Selexipag as Add-on Therapy for Patients with Pulmonary Arterial Hypertension Associated with Congenital heart disease:A Single-center Retrospective Study

Purpose:This study examined the efficacy and safety of selexipag in treating pulmonary arterial hypertension(PAH)associated with congenital heart disease(CHD).Materials and Methods:We conducted a retrospective study of patients with CHD-associated PAH,treated with selexipag since December *** adult patients(mean age,45.4 years;women,77%)were treated with selexipag as add-on *** characteristics,World Health Organization functional class,6-minute walking distance(6MWD)test results,N-terminal pro-B-type natriuretic peptide levels,echocardiographic data,and incidence of side effects were ***:The majority of patients(12/13,92.3%)experienced more than one treatment-associated complication;one patient dropped out of the study due to intolerable *** results of 6MWD test(from 299.2±56.2 m to 363.8±86.5 m,p=0.039)and tricuspid regurgitation(TR)pressure gradient(from 84.7±20.5 mmHg to 61.6±24.0 mmHg,p=0.018)improved and remained improved after selexipag treatment in 12 *** on the results of a non-invasive risk assessment,8(66.7%)patients showed improvement,3(25.0%)showed no interval change,and the status of one patient(8.3%)***,compared to patients treated with a low dosage,patients treated with a medium-to-high dosage showed a greater increase in 6MWD results(88.3±26.4 m vs.55.3±27.6 m,p=0.043)and a greater reduction in the TR pressure gradient(-33.7±10.9 mmHg vs.-12.5±12.0 mmHg,p=0.015).Conclusion:Selexipag is an efficient pulmonary vasodilator as add-on therapy in treating CHD-associated PAH.

Association of serum complement C1q with cardiovascular outcomes among patients with acute coronary syndrome undergoing percutaneous coronary intervention

OBJECTIVE To determine the association of serum complement C1q levels with cardiovascular outcomes among patients with acute coronary syndrome(ACS)undergoing percutaneous coronary intervention(PCI),and evaluate the value of C1q modified by high-sensitivity C-reactive protein(hs-CRP)levels as an independent *** As a single-center prospective observational study,we analyzed 1701 patients who had received primary or elective PCI for ACS at Beijing Anzhen Hospital,Capital Medical University,Beijing,China between June 1,2016 and November 30,*** associations of C1q modified by hs-CRP with major adverse cardiovascular events(MACE)were determined in survival *** Patients with the lowest C1q tertile had the highest cumulative risk of MACE(log-rank P=0.007).In fully adjusted Cox regression models,stratifying the total population according to hs-CRP dichotomy,C1q was significantly associated with MACE in patients with hs-CRP levels less than 2 mg/L but not in those with 2 mg/L or more(P_(interaction)=0.02).In patients with hs-CRP levels less than 2 mg/L,with the lowest C1q tertile as reference,the risk of MACE was reduced by 40.0%in the middle C1q tertile[hazard ratio(HR)=0.600,95%CI:0.423–0.852,P=0.004]and by 43.9%in the highest C1q tertile(HR=0.561,95%CI:0.375–0.840,P=0.005).CONCLUSIONS Serum complement C1q is significantly associated with cardiovascular outcomes in patients with ACS undergoing PCI,only when hs-CRP levels are less than 2 mg/*** finding implicates the usefulness of C1q for the risk stratification in ACS patients with reduced systemic inflammation.

Cardiovascular diseases(CVD)are the leading cause of death in the world and one of the most common diseases in the elderly,with high prevalence and poor prognosis,which seriously affect the health of the elder *** report showed that global deaths from CVD increased from 12.4 million in 1990 to 19.8 million in 2022,reflecting the global population growth and aging,as well as the impact of metabolic,environmental,and behavioral risks.[1]With the progress of aging population,the burden of CVD in the elderly in China has increased year by year,and factors including multiple complications,organ function decline,and high complication rate have made the interventional treatment of CVD in the elderly one of the difficulties and focuses in the field of cardiology in China.

Effects of chronic obstructive pulmonary disease on longterm prognosis of patients with coronary heart disease postpercutaneous coronary intervention

BACKGROUND Chronic obstructive pulmonary disease(COPD)and cardiovascular diseases are often comorbid conditions,their co-occurrence yields worse outcomes than either condition *** study aimed to investigate COPD impacts on the five-year prognosis of patients with coronary heart disease(CHD)after percutaneous coronary intervention(PCI).METHODS Patients with CHD who underwent PCI in 2013 were recruited,and divided into COPD group and non-COPD *** events occurring among those groups were recorded during the five-year follow-up period after PCI,including all-cause death and cardiogenic death,myocardial infarction,repeated revascularization,as well as stroke and bleeding *** adverse cardiac and cerebral events were a composite of all-cause death,myocardial infarction,repeated revascularization and *** A total of 9843 patients were consecutively enrolled,of which 229 patients(2.3%)had *** to non-COPD patients,COPD patients were older,along with poorer estimated glomerular filtration rate and lower left ventricular ejection ***-year follow-up results showed that incidences of all-cause death and cardiogenic death,as well as major adverse cardiac and cerebral events,for the COPD group were significantly higher than for non-COPD group(10.5%vs.3.9%,7.4%vs.2.3%,and 30.1%vs.22.6%,respectively).COPD was found under multivariate Cox regression analysis,adjusted for confounding factors,to be an independent predictor of all-cause death[odds ratio(OR)=1.76,95%CI:1.15-2.70,P=0.009]and cardiogenic death(OR=2.02,95%CI:1.21-3.39,P=0.007).CONCLUSIONS COPD is an independent predictive factor for clinical mortality,in which CHD patients with COPD are associated with worse prognosis than CHD patients with non-COPD.

BACKGROUND Several studies have proved the safety and feasibility of robot-assisted percutaneous coronary intervention(PCI)in reducing the occupational hazards of interventionists while achieving precision ***,an independently developed robot-assisted system for PCI in China has not yet *** study aimed to evaluate the safety and feasibility of a robot-assisted system for elective PCI in *** This preclinical trial included 22 experimental pigs and preliminarily supported the safety and feasibility of the ETcath200 robot-assisted system for ***,eleven patients with coronary heart disease who met the inclusion criteria and had clinical indications for elective PCI were *** was performed using a robot-assisted *** primary outcomes were clinical success(defined as visual estimated residual stenosis<30%after PCI and no major adverse cardiovascular events during hospitalization and within 30 days after PCI)and technical success(defined as the ability to use the robot-assisted system to complete PCI successfully without conversion to the traditional manual PCI).RESULTS Eleven patients were included in this clinical trial.A drug-eluting stent with a diameter of 3 mm(interquartile range:2.75–3.5 mm)and a length of 26 mm(interquartile range:22–28 mm)was deployed in all *** clinical success rate was 100%,with no PCI-related complications and no in-hospital or 30-day major adverse cardiovascular events,and the technical success rate was 100%.CONCLUSIONS The results strongly suggest that the use of the independently developed robot-assisted system in China for elective PCI is feasible,safe,and effective.
