The emerging role of mesenchymal stem cell-derived extracellular vesicles to ameliorate hippocampal NLRP3 inflammation induced by binge-like ethanol treatment in adolescence

Our previous studies have reported that activation of the NLRP3(NOD-,LRR-and pyrin domain-containing protein 3)-inflammasome complex in ethanol-treated astrocytes and chronic alcohol-fed mice could be associated with neuroinflammation and brain *** stem cell-derived extracellular vesicles(MSC-EVs)have been shown to restore the neuroinflammatory response,along with myelin and synaptic structural alterations in the prefrontal cortex,and alleviate cognitive and memory dysfunctions induced by binge-like ethanol treatment in adolescent *** the therapeutic role of the molecules contained in mesenchymal stem cell-derived extracellular vesicles,the present study analyzed whether the administration of mesenchymal stem cell-derived extracellular vesicles isolated from adipose tissue,which inhibited the activation of the NLRP3 inflammasome,was capable of reducing hippocampal neuroinflammation in adolescent mice treated with binge *** demonstrated that the administration of mesenchymal stem cell-derived extracellular vesicles ameliorated the activation of the hippocampal NLRP3 inflammasome complex and other NLRs inflammasomes(e.g.,pyrin domain-containing 1,caspase recruitment domain-containing 4,and absent in melanoma 2,as well as the alterations in inflammatory genes(interleukin-1β,interleukin-18,inducible nitric oxide synthase,nuclear factor-kappa B,monocyte chemoattractant protein-1,and C–X3–C motif chemokine ligand 1)and miRNAs(miR-21a-5p,miR-146a-5p,and miR-141-5p)induced by binge-like ethanol treatment in adolescent *** analysis further revealed the involvement of miR-21a-5p and miR-146a-5p with inflammatory target genes and NOD-like receptor signaling *** together,these findings provide novel evidence of the therapeutic potential of MSC-derived EVs to ameliorate the hippocampal neuroinflammatory response associated with NLRP3 inflammasome activation induced by binge drinking in adolescence.

Evaluation of Impact of Major Causes of Death on Life Expectancy Changes in China,1990-2005

Objective To evaluate the impact of major causes of death on changes of life expectancy in China. Methods Life expectancy was calculated by standard life table techniques using mortality data from the national censuses in 1990 and 2000 and the 1% National Population Sampling Surveys in 1995 and 2005, Mortality data about the major causes of death from VR-MOH were used as reference values to estimate their death proportions of the specific age groups by sex and regions, as well as all-cause mortality and age-specific mortality rates of major causes of death. Decomposition method was used to quantitatively evaluate the impact. Results Three key findings were identified in our study. First, China's health challenge was shifted from diseases related to living conditions to those related to behavior and lifestyle, with rural areas relatively lagged behind urban areas, Second, the impacts of cardiovascular diseases and neoplasm on the middle aged and elderly population were stressed. Third, compared to the urban population, the rural population tended to have increasing mortality of neoplasm and cardiovascular diseases, especially in adults at the age of 15-39 years. Conclusion Further efforts should be made to reduce the incidence of neoplasm and cardiovascular diseases, especially in rural areas, by promoting healthy behavior and lifestyle and providing appropriate therapies for all patients in need.

Psychometrics of chronic liver disease questionnaire in Chinese chronic hepatitis B patients

