Objective: To probe the effects of qi-supplementing and yin-nourishing therapy (blood-increasing decoction and blood generating powder) on chronic thrombocytopenia. Methods: Two hundred patients with chronic thrombocytopenia were randomly divided into control (n=100) and test groups (n = 100) with Amino-polypeptide as a basic treatment for both. Test group patients consumed a blood-increasing decoction and blood-generating powder for 1-3 months. Improvements in platelet counts and tcm syndrome were observed. Results: One hundred and sixty-four (80 in the test group and 84 in the control group) of 189 total participants were treated for 3 months. The total effective rate in improving tcm syndrome was 95.00% in the test group and 79.76% in the control group (Ptcm syndrome between the two groups treated at different time points. The total effective rate of platelet counts was 86.25% in the test group and 59.52% in the control group (Ptcm syndrome was improved and platelet counts increased with no obvious side effects, and the quality of life of the participants was enhanced with noticeable long-term curative effects.

Objective:To explore the influence of artificial luminous environment and preventive function of traditional Chinese medicine(tcm) intervention based on "Theory of yin-yang clock" on myopia.Methods:A total of 45 New Zealand young rabbits were randomly divided into 5groups,9 for each group.Control group was exposed in natural light.Fluorescent group and full spectrum group were exposed in fluorescent light and full spectrum light,on which basis fluorescent tcm group and full spectrum tcm group were added with "Rizhong Yinyang Formulas",respectively.Optical parameters were measured and the influence of different lights on the serum and retinal dopamine(DA) levels as well as the retinal histopathological tissues was observed.Results:The spectrum of fluorescent light mainly focused at 420-490 nm with the peak value of wavelength near 450 nm,whereas that of full spectrum was wider(400-800 nm) with the peak value near 600 nm.After 4 and 12 weeks,fluorescent group was evidently lower in serum and retinal DA levels(Ptcm group and full spectrum tcm group(P>0.05).Histopathological observation showed that there was significant difference in pigment epithelium layer,photoreceptor and nerve fiber layer between fluorescent group and control group,but the difference among the test groups was not significant.Conclusions:Fluorescent light has certain influence on retinal histological construction and visual performance.However,tcm intervention may have some degree of protective function on retina.

OBJECTIVE:To reveal that mirror-like tongue,observed via a noninvasive inspection,is a powerful indicator of the severity and prognosis of patients with acute heart failure(AHF).METHODS:This was an observational,prospective study.A total of 408 patients who met the inclusion criteria and were diagnosed with AHF for the first time at Taicang Traditional Chinese Medicine hospital affiliated to nanjing university of Chinese Medicine from August 2019 to January 2021 were selected as the research subjects.According to their tongue fur,the patients were divided into group A(mirror-like tongue group)and group B(non-mirror-like tongue group).The clinical characteristics and incidence of major adverse cardiovascular events(MACEs)within 1 year of follow-up were systematically compared between the two groups.RESULTS:Sixty-five patients were included in group A,and 343 patients were included in group B.There were 32 males and 33 females in group A and 168 males and 175 females in group B.The average age of the overall population was 64 years old,and the average age of group A was significantly higher than that of group B(74 vs 62 years,Phospital stay in group A was significantly longer than that in group B(13.1 vs 7.6,P<0.001).The incidence of MACEs,such as recurrence of AHF,new myocardial infarction and stroke,in group A within one year was higher than that in group B(P=0.007,0.009,<0.001).The incidence of cumulative MACEs in group A was significantly high

