Seroprevalence and Risk Factors of Syphilis Infection among Antiretroviral Therapy Naive HIV Patients at the cape coast teaching hospital, Ghana

Diagnosis of sexually transmitted infections is very important considering the spread of HIV and the extensive use of highly active antiretroviral therapy worldwide. This will assist in planning of treatment schedule in controlling these infections. The study therefore aimed at determining the prevalence of syphilis in HIV positive antiretroviral therapy naive patients in cape coast and the associated risk factors involved in infection. A cross-sectional study was carried out using initial HIV rapid and confirmation tests, and then Venereal Disease Research Laboratory test with the Ultra Rapid Test Kits for syphilis. Demographic data, risky sexual behaviours capable of co-transmission of both HIV and Syphilis, were also collected through the use of questionnaires. In all, 150 HIV positive antiretroviral naive subjects were studied and 15 (10%) were positive for VDRL test, with females (6.00%) and males (4.00%), who were mainly within the age group of 20 - 39 years. A significant number of males (p = 0.019) and females (p = 0.015) participants were not smoking with a fewer number of the females (p = 0.002) having multiple sexual partners. Also a smaller number of those who were infected with the bacteria (p = 0.004) did not support the control of sexually transmitted infection (p = 0.022). The result showed that co-infection of Syphilis in HIV positive antiretroviral therapy naive patients persists in the cape coast Metropolis, which is an indication of prominence of STIs that require further study on a larger scale to ascertain the extent of co-infection and to formulate policy for treatment to help minimize the rate of infection.

Scatter Radiation Dose Assessment in the Radiology Department of cape coast teaching hospital-Ghana

During the operation of an X-ray machine, if the radiation protection of X-ray room is insufficient, not only the patient but also clinical staffs as well as public are exposed to high X-ray exposures. In this research work, leakage and scattered radiations were measured from X-ray machine in the radiology department of cape coast teaching hospital in the cape coast Municipality of Ghana. The scattered radiation exposures of X-rays in some selected areas within the facility were measured. The X-ray machine was operated for a range of kilo voltage peaks of kVp 77, 70, 63 and 66 with intensities 20, 6.3 and 8.0 mAs, which represents the main technique factors of some body parts such as chest, lumbar spine and extremities during radiographic examinations. The measurements were performed using a RADOS-120 Universal survey meter. The radiation doses rates measured in the various locations in the Radiology Department of cape coast teaching hospital of Ghana were in the range of 0.10 μSv/hr to 0.12 μSv/hr. These values were all within the background measurement of 0.10 μSv/hr. Moreover, there were no risks of high radiation doses to patients, staffs and people visiting the X-ray department. The results obtained indicated that within the radiology department of the hospital, all the selected locations were very safe to patients, occupational workers and the general public which could be attributed to adequate shielding in the facility.

Achieving effective municipal solid waste （MSW） management remains a major challenge and waste generation and accumulation continue to constitute important environmental and public health concern, particularly in most developing countries. Although the general population is at risk of adverse health consequences and hazards associated with exposure to MSW, the waste management workers （WMWs） are the most vulnerable because of their direct involvement in the disposal of waste, with increasing evidence of work-related health and safety risks among these individuals. Among the numerous work-related health hazards prevalent in WMWs, development of toxic neuropathies following chronic occupational exposure remains poorly recognized. However, the risk or predisposition to toxic neuropathies is becoming evident considering the increasing recognition of large amount of neurotoxic heavy metals and hazardous industrial materials present in MSW in mostparts of the world. The present review seeks to draw attention to the continuous vulnerability of the WMWs to developing toxic neuropathies. This is aimed at facilitating conscious efforts by relevant governmental and non- governmental agencies towards promoting risk reduction and ensuring adequate protection against possible toxic polyneuropathies associated with occupational exposure to solid wastes. While continuous education of the WMWs on the need for adequate compliance to safety regulations and practice remains sacrosanct towards achieving significant reduction in toxic neuropathies and related adverse health consequences of waste handling, it is also our intention in this review to underscore the possible relevance of nutrient supplementation and alternative medicines in chemoprevention.

Penile Fracture—Report on Three Cases from cape coast, Ghana

Introduction: Penile fracture is a urological emergency that occurs when the penis in an erect state suffers a blunt trauma resulting in a rupture of the tunica albuginea of either one or both corpora cavernosa. It is often caused by vigorous sexual intercourse, hence the incidence of penile fracture is under-reported. We therefore present our experience of the clinical presentation and surgical management of penile fracture. Presentation of Cases: We report three cases of penile fracture and all were diagnosed based on their clinical presentation and examination findings. The patients include two middle aged men and one young man, all with history of hearing a popping sound and experiencing sudden onset pain with detumescence of the penis. No radiological investigations were carried out. An emergency surgical repair was done for all patients. Discussion: All the patients had no urethral injury on presentation and underwent immediate surgical repair. The erectile and voiding function of each patient was preserved. Conclusion: The management of penile fracture involves early diagnosis and immediate surgical repair. Early intervention is necessary to preserve penile function.