AIM: To evaluate psychometrics of the Chinese (mainland) chronic liver disease questionnaire (CLDQ) in patients with chronic hepatitis B (CHB). METHODS: A cross-sectional sample of 460 Chinese patients with CHB was selected from the Outpatient Department of the Eighth Hospital of Xi'an, including CHB (CHB without cirrhosis) (n = 323) and CHB-related cirrhosis (n = 137). The psychometrics includes reliability, validity and sensitivity. Internal consistency reliability was measured using Cronbach's α. Convergent and discriminant validity was evaluated by item-scale correlation. Factorial validity was explored by principal component analysis with varimax rotation. Sensitivity was assessed using Cohen's effect size (ES), and independent sample t test between CHB and CHB-related cirrhosis groups and between alanine aminotransferase (ALT) normal and abnormal groups after stratifying the disease (CHB and CHB-related cirrhosis).RESULTS: Internal consistency reliability of the CLDQ was 0.83 (range: 0.65-0.90). Most of the hypothesized item-scale correlations were 0.40 or over, and all of such hypothesized correlations were higher than the alternative ones, indicating satisfactory convergent and discriminant validity. Six factors were extracted after varimax rotation from the 29 items of CLDQ. The eligible Cohen's ES with statistically significant independent sample t test was found in the overall CLDQ and abdominal, systematic, activity scales (CHB vs CHBrelated cirrhosis), and in the overall CLDQ and abdominal scale in the stratification of patients with CHB (ALT normal vs abnormal). CONCLUSION: The CLDQ has acceptable reliability, validity and sensitivity in Chinese (mainland) patients with CHB.

Genetic and environmental risk factors for primary open-angle glaucoma

Background Primary open-angle glaucoma (POAG) is characterized by optic nerve damage and consists of a group of genetically heterogeneous disorders. This study was to investigate the associations of genetic and environmental factors with POAG in a hospital-based Chinese population Methods Thirty-two adult onset POAG patients and 96 age-sex matched control subjects were studied by multivariable logistic regression analysis for the relationships between POAG and its risk factors including family history, diabetes, hypertension, cardiovascular diseases, cigarette smoking, alcohol consumption and polymorphisms of the myocilin and the optineurin genes Results Univariate analysis showed that POAG was related to family history, cardiovascular disease, alcohol consumption and a myocilin sequence alteration (T353I) ( P <0 04) Multivariable logistic regression analysis confirmed that POAG was significantly associated with family history ( OR =20 2), hypertension ( OR =3 58), cigarette smoking ( OR =10 8), alcohol consumption ( OR =0 028) and T353I ( OR =6 03, all P <0 05) Conclusions Family history, hypertension, cigarette smoking and T353I in the myocilin gene are risk factors for POAG Alcohol consumption, however, has a protective effect

Totally robotic vs 3D laparoscopic colectomy: A single centers preliminary experience

AIM: to compare robotic and three-dimensional(3D) laparoscopic colectomy based on the literature and our preliminary ***: This retrospective observational study compared operative measures and postoperative outcomes between laparoscopic 3D and robotic colectomy for cancer. From September 2013 to September 2014,24 robotic colectomies and 23 3D laparoscopic colectomy were performed at our Department. Data were analyzed and reported both by approach and by colectomy side. Robotic left colectomy(RL) vs laparoscopic 3D left colectomy(LL 3D) and Robotic right colectomy(RR) vs laparoscopic 3D(LR 3D). Rectal cancer procedures were not ***: There were 18 RR and 11 LR 3D,6 RL and 12 LL 3D. As regards LR 3D,extracorporeal anastomosis(EA) was performed in 7 patients and intracorporeal anastomosis(IA) in 4; the RR group included 14 IA and 4 EA. There was no mortality. Median operative time was higher for the robotic group while conversion rate(12.5% vs 13%) and lymph nodes removed(14 vs 13) were similar for both. First flatus time was 1 d for RR and 2 d the other patient groups. Oral intake was resumed in 1 d by LR and in 2 d by the other patients(P = 0.012). Overall cost was €4950 and €1950 for RL and LL 3D,and €4450 and €1450 for RR and LR 3D,respectively. CONCLUSION: There were no differences betweenRR and LR 3D,except that IA was easier with RR,and probably contributed with the learning curve to the longer operative time recorded. Both techniques offer similar advantages for the patient with significantly different costs. In left colectomies robotic colectomy provided better outcomes,especially in resections approaching the rectum.

