What is already known about this topic?The inclusion of meningococcal vaccines in the National Immunization Program(NIP)over several years has significantly reduced the incidence of meningococcal meningitis in China to historic lows.Worldwide,there has been a diversification of meningococcal serogroups,leading to a shift in dominant serogroups in China from serogroup A to serogroups C and B,accompanied by a rise in reports of serogroups Y and W.What is added by this report?An outbreak of serogroup Y Neisseria meningitidis(Nm)in a secondary vocational school involved a single confirmed severe case and 24 individuals with laboratory-confirmed Nm carriage.Epidemiological investigation revealed that the outbreak was localized to the classroom of the confirmed case.Prolonged close contact within a confined space was identified as a significant risk factor for Nm transmission.The genotype sequence identified was type 1655(ST-1655),which is categorized under clonal complex 23(CC-23)and bears resemblance to 8 previously confirmed cases of serogroup Y meningococcal meningitis within Guangdong province.This suggests that serogroup Y infections continue to sporadically emerge and have become prevalent strains.What are the implications for public health practice?This outbreak underscores the critical need to enhance surveillance of meningococcal serogroups and population carrier,and advocate for vaccination with MenY-containing vaccines.

Introduction:Plague is a zoonotic disease that occurs naturally in specific geographic areas.Climate change can influence the populations of the plague host or vector,leading to variations in the occurrence and epidemiology of plague in animals.Methods:In this study,we collected meteorological and plague epidemiological data from the Marmota himalayana plague focus in the Altun Mountains of the Qinghai-Xizang Plateau.The data spanned from 2000 to 2022.We describe the climatic factors and plague epidemic conditions and we describe their analysis by Pearson’s correlation.Results:During the period from 2000 to 2022,the isolation rates of Yersinia pestis(Y.pestis)from marmots and fleas were 9.27%(451/4,864)and 7.17%(118/1,646),respectively.Additionally,we observed a positive rate of F1 antibody of 11.25%(443/3,937)in marmots and 18.16%(142/782)in dogs.With regards to climate,there was little variation,and a decreasing trend in blowing-sand days was observed.The temperature in the previous year showed a negative correlation with the Y.pestis isolation rate in marmots(r=−0.555,P=0.011)and the positive rate of F1 antibody in marmots(r=−0.552,P=0.012)in the current year.The average annual precipitation in the previous two years showed a positive correlation with marmot density(r=0.514,P=0.024),while blowingsand days showed a negative correlation with marmot density(r=−0.701,P=0.001).Furthermore,the average annual precipitation in the previous three years showed a positive correlation with the isolation rate of Y.pestis from marmots(r=0.666,P=0.003),and blowing-sand days showed a negative correlation with marmot density(r=−0.597,P=0.009).Conclusions:The findings of this study indicate that there is a hysteresis effect of climate change on the prevalence of plague.Therefore,monitoring climate conditions can offer significant insights for implementing timely preventive and control measures to combat plague epidemics.

Introduction:A retrospective study based on sentinel surveillance was conducted in 10 provinciallevel administrative divisions(PLADs)in China to enhance the understanding of the epidemiological characteristics of human parainfluenza viruses(HPIVs).Methods:From January 2019 to June 2023,respiratory specimens were collected from individuals with acute respiratory infections(ARIs)and screened for four HPIVs serotypes and other common respiratory viruses using multiplex real-time polymerase chain reaction(PCR).This study analyzed the association of HPIVs infections with seasonal patterns,geographical distribution,demographic profiles,clinical features,and co-infection status.Results:During the study period,a total of 12,866 ARIs were included.The overall detection rate of HPIVs was 6.15%,varying from 5.04%in 2022 to 9.70%in 2020.The median age of HPIVs-infected patients was 3 years.HPIV2 was more prevalent among individuals aged 5–17 years(42.57%),while HPIV4 was more common in those over 65 years(12.24%).HPIV3(54.16%)and HPIV1(27.18%)were the predominant serotypes,and their prevalence exhibited significant seasonal fluctuations postcoronavirus disease 2019(COVID-19)pandemic.The peak of HPIV3 shifted three months later in 2020 compared to 2019 and returned to a summer peak thereafter.Two peaks of HPIV1 were observed in 2021 following the peak of HPIV3.Additionally,coinfections were frequent in HPIVs cases(overall rate:22.12%),with human rhinovirus being the most common co-infecting virus.Conclusions:The prevalence of HPIVs in China was predominantly due to HPIV3 and HPIV1,and their seasonal patterns were altered by pandemic restrictions.Hence,continuous surveillance of HPIVs is essential.

Background The thirteen-valent pneumococcal conjugate vaccine(PCV13)is not included in the national immu-nization program and is administered voluntarily with informed consent in China.In preparation for assessing the impact of pilot introduction in Hainan province,we conducted a carriage study among children under 5 years of age from four locations in Hainan province,China.Methods From March to June 2022,nasopharyngeal(NP)swabs,collected from healthy children aged younger than 59 months who lived in the 4 different locations(Haikou,Wanning,Baisha and Qiongzhong)in Hainan province,were tested for pneumococcus using conventional culture.Pneumococcal isolates were serotyped using the Quel-lung reaction.Risk factors associated with pneumococcal colonization were assessed using univariate analysis and multivariable logistic regression adjusting for age,daycare attendance and other factors.Results Pneumococcus was isolated in 710(30.4%)of the 2333 children enrolled.Of 737 pneumococci,29 serotypes were identifed;60.9%were PCV13 serotypes;the most common vaccine serotypes were 6B(20.4%),19F(13.0%),6A(11.9%)and 23F(6.1%);and the most common nonvaccine serotypes were 23A(12.9%),34(6.1%)and nontypeable(NT)pneumococci(5.6%).Children vaccinated with PCV13 had lower carriage(17.7%vs 32.5%;P=0.0001)and fewer PCV13 serotypes(41.9%vs 62.7%;P=0.0017)compared to unimmunized children.After adjustment,NP carriage Was higher among children attending daycare(aOR=2.3,95%CI:1.7-3.2),living in rural areas(aOR=1.4,95%CI:1.1-1.8),living with siblings(aOR=1.3,95%CI:1.0-1.6)and whose mothers had completed senior high/technical sec-ondary school(aOR=1.5,95%CI:1.1-2.0).In contrast,completion of 3-4 doses of PCV13 were associated with a lower carriage rate(aOR=0.6,95%CI:0.4-0.9).Conclusions We established the baseline of pneumococcal carriage,serotype distribution and PCV13 immuni-zation rates among healthy children under 5 years of age in Hainan province,prior to the introduction of PCV13 into the national immunization program.The high proport
