Introduction: Insomnia is a common but often underdiagnosed and undertreated condition among cancer patients, significantly impacting their quality of life (QoL). Oncologists play a crucial role in managing insomnia, yet their knowledge and practices in this area remain unclear. This study aims to assess the current practices of oncologists in Singapore regarding insomnia management and to identify areas where additional education may be needed. Methods: We conducted a cross-sectional survey of practicing medical and radiation oncologists in Singapore. The survey, based on the 2023 ESMO Clinical Practice Guideline for managing insomnia in adult cancer patients, included 12 multiple-choice, checkbox, and open-ended questions. Data were analyzed descriptively to identify trends in screening, assessment, and management of insomnia among oncologists. Results: A total of 25 oncologists responded to the survey. The majority (84%) do not routinely screen for sleep disturbances, and only a small fraction uses validated tools to assess insomnia severity. Time constraints and a lack of knowledge were identified as the primary barriers to comprehensive sleep assessments. Pharmacological treatments, particularly melatonin and Z-class drugs, were commonly initiated, with 32% of oncologists using them as first-line therapy. Non-pharmacological strategies, such as cognitive behavioral therapy for insomnia, were underutilized. Additionally, many oncologists expressed uncertainty about when to refer patients to sleep specialists, indicating a gap in their training and confidence in managing insomnia. Conclusion: This study reveals significant gaps in the routine screening and comprehensive management of insomnia among oncologists in Singapore. Despite the high prevalence of sleep disturbances in cancer patients, oncologists often prioritize other symptoms and lack the necessary tools and knowledge to address insomnia effectively. Background:?According to the GLOBOCAN 2018 report, the estimated incidence of lung cancer in India was 67,795 in both sexes. The treatment of advanced Non-small cell lung cancer (NSCLC) saw a major paradigm shift with recent advances in molecular-targeted therapy. Immune checkpoint blockade therapy is one such novel strategy with promising clinical benefits in advanced NSCLC. Programmed cell death receptor-1/Programmed cell death ligand-1 (PD-1/PD-L1) pathway is one such checkpoint and has thus become the current area of interest in the treatment of lung carcinoma. The PD-1/?PD-L1 pathway is under active investigation as it represents a promising therapeutic target in NSCLC. The expression of PD-L1 in tumor cells has been suggested as a predictive marker of the clinical response to PD-1/?PD-L1-targeted therapy. Methods: This study was carried out at the Department of Pathology, Health care Global Specialty Hospital, Bangalore from May 2018 to May 2019. In this study, we analyzed pattern of PD-L1 expression by immunohistochemistry testing using our own Laboratory Developed Test (LDT) in NSCLC patients. Results:?Our study group comprised 50 patients of NSCLC. Among our study population, 40% of the patients exhibited PD-L1 immunopositivity (≥1%) with 28% of them having any expression (TPS 1% - 49%) and 12% of them having a high expression (TPS ≥ 50%) of PD-L1. Majority of them exhibited adenocarcinoma type of NSCLC under which the solid subtype showed a direct correlation with PD-L1 positivity (p-value: 0.004) with a poorly differentiated tumor histology being common in our population in relation to PD-L1 positivity (p-value: 0.043). PD-L1 expression did not correlate with age, gender, smoking status or clinical stage in our study. No association was found between tumor histology (SCC or AC) and driver mutation status with expression of PD-L1 in the present study. The number of long-term colorectal cancer (CRC) survivors has increased substantially over the past three decades due to both ongoing advances in early detection and improvements in cancer therapies. Adult survivors of CRC experience chronic health conditions due to normal issues associated with aging, which is further compounded by the long-term adverse effects of having had cancer and anti-cancer therapies. In addition, they are at a higher risk for CRC recurrences, new primary cancers, and other co-morbidities. This article will provide an overview of the clinical care of adult survivors of CRC. Epidemiologic data will be presented followed by a discussion of the approach to the care of long-term adult survivors of CRC, including surveillance of recurrences and new primary cancers, interventions to manage both physical and psychological consequences of cancer and its treatments, and strategies to address concerns related to unemployment and disability. Finally, we will explore the challenges of healthcare delivery, especially with respect to the coordination of follow-up between cancer specialists and primary care physicians, so as to ensure that all of the survivor’s health needs are met promptly and appropriately.

