Evaluation of PIMA Point-of-care CD4 Analyzer in Yunnan, China

Background： CD4 count is used to determine antiretroviral therapy （ART） eligibility. In China, flow cytometers are mostly located in urban areas with limited access by patients residing in remote areas. In an attempt to address this issue, we conducted a study to validate the performance ofAlere PIMA point-of-care CD4 analyzer. Methods： venous and finger-prick blood specimens were collected from HIV-positive participants from two voluntary counseling and testing sites in Yunnan Province. Both venous and finger-prick blood specimens were tested with the PIMA analyzer. venous blood specimens tested with the Becton Dickinson FACSCalibur were used as a reference. Results： venous specimens from 396 and finger-prick specimens from 387 persons were available for analysis. CD4 counts by PIMA correlated well with those from FACSCalibur with an R2 of 0.91 for venous blood and 0.81 for finger-prick blood. Compared to FACSCalibur, the PIMA analyzer yielded lower counts with a mean bias of- 47.0 cells/μl （limit of agreement, [LOA]： -204-110 cells/μl） for venous blood and -71.0 cells/μl （LOA： -295-153 cells/μl） for finger-prick blood. For a CD4 threshold of 350 cells/μl, the positive predictive value （PPV） of PIMA was 84.2% and 75.7% and the negative predictive value （NPV） was 97.6% and 95.8% for venous and finger-prick blood, respectively. For an ART threshold of 500 cells/μl, the corresponding PPV was 90.3% and 84.0% and NPV was 94.3% and 93.4%, respectively. Conclusions： CD4 counting using venous blood with PIMA analyzers is a feasible alternative to a large flow cytometer to determine ART eligibility.

Polymorphism of apolipoprotein E gene and post-stroke vascular dementia

BACKGROUNO： Studies have indicated that the more certain etiological factors of vascular dementia are the sites, size and number of cerebrovascular lesions, but there are arguments all the time in the relationship of genetic factors, especially apolipoprotein E gene, with the occurrence and development of vascular dementia. OBJECTIVE： To analyze the relationship between the polymorphism of apolipoprotein E gene and vascular dementia in stroke patients. OESIGN ： A non-randomized grouped comparative observation at the same time SETTING ： The Laboratory of Neurology, the Second Affiliated Hospital of Chongqing University of Medical Sciences. PARTICIPANTS： Totally 121 inpatients with stroke were selected from the Department of Neurology, the Second Affiliated Hospital of Chongqing University of Medical Sciences from January 2000 to December 2001. All the patients were accorded with the diagnostic standards for cerebrovascular diseases set by the Second National Academic Meeting for Cerebrovascular Disease, and confirmed by cranial CT or MRI. According to with vascular dementia or not, they were divided into vascular dementia group （n =58） and non-vascular dementia group （n =63）. In the vascular dementia group, there were 37 males and 21 females, the average age was （59＋8） years. They were all in accordance with the standard of the third edition of Diagnostic and Statistical Manual of Mental Disorders （DMS-Ⅲ）, without conscious disturbance and language disorder; hospitalized within 3 days after the attack of cerebrovascular disease. In the non-vascular dementia group, there were 37 males and 26 females, the average age was （59±9.5） years. They all had no intellectual disturbance within 3 months after the attack of cerebrovascular disease. All the subjects agreed to supply blood samples for the experiment. Serum level changes of insulin-like growth factor-1 and amino acids in children with cerebral palsy following functional exercise plus head acupuncture therapy

BACKGROUND： In the past few years, there were many studies about the pathophysiology of insulin-like growth factor-1 （IGF-1）, as well as glutamic acid （GLU） and γ-aminobutyric acid （GABA） in hypoxic-ischemic brain damage （HIBD）. IGF-1 plays a protective role in brain damage. The over release of excitatory amino acids （EAA） plays an important role in acute neuronal death, which delays neuronal death. The disproportion of increasing of excitatory and inhibitory amino acids can cause different extent HIBD. However, there is rare report about the change of IGF-1 and neurotransmitter level in serum of cerebral palsy （CP） children. OBJECTIVE ： To observe the levels of serum IGF-1, GABA and GLU before and after functional exercise plus head acupuncture therapy and single functional exercise, then study the effective mechanism of IGF-1, GABA and GLU in the occurrence and development of CP, and compare those with normal control group. DESIGN： Case-control study SETTING： College of Rehabilitation Medicine, Jiamusi University, Prevention and Treatment Center of Child Cerebral Palsy in Heilongjiang Province. PARTICIPANTS： CP group： Sixty CP children came from Prevention and Treatment Center of Child Cerebra Palsy in Heilongjiang Province between April 2005 and March 2006 were selected in this study. All the cases were consistent with the diagnostic criteria and the type of cerebral palsy and finally diagnosed with CT and MRI examinations. There were 35 males and 25 females aged from 8 months to 4 years with the mean age o （2.0±0.5） years. And then, they were randomly divided into two groups. Twenty-six cases were received functional exercise, and the other 34 cases were treated with functional exercise and head acupuncture. Level of serum neuron-specific enolase and brain damage in children with febrile seizures

BACKGROUND ： Febrile seizure （FS） has good prognosis in the majority of cases. But there is an ongoing debate on the relationship between complicated febrile seizure （CFS） and later development of temporal lobe epilepsy （TLE） due to hippocampal sclerosis.OBJECTIVE： To evaluate the level of serum neuron-specific enolase （S-NSE） of children with simple febrile seizure （SFS） and complicated febrile seizure and compare with children with non-FS respiratory tract infection. DESIGN： Contrast observation.SETTING ： Department of Pediatrics and Department of Laboratory, Fujian Provincial Hospita. PARTICIPANTS： Forty-nine patients who were admitted to Department of Pediatrics of Fujian Provincial Hospital from June 2002 to September 2003 with FS were enrolled in this study. There were 28 boys and 21 girls aged from 5 to 72 months. All children were divided into 2 groups based on frequency and duration. Thirty-two children whose FS occurred within 24 hours （lasting shorter than single and twice durations and also shorter than 10 minutes） were regarded as SFS group; meanwhile, 17 children whose FS occurred within 24 hours （lasting longer than single and twice durations and also longer than 10 minutes） were regarded as CFS group. Another 23 patients who were admitted to our hospital with respiratory tract infection in the same period, without the history and positive symptoms/features of neurological dysfunction, were enrolled as control group. There were 13 boys and 10 girls aged from 5 months to 86 months. All parents were told the facts. METHODS： 2 mL venous blood was collected from FS children within 24 hours after the seizures. Meanwhile, 2 mL venous blood was also collected from children in the control group. Level of S-NSE was measured with enzyme immunoassay procedure, and differences among groups were compared with ttest.