Incidence of Human Immunodeficiency Virus in Sickle Cell Patients in the cape coast Metropolis, Ghana

Context: Human immunodeficiency virus (HIV) is a well-known pathogen that causes acquired immunodeficiency syndrome (AIDS). The course of HIV infection and progression may be influenced by sickle cell traits. Objectives: The aim of this study was to determine the incidence of HIV among sickle cell patients in the cape coast metropolis. Methods: A cross sectional study was conducted among patients who visited the Central Regional hospital, cape coast. About 2 ml of blood sample was drawn from each participant for sickle cell test using sodium metabisulphite solution. Sickle cell positive samples were selected and tested for the presence of HIV using Alere DetermineTM?HIV-1/2. Results: A total of 75 patients made up of 25 males and 50 females were enrolled. Their age ranges between 4 to 59 years (27, 1 - 15 years;24, 16 - 30 years;17, 31 - 45 years;7, 46 - 60 years). Fifteen (15) (20%) had received at least a single blood transfusion while the remaining 60 (80%) had never received blood transfusion before the study. Thirty-six patients (48.0%) were married and the other 39 patients (52.0%) were single. Five patients (6.7%) had antibodies to HIV whereas 70 patients (93.3%) were not with the virus. Conclusion: The incidence rate of HIV among sickle cell trait children and the youths of cape coast metropolis were found to be 6.7%. This may suggest that sickle cell traits in children and the youth may be associated with HIV transmission.

Effect of pre-existing Schistosoma haematobium infection on Plasmodium berghei multiplications in imprinting control region mice

Objective: To investigate the effect of pre-existing Schistosoma haematobium(S. haematobium) infection on malaria disease ***: The study involved the use of twenty-i ve imprinting control region mice, i fteen of which were initially infected with S. haematobium. Five of the remaining ten schistouninfected mice together with i ve schisto-infected mice were infected with Plasmodium berghei(P. berghei) after four weeks(acute stage) of schistosoma infection. The remaining i ve schisto-uninfected mice together with i ve schisto-infected mice were also infected with P. berghei after seven weeks(chronic stage) of schistosoma infection. The last i ve schistoinfected mice were used as control group. They were then monitored for changes in P. berghei parasitaemia on Days 3, 5, 7, 9 and 11 post-infection. Records on their survivability were also ***: The co-infected mice had signii cantly higher malaria parasitaemia, compared with the mono-infected mice during acute S. haematobium infection. In contrast, the co-infected mice had signii cantly lower malaria parasitaemia during chronic S. haematobium infection and a higher survival ***: Co-infection of mice with P. berghei during acute S. haematobium infection resulted in rapid P. berghei development and increased malaria parasitaemia. However, the co-infection resulted in slower P. berghei development and decreased malaria parasitaemia with enhanced survivability of the mice during chronic S. haematobium infection. Therefore, pre-existing chronic S. haematobium infection may provide some protection to the host by reducing parasitaemia.

Anti-inflammatory effect of Heliotropium indicum Linn on lipopolysaccharide-induced uveitis in New Zealand white rabbits

AIM： To investigate the anti-inflammatory effect of an aqueous whole plant extract of Heliotropium indicum（HIE） on endotoxin-induced uveitis in New Zealand white rabbits.·METHODS： Clinical signs of uveitis including flares,iris hyperemia and miosis, were sought for and scored in1.0 mg/kg lipopolysaccharide（LPS）-induced uveitic rabbits treated orally with HIE（30-300 mg/kg）,prednisolone（30 mg/kg）, or normal saline（10 m L/kg）. The number of polymorphonuclear neutrophils infiltrating, the protein concentration, as well as levels of tumor necrosis factor-α（TNF-α）, prostaglandin E2（PGE2）, and monocyte chemmoattrant protein-1（MCP-1） in the aqueous humor after the various treatments were also determined. A histopathological study of the anterior uveal was performed.· RESULTS： The extract and prednisolone-treatment significantly reduced（P ≤0.001） both the clinical scores of inflammation（1.0-1.8 compared to 4.40 ±0.40 in the normal saline-treated rabbits） and inflammatory cells infiltration. The level of protein, and the concentrationsof TNF-α, PGE2 and MCP-1 in the aqueous humor were also significantly reduced（P ≤0.001）. Histopathological studies showed normal uveal morphology in the HIE and prednisolone-treated rabbits while normal saline-treated rabbits showed marked infiltration of inflammatory cells.· CONCLUSION： The HIE exhibits anti-inflammatory effect on LPS-induced uveitis possibly by reducing the production of pro-inflammatory mediators.
