Amyotrophic lateral sclerosis(ALS)is a neuromuscular condition resulting from the progressive degeneration of motor neurons in the cortex,brainstem,and spinal *** the typical clinical phenotype of ALS involves both upper and lower motor neurons,human and animal studies over the years have highlighted the potential spread to other motor and non-motor regions,expanding the phenotype of *** superoxide dismutase 1(SOD1)mutations represent a minority of ALS cases,the SOD1 gene remains a milestone in ALS research as it represents the first genetic target for personalized *** numerous single case reports or case series exhibiting extramotor symptoms in patients with ALS mutations in SOD1(SOD1-ALS),no studies have comprehensively explored the full spectrum of extramotor neurological manifestations in this *** this narrative review,we analyze and discuss the available literature on extrapyramidal and non-motor features during *** multifaceted expression of SOD1 could deepen our understanding of the pathogenic mechanisms,pointing towards a multidisciplinary approach for affected patients in light of new therapeutic strategies for SOD1-ALS.

Acupuncture is an alternative therapy for tinnitus in clinical *** mechanism by which acupuncture can alleviate tinnitus is still *** nervous system was reported to be responsible for *** aim of this study was to explore the effect of acupuncture on autonomic balance in adult tinnitus *** patients were randomly assigned into either the deep acupuncture(DA)group or the shallow acupuncture(SA)*** patient received 6 acupuncture sessions(a-f phase)over three *** of heart rate variability and Tinnitus Handicap Inventory(THI)were obtained at baseline and after the sixth acupuncture session in all *** results showed that the low frequency/high frequency(LF/HF)pattern was increased at b-f phase until the sixth acupuncture session when compared with that at the first acupuncture session in DA ***,it continuously increased at b-f phase in SA group even at the sixth acupuncture session,which was not significantly different from that at the first acupuncture *** decrease in THI in DA group was greater than that in SA group after 3-week treatment(P=0.043).Our preliminary study suggests three-week deep acupuncture can improve tinnitus symptoms in adult tinnitus patients,which may be related to the regulation of autonomic nervous system balance.

Development and validation of a risk score for advanced colorectal adenoma recurrence after endoscopic resection

AIM: To develop and validate a risk score for advanced colorectal adenoma(ACA) recurrence after endoscopic ***: Out of 3360 patients who underwent colon polypectomy at University of Foggia between 2004 and 2008, data of 843 patients with 1155 ACAs was retrospectively reviewed. Surveillance intervals were scheduled by guidelines at 3 years and primary endpoint was considered 3-year ACA recurrence. Baseline clinical parameters and the main features of ACAs were entered into a Cox regression analysis and variables with P < 0.05 in the univariate analysis were then tested as candidate variables into a stepwise Cox regression model(conditional backward selection). The regression coefficients of the Cox regression model were multiplied by 2 and rounded in order to obtain easy to use point numbers facilitating the calculation of the score. To avoid overoptimistic results due to model fitting and evaluation in the same dataset, we performed an internal 10-fold cross-validation by means of bootstrap sampling. RESULTS: Median lesion size was 16 mm(12-23) while median number of adenomas was 2.5(1-3), whereof the number of ACAs was 1.5(1-2). At 3 years after polypectomy, recurrence was observed in 229 ACAs(19.8%), of which 157(13.5%) were metachronous neoplasms and 72(6.2%) local recurrences. Multivariate analysis, after exclusion of the variable "type of resection" due to its collinearity with other predictive factors, confirmed lesion size, number of ACAs and grade of dysplasia as significantly associated to the primary outcome. The score was then built by multiplying the regression coefficients times 2 and the cut-off point 5 was selected by means of a Receiver Operating Characteristic curve analysis. In particular, 248 patients with 365 ACAs fell in the higher-risk group(score ≥ 5) where 3-year recurrence was detected in 174 ACAs(47.6%) whereas the remaining 595 patients with 690 ACAs were included in the low-risk group(score < 5) where 3-year recurrence rat

Post-COVID-19 Late Pulmonary Embolism in a Young Woman about a Case

COVID-19 appears to be associated with a disproportionate risk of thrombosis. The occurrence of thrombosis is a frequent complication of many medical and surgical conditions. Their prevention by LMWH is the option of choice as well as their early diagnosis in this pandemic context. In addition, several recent observations support that severe pulmonary embolism is very common in hospitalized patients with COVID-19. We report the case of late pulmonary embolism in a 26-year-old patient with a medical history of (SARS-CoV2) and newly discovered diabetes. The clinical examination found exertional dyspnea, intense chest pain, blood pressure at 145/85mmHg, heart rate at 129 bpm, respiratory rate at 25 cycles/min, blood sugar at 1.45 g/l, oxygen saturation at 92%. In front of this clinical picture, the thoracic CT angiography to ask shows a pulmonary embolism of the right segmental and sub-segmental branches less marked on the left, involving the lingular and basal branches; the management was limited to anticoagulation associated with rehabilitation sessions (respiratory physiotherapy).