Eliminating mother-to-child transmission of HBV:progress and challenges in China

China has the world’s largest burden of hepatitis B virus(HBV)infection,but the country has made considerable progress in preventing its mother-to-child transmission(MTCT)in the past three *** feat is made possible due to the high coverage of birth-dose hepatitis B vaccine(HepB,>95%),hepatitis B surface antigen(HBsAg)screening for pregnant women(>99%),and hepatitis B immunoglobulin plus HepB for newborns whose mothers are HBsAg positive(>99%).Studies on the optimal antiviral treatment regimen for pregnant women with high HBV-DNA load have also been ***,China still faces challenges in eliminating MTCTof *** overall HBsAg prevalence among pregnant women is considered an intermediate *** prevalence of HBsAg among pregnant women from remote,rural,or ethnic minority areas is higher than that of the national level because of limited health resources and public health education for *** coverage for maternal and child healthcare and immunization services should be improved,especially in western *** of current services to prevent MTCTof HBV with other relevant health services can increase the acceptability,efficiency,and coverage of these services,particularly in remote areas and ethnic minority *** doing so,progress toward key milestones and targets to eliminate hepatitis B as the main public health threat by 2030 can be achieved.

Genetic association between the polymorphism of cytosolicPLA2 gene family and schizophrenia

Abnormal phospholipid metabolism in the brain plays an important role in neuropsychiatric *** A2(PLA2)is a crucial element for normal neuro-physiological *** study aims to investi-gate the genetic association between the polymorphism of cytosolic phospholipase A2(cPLA2)family genes and schizophrenia among Han Chinese in the northern part of *** polymerase chain reaction-based ligase detec-tion reaction(PCR-LDR)was applied to detect the genotype ten single nucleotide polymorphisms(SNPs)of cPLA2 family genes among 201 pedigrees consisting of fathers,mothers and affected offsprings with *** pedigrees were collected from 2000 to *** relative risk(HRR)test,transmission disequili-brium test(TDT),haplotype transmission analysis and multiple locus analysis were conducted to analyze the genotyping *** genotypic frequency of cPLA2 gene did not deviate from Hardy-Weinberg equilibrium either in case or control *** and TDT showed that the ten SNPs were not associated with schizophrenia(P>0.05).Analysis for haplotype transmission showed that no haplotype system was associated with schizophrenia(P>0.05).The conditioning on allele(COA)and conditioning on gene(COG)tests showed disease associations with the haplotype of rs2162886-rs1668589,rs891014-rs1668589 and rs2307279-rs7542180(χ2=6.913,P=0.032;χ2=8.393,P=0.015;χ2=8.447,P=0.038).Our data suggest that many loci in the cPLA2 family genes may be associated with schizophrenia.

Prevalence of diabetic retinopathy in Iran： a systematic review and Meta-analysis

AIM：To estimate the overall prevalence of diabetic retinopathy（DR） in Iran by a systematic review and ***：We conducted a search of all published literature on diabetic patients for the prevalence of DR using Web of Sciences, Pub Med, Scopus, Google Scholar, and national electronic databases SID, Magiran, and Iranmedex from their inception until September 2016 with standard keywords. Pooled estimates of the DR prevalence and the corresponding 95% confidence intervals（CI） were calculated using random effects ***：Thirty-one studies involving 23 729 patients with type Ⅰ and Ⅱ diabetes were included. The publication bias assumption for prevalence of DR was rejected by Begg and Egger tests（P=0.825, P=0.057, respectively）. The results of Cochran test and I^2 statistics showed considerable heterogeneity for prevalence of DR（Q=1278.21, d.f.=30, P〈0.001 and I^2=97.7%）. The prevalence of DR, non-proliferative diabetic retinopathy（NPDR） and proliferative diabetic retinopathy（PDR） in Iranian diabetic patients were 41.9%（95% CI：35.6-48.2）, 32.2%（95% CI：28.7-35.8）, and 13.2%（95% CI：8.3-18.1）, respectively. CONCLUSION：The prevalence of DR in Iran appears a little high. NPDR was more common. This study highlights the necessity for DR screening and management in diabetic patients in Iran.
