Communication between sympathetic nerves and the immune system is a crucial and active process during myocardial ischemia(MI), as myocardial damage and inflammatory stimuli concurrently occur. Sympathetic nerves undergo structural and functional changes after MI,leading to adverse left ventricular(LV) remodeling and heart failure(HF). The complex inflammatory response to MI, including local myocardial anti-inflammatory repair and systemic immune reactions, plays a key role in adverse LV remodeling. Here, we review the progressive structural and electrophysiological remodeling of the LV and the involvement of sympathetic tone in complex and dynamic processes that are susceptible to MI pathological conditions. acupuncture has been reported to effectively improve cardiac function, eliminate arrhythmia, and mitigate adverse LV remodeling via somatosensory regulation after MI. Moreover,acupuncture has an anti-inflammatory effect on the pathological process of myocardial ischemia. In this Review, we aim to summarize the involvement of sympathetic nerve activation in the neuro-immune modulation of structural and functional cardiac changes after MI. As a noninvasive method for sympathetic regulation, acupuncture is an ideal option because of its anti-ischemic efficacy. A better understanding of the neural circuitry that regulates cardiac function and immune responses following MI could reveal novel targets for acupuncture treatment.

acupuncture, a therapeutic treatment defined as the insertion of needles into the body at specific points(ie, acupoints), has growing in popularity world-wide to treat various diseases effectively, especially acute and chronic pain. In parallel, interest in the physiological mechanisms underlying acupuncture analgesia, particularly the neural mechanisms have been increasing. Over the past decades, our understanding of how the central nervous system and peripheral nervous system process signals induced by acupuncture has developed rapidly by using electrophysiological methods. However, with the development of neuroscience, electrophysiology is being challenged by calcium imaging in view field, neuron population and visualization in vivo. Owing to the outstanding spatial resolution, the novel imaging approaches provide opportunities to enrich our knowledge about the neurophysiological mechanisms of acupuncture analgesia at subcellular, cellular, and circuit levels in combination with new labeling, genetic and circuit tracing techniques. Therefore, this review will introduce the principle and the method of calcium imaging applied to acupuncture research. We will also review the current findings in pain research using calcium imaging from in vitro to in vivo experiments and discuss the potential methodological considerations in studying acupuncture analgesia.

OBJECTIVE: To investigate the role of satellite glial cells in irritable bowel syndrome(IBS) and the effect of electroacupuncture(EA) at the Tianshu(ST25) and Shangjuxu(ST37) combination. METHODS: A model for visceral hypersensitivity in IBS was induced through colorectal distension(CRD) stimulation. Clean-grade male Sprague-Dawley(SD) rats were randomly divided into four groups: a normal group(NG), a model group(MG), an electroacupuncture group(EA), and a glial cell inhibitor group(FCA). Bilateral EA(2/100 Hz, 1 mA, 30 min) was administered at the Tianshu(ST25) and Shangjuxu(ST37) in week 6. Abdominal withdrawal reflex(AWR) scores were used to assess the behavioral response associated with visceral hyperalgesia, while hematoxylin-eosin staining was employed to evaluate pathological changes in the colon. The protein and mRNA levels of glial fibrillary acidic protein(GFAP) in the colon and colon-related dorsal root ganglion(DRG) were analyzed using immun-ofluorescence, immunohistochemistry, Western blotting, real-time polymerase chain reaction. The impact of EA on electrophysiological properties of colon-related DRG neurons was observed through whole-cell patch clamp analysis. RESULTS: EA significantly reduced the visceral pain behavior scores in rats with IBS in response to graded(20, 40, 60, 80 mm Hg) CRD stimulation. Additionally, EA downregulated the protein and mRNA expression levels of GFAP in the colon and colon-related DRG of rats with IBS. EA also regulated the resting membrane potential, rheobase and action potential of colon-related DRG neurons in rats with IBS. CONCLUSIONS: EA can regulate the excitatory properties of colon-related DRG neurons by downregulating the protein and mRNA expression of GFAP in the colon and colon-related DRG, indicating a potential neurobiological mechanism by which EA relieves visceral hypersensitivity in rats with IBS.

Urinary incontinence (UI) is a common problem *** has a major impact on physical and social activities and interpersonal *** is common in women,but is under-reported and *** affects the quality of life of female patients *** and moxibustion have been proposed as potentially effective interventions for female ***,for the benefit of acupuncture practitioners around the world,the World Federation of acupuncture-moxibustion Societies initiated a project to develop a clinical practice guideline (CPG) for the use of acupuncture and moxibustion to treat female *** CPG was developed according to the Grading of Recommendations Assessment,Development,and Evaluation (GRADE) methodology,referring to the principles of the World Health Organization Handbook for Guideline *** the development of the CPG,the guideline development group (GDG) played an important *** clinical questions,recommendations and therapeutic protocols were all formulated by GDG using the modified Delphi *** CPG contains ten recommendations about the use of acupuncture and moxibustion interventions for ten clinical questions,which include nine conditional recommendations for the intervention and one conditional recommendation for either the intervention or the *** CPG also provides one protocol for conventional filiform needle therapy,two therapy protocols for deep needling stimulation on lumbosacral acupoints,and four moxibustion therapy protocols,based on the protocols presented in randomized controlled trials reviewed by the GDG.