MYOCARDIAL PROTECTIVE EFFECT OF CAPTOPRIL CARDIOPLEGIA ON ISCHEMIA AND REPERFUSION INJURY IN THE ISOLATED RABBIT HEART

The myocardial protective effect of captopril cardioplegia was investigated through the recovery of cardiac functions, biochemical changes and ultrastructural assays in the isolated working rabbit heart after hypothermic ischemic arrest for 3 hours. Hearts were randomly divided into 3 groups with 8 in each. Another 3 groups of hearts (8 in each) were arranged for studying the biochemical changes and ultrastructure during the ischemic period. In the control group (Ⅰ), the hearts were protected by the modified St. Thomas’ cardioplegic solution No. Ⅰ, while in Group Ⅱ and Ⅲ the cardioplegic solutions contained 2.3 μmol/L and 23 μmol/L captopril respectively. The results showed that the percent recoveries of cardiac function in the 23 μmol/L captopril group compared with the control group were 97.9±9.8% vs 84.9±7.3% in left ventricular peak systolic pressure (P<0.01), 118±33% vs 79±20% in +dp/dt max (P<0.01), 111±22% vs 75±18% in -dp/dt max (P<0.01), 145.6±44.8% vs 76.7±18% in coronary flow (P<0.01), 120.2±45.7% vs 36.4±22.2% in aortic flow (P<0.01), 127.4±27.3% vs 56.1±14% in cardiac output (P<0.01), and 118.2±33.5% vs 52.5±19.9% in stroke volume (P<0.01) respectively. The creatine kinase release at 180 minutes of ischemia was 0.17±0.14 IU/min/*** in 23 μmol/L captopril group vs 0.39±0.14 IU/min/*** in the control group (P<0.05), and it was 11.6±7.5 IU/min/*** vs 41.4±10.2 IU/min/*** 35 minutes on reperfusion (P<0.01). The myocardial MDA in Group Ⅲ after reperfusion was 50.0±9.2 pmol/mg which was much less than 85.1±16.1 pmol/mg in the control group (P<0.01). The mitochondrial ultrastructure was also much better preserved in the captopril groups than that in the control group. We conclude that captopril can greatly intensify the myocardial protective effect of the modified St. Thomas’ No. Ⅰ cardioplegia against the global ischemia and reperfusion injury.

Chemotherapy-induced peripheral neuropathy （CIPN） is one of the most common and severe adverse effects related to cancer treatment. Unfortunately, although several agents and protocols have been proposed, no prophylactic strategies have yet to be proven useful. Therefore, new alternative therapies have been considered for CIPN prevention. Herbal medicine in Japan, called Kampo medicine, is derived from traditional Chinese medicine. Goshajinkigan （GJG） is a Kampo medicine, that is comprised of ten herbs. The aim of this work is to analyse the results of pre-clinical and clinical studies on the potential applications of GJG in CIPN prevention.

Automatic web services classification based on rough set theory

With development of web services technology, the number of existing services in the internet is growing day by day. In order to achieve automatic and accurate services classification which can be beneficial for service related tasks, a rough set theory based method for services classification was proposed. First, the services descriptions were preprocessed and represented as vectors. Elicited by the discernibility matrices based attribute reduction in rough set theory and taking into account the characteristic of decision table of services classification, a method based on continuous discernibility matrices was proposed for dimensionality reduction. And finally, services classification was processed automatically. Through the experiment, the proposed method for services classification achieves approving classification result in all five testing categories. The experiment result shows that the proposed method is accurate and could be used in practical web services classification.

Aspartic acid in the hippocampus:a biomarker for postoperative cognitive dysfunction

This study established an aged rat model of cognitive dysfunction using anesthesia with 2% iso- flurane and 80% oxygen for 2 hours. Twenty-four hours later, Y-maze test results showed that isoflurane significantly impaired cognitive function in aged rats. Gas chromatography-mass spectrometry results showed that isoflurane also significantly increased the levels of N,N-diethy- lacetamide, n-ethylacetamide, aspartic acid, malic acid and arabinonic acid in the hippocampus of isoflurane-treated rats. Moreover, aspartic acid, N,N-diethylacetamide, n-ethylacetamide and malic acid concentration was positively correlated with the degree of cognitive dysfunction in the isoflurane-treated rats. It is evident that hippocampal metabolite changes are involved in the formation of cognitive dysfunction after isoflurane anesthesia. To further verify these results, this study cultured hippocampal neurons in vitro, which were then treated with aspartic acid （100 μmol/L）. Results suggested that aspartic acid concentration in the hippocampus may be a biomarker for predicting the occurrence and disease progress of cognitive dysfunction.
