AiM:To compare success rates and complications of Densiron 68 and 1000cSt silicone oil (SO) in the management of rhegmatogenous retinal detachment (RRD) with inferior breaks (iBs). METHODS:Totally 61 eyes of 61 consecutive patients with RRD with iBs were assigned to pars plana vitrectomy (PPV) with Densiron (n =31) or PPV with SO (n=30) in order of presentation. SO and Densiron removal was performed 3 months after initial surgery. Follow up visits were terminated 6 months after SO removal. ·RESULTS:With a single operation, the Densiron group showed 84% and SO 74% reattachment. With further surgery, both groups showed 90% reattachment. Complications such as cataract, raised intraocular pressure (iOP), inflammatory reaction, macular epiretinal membranes, and emulsification of SO were seen in both groups. CONCLUSiON:Densiron and SO are found to have similar success rates and complications.

The risk factors for the transmission of hepatitis C virus(HCV)infection varies substantially between countries and geographic *** aim of this investigation was to determine the risk factors which may be involved in the transmission of HCV infection in the Turkish *** study included patients who were admitted to the Department of Gastroenterohepatology,istanbul Medical Faculty,istanbul University,between 1996 and 2002 and found to be anti-HCV positive during hospitalization or during follow-up as *** patients were asked about risk factors for HCV transmission including transfusion,history of operation,hospitalization,hemodialysis,intravenous drug use,suspected sexual contact,tattooing,acupuncture,dental procedures,manicure and pedicure,blood brotherhood rituals,perinatal risk factors,common circumcision rituals,and history of *** our study,total of 320 patients with anti-HCV seropositivity were *** numbers and percentages of male and female patients were 139(43.4%)and 181(56.6%),*** mean age of the patients was 49.7 ±12.4 years(range:18-73 years).HCV-RNA was found to be positive in 297(92.8%)*** most common risk factor was a history of surgery(305;98%),and the second most common was blood transfusion(123;39.7%).The numbers and percentages of patients for the other risk factors were as follows:dental procedure,86(27.5%);abortion,66(21.2%);long-term hospitalization,37(11.6%);hemodialysis,31(10%);history of jaundice,15(4.6%);history of intravenous drug abuse,10(3.1%);history of suspected sexual contact,5(1.5%);history of manicure and pedicure,4(1.2%);history of occupational transmission,3(0.9%);history of tattooing,2(0.6%);history of acupuncture,2(0.6%);circumcision in a common circumcision ritual,1(0.3%);and percutaneous needle puncture,1(0.3%).None of the patients had a history of blood brotherhood ritual or perinatal *** one risk factor was detected in 73(22.8%)patients,two risk fac

Gastrointestinal endoscopy has a major diagnostic and therapeutic role in most gastrointestinal disorders; however, limited information is available about clinical efficacy and safety in pregnant patients. The major risks of endoscopy during pregnancy include potential harm to the fetus because of hypoxia, premature labor, trauma and teratogenesis. in some cases, endoscopic procedures may be postponed until after delivery. When emergency or urgent indications are present, endoscopic procedures may be considered with some precautions. United States Food and Drug Administration category B drugs may be used in low doses. Endoscopic procedures during pregnancy may include upper gastrointestinal endoscopy, percutaneous endoscopic gastrostomy, sigmoidoscopy, colonoscopy, enteroscopy of the small bowel or video capsule endoscopy, endoscopic retrograde cholangiopancreatography and endoscopic ultrasonography. All gastrointestinal endoscopic procedures in pregnant patients should be performed in hospitals by expert endoscopists and an obstetrician should be informed about all endoscopic procedures. The endoscopy and flexible sigmoidoscopy may be safe for the fetus and pregnant patient, and may be performed during pregnancy when strong indications are present. Colonoscopy for pregnant patients may be considered for strong indications during the second trimester. Although therapeutic endoscopic retrograde cholangiopancreatography may be considered during pregnancy, this procedure should be performed only for strong indications and attempts should be made to minimize radiation exposure.

Simulation can be defined as malingering, or sometimes functional visual loss (FVL). it manifests as either simulating an ophthalmic disease (positive simulation), or denial of ophthalmic disease (negative simulation). Conscious behavior and compensation or indemnity claims are prominent features of simulation. Since some authors suggest that this is a manifestation of underlying psychopathology, even conversion is included in this context. in today's world, every ophthalmologist can face with simulation of ophthalmic disease or disorder. in case of simulation suspect, the physician's responsibility is to prove the simulation considering the disease/disorder first, and simulation as an exclusion. in simulation examinations, the physician should be firm and smart to select appropriate test(s) to convince not only the subject, but also the judge in case of indemnity or compensation trials. Almost all ophthalmic sensory and motor functions including visual acuity, visual field, color vision and night vision can be the subject of simulation. Examiner must be skillful in selecting the most appropriate test. Apart from those in the literature, we included all kinds of simulation in ophthalmology. in addition, simulation examination techniques, such as, use of OCT (optical coherence tomography), frequency doubling perimetry (FDP), and modified polarization tests were also included. in this review, we made a thorough literature search, and added our experiences to give the readers up-to-date information on malingering or simulation in ophthalmology.

