Background and Purpose -Endovascular coil embolization has become an accepted alternative for the treatment of intracranial aneurysms. The purpose of this st udy was to evaluate the clinical safety and the angiographic stability of aneury sm occlusion with a new class of biologically active platinum coils after a 6-m onth period. Methods -Twenty-five patients with 25 intracranial aneurysms were treated by selective embolization with a new coated bioactive coil (Matrix; Bos ton Scientific Neurovascular). Matrix coils were used alone in 6 patients and in combination with Guglielmi detachable coils (GDcs; Boston Scientific Neurovascu lar) in 19. Angiographic results, procedure-related complications, and adverse neurological events during the follow-up period were recorded. Magnetic resonan ce angiography was performed at 6 months in all patients. results -Initial angi ographies demonstrated complete occlusion in 17 patients, residual neck in 7, an d a persisting aneurysm in 1. The clinical follow-up showed stable results in a ll patients. The grade of aneurysm occlusion at 6 months improved in 4 patients, remained stable in 15, and deteriorated in 6. Three patients needed retreatment because of a major aneurysm recanalization. Angiographic recurrences in cases o f aneurysms treated with a combination of Matrix coils and GDcs occurred within the expected range for bare platinum coils. Two of 3 patients needing retreatmen t had been treated with Matrix coils alone. conclusion -Stable results were obt ained predominantly when Matrix coils were combined with bare platinum coils. A prospective, randomized study is necessary to assess the potential benefit of Ma trix coils for patients treated by endovascular techniques.

Background In multisystem inflammatory syndrome in children(MIS-c),diagnostic delay could be associated with severity.This study aims to measure the time to diagnosis in MIS-c,assess its impact on the occurrence of cardiogenic shock,and specify its determinants.Methods A single-center prospective cohort observational study was conducted between May 2020 and July 2022 at a tertiary care hospital.children meeting the World health Organization MIS-c criteria were included.A long time to diagnosis was defined as six days or more.Data on time to diagnosis were collected by two independent physicians.The primary outcome was the occurrence of cardiogenic shock.Logistic regression and receiver operating characteristic curve analysis were used for outcomes,and a cox proportional hazards model was used for determinants.results Totally 60 children were assessed for inclusion,and 31 were finally analyzed[52%males,median age 8.8(5.7-10.7)years].The median time to diagnosis was 5.3(4.2-6.2)days.In univariable analysis,age above the median,time to diagnosis,high c-reactive protein,and high N-terminal pro-B-type natriuretic peptide(NT-proBNP)were associated with cardiogenic shock[odds ratio(Or)6.13(1.02-36.9),2.79(1.15-6.74),2.08(1.05-4.12),and 1.70(1.04-2.78),respectively].In multivariable analysis,time to diagnosis≥6 days was associated with cardiogenic shock[adjusted Or(aOr)21.2(1.98-227)].Time to diagnosis≥6 days had a sensitivity of 89% and a specificity of 77% in predicting cardiogenic shock;the addition of age>8 years and NT-proBNP at diagnosis≥11,254 ng/L increased the specificity to 91%.Independent determinants of short time to diagnosis were agers[ahr 0.34(0.13-0.88)],short distance to tertiary care hospital[ahr 0.27(0.08-0.92)],and the late period of the cOVID-19 pandemic[ahr 2.48(1.05-5.85)].conclusions Time to diagnosis≥6 days was independently associated with cardiogenic shock in MIS-c.Early diagnosis and treatment are crucial to avoid the use of inotropes and limit morbidity,especially in older children.

AIM:To investigate the polymorphic simple sequencerepeat in intron 1 of the epidermal growth factor receptorgene(EGFr)(cA-SSrⅠ),which is known to affect theefficiency of gene transcription as a putative target of themismatch repair(MMr)machinery in colorectal tumors.METhODS:The cA-SSrⅠgenotype was analyzed ina total of 86 primary colorectal tumors,selected upontheir microsatellite instability(MSI)status[42 with highfrequency MSI(MSI-h)and 44 microsatellite stable(MSS)]and their respective normal tissue.The effect of the cA-SSrⅠgenotype on the expression of the EGFr gene wasevaluated in 18 specimens using quantitative real-timereverse transcription Pcr and immunohistochemistry.rESuLTS:Mutations in cA-SSrⅠwere detected in 86%(36 of 42)of MSI-h colorectal tumors and 0%(0 of 44)ofMSS tumors,indicating the EGFr gene as a novel putativespecific target of the defective MMr system(Pred expression of EGFr was detected in most ofthe colorectal tumors analyzed[6/12(50%)at the mrNAlevel and 15/18(83%)at the peptide level].however,noassociation was apparent between EGFr expression andcA-SSrⅠstatus in tumors or normal tissues.cONcLuSION:Our results suggest that cA-SSrⅠsequence does not contribute to the regulation of EGFrtranscription in colon,and should thus not be consideredas a promising predictive marker for response to EGFrinhibitors in patients with colorectal cancer.

dear Editor, The cytosolic protein Tau is naturally present in human neurons, where it has a pivotal role in controlling microtubule stability. hyperphosphorylation of Tau （observed during neurodegenerative diseases, such as Alzheimer＇s disease） impairs the protein＇s ability to bind microtubules. This results in microtubule disassembly and the formation of Tau aggregates, Tau protein is also widely expressed in peripheral tissues. In the male reproductive system, screening for Tau has focused solely on the rodent and bovine testis. In the present study, we used immunofluorescence and immunoenzymatic techniques （with a Tau-specific antibody） to investigate the presence of Tau protein in human ejaculated sperm and testicular tissue.