BACKGROUND Colorectal mucosa-associated lymphoid tissue lymphoma(MALToma),a rare kind of nongastric MALToma,lacks consensus on its endoscopic features and standard therapies.According to previous studies on the clinical characteristics and outcomes of colorectal MALToma,endoscopic resection remains a good therapeutic strategy.CASE SUMMARY A 71-year-old woman suffered intermittent hematochezia for 1 mo,accompanied with abdominal pains but without weight loss,fever,chills or fatigue.Colonoscopy showed a massive hemispheric mass with rough and hyperemic mucosa in the lower rectum.Narrow-band imaging magnifying endoscopy detected some branching abnormal blood vessels and disappearance of glandular structure,which was similar with the tree-like appearance sign in gastric MALToma.Endoscopic ultrasonography revealed the lesion to be hypoechoic,boundary-defined,and echo uniform inside,originating from the muscularis propria.Abdominal enhanced computed tomography(CT)demonstrated a soft tissue mass with defined boundary.No enlarged superficial lymph nodes were detected by B-mode ultrasound.C13-urea breath test and serum Helicobacter pylori antibody were both negative.The patient underwent endoscopic full-thickness resection.Postoperative pathological analysis indicated colorectal MALToma.The patient remained asymptomatic after discharge,and follow-up positron emission tomography–CT and colonoscopy showed no residual lesion,remnants or lymph node metastasis.CONCLUSION This case provides new information on the specific endoscopic features of colorectal MALToma and an alternative treatment for patients.

AIM: To prove anthrax lethal toxin(Le Tx) blocks the mitogen activated protein kinases(MAPKs) activation by degrading the MAPK/ERK kinases(MEKs) to suppress vascular endothelial growth factor(VEGF) secretion.METHODS: Human adult retinal pigmented epithelium(ARPE) cells were cultured and treated with normal glucose, high glucose or high glucose with Le Tx for additional 24, 48 or 72 h for viable cell count. Total RNA from the ARPE was isolated for reverse transcription polymerase chain reaction(RT-PCR). The conditioned medium of ARPE cells treated in different group for 48 h was filtered and diluted to detect the concentration of VEGF by enzyme-linked immunosorbant assays.Evaluate the role of MEK/MAPK pathway in the secretion of VEGF by immunoblotting. RESULTS: In this study, we proved high glucose induced activation of the MAPK extracellular signal-regulated kinase(ERK1/2) and p38 in the ARPE cell line was blocked by anthrax Le Tx. Le Tx also inhibited high glucose induced ARPE cell over proliferation.CONCLUSION: Le Tx suppressed high glucose induced VEGF over secretion in the ARPE cells, mainly through a post-translational mechanism.

Pseudo-allergic reactions(PARs)widely occur upon application of drugs or functional foods.Anti-pseudo-allergic ingredients from natural products have attracted much attention.This study aimed to investigate anti-pseudo-allergic compounds in licorice.The anti-pseudo-allergic effect of licorice extract was evaluated in rat basophilic leukemia 2H3(RBL-2H3)cells.Anti-pseudo-allergic compounds were screened by using RBL-2H3 cell extraction and the effects of target components were verified further in RBL-2H3 cells,mouse peritoneal mast cells(MPMCs)and mice.Molecular docking and human MRGPRX2-expressing HEK293T cells(MRGPRX2-HEK293T cells)extraction were performed to determine the potential ligands of MAS-related G protein-coupled receptor-X2(MRGPRX2),a pivotal target for PARs.Glycyrrhizic acid(GA)and licorice chalcone A(LA)were screened and shown to inhibit Compound48/80-induced degranulation and calcium influx in RBL-2H3 cells.GA and LA also inhibited degranulation in MPMCs and increase of histamine and TNF-a in mice.LA could bind to MRGPRX2,as determined by molecular docking and MRGPRX2-HEK293T cell extraction.Our study provides a strong rationale for using GA and LA as novel treatment options for PARs.LA is a potential ligand of MRGPRX2.

Infliximab(IFX),as a drug of first-line therapy,can alter the natural progression of Crohn’s disease(CD),promote mucosal healing and reduce complications,hospitalizations,and the incidence of surgery.Perianal fistulas are responsible for the refractoriness of CD and represent a more aggressive disease.IFX has been demonstrated as the most effective drug for the treatment of perianal fistulizing CD.Unfortunately,a significant proportion of patients only partially respond to IFX,and optimization of the therapeutic strategy may increase clinical remission.There is a significant association between serum drug concentrations and the rates of fistula healing.Higher IFX levels during induction are associated with a complete fistula response in these patients.Given the apparent relapse of perianal fistulizing CD,maintenance therapy with IFX over a longer period seems to be more beneficial.It appears that patients without deep remission are at an increased risk of relapse after stopping anti-tumor necrosis factor agents.Thus,only patients in prolonged clinical remission should be considered for withdrawal of IFX treatment when biomarker and endoscopic remission is demonstrated,especially when the hyperintense signals of fistulas on T2-weighed images have disappeared on magnetic resonance imaging.Fundamentally,the optimal timing of IFX use is highly individualized and should be determined by a multidisciplinary team.

