Very recently,unusual thrombotic events in combination with severe thrombocytopenia have been reported 1–2 weeks following SARS-CoV-2 vaccination with ChAdOx1 nCov-19(AstraZeneca).This condition,termed VITT(vaccine-induced immune thrombotic thrombocytopenia),has been related to high risk of fatal outcome with both ischaemic and haemorrhagic complications.12 Optimal treatment strategies still need to be elucidated—especially in case of cerebral sinus venous thrombosis(CSVT)with associated brain haemorrhage as the underlying thrombotic event.We here report our clinical experience with two young women diagnosed with VITT-associated CSVT,treated with high-dose intravenous immunoglobulins(IVIGs),corticosteroids and argatroban in the hyperacute phase,followed by dabigatran resulting in excellent outcome.

To the Editor:To control the coronavirus disease 2019(COVID-19)pandemic and reduce the complications and deaths resulting from the transmission of the disease,a variety of COVID-19 vaccines,such as mRNA vaccines,adenovirus-vector vaccines,inactivated viral vaccines,and others,have been licensed for emergency use.Most of these vaccines were rolled out in different countries following the successful completion of phase 3 clinical trials.Scale-up of vaccine production and vaccination have provided notable protection to large populations,although some vaccines may cause side effects.Furthermore,as the severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)has mutated and spread further over time,some notable novel variants have emerged,but whether these variants are vulnerable to currently available vaccines is yet to be determined.

Dear Editor,The emergence of Omicron subvariants has required additional boosters to induce a highly neutralizing antibody response.1 However,the new Omicron subvariants BQ.1 and XBB,evade the humoral response induced by multiple mRNA vaccinations and infections.2,3 CansinoBio(Ad5-nCoV)is a single-dose adenovirus vector-based vaccine with high protection against severe disease4 but lower efficacy and antibody response than mRNA vaccines.Fortunately,hybrid immunity induced by infection plus vaccine or heterologous vaccines has produced a robust neutralizing antibody response.

The digitalization of healthcare-related information service systems has become a trend across the world.However,several crucial services are still provided manually due to a lack of trust in digital solutions.One such service is keeping records of children’s vaccination,which still relies on a paper-based file system in most parts of the world.This approach causes serious data integrity problems.Recently,healthcare has become a potential application area of the blockchain,as it can preserve and protect highly sensitive private medical records while sharing these records in a decentralized manner without losing personal ownership.Therefore,we propose a new digital model to track a child’s vaccination records using blockchain.In particular,this proposed application helps improve the vaccination record-keeping process by ensuring the integrity of the preserved data in a more secure way.In an emerging pandemic situation,our approach can be extended to manage the overall vaccination process effectively.

BACKGROUND After vaccination was mandated worldwide,various adverse effects associated with the coronavirus disease 2019(COVID-19)vaccination,including shoulder pain,have been reported.Here,we report a case of new-onset shoulder pain after BNT162b2(Comirnaty,Pfizer-BioNTech)mRNA vaccination.CASE SUMMARY A 50-year-old man visited our rehabilitation center with left shoulder range of motion(ROM)limitation that had persisted for more than 5 mo.The history included no specific noteworthy events,except vaccination.The pain in the patient’s left deltoid muscle appeared 1 day after the second BNT162b2 vaccination and intensified to severe pain.The patient self-administered aspirin,with which the pain subsided immediately,whereas ROM limitation persisted.At the first visit,the patient complained of dull pain and ROM restriction of the left shoulder(flexion 130°,abduction 110°,and external rotation 40°).Among the diagnostic studies conducted for the evaluation of the shoulder,magnetic resonance imaging showed a thickened coracohumeral ligament.Nerve conduction studies and needle electromyography showed no electrodiagnostic abnormalities.The patient received comprehensive rehabilitation for 7 mo and had an overall improvement in pain and ROM of the left shoulder.CONCLUSION In this case of severe shoulder pain after COVID-19 vaccination that subsided immediately with aspirin treatment,the exact cause and mechanism of pain are unclear.However,the clinical symptoms and diagnostic workups in our report suggest the possibility that the COVID-19 vaccination triggered an immunochemical response that resulted in shoulder pathology.

In this paper,a deterministic compartmental model is presented to assess the impact of vaccination and non-pharmaceutical interventions(social distance,awareness,face mask,and quarantine)on the transmission dynamics of COVID-19 with co-morbidity and reinfection.An expression for the basic reproduction number is then derived for this model.Theoretical analysis shows that the model exhibits backward bifurcation phenomenon when the basic reproduction number is less than unity.But for the case of no reinfection,the model has a globally asymptotically stable disease-free equilibrium(DFE)when the basic reproduction number is less than unity.Furthermore,it is shown that in the case of no re-infection,a unique endemic equilibrium point(EEP)of the model exists which is globally asymptotically stable whenever the reproduction number is greater than unity.From the global sensitivity and uncertainty analysis,we have identified mask coverage,mask efficacy,vaccine coverage,vaccine efficacy,and contact rate as the most influential parameters influencing the spread of COVID-19.Numerical simulation results show that the use of effective vaccines with proper implementation of non-pharmaceutical interventions could lead to the elimination of COVID-19 from the community.Numerical simulations also suggest that the control strategy that ensures a continuous and effective mass vaccination program is the most cost-effective control strategy.The study also shows that in the presence of any co-morbidity and with the occurrence of re-infection,the disease burden may increase.