Use of Evidence-Based research Approach in RCTs of acupuncture-Related Therapies for Primary Dysmenorrhea: A Meta-research

Objective To assess the use of evidence-based research(EBR)approach in randomized controlled trials(RCTs)of acupuncture-related therapies for primary dysmenorrhea(PD).Methods PubMed,EMBASE,Cochrane Library,China National Knowledge Infrastructure,Wanfang Database,Chinese Biomedical Database,and China Science and Technology Journal Database were searched from January 2013 to December 2022 for RCTs of acupuncture on *** full text and references of each RCT were read to assess whether systematic reviews(SRs)or other types of studies with similar research questions and end-users’perspectives were cited to justify and design the *** addition,the discussion section were analyzed to evaluate whether trials placed the new result in the existing SRs to draw a *** logistic regression was used to find variables that associated with 3 aspects of EBR approach:(1)citing clinical studies for justification,(2)citing relevant studies that obtain the perspectives of end users,and(3)citing clinical studies for results *** Of 473 RCTs included,45.67%(216)of the trials cited relevant similar studies,21.56%(102)referenced to the studies that collected end-users’perspectives,and 10.99%(52)placed result in the context of the previous *** RCTs appropriately applied EBR *** all the included studies,3.17%(15)of the trials used SRs to inform study questions but none of them used updated SRs with acceptable quality;1.05%(5)of the trials cited SRs of end-user’s perspectives in the justification and design of the study,and only 1 trial added results in existing SR to draw a *** of publication,language,funding,registration,ethical approval and number of sites were significantly associated with 1 of the 3 aspects of EBR *** Few RCTs in acupuncture-related therapies for PD used the EBR approach to minimize research ***,research institutes,funding agencies,ethics committees,journals and peer

It has historically been very difficult to trace the history of the westward transmission of Chinese medicine through the accounts of its protagonists. Many of the early scholars such as Jack Worsley, Dick Van Buren, and Joe Goodman were reluctant to divulge information about the source of their knowledge, or their professional qualifications. Others, such as John Shen and Hong Yuan-bain were early 20th century immigrants who transmitted highly personalized versions of acupuncture and Chinese medicine to select disciples. Eventually, a new class of scholars appeared, including names such as Ted Kaptchuk, Peter Deadman, Nigel Wiseman, William Morris, Peter Eckman, John Mc Donald, Charles Buck, and the late Giovanni Maciocia who looked for answers back in China, developed translation methodologies and terminology, compiled the main textbooks currently in use at TCM colleges, overcame enormous scholastic adversity, developed courses and pursued the regulation and accreditation of TCM in various countries. This special issue synopsizes the path towards the global acculturation of TCM over the last 50 years, the main protagonists, the enormous accomplishments they have achieved for the profession, their philosophy, their clinical perspectives and visions for the future.

acupuncture with different frequencies for tobacco dependence: a randomized controlled trial

OBJECTIVE: To observe the effect of different acupuncture frequencies on the abstinence rate which could be used as a reference for optimizing acupuncture cessation ***: From July 2018 to June 2022， a total of 220 smokers were recruited based on inclusion criteria and randomly divided into the high-frequency acupuncture group (HF group， n = 110): 5 times a week from the 1st to the 4th week， from weeks 5 to 8， three times a week and the low-frequency acupuncture group (LF group， n = 110): 3 times a week from the 1st to the 4th week， from weeks 5 to 8， twice a week， then treated for 8 weeks and follow-up at 1 month in ***: In total， 162 subjects completed the whole study with a drop-out rate of 20.45%. The expiratory CO point abstinence rate was HF group 53/110 (48.18%) vs LF group 41/110 (37.27%) in Week 8 (P = 0.102) and HF group 46/110 (41.82%) vs LF group 45/110 (40.91%) in Week 12 (P = 0.891) and the HF acupuncture and LF acupuncture were nearly equal in the 8-week abstinence rate. In addition， both HF and LF acupuncture significantly reduced Fagerstr?m test for nicotine dependence scale (FTND) scores (P acupuncture showed some superiority over LF acupuncture in relieving patients' smoking cravings (P acupuncture and low-frequency acupuncture treatment were safe and effective on smoking cessation for 8 weeks， but high-frequency acupuncture was advantageous in reducing smoking cravings. More accurate acupuncture frequency needs to be validated through larger clinical studies to optimize acupuncture smoking cessation programs.
