Introduction: covid-19 is an infectious disease that has been causing a global pandemic since 2019. Although clinical forms are generally less severe in children than in adults, children nevertheless present polymorphous clinical forms and severe cases that can lead to death. Objective: To describe the clinical presentations found in the different waves of covid-19, and to highlight the different factors of severity. Materials and Methods: We conducted a cross-sectional study with retrospective and prospective data collection which lasted 7 months (from November 2021 to June 2022) and covered a study period from 6 March 2020 to 22 June 2022, i.e. 27 months. All patients aged 0 to 18 years, suspected of having covid-19, confirmed by real-time RT-PCR or an antigenic Rapid diagnostic Test or antibody were included. These patients were to be managed in the Mother and Child Centre of the Chantal Biya Foundation, as well as in the Specialised Centre for the Management of covid patients, annex number II of the Yaoundé Central Hospital. The results were analysed using IBM SPSS.23 software. Results: We included 163 patients in our study. No paediatric patients were registered during the 3rd wave. The mean age of the patients in the study population was 13 ± 5 years with extremes from 15 days to 18 years. We had a female predominance with a sex ratio of 0.83. The most common comorbidity was asthma. The first wave presented mainly with respiratory symptoms such as dry cough and signs of respiratory distress. The second wave presented mainly with digestive symptoms such as diarrhoea, abdominal pain and vomiting. The fourth wave presented with ENT signs such as sore throat, and rhinorrhea. Factors associated with severity were mainly age less than five years (OR = 17.69), vomiting (OR = 6.50), presence of comorbidities (OR = 3.39), and alteration of vital parameters such as bradypnoea (OR = 19.68), bradycardia (OR = 6.34), tachycardia (OR = 3.73), oxygen saturation Co

The coronavirus disease 2019（covid-19） outbreak caused by the severe acute respiratory syndrome coronavirus-2（SARS-CoV-2） developed into a global health emergency. Systemic microthrombus caused by SARS-CoV-2 infection is a common complication in patients with covid-19. Cardiac microthrombosis as a complication of SARS-CoV-2 infection is the primary cause of cardiac injury and death in patietns with severe covid-19. In this study, we performed single-cell sequencing analysis of the right ventricular free wall tissue from healthy donors,patients who died during the hypercoagulable period of characteristic coagulation abnormality（CAC）, and patients who died during the fibrinolytic period of CAC. We collected 61,187 cells enriched in 24 immune cell subsets and 13 cardiac-resident cell subsets. We found that in the course of SARS-CoV-2 infected heart microthrombus, MYO1EhighRASGEF1Bhighmonocyte-derived macrophages promoted hyperactivation of the immune system and initiated the extrinsic coagulation pathway by activating chemokines CCL3, CCL5. This series of events is the main cause of cardiac microthrombi following SARS-CoV-2 infection. In a SARS-CoV-2 infected heart microthrombus, excessive immune activation is accompanied by an increase in cellular iron content, which in turn promotes oxidative stress and intensifies intercellular competition. This induces cells to alter their metabolic environment, resulting in increased sugar uptake via the glycosaminoglycan synthesis pathway. In addition, high levels of reactive oxygen species generated by elevated iron levels promote increased endogenous malondialdehyde synthesis in a subpopulation of cardiac endothelial cells. This exacerbates endothelial cell dysfunction and exacerbates the coagulopathy process.

BACKGROUND The clinical efficacy and safety of vaccination against novel coronavirus disease 2019(covid-19)in patients with cirrhosis have not been evaluated *** To evaluate the clinical efficacy and safety of vaccination against covid-19 in patients with *** This was a retrospective cohort study of patients with *** first cohort included patients vaccinated with Gam-covid-Vac(Sputnik V);the second one consisted of unvaccinated *** The study included 89 vaccinated patients and 148 unvaccinated *** were 4 cases of covid-19 in the vaccinated group and 24 cases in the unvaccinated group(P=0.035).No severe cases of covid-19 were revealed in the vaccinated group,while there were 12 ones in the unvaccinated group(P=0.012)with 10 deaths detected(P=0.012).The vaccine efficacy was 69.5%(95%confidence interval[CI]:18.5%-94.4%)against symptomatic cases of covid-19,100%(95%CI:25.1%-100.0%)against severe cases,and 100%(95%CI:1.6%-100.0%)against death associated with *** efficacy of full vaccination with revaccination against symptomatic cases of covid-19 was 88.3%(95%CI:48.0%-99.6%).The overall mortality rate was higher in the unvaccinated group than in the vaccinated group(17.1%vs 3.0%;P=0.001).Higher Child-Turcotte-Pugh class cirrhosis(hazard ratio[HR]=4.13,95%CI:1.82-9.35)and higher age(HR=1.08,95%CI:1.04-1.15)were independent predictors of overall mortality,while vaccination had a protective effect(HR=0.09,95%CI:0.01-0.76).There was no significant difference in liver-related mortality(P=0.135)or the incidence of liver decompensation(P=0.077),bleeding esophageal varices(P=0.397),and vascular events(P=0.651)between the two groups of *** Vaccination against covid-19 in patients with cirrhosis is effective and safe.
