Aims: Although the copper intrauterine device (Cu-IUD) is an effective contraceptive device, cases of pregnancy under the Cu-IUD have been reported. We here report 1. women who got pregnant under this condition, with special reference to etiologies and pregnancy outcomes. Methodology: We analyzed all women who got pregnant under Cu-IUD at ATBEF Main Clinic from July 1. 201. to June 30, 2020. Results: We retrieved 1. patients. The durations of Cu-IUD use were 3 months to 6 years. The etiologies were poor insertion;migration and spontaneous expulsion. The occurrence of pregnancy was poorly accepted by the women: 20% of women considered it was due to the providers’ incompetence. Of the 1. pregnancies, 5 women had given vaginal birth spontaneously;2 induced abortions, 2 spontaneous abortions and 1.ectopic pregnancy. The contraceptive method adopted after pregnancy was jadelle implants in 8 cases and spousal vasectomy in 2 cases. Conclusion: The prevention of IUD pregnancies may require adequate insertion time, insertion technique and follow-up.

Gonadal somatic cells are the main players in gonad development and are important for sex determination and germ cell ***,using a time-series single-cell RNA sequencing(scRNA-seq)strategy,we analyzed fetal germ cells(FGCs)and gonadal somatic cells in human embryos and *** analysis of testes and ovaries revealed several novel cell subsets,including POU5F1.(+)SPARC^(+)FGCs and KRT1.^(+) somatic ***,our data indicated that the bone morphogenetic protein(BMP)signaling pathway plays cell type-specific and developmental stage-specific roles in testis development and promotes the gonocyte-to-spermatogonium transition(GST)in late-stage testicular mitotic arrest ***,testosterone synthesis function transitioned from fetal Sertoli cells to adult Leydig cells in a stepwise *** our study,potential interactions between gonadal somatic cells were systematically explored and we identified cell type-specific developmental defects in both FGCs and gonadal somatic cells in a Turner syndrome embryo(45,XO).Our work provides a blueprint of the complex yet highly ordered development of and the interactions among human FGCs and gonadal somatic cells.

Objective:This study was aimed to determine the changes in CXCR2 expression in preeclampsia placenta and its correlation with clinical ***:Sixty-four gravidas ranging in age from 25 to 42 years referred to the obstetrics unit of the West China Second University Hospital from April 201. to October 201. were recruited in this case-control study;women were diagnosed and divided into early-onset preeclampsia group(n=22),late-onset preeclampsia group(n=22),and healthy pregnancy group(n=20).After immunolocalized in human placenta,the levels of CXCR2 protein and messenger ribonucleic acid(mRNA)were detected by enzyme-linked immunosorbent assay and real-time quantitative polymerase chain *** between placental CXCR2 protein expression with systolic blood pressure and lactate dehydrogenase(LDH)in early-onset preeclampsia were examined using Pearson or Spearman’s correlation ***:Placental CXCR2 protein and mRNA expression in early-onset preeclampsia was significantly lower than it was in placentas from healthy pregnancy pregnancies and late-onset preeclampsia(P1.P<0.05;r=-0.43,P<0.05).Conclusion:Significant abnormal placental CXCR2 expression in early-onset preeclampsia,and its correlations with some clinical parameters(systolic blood pressure and LDH)were discovered,suggesting that CXCR2 may play a role in the pathogenesis of early-onset preeclampsia.

The increasing incidence of morbidly adherent placenta （MAP） is placing women at a higher risk of life-threatening massive hemorrhage. The involvement of interventional radiology to manage this complex condition by performing prophylactic lilac artery balloon occlusion has been reported recently. However, the effectiveness and safety of this technique have not been fully determined. Here we report the case of a 25-year-old woman with placenta increta with preemptive bilateral internal lilac artery balloons who had external lilac artery thrombosis detected by computed tomography angiography （CTA） 72 h post cesarean section. A digital subtraction angiogram （DSA） and intra-arterial thrombolysis were instantly performed followed by supplementary conservative treatments, leading to a desirable resolution, of thrombus without sequela. This is the first report of vascular complications with successful interventional thrombolysis in this setting. Our experience suggests that prophylactic lilac artery balloon occlusion should be used cautiously in cases of MAP and consideration given to minimizing vascular complications given the hypercoagulable state of pregnancy.

