Development and validation of a novel non-invasive test for diagnosing nonalcoholic fatty liver disease in Chinese children

With the exploding prevalence of obesity,many children are at risk of developing nonalcoholic fatty liver *** anthropometric and laboratory parameters,our study aimed to develop a model to quantitatively evaluate liver fat content(LFC)in children with *** A well-characterized cohort of 181 children between 5 and 16 years of age were recruited to the study in the Endocrinology Department as the derivation *** external validation cohort comprised 77 *** assessment of liver fat content was performed using proton magnetic resonance *** and laboratory metrics were measured in all subjects.B-ultrasound examination was carried out in the external validation *** Kruskal-Wallis test,Spearman bivariate correlation analyses,univariable linear regressions and multivariable linear regression were used to build the optimal predictive *** The model was based on indicators including alanine aminotransferase,homeostasis model assessment of insulin resistance,triglycerides,waist circumference and Tanner *** adjusted R^(2)of the model was 0.589,which presented high sensitivity and specificity both in internal[sensitivity of 0.824,specificity of 0.900,area under curve(AUC)of 0.900 with a 95%confidence interval:0.783–1.000]and external validation(sensitivity of 0.918 and specificity of 0.821,AUC of 0.901 with a 95%confidence interval:0.818–0.984).Conclusions Our model based on five clinical indicators was simple,non-invasive,and inexpensive;it had high sensitivity and specificity in predicting LFC in ***,it may be useful for identifying children with obesity who are at risk for developing nonalcoholic fatty liver disease.

Hepatectomy guided by the diseased bile duct and hepatic vein for hepatolithiasis

Hepatolithiasis(HL)is a common and serious disease with high incidence in Southeast Asian countries(1),especially in Southwest China,South China,and Southeast coastal areas of *** on the pathological characteristics of the segmented stone distribution and atrophy of the hepatic parenchyma,resection of the atrophied liver involving the diseased bile duct is the most effective and common surgical treatment for *** proposed by Shindoh et al.,hepatobiliary surgeons have used anatomic liver resection(AR)for hepatocellular carcinoma(HCC),which involves complete resection of the anatomic area supplied by the portal vein of the Glissonean branch(2).However,factors such as changes in bile fluid dynamics,stone compression,and chronic inflammation associated with HL lead to portal vein and hepatic artery stenosis and occlusion in some diseased areas,resulting in the diseased bile duct and hepatic vein(HV)becoming significant characteristic anatomical *** such cases,the effect and success rate of indocyanine green fluorescence staining are poor for navigation during HL *** imaging techniques such as 3D visualization,3D printing,and artificial intelligence can facilitate preoperative evaluation but not accurate evaluation of all real anatomical structures and guidance of the transection *** such patients with HL,the lack of effective intrahepatic anatomic landmarks used during AR results in residual stones and diseased bile ducts and recurrence of *** necessitates repeated surgeries and affects postoperative recovery and patients’quality of ***,to allow for standardized and more accurate hepatectomy for HL and provide a theoretical basis for formulating corresponding treatment norms,we proposed and applied hepatectomy guided by the bile duct and HV(H-BV),that is by using the diseased bile duct tree and HV as anatomic landmarks.

Although big data is publicly available on water quality parameters,virtual simulation has not yet been adequately adapted in environmental chemistry *** twin is different from conventional geospatial modeling approaches and is particularly useful when systematic laboratory/field experiment is not realistic(e.g.,climate impact and water-related environmental catastrophe)or difficult to design and monitor in a real time(e.g.,pollutant and nutrient cycles in estuaries,soils,and sediments).Data-driven water research could realize early warning and disaster readiness simulations for diverse environmental scenarios,including drinking water contamination.

As ethnic medicine,the whole grass of plants in Cirsium was used as *** review focuses on the antimicrobial activity of plants in Cirsium,including antimicrobial components,against different types of microbes and bacteriostatic *** results showed that the main antimicrobial activity components in Cirsium plants were flavonoids,triterpenoids and phenolic acids,and the antimicrobial ability varied according to the species and the content of *** them,phenolic acids showed a strong antibacterial ability against Acinetobacter baumannii,Pseudomonas aeruginosa,and Enterococcus *** antibacterial mechanisms include:(1)damaging the cell membrane,cell walls,mitochondria and nucleus of bacteria;(2)inhibiting the synthesis of proteins and nucleic acids;(3)suppressing the synthesis of enzymes for tricarboxylic acid cycle pathways and glycolysis,and then killing the bacteria via inhibition of energy ***,most research results on antimicrobial activity of Cirsium plants are reported based on in vitro *** evidence from clinical data and comprehensive evaluation are needed.

