Timely monitoring of anesthesia status during surgery is important to prevent an overdose of isoflurane ***,in-depth studies of the neural mechanisms of anesthetics are *** CA1 plays an important role during ***,a high spatiotemporal resolution microdevice technology for the accurate detection of deep brain nuclei is *** this research,four-shank 32-channel implantable microelectrode arrays(MEAs)were developed for the real-time recording of single-cell level neural information in rat hippocampal *** nanoparticles were modified onto the microelectrodes to substantially enhance the electrical properties of the microelectrode *** modified MEAs exhibited low impedance(11.5±1 kΩ)and small phase delay(-18.5°±2.54°),which enabled the MEAs to record single-cell level neural information with a high signal-to-noise *** MEAs were implanted into the CA1 nuclei of the anesthetized rats,and the electrophysiological signals were recorded under different degrees of anesthesia mediated by low-dose concentrations of *** recorded signals were analyzed in *** caused an inhibition of spike firing rate in hippocampal CA1 neurons,while inducing low-frequency oscillations in CA1,thus enhancing the low-frequency power of local field *** this manner,the spike firing rate and the power of local field potentials in CA1 could characterize the degree of isoflurane *** present study provides a technical tool to study the neural mechanisms of isoflurane anesthesia and a research method for monitoring the depth of isoflurane anesthesia in clinical practice.

During cardiac arrest (CA),severe ischemia and hypoxia occur in tissues and organs of the entire body,inflammatory cytokines are released,and ischemiareperfusion injury occurs after the return of spontaneous circulation (ROSC),leading to multiple organ dysfunction in the body;this condition is called post-CA syndrome(PCAS).^([1])According to the BASeline Investigation of Out-of-Hospital Cardiac Arrest (BASIC-OHCA) study,the crude incidence of emergency medical service (EMS)-assessed OHCA was 95.7 per 100,000 individuals,and only1.2%of those individuals survived to hospital discharge or30 d after being resuscitated by the EMS in China.

This review summarizes the current research advances and guideline updates in neurocritical care. For the therapy of ischemic stroke, the extended treatment time window for thrombectomy and the emergence of novel thrombolytic agents and strategies have brought greater hope for patient recovery. Minimally invasive hematoma evacuation and goal-directed bundled management have shown clinical benefits in treating cerebral hemorrhage. In the treatment of aneurysmal subarachnoid hemorrhage (aSAH), early lumbar drainage can reduce the risk of infarction. Decompressive craniectomy for severe traumatic brain injury has also obtained high-quality evidence support. However, multimodal brain monitoring strategies for patients with traumatic brain injury need further optimization. For patients with cardiac arrest, extracorporeal cardiopulmonary resuscitation can reduce in-hospital mortality and improve long-term neurological prognosis. For neurocritical care patients, abundant high-quality studies have emerged in areas including multimodal neuromonitoring, hemodynamic management, airway management and respiratory therapy, and antiepileptic treatment. In 2023, the guidelines for aSAH have been updated for the first time in the past decade, aiming to provide evidence-based practice recommendations for clinical care. chinese expert consensuses have also been formulated to guide analgesia and sedation for neurocritical care patients and developed a set of medical quality indicators on neurocritical care, which will enhance standardization and homogenization improvement in neurocritical care quality.

BACKGROUND: Sepsis, a common acute and critical disease, leads to 11 million deaths annually worldwide. Probiotics are living microorganisms that are beneficial to the host and may benefit sepsis outcomes, but their effects are stil inconclusive. This study aimed to evaluate the overal eff ect of probiotics on the prognosis of patients with *** RESOURCES: We searched several sources for published/presented studies, including Pub Med, EMBASE, Web of Science, the Cochrane Library and the US National Library of medicine Clinical Trials Register(***) updated through July 30, 2023, to identify all relevant randomized controlled trials(RCTs) or observational studies that assessed the effectiveness of probiotics or synbiotics in patients with sepsis and reported mortality. We focused primarily on mortality during the study period and analyzed secondary outcomes, including 28-day mortality, in-intensive care unit(ICU) mortality and other ***: Data from 405 patients in five RCTs and 108 patients in one cohort study were included in the analysis. The overall quality of the studies was satisfactory, but clinical heterogeneity existed. All adult studies reported a tendency for probiotics to reduce the mortality of patients with sepsis, and most studies reported a decreasing trend in the incidence of infectious complications, length of ICU stay and duration of antibiotic use. There was only one RCT involving ***: Probiotics show promise for improving the prognosis of patients with sepsis, including reducing mortality and the incidence of infectious complications, particularly in adult patients. Despite the limited number of studies, especially in children, these findings will be encouraging for clinical practice in the treatment of sepsis and suggest that gut microbiota-targeted therapy may improve the prognosis of patients with sepsis.

