Differential diagnosis of covid-19 at the chest computed tomography scan:A review with special focus on cancer patients

BACKGROUND Given the several radiological features shared by coronavirus disease 2019 pneumonia and other infective or non-infective diseases with lung involvement,the differential diagnosis is often tricky,and no unequivocal tool exists to help the radiologist in the proper *** tomography is considered the gold standard in detecting pulmonary illness caused by severe acute respiratory syndrome coronavirus *** To conduct a systematic review including the available studies evaluating computed tomography similarities and discrepancies between coronavirus disease 2019 pneumonia and other pulmonary illness,then providing a discussion focus on cancer *** Using pertinent keywords,we performed a systematic review using PubMed to select relevant studies published until October 30,*** Of the identified 133 studies,18 were eligible and included in this *** Ground-glass opacity and consolidations are the most common computed tomography lesions in coronavirus disease 2019 pneumonia and other respiratory *** two studies included cancer patients,and the differential diagnosis with early lung cancer and radiation pneumonitis was performed.A single lesion associated with pleural effusion and lymphadenopathies in lung cancer and the onset of the lesions in the radiation field in the case of radiation pneumonitis allowed the differential ***,the studies were heterogeneous,and the type and prevalence of lesions,distributions,morphology,evolution,and additional signs,together with epidemiological,clinical,and laboratory findings,are crucial to help in the differential diagnosis.

Media power,the impact that media have on public opinion and perspectives,plays a significant role in maintaining internal stability,exerting external influence,and shaping international dynamics for nations/***,prior research has primarily concentrated on news content and reporting time,resulting in limitations in evaluating media *** more accurately assess media power,we use news content,news reporting time,and news emotion simultaneously to explore the emotional contagion between *** use emotional contagion to measure the mutual influence between media and regard the media with greater impact as having stronger media *** propose a framework called Measuring Media Power via Emotional Contagion(MMPEC)to capture emotional contagion among media,enabling a more accurate assessment of media power at the media and national/regional *** also interprets experimental results through correlation and causality analyses,ensuring *** analyses confirm the higher accuracy of MMPEC compared to other baseline models,as demonstrated in the context of covid-19-related news,yielding compelling and interesting insights.

Time series analysis is a valuable tool in epidemiology that complements the classical epidemiological models in two different ways:Prediction and *** is related to explaining past and current data based on various internal and external influences that may or may not have a causative *** is an exploration of the possible future values based on the predictive ability of the model and hypothesized future values of the external and/or internal *** time series analysis approach has the advantage of being easier to use(in the cases of more straightforward and linear models such as Auto-Regressive Integrated Moving Average).Still,it is limited in forecasting time,unlike the classical models such as *** applicability in forecasting comes from its better accuracy for short-term *** its basic form,it does not assume much theoretical knowledge of the mechanisms of spreading and mutating pathogens or the reaction of people and regulatory structures(governments,companies,etc.).Instead,it estimates from the data *** predictive ability allows testing hypotheses for different factors that positively or negatively contribute to the pandemic spread;be it school closures,emerging variants,*** can be used in mortality or hospital risk estimation from new cases,seroprevalence studies,assessing properties of emerging variants,and estimating excess mortality and its relationship with a pandemic.

Ecological studies suggested a link between air pollution and severe covid-19 outcomes,while studies accounting for individual-level characteristics are *** the present study,we aimed to investigate the impact of short-term ambient air pollution exposure on disease severity among a cohort of 569 laboratory confirmed covid-19 patients admitted to designated hospitals in Zhejiang province,China,from January 17 to March 3,2020,and elucidate the possible biological processes involved using *** with mild cases,severe cases had higher proportion of medical conditions as well as unfavorable results in most of the laboratory tests,and manifested higher air pollution exposure *** exposure to air pollutants was associated with increased risk of severe covid-19 with odds ratio(OR)of 1.89(95%confidence interval(CI):1.01,3.53),2.35(95%CI:1.20,4.61),2.87(95%CI:1.68,4.91),and 2.01(95%CI:1.10,3.69)for PM_(2.5),PM_(10),NO_(2)and CO,*** for NO_(2)remained significant in two-pollutant models after adjusting for other *** analysis showed 884 differentially expressed genes which mainly were enriched in virus clearance related biological processes between patients with high and low NO_(2)exposure levels,indicating that compromised immune response might be a potential underlying mechanistic *** findings highlight the impact of shortterm air pollution exposure,particularly for NO_(2),on covid-19 severity,and emphasize the significance in mitigating the covid-19 burden of commitments to improve air quality.

Challenges in hyperglycemia management in critically ill patients with covid-19

Hyperglycemia is commonly associated with adverse outcomes especially in patients requiring intensive care unit *** from the corona virus disease 2019(covid-19)pandemic indicates that individuals with diabetes appear to be at similar risk for covid-19 infection to those without diabetes but are more likely to experience increased morbidity and *** proposed hypothesis for hyperglycemia in covid-19 include insulin resistance,critical illness hyperglycemia(stress-induced hyperglycemia)secondary to high levels of hormones like cortisol and catecholamines that counteract insulin action,acute cytokine storm and pancreatic cell *** patients are more likely to have severe hyperglycemic complications including diabetic ketoacidosis and hyperosmolar hyperglycemic *** of hyperglycemia in covid-19 is often complicated by use of steroids,prolonged total parenteral or enteral nutrition,frequent acute hyperglycemic events,and restrictions with fluid management due to acute respiratory distress *** managing hyperglycemia special attention should be paid to mode of insulin delivery,frequency of glucose monitoring based on patient and caregiver safety thereby minimizing exposure and conserving personal protective *** this article we describe the pathophysiology of hyperglycemia,challenges encountered in managing hyperglycemia,and review some potential solutions to address them.