Weight regain after intragastric balloon for pre-surgical weight loss

BACKGROUND Over one-third of Americans carry the diagnosis of obesity,many also with obesity-related *** can place patients at increased risk of operative and postoperative *** intragastric balloon has been shown to aid in minor weight loss,however its weight recidivism in patients requiring short interval weight loss has not been well *** To evaluate weight loss,ability to undergo successful elective surgery after intragastric balloon placement,and weight management after balloon *** This study is a retrospective review of patients in a single academic institution undergoing intragastric balloon placement from 2019-2023 to aid in weight loss prior to undergoing elective *** outcomes including weight loss,duration of balloon placement,successful elective surgery,weight regain postballoon and post-procedure complications were *** criteria included those with balloon in place at time of *** Thirty-three patients completed intragastric balloon therapy from 2019-2023 as a bridge to elective *** patients were required to participate in a 12-month weight management program to be eligible for balloon *** surgeries included incisional hernia repair,umbilical hernia repair,inguinal hernia repair,and knee and hip *** average age at placement was 53 years±11 years,majority(91%)were *** average duration of intragastric balloon therapy was 186 days±41 *** average weight loss was 14.0 kg±7.4 kg and with an average percent excess body weight loss of 30.0%(7.9%-73.6%).Over half of the patients(52.0%)achieved the goal of 30-50 lbs(14-22 kg)weight ***-one patients(64%)underwent their intended elective surgery,2 patients(6%)deferred surgery due to symptom relief with weight loss ***-one of the patients(64%)have documented weights in 3 months after balloon removal,in these patients the majority(76%)gained weight after balloon *** pat

Impaired pericyte-Müller glia interaction via PDGFRβsuppression aggravates photoreceptor loss in a rodent model of light-induced retinal injury

AIM:To investigate the involvement of pericyte-Müller glia interaction in retinal damage repair and assess the influence of suppressing the platelet-derived growth factor receptorβ(PDGFRβ)signaling pathway in retinal pericytes on photoreceptor loss and Müller glial ***:Sprague-Dawley rats were exposed to intense light to induce retinal *** antibody against PDGFRβwere deployed to block the signaling pathway in retinal pericytes through intravitreal *** histology and Müller glial reaction were assessed following light *** vitro,normal and PDGFRβ-blocked retinal pericytes were cocultured with Müller cell line(rMC-1)to examine morphological and protein expression changes upon supplementation with light-injured supernatants of homogenized retinas(SHRs).RESULTS:PDGFRβblockage 24h prior to intense light exposure resulted in a significant exacerbation of photoreceptor *** upregulation of GFAP and p-STAT3,observed after intense light exposure,was significantly inhibited in the PDGFRβblockage *** ther upregulation of cytokines monocyte chemoattractant protein 1(MCP-1)and interleukin-1β(IL-1β)was also observed following PDGFRβ*** the in vitro coculture system,the addition of light-injured SHRs induced pericyte deformation and upregulation of proliferating cell nuclear antigen(PCNA)expression,while Müller cells exhibited neuron-like morphology and expressed ***,PDGFRβblockage in retinal pericytes abolished these cellular responses to light-induced damage,consistent with the in vivo PDGFRβblockage ***:Pericyte-Müller glia interaction plays a potential role in the endogenous repair process of retinal *** of this interaction exacerbates photoreceptor degeneration in light-induced retinal injury.

AIM:To provide a comprehensive and more representative national data on the disease,especially on treatment options and outcomes,and to determine access of retinoblastoma patients from Luzon,Visayas and Mindanao to eye care,and determine if access is associated with delay in consultation,staging and ***:Cohort study of retinoblastoma patients seen in eleven institutions located in the three major areas of the Philippines namely Luzon,Vizayas and Mindanao from ***:Totally 636 patients,involving 821 eyes,were ***(57%)were from Luzon and were seen in institutions in Luzon(72%).Annually,58±10 new cases were seen with 71%having unilateral *** delay of consultation remained long at 9(3,17)mo,longest in patients with unilateral disease(P<0.02)and those from the Visayas(P<0.003).Based on the International Retinoblastoma Staging System,only 35%of patients had Stage 1 while 47%already had extraocular *** was the most common treatment received by 484 patients while intravenous chemotherapy was received by *** were 250(39%)patients alive,195(31%)dead,85(13%)abandoned,17(3%)refused and 89(14%)with no ***:This study presents the largest cohort of retinoblastoma patients in the Philippines in terms of patients’and participating institutions’number and geographical location and type of institution(private and public).It also presents more comprehensive data on the treatments used and outcomes(survival,globe salvage,and vision retention rates).Delay in consultation was still long among patients leading to advanced disease stage and lower survival *** increasing capacity to diagnose and manage retinoblastoma in the country,the delay of consultation remains long primarily due to accessibility issues to eye care institutions especially in the Visayas and financial *** delay was still significant that overall survival rate remain low.