BACKGROUND Coronavirus disease 2019(COVID-19)is one of the current global public health threats and vaccination is the most effective tool to reduce the spread and decrease the severity of COVID-19.Diabetes is one of the important chronic diseases threatening human health and is a common comorbidity of COVID-19.What is the impact of diabetes on the immunization effect of COVID-19 vaccination?Conversely,does vaccination against COVID-19 exacerbate the severity of pre-existing diseases in patients with diabetes?There are limited and conflicting data on the interrelationship between diabetes and COVID-19 vaccination.AIM To explore the clinical factors and possible mechanisms underlying the interaction between COVID-19 vaccination and diabetes.METHODS We conducted a comprehensive search of PubMed,MEDLINE,EMBASE,and Reference Citation Analysis(https://***)online datab-ases,and medRxiv and bioRxiv gray literature using the keywords"SARSCoV-2","COVID-19","vaccine","vaccination","antibody",and"diabetes"individually or in combination,with a cut-off date of December 2,2022.We followed inclusion and exclusion criteria and after excluding duplicate publications,studies with quantifiable evidence were included in the full-text review,plus three manually searched publications,resulting in 54 studies being included in this review.RESULTS A total of 54 studies were included,from 17 countries.There were no randomized controlled studies.The largest sample size was 350963.The youngest of the included samples was 5 years old and the oldest was 98 years old.The included population included the general population and also some special populations with pediatric diabetes,hemodialysis,solid organ transplantation,and autoimmune diseases.The earliest study began in November 2020.Thirty studies discussed the effect of diabetes on vaccination,with the majority indicating that diabetes reduces the response to COVID-19 vaccination.The other 24 studies were on the effect of vaccination on diabetes,which included 18 case reports/series.Most of

The COVID-19 pandemic provides an opportunity to explore the impact of government mandates on movement restrictions and non-pharmaceutical interventions on a novel infection,and we investigate these strategies in early-stage outbreak dynamics.The rate of disease spread in South Africa varied over time as individuals changed behavior in response to the ongoing pandemic and to changing government policies.Using a system of ordinary differential equations,we model the outbreak in the province of Gauteng,assuming that several parameters vary over time.Analyzing data from the time period before vaccination gives the approximate dates of parameter changes,and those dates are linked to government policies.Unknown parameters are then estimated from available case data and used to assess the impact of each policy.Looking forward in time,possible scenarios give projections involving the implementation of two different vaccines at varying times.Our results quantify the impact of different government policies and demonstrate how vaccinations can alter infection spread.

BACKGROUND The clinical efficacy and safety of vaccination against novel coronavirus disease 2019(COVID-19)in patients with cirrhosis have not been evaluated yet.AIM To evaluate the clinical efficacy and safety of vaccination against COVID-19 in patients with cirrhosis.METHODS This was a retrospective cohort study of patients with cirrhosis.The first cohort included patients vaccinated with Gam-COVID-Vac(Sputnik V);the second one consisted of unvaccinated controls.RESULTS The study included 89 vaccinated patients and 148 unvaccinated ones.There were 4 cases of COVID-19 in the vaccinated group and 24 cases in the unvaccinated group(P=0.035).No severe cases of COVID-19 were revealed in the vaccinated group,while there were 12 ones in the unvaccinated group(P=0.012)with 10 deaths detected(P=0.012).The vaccine efficacy was 69.5%(95%confidence interval[CI]:18.5%-94.4%)against symptomatic cases of COVID-19,100%(95%CI:25.1%-100.0%)against severe cases,and 100%(95%CI:1.6%-100.0%)against death associated with COVID-19.The efficacy of full vaccination with revaccination against symptomatic cases of COVID-19 was 88.3%(95%CI:48.0%-99.6%).The overall mortality rate was higher in the unvaccinated group than in the vaccinated group(17.1%vs 3.0%;P=0.001).Higher Child-Turcotte-Pugh class cirrhosis(hazard ratio[HR]=4.13,95%CI:1.82-9.35)and higher age(HR=1.08,95%CI:1.04-1.15)were independent predictors of overall mortality,while vaccination had a protective effect(HR=0.09,95%CI:0.01-0.76).There was no significant difference in liver-related mortality(P=0.135)or the incidence of liver decompensation(P=0.077),bleeding esophageal varices(P=0.397),and vascular events(P=0.651)between the two groups of patients.CONCLUSION vaccination against COVID-19 in patients with cirrhosis is effective and safe.