Polyprenylated acylphloroglucinols represent an important class of natural products found in many plants.Among them,the two related products oblongifolin c(Ob-c)and guttiferone K(Gt-K)isolated from Garcinia species(notably from edible fruits),have attracted attention due to their marked anticancer properties.The two compounds only differ by the nature of the c-6 side chain,prenyl(Gt-K)or geranyl(Ob-c)on the phloroglucinol core.Their origin,method of extraction and biological properties are presented here,with a focus on the targets and pathways implicated in their anticancer activities.Both compounds markedly reduce cancer cell proliferation in vitro,as well as tumor growth and metastasis in vivo.They are both potent inducer of tumor cell apoptosis,and regulation of autophagy flux is a hallmark of their mode of action.The distinct mechanism leading to autophagosome accumulation in cells and the implicated molecular targets are discussed.The specific role of the chaperone protein hSPA8,known to interact with Ob-c,is addressed.Molecular models of Gt-K and Ob-c bound to hSPA8 provide a structural basis to their common hSPA8-binding recognition capacity.The review shed light on the mechanism of action of these compounds,to encourage their studies and potential development.

BAcKGrOuNDIrritable bowel syndrome(IBS),defined according to the rome IV diagnostic criteria,is a chronic functional gastrointestinal disorder characterized by recurrent abdominal pain related to altered bowel habits.First-line recommended treatments are limited to combining drugs targeting predominant symptoms,particularly pain(antispasmodics),constipation(laxatives),and diarrhea(loperamide),yielding only a limited therapeutic gain.GASTrAP^(■)DIrEcT is a class IIa medical formulation composed of a combination of chitin-glucan and simethicone indicated for the symptomatic treatment of gas-related gastrointestinal disorders by combining different mechanisms of action.AIM To evaluate the efficacy,tolerability,and safety of 4-week GASTrAP^(■)DIrEcT treatment in patients with IBS.METhODS In this prospective,multicenter,open-label trial,120 patients with IBS received three sticks of GASTrAP^(■)DIrEcT(1.5 g/d of chitin-glucan and 0.75 mg/d of simethicone)per day for 4 weeks.The primary endpoint was the responder rate,defined as the number of patients whose abdominal pain score decreased by≥30%from baseline to week(W)4.The analysis was performed using the per-protocol set.cardinal symptoms,impact of global symptoms on daily life,change in stool consistency,and improvement in defecatory disorders were evaluated.rESuLTS Overall,100 patients were evaluated.At W4,67%(95%cI:57-75)showed improvement in abdominal pain(score:5.8±2.4 vs 2.9±2.0,Pr improvements were observed for bloating[8.0±1.7 vs 4.7±2.9,PcI:50-70)responders],abdominal distension[7.2±2.1 vs 4.4±3.1,PcI:43-63)responders],and impact of global symptoms on daily life[7.1±2.0 vs 4.6±2.9,PcI:44-64)responders].Stool consistency improved in most patients(90%and 57%for patients with liquid and hard stools,respectively).Overall,42%of patients with defecatory disorders reported very much/considerable improvements by W2.No severe adverse event occurred,and tolerability was rated“good”or“very good”by 93%of patients

BAcKGrOuND Irritable bowel syndrome(IBS)is one of the most frequent and debilitating conditions leading to gastroenterological referrals.however,recommended treatments remain limited,yielding only limited therapeutic gains.chitin-glucan(cG)is a novel dietary prebiotic classically used in humans at a dosage of 1.5-3.0 g/d and is considered a safe food ingredient by the European Food Safety Authority.To provide an alternative approach to managing patients with IBS,we performed preclinical molecular,cellular,and animal studies to evaluate the role of chitin-glucan in the main pathophysiological mechanisms involved in IBS.AIM To evaluate the roles of cG in visceral analgesia,intestinal inflammation,barrier function,and to develop computational molecular models.METhODS Visceral pain was recorded through colorectal distension(crD)in a model of long-lasting colon hypersensitivity induced by an intra-rectal administration of TNBS[15 milligrams(mg)/kilogram(kg)]in 33 Sprague-Dawley rats.Intracolonic pressure was regularly assessed during the 9 wk-experiment(weeks 0,3,5,and 7)in animals receiving cG(n=14)at a human equivalent dose(hED)of 1.5 g/d or 3.0 g/d and compared to negative control(tap water,n=11)and positive control(phloroglucinol at 1.5 g/d hED,n=8)groups.The anti-inflammatory effect of cG was evaluated using clinical and histological scores in 30 c57bl6 male mice with colitis induced by dextran sodium sulfate(DSS)administered in their drinking water during 14 d.hT-29 cells under basal conditions and after stimulation with lipopolysaccharide(LPS)were treated with cG to evaluate changes in pathways related to analgesia μ-opioid receptor(MOr),cannabinoid receptor 2(cB2),peroxisome proliferator-activated receptor alpha,inflammation[interleukin(IL)-10,IL-1b,and IL-8]and barrier function[mucin 2-5Ac,claudin-2,zonula occludens(ZO)-1,ZO-2]using the real-time Pcr method.Molecular modelling of cG,LPS,lipoteichoic acid(LTA),and phospholipomannan(PLM)was developed,and the ability of cG to chelate microbial pathogenic lipids

Myositis ossificans circumscribed is a bone and cartilage heterotopic non neoplastic proliferation inside the soft tissues. It is a benign focal heterotopic ossification process of soft tissues, and a rare disorder that occurs spontaneously or after local trauma. clinical and radiographic appearances are quite hustler. A careful histological examination of biopsy straightens diagnosis is necessary. There is no consensus in support (surgical or/and medical). From Benign prognosis, evolution of this pathology is usually favorable. The authors report a case of giant myositis ossificans circumscribed post-traumatic localized on gluteus and adductor muscles of the right hip on a 26-year-old man. Through a review of literature the mechanism, the diagnostic methods and therapeutic will be discussed.
