立于SED时间，然而在中等水平MVPA时间组没有观察到这种关联。与低水平身体活动组的参与者相比，高水平身体活动组的人更容易达到BMI的目标水平。进一步评估二者对CVD风险的联合作用，与参照组(即SED时间<4 h·d–1和MVPA时间≥450 min·week–1)相比，低水平身体活动组(MVPA时间<150 min·week–1)的CVD患病风险显著升高。其中，与SED时间<4 h·d–1相关的OR值为1.270(95%CI为1.040～1.553)，与SED时间≥8 h·d–1相关的OR值为1.499(95%CI为1.149～1.955)。在T2DM患者中，长时间的久坐行为(即≥8 h·d–1的SED时间)与CVD风险增加有关；高水平的身体活动(即MVPA时间≥450 min·week–1)与CVD患病风险降低有关，且独立于SED时间。

Canine parvovirus 2(CPV-2)is a highly contagious virus in dogs that typically causes hemorrhagic enteritis and a high mortality rate in unvaccinated *** genetic variability and antigenic diversity of CPV-2 hinder its efective prevention of infection by *** investigate the epidemiology and genetic characteristics of CPV-2 in China,rectal swabs from afected dogs were collected from diferent animal clinics in Kunshan from 2022 to *** detection and capsid gene sequencing of CPV-2 were performed using previously described primers and *** overall detection rate for CPV-2 was 16.5%(33/200).A signifcant association was found between the CPV-2-positivity and clinical signs,age,breed and vaccination *** analysis revealed the presence of CPV-2c genotypes in all positive samples,which were genetically similar to other Asian CPV-2c ***,four key mutations(A5G,F267Y,Y324I and Q370R)were detected in all isolates,and one novel mutation(I447M)was detected in three CPV-2 *** mutations in the CPV-2 strains could impact vaccine efcacy and the efectiveness of the virus immune ***,no recombination events were observed between the identifed CPV-2c strains and reference strains from *** data revealed that amino acid residues 324,426 and 440 of VP2 may under strong selection *** pattern of genetic variation in the CPV-2 lineage warrants continuous laboratory-based surveillance programs in other parts of China to better understand the pattern of seasonal distribution and association between emerging genotypes and the intensity of disease severity.