The occurrence and development of colorectal cancer involve multiple genes and pathways as a result of accumulated mutations at several sites that control growth and differentiation.Baicalin is a flavonoid extracted from Scutellaria baicalensis Georgi with antioxidant,anti-inflammatory and antiviral activities.Here,we discuss its possible clinical development and perspectives for future research.We also summarize the literature on colorectal cancer as well as the anticancer effect of Scutellaria baicalensis Georgi in the last 20 years.As research progresses,the therapeutic effect of baicalin in combating colorectal cancer has gradually been recognized.Its impact on colorectal cancer and anticancer mechanism,such as blocking the tumor cell cycle,inducing tumor cell apoptosis,preventing and treating tumor metastasis and anti-inflammatory activity,have become new hot topics in the study of antitumor agents in Chinese medicine.The present review surveys and summarizes studies of baicalin in anti-colorectal cancer treatment to enhance the understanding of its tumoricidal mechanism and to provide new therapeutic options for colorectal cancer.

Jug-PLGA-NPs, a New Form of Juglone with Enhanced Efficiency and Reduced Toxicity on Melanoma

Objective: To verrify the anti-tumor efficacy and toxicity between juglone(Jug) and Jug-loaded poly lactic-co-glycolic acid(PLGA) nanoparticles(Jug-PLGA-NPs). Methods: Jug-PLGA-NPs were prepared by ultrasonic emulsification. The anti-tumor activity of Jug(2, 3, 4 μg/mL) and Jug-PLGA-NPs(Jug: 2, 3, 4 μg/mL) in vitro was measured by MTT assay and cell apoptosis analysis. The distribution, anti-tumor effect and biological safety in vivo was evaluated on A375 nude mice. Results: With the advantage of good penetration and targeting properties, Jug-PLGA-NPs significantly inhibited proliferation and migration of melanoma cells both in vitro and in vivo(P<0.05 or P<0.01) with acceptable biocompatibility. Conclusions: Jug can inhibit the growth of melanoma but is highly toxic. With the advantage of sustained release, tumor targeting, anti-tumor activity and acceptable biological safety, Jug-PLGA-NPs provide a new pharmaceutical form for future application of Jug.

Electroacupuncture alleviates water avoidance stress-induced irritable bowel syndrome in mice by improving intestinal barrier functions and suppressing the expression of inflammatory cytokines

OBJECTIVE:To evaluate the effects and related mechanisms of electroacupuncture(EA)on irritable bowel syndrome(IBS).METHODS:Male C57BL/6 mice were randomly allocated into normal,model,and EA groups.Experimental IBS mice models were established by exposure to water avoidance stress(WAS).Mice in the EA group were treated with EA at bilateral Tianshu(ST 25)and Zusanli(ST 36)for 7 consecutive days,15 min each day.Abdominal withdrawal reflex(AWR)tests and intestinal motility tests were performed to evaluate visceral sensitivity and intestinal motility of mice.Expression levels of tight junction proteins(TJPs)and inflammatory cytokines in colon tissues were determined through immunofluorescence,real-time polymerase chain reactions(PCR)and Western blot assays.RESULTS:EA alleviated visceral hypersensitivity and intestinal hypermotility in WAS-induced IBS mice.Moreover,EA promoted the expression of zonula occludens(ZO)-1,claudin-1,and occludin while suppressing the expression of interleukin(IL)-8,interferon(IFN)-γ,and tumor necrosis factor(TNF)-αin water avoidance stress(WAS)-induced irritable bowel syndrome(IBS)mice.CONCLUSION:EA alleviated WAS-induced IBS in mice by promoting intestinal barrier functions and suppressing the expression of inflammatory cytokines.