Celiac disease can be triggered by upper abdominal surgery, such as vagotomy, oesophagectomy, pancreatico-duodenectomy, and gastrojejunal anastomosis. Here we report a case of a 24 year-old woman who developed celiac disease after an ileal resection for perforated Meckel's diverticula. This is the first reported celiac case that has been triggered, not by upper abdominal surgery, but after ileal resection for Meckel's diverticula.

Folate deficiency and its association with cancer have been studied in the literature, but its clinical impact is still unknown. Folate deficiency and its result on gastric cancer is a mysterious part of oncology, with ongoing studies hopefully clarifying its impact on gastric cancer management. Lee et al studied folate deficiency and its impact on staging and clinical results. Here we try to contribute to the field by expressing our own thoughts about the paper.

AiM: To evaluate the effect of topical dorzolamide-timolol fixed combination prophylaxis on short term intraocular pressure(iOP) changes in patients who had intravitreal bevacizumab ***: One hundred and fifty one eyes of 151 patients which were followed up in retina clinic in Ulucanlar Eye Training and Research Hospital were evaluated in this study. Patients were divided into two groups. Group 1 consists of 75 patients who had topical dorzolamid-timolol medication two hours before injection;while Group 2 consists of 76 patients without prophylaxis. Demographic data, iOP measurements prior to the injection and one, thirty and sixty minutes and twenty-four hours after the injection were recorded. The data were analyzed using SPSS software version 15.0(SPSS inc., Chicago, iL, USA).RESULTS: There were no significant difference between two groups in age, gender distrubition and indications for injections. The mean iOPs in Groups 1 and 2 prior to the injection(T0) were 17.84±0.43 and 18.15±0.43 mm Hg,one minute after the injection(T1) were 29.75 ±1.6 and34.44 ±1.59 mm Hg, 30 minutes after the injection(T30)were 20.06 ±0.6 and 21.71 ±0.59 mm Hg respectively. The mean iOPs were 18.26 ±0.56 mm Hg in Group 1 and19.78 ±0.56 mm Hg in Group 2 sixty minutes after the injection(T60). All iOP values after the injection were compared between two groups, there was a significant difference between two groups only on T1; one minute after the injection(P =0.04). There were a statisciallysignificant difference between the baseline values and other recorded values; except on T60, in Groups 1 and 2(P iON: After intravitreal bevacizumab injection;we observe a transient iOP elevation which normalizes about one hour after intravitreal injection. in patients who had topical dorzolamid-timolol combination prophylaxis before injections, a significant decrease is seen in iOP spikes due to this injection. ·AiM:To study the relationship between corneal white-to- white (WTW) distance and mesopic pupil diameter. ·METHODS:This study is composed of 30 cases that underwent photorefractive keratotomy (PRK). Pupil size measurements were performed with Schwind ORK wavefront analyzer in mesopic conditions. WTW distance was measured with a measuring caliper. Also, A-scan ultrasound examination was performed in all patients. The relationship among the mesopic pupil diameter and age, sex, axial length, lens thickness, anterior chamber depth(ACD), horizontal WTW distance, vertical WTW distance, spherical equivalent, and average keratometry were analyzed with univariate and multivariate regression analysis. ·RESULTS:Mean pupil diameter was (6.39±0.80)mm (range:3.70mm to 7.73mm. Horizontal WTW distance measurements were between 11.00mm and 12.50mm and mean horizontal WTW distance was (11.79±0.43)mm. On the other hand, vertical WTW distances ranged between 10.00mm and 13.00mm, and their mean was(11.42±0.72)mm. Bivariate correlation between pupil diameter and other variables showed that the axial length, ACD, spherical equivalent, and horizontal WTW distance had a moderate correlation with mesopic pupil diameter. Multiple regression analysis revealed that spherical equivalent and horizontal WTW distances were significantly associated with mesopic pupil diameter (R =0.598, R 2=0.358 P =0.02). ·CONCLUSiON:This study shows that mesopic pupil diameter is closely related to horizontal WTW distance. These two factors must be taken in consideration together in preoperative ablation zone planning.