Pain is one of the most common and distressing symptoms experienced by cancer patients and can cause significant physical and psychological complications for patients and their families. Adequate pain management requires an understanding of pain, its assessment, and the use of analgesics according to the WHO analgesic ladder;information that is often lacking in routine medical education. Understanding the level of knowledge, attitudes and practices of clinicians who treat cancer pain can help us prepare relevant educational programs to address this need. A cross-sectional descriptive study was conducted among randomly selected eligible clinical staff using self-administered questionnaires that contained close-ended and open-ended questions testing factual knowledge on pain, attitudes towards pain management, and practices related to pain management and opioid use. Results were analyzed using SPSS Version 28. Of 66 participants of this study, 78% were female and 65% were nurses. Only 10% had received any additional training in pain management. Knowledge on pain was found to be adequate in some areas, but deficient in others such as “types of pain” and “adjuvant drugs” (35% and 26% respectively). A significant proportion of responses (73%) showed incorrect attitudes towards pain and opioid use. Recommended practices such as prescribing laxatives with opioids were prevalent (72%), however using a tool to assess pain or the WHO ladder to prescribe analgesics was not as common. The results of this study show that despite being a key component of clinical care, pain management is not adequately understood by many clinicians. Gaps in knowledge, and incorrect attitudes could contribute to inadequate pain management for patients. This study provides valuable information for an educational workshop planned on pain management.

Background: Lung cancer is the leading cause of cancer death in Australia, with only modest improvements in survival. This study aims to identify factors impacting on diagnosis and management of lung cancer with particular reference to Australian primary care. Methods: A sequential mixed method modified approach employing interview and a two-phased survey technique. Following telephonic interviews with 31 health professionals (individuals representing general practitioners, specialized physicians, nurses and allied health practitioners), interview data was analysed using qualitative thematic analysis, and surveys using descriptive statistics. Emergent themes were organised under patient, provider and system factors. Interviews ceased upon saturation of data. Results: Multiple patient, provider and systems issues were seen to contribute to adverse health outcomes. There is a strong relationship between smoking and outcomes, and factors related to higher smoking rates such as a lower socioeconomic status. For smokers, guilt and/or denial was considered a reason for delay in the decision to seek medical care for cough or shortness of breath. Aboriginal people under-report morbidity related to smoking and chronic obstructive pulmonary disease;other patients fail to recognise the significance of their symptoms. Discussion: Despite the poor prognosis of lung cancer diagnosis, increased awareness of presentation and treatment options can address disparities in health outcomes.

Approximately 1 in 8 women will develop breast cancer during their lifetime and the risk factors include age, family history, and reproductive factors. In women with a family history of breast cancer, there is a proportion in which a gene mutation can be the cause of the predisposition for breast cancer. A careful assessment of family and clinical history should be performed in these women in order to determine if a genetic counseling referral is indicated. In cases of hereditary breast cancer, genetic testing with a multigene panel can identify specific genetic mutations in over 100 genes. The most common genes mutated in hereditary breast cancer are the high-penetrance BRCA1 and BRCA2 genes. In addition, other mutations in high-penetrance genes in familial cancer syndromes and mutations in DNA repair genes can cause hereditary breast cancer. Mutations in low-penetrance genes and variants of uncertain signifcance may play a role in breast cancer development, but the magnitude and scope of risk in these cases remain unclear, thus the clinical utility of testing for these mutations is uncertain. In women with high-penetrance genetic mutations or lifetime risk of breast cancer 〉 20%, risk-reducing interventions, such as intensive screening, surgery, and chemoprevention, can decrease the incidence and mortality of breast cancer.