Objective: To develop and demonstrate a comprehensive methodological framework for target trial emulation (TTE) implementation via post-laparoscopic septic shock fluid management as a clinical case study. Methods: We designed a simulated study of 10,000 patients in which colloid resuscitation was compared with crystalloid resuscitation. The framework incorporated precise eligibility criteria, standardized intervention protocols, and balanced treatment allocation with temporal anchoring. The primary analyses focused on 28-day mortality and were evaluated through survival analysis and comparative statistics. Results: The methodological framework successfully demonstrated the systematic implementation of TTE principles, providing reproducible protocols for eligibility determination, intervention standardization, and outcome assessment in observational studies. Conclusion: This work bridges the gap between TTE theory and practice by providing a detailed technical roadmap that enhances methodological transparency and reproducibility in comparative effectiveness research. © 2025 Zhejiang university

Objective: To explore the methodology of the evidence-based expert consensus formulation process of traditional chinese medicine(TCM) nursing taking stroke as an ***: First, preliminary and comprehensive presentation of all stroke-related symptoms and corresponding TCM nursing techniques involved were revealed through bibliometric analysis. Then, selection of stroke symptoms and TCM nursing techniques for inclusion in the consensus was performed using an expert consultation method. Next, we determined the search strategy for a precise evidence search;conducted an evaluation of evidence quality and the grade of the evidence;and completed evidence extraction, evidence analysis, and evidence synthesis based on the included symptoms and TCM nursing techniques. The Delphi method was then applied to determine the strength of each recommendation and the choice of nursing care points by referring to the Grading of Recommendations, Assessment, Development, and Evaluations grid. Finally, we conducted an external expert validation of the Delphi results to form an expert consensus ***: Through the bibliometric analysis, 22 stroke symptoms and 18 TCM nursing techniques were identified in the literature. Then, after expert consultation, 22 symptoms and 111 pairs of symptoms combined with TCM nursing techniques were selected for the evidence search. Evidence integration yielded 10 stroke symptoms corresponding to 29 bodies of evidence;these 10 symptoms were retained through the Delphi consultation, and recommendation strength results for 26 recommendations were obtained. A total of 9 symptoms were further retained for expert external validation to form 24 recommendations, with a recommendation process score range of 7.64-9.99 points and a more scientific and standardized recommendation-formation ***: Owing to the current limited conditions of evidence-based resources for TCM nursing, the present consensus-building process represents only a

Type I interferon(IFN)is considered as a bridge between innate and adaptive *** activation or inhibition of type I IFN signaling is essential for host defense against pathogen invasion,tumor cell proliferation,and overactive immune *** to intricate and diverse chemical structures,natural products and their derivatives have become an invaluable source inspiring innovative drug *** addition,some natural products have been applied in clinical practice for infection,cancer,and autoimmunity over thousands of years and their promising curative effects and safety have been ***,whether these natural products are primarily targeting type I IFN signaling and specific molecular targets involved are not fully *** the current review,we thoroughly summarize recent advances in the pharmacology researches of natural products for their type I IFN activity,including both agonism/activation and antagonism/inhibition,and their potential application as ***,the source and chemical nature of natural products with type I IFN activity are highlighted and their specific molecular targets in the type I IFN pathway and mode of action are *** conclusion,natural products possessing type I IFN activity represent promising therapeutic strategies and have a bright prospect in the treatment of infection,cancer,and autoimmune diseases.