Objective: This study was undertaken to analyze bleeding problems with tension-free vaginal tape (TVT) operations in a national registry. Study design: We studied patients for whom increased intraoperative bleeding or reoperation for bleeding/hematoma with TVT operation were reported to the registry. Results: Bleeding problems were reported for 1.1.of 5578 (2.7% )TVT operations. Increased intraoperative bleeding was reported for 1.6 (1.9% ) and reoperation (or conversion) in 45 (0.8% ) patients (both in 5 patients). Increased intraoperative bleeding was managed conservatively in 1.3 patients (95% ). Overall, 45 (0.8% ) patients required reintervention for bleeding or hematoma. The reinterventions comprised 34 laparotomies, 4 transvaginal evacuations of hematomas, 3 revisions for bleeding from a suprapubic catheter site, and 2 revisions of the vaginal incision (details unclear in 1.patient). Bleeding was considered arterial in 1.% (including 1.external iliac artery injury and 1.obturator artery injury) and venous or unknown in 88% . Of reoperated patients, 39% were reoperated within 24 hours, 20% within 2 to 1. days, and 41. within 1. to 56 days after TVT placement. Overall, 1. patients received blood transfusions (range, 1. 1. units). There were no deaths from bleeding complications. Conclusion: Bleeding complications were reported with less than 3% of 5578 TVT operations. Most cases of increased intraoperative bleeding were managed conservatively; 0.8% of patients required conversion or reoperation.

在全球肆虐的严重急性呼吸综合征冠状病毒2(severe acute respiratory syndrome coronavirus 2,SARS-CoV-2)导致了大量人群感染,血管紧张素转化酶2(angiotensin-converting enzyme 2, ACE2)是SARS-CoV和SARS-CoV-2的受体蛋白。然而是否存在SARS-CoV-2的垂直传播仍颇有争议。为了探讨SARS-CoV-2垂直传播的潜在风险,我们利用已发表的单细胞转录组数据观察了围着床期胚胎和母胎界面上ACE2和TMPRSS2(编码跨膜丝氨酸蛋白酶2)的转录表达情况。结果显示,ACE2和TMPRSS2在第6天(D6)围着床期胚胎的滋养外胚层(trophectoderm, TE)细胞、妊娠第8周的合胞滋养层(syncytiotrophoblast at 8 weeks of gestation, STB8W)细胞以及妊娠第24周的绒毛外滋养层(extravillous trophoblasts at 24 weeks of gestation, EVT24W)细胞中均存在明显的共表达现象,表明这些细胞类群可能易被SARS-CoV-2感染。在这三个细胞类群中,ACE2阳性表达细胞相对于阴性细胞在自噬和免疫相关过程中存在差异。尽管ACE2的表达水平在围着床期胚胎中没有表现出性别偏差,但是其在D6 TE、第6天围着床期胚胎的原始内胚层(D6 primitive endoderm, D6 PE)细胞以及ACE2阳性表达的STB细胞中存在明显的性别差异。这表明围着床期发育第6天以及妊娠期胚胎对SARS-CoV-2的易感性可能存在性别差异。我们的结果揭示了胚胎移植过程中,围着床期以及妊娠期胚胎面临着SARS-CoV-2的潜在感染风险。

Pregnancy in patients with Eisenmenger syndrome(ES)is associated with high maternal mortality rates of 30%‒50%,or even up to 65%in the case of a cesarean section(Yuan,201.).Here,we report a case of term pregnancy complicated with ES and severe pulmonary artery hypertension(PAH),which was managed by a multidisciplinary team(MDT)and resulted in an uncomplicated delivery via elective cesarean *** goal of this study is to emphasize the importance of multidisciplinary approach in the management of pregnancy with ES,which can profoundly improve maternal and infant outcomes.

Neural regeneration after spinal cord injury (SCI) closely relates to the microvascular endothelial cell (MEC)- mediated neurovascular unit formation. However, the effects of central nerve system-derived MECs on neovascularization and neurogenesis, and potential signaling involved therein, are unclear. Here, we established a primary spinal cord-derived MECs (SCMECs) isolation with high cell yield and purity to describe the differences with brain-derived MECs (BMECs) and their therapeutic effects on SCI. Transcriptomics and proteomics revealed differentially expressed genes and proteins in SCMECs were involved in angiogenesis, immunity, metabolism, and cell adhesion molecular signaling was the only signaling pathway enriched of top 1. in differentially expressed genes and proteins KEGG analysis. SCMECs and BMECs could be induced angiogenesis by different stiffness stimulation of PEG hydrogels with elastic modulus 50-1.50 Pa for SCMECs and 50-300 Pa for BMECs, respectively. Moreover, SCMECs and BMECs promoted spinal cord or brain-derived NSC (SNSC/BNSC) proliferation, migration, and differentiation at different levels. At certain dose, SCMECs in combination with the NeuroRegen scaffold, showed higher effectiveness in the promotion of vascular reconstruction. The potential underlying mechanism of this phenomenon may through VEGF/AKT/eNOS- signaling pathway, and consequently accelerated neuronal regeneration and functional recovery of SCI rats compared to BMECs. Our findings suggested a promising role of SCMECs in restoring vascularization and neural regeneration.
