Triglyceride-glucose index predicts the prognosis of patients with acute myocardial infarction

Background Research has indicated that the triglyceride-glucose(Ty G)index is associated with the development and prognosis of cardiovascular ***,there is still uncertainty regarding whether the Ty G index can predict adverse cardiovascular events in patients with acute myocardial infarction(AMI).The present study aimed to explore the prognostic value of the Ty G index in patients with *** AMI patients who underwent emergency percutaneous coronary intervention(PCI)in Guangdong General hospital from February 2021 to June 2022 were *** study population was grouped based on the median Ty G *** end point of the study was major adverse cardiovascular events(MACEs),defined as all-cause mortality,cardiogenic shock,recurrent myocardial infarction,acute attack of heart failure,repeat revascularization induced by cardiac ischemia,and urgent visit due to aggravation of chest *** Ty G index was calculated as follows:LN[fasting triglycerides(mg/d L)×fasting blood glucose(mg/d L)x1/2].Results A total of 196 patients were *** the multivariate logistic regression analysis,Ty G index[odds ratio(OR):0.99,95%confidence interval(CI):0.45-2.16,P=0.98]was not significantly correlated with the occurrence of MACEs in AMI patients during *** a total of 178 patients were followed up for an average of 12.66±5.35 *** the multivariate Cox proportional hazards model,Ty G index[hazard ratio(HR):1.91,95%CI:1.02-3.6,P<0.05]were independent predictor of adverse events during follow-up,and the area under the receiver operating characteristic(ROC)curve(AUC)was0.63(0.53-0.73,P<0.05).Conclusions Ty G index can independently predict the prognosis of AMI patients during the follow-up period.[S Chin J Cardiol 2024;25(1):11-20]

High blood pressure is a major public health issue and a leading risk factor for death worldwide,with the number of attributable deaths increased from 6.8 million in 1990 to 10.8 million in 2019.1,2 According to the World Health Organization,the age-standardized prevalence of hypertension was estimated to be 33%,affecting approximately 1.3 billion adults aged 30–79 years in 2019 worldwide,doubling from 1990.3 In addition to the heavy disease burden,the economic burden associated with hypertension is also substantial,accounting for approximately 10%of the global healthcare expenditure.4 Furthermore,despite improvements in diagnostic and treatment capacities,the status of hypertension care remains suboptimal,especially in low-and middle-income *** instance,in China,in 2019,the awareness,treatment,and control rates of hypertension were 38.3%,34.6%,and 12.0%,respectively.5 Accumulating evidence calls for more comprehensive and effective prevention and management of hypertension.

Potential preservative mechanisms of cardiac rehabilitation pathways on endothelial function in coronary heart disease

Cardiac rehabilitation, a comprehensive exercise-based lifestyle and medical management, is effective in decreasing morbidity and improving life quality in patients with coronary heart disease. Endothelial function, an irreplaceable indicator in coronary heart disease progression, is measured by various methods in traditional cardiac rehabilitation pathways, including medicinal treatment, aerobic training, and smoking cessation. Nevertheless, studies on the effect of some emerging cardiac rehabilitation programs on endothelial function are limited. This article briefly reviewed the endothelium-beneficial effects of different cardiac rehabilitation pathways, including exercise training, lifestyle modification and psychological intervention in patients with coronary heart disease, and related experimental models, and summarized both uncovered and potential cellular and molecular mechanisms of the beneficial roles of various cardiac rehabilitation pathways on endothelial function. In exercise training and some lifestyle interventions, the enhanced bioavailability of nitric oxide, increased circulating endothelial progenitor cells(EPCs), and decreased oxidative stress are major contributors to preventing endothelial dysfunction in coronary heart disease. Moreover, the preservation of endothelial-dependent hyperpolarizing factors and inflammatory suppression play roles. On the one hand, to develop more endothelium-protective rehabilitation methods in coronary heart disease, adequately designed and sized randomized multicenter clinical trials should be advanced using standardized cardiac rehabilitation programs and existing assessment methods. On the other hand, additional studies using suitable experimental models are warranted to elucidate the relationship between some new interventions and endothelial protection in both macro-and microvasculature.

Mediating Role of Glucose-Lipid Metabolism in the Association between the Increased Risk of Coronary Heart Disease and Exposure to Organophosphate Esters,Phthalates,and Polycyclic Aromatic Hydrocarbons

There is a lack of human evidence concerning the cardiovascular effects of combined exposure to endocrine *** case-control study sought to investigate coronary heart disease(CHD)associations with exposure to organophosphate flame retardants(OFRs),phthalates(PAEs),and polycyclic aromatic hydrocarbons(PAHs)among 148 adults with coronary-angiography-diagnosed CHD and 320 healthy adults from southern *** mediating role of glucose-lipid metabolism was also *** kernel machine regression suggested that when exposure status was fixed to the 75th percentile with the median value as the reference,exposure to OFRs,PAEs,and PAHs was associated with an 84%(95%CI:36%−134%),132%(12%−252%),and 214%(89%−331%)increased risk of developing CHD,*** quantile sum regression indicated urinary bis(2-butoxyethyl)phosphate(BBOEP),dibutyl phosphate(DBP),monoisononyl phthalate(miNP),and metabolites of phenanthrene may be major contributors to the overall effect of *** further analyses on identified chemical risk factors,mediation analyses suggested exposure to phenanthrene may increase the risk of CHD via elevating total cholesterol and blood glucose,while exposure to DiNP mainly associates with serum ***,we observed a slight mediation effect of oxidative DNA damage between urinary BBOEP and risk of *** results provide potential direction for further experimental *** evidence is needed to clarify the causation of the results.
