BACKGROUND Older men are more vulnerable to fatal falls than women,and gait disturbances contribute to the risk of falls.Studies have assessed the association between arterial stiffness and gait dysfunction,but the results have been inconclusive.This study aimed to conduct a cross-sectional analysis to evaluate the association between brachial–ankle pulse wave velocity(baPWV)and gait assessment in older men.METHODS Data from the 2014–2015 Korea Institute of Sport Science Fitness Standards project were used for the analysis.The inclusion criteria were men aged>65 years with gait assessment[the 30-s chair stand test(30s-CST),the timed up and go(TUG)test,the figure-of-8 walk(F8W)test,the 2-min step test(2MST),and the 6-min walk test(6MWT)]and baPWV measurement data.Generalized linear regression analysis was conducted with multiple confounding factor adjustments,including lower extremity isometric strength.RESULTS A total of 291 participants were included in the analysis.The mean age was 71.38±4.40 years.The mean values were as follows:(1)30s-CST,17.48±5.00;(2)TUG test,6.01±1.10 s;(3)F8W test,25.65±4.71 s;(4)2MST,102.40±18.83 per 2 min;and(5)6MWT,500.02±85.65 m.After multivariable adjustment,baPWV was associated with the 6MWT(β=−0.037,95%CI:−0.072–−0.002),TUG test(β=0,95%CI:0.000–0.001),and F8W test(β=0.002,95%CI:0.000–0.004).baPWV was not associated with the 30s-CST and 2MST.CONCLUSIONS The current study showed a statistically significant association between gait assessments and arterial stiffness,independent of lower extremity strength.However,this association was modest.Future prospective studies are needed to elucidate the complex relationship between arterial stiffness and gait dysfunction.

To the Editor:Systemic lupus erythematosus(SLE)is an autoimmune disease that tends to affect females of childbearing age.There are nearly 4 million people in China currently suffering from SLE,which can cause multiple organ damage in its early stages and seriously affect patients’quality of life.[1-3]Menstrual disorders are common in SLE patients and vary from heavy menstrual bleeding to amenorrhea.The etiology of menstrual irregularities includes the disease activity itself,glucocorticoids(GCs)treatment,and the use of some immunosuppressants.[4]This multicenter,cross-sectional,web-based observational study investigated menstrual irregularity,pregnancy outcomes,and birth outcomes on SLE patients across China.The research was approved by the Ethics Committee of Peking University People’s Hospital,China(No.2019PHB253-01).The patients were informed that consent was implied with completion of the questionnaire,and the process was anonymous and voluntary.

To the Editor:Augmented renal clearance(ARC)refers to an enhanced elimination of circulating solutes,including drugs,by the kidneys at a rate statistically higher than normal.On the first day of intensive care unit(ICU)admission,ARC occurrence may predict continued creatinine clearance(CrCl)elevation for 1 day to several weeks.[1]ARC has a substantial adverse effect on renally cleared antibiotics'pharmacokinetics/pharmacodynamics.Treatment failure or worse outcomes are more frequent in ARC patients.Thus,a"one size fits all"approach to drug dosing in ARC patients is inappropriate;dosing requires adjustment for some variables.

BACKGROUND Epidemiologic studies have explored the association between a single cardiovascular risk factor(CVRF)and resting heart rate(RHR),but the research on the relation of multiple risk factors with RHR remains scarce.This study aimed to explore the associations between CVRFs clustering and the risk of elevated RHR.METHODS In this cross-sectional study,adults aged 35-75 years from 31 provinces were recruited by the China PEACE Million Persons Projects from September 2015 to August 2020.We focused on seven risk factors:hypertension,diabetes mellitus,dyslipidemia,obesity,smoking,alcohol use,and low physical activity.Multivariate logistic regression was used to calculate odds ratios(OR)for elevated RHR(>80 beats/min).RESULTS Among 1,045,405 participants,the mean age was 55.67±9.86 years,and 60.4%of participants were women.The OR(95%CI)for elevated RHR for the groups with 1,2,3,4 and≥5 risk factor were 1.11(1.08-1.13),1.36(1.33-1.39),1.68(1.64-1.72),2.01(1.96-2.07)and 2.58(2.50-2.67),respectively(Ptrend<0.001).The association between the CVRFs clustering number and elevated RHR was much more pronounced in young males than in other age-sex subgroups.Clusters comprising more metabolic risk factors were associated with a higher risk of elevated RHR than those comprising more behavioral risk factors.CONCLUSIONS There was a significant positive association between the CVRFs clustering number and the risk of elevated RHR,particularly in young males.Compared clusters comprising more behavioral risk factors,clusters comprising more metabolic risk factors were associated with a higher risk of elevated RHR.RHR may serve as an indicator of the cumulative effect of multiple risk factors.

To the Editor:Cognitive impairment in older adults is becoming a more prominent issue as the population ages,affecting their social functioning and quality of life.The prevalence of mild cognitive impairment(MCI),as determined in population-based epidemiological studies,ranges from 3%to 19%in adults>65 years.[1]Many risk factors for cognitive decline have been identified,with research identified hearing loss(HL)as an independent risk factor.Age-related hearing loss(ARHL),the third leading cause of chronic disability in older adults,predisposes individuals to cognitive impairment and dementia.

Stroke imposes a substantial burden worldwide.With the rapid economic and lifestyle transition in China,trends of the prevalence of stroke across different geographic regions in China remain largely unknown.Capitalizing on the data in the National Health Services Surveys(NHSS),we assessed the prevalence and risk factors of stroke in China from 2003 to 2018.In this study,data from 2003,2008,2013,and 2018 NHSS were collected.Stroke cases were based on participants’self-report of a previous diagnosis by clinicians.We estimated the trends of stroke prevalence for the overall population and subgroups by age,sex,and socioeconomic factors,then compared across different geographic regions.We applied multivariable logistic regression to assess associations between stroke and risk factors.The number of participants aged 15 years or older were 154,077,146,231,230,067,and 212,318 in 2003,2008,2013,and 2018,respectively,among whom,1435,1996,3781,and 6069 were stroke patients.The age and sex standardized prevalence per 100,000 individuals was 879 in 2003,1100 in 2008,1098 in 2013,and 1613 in 2018.Prevalence per 100,000 individuals in rural areas increased from 669 in 2003 to 1898 in 2018,while urban areas had a stable trend from 1261 in 2003 to 1365 in 2018.Across geographic regions,the central region consistently had the highest prevalence,but the western region has an alarmingly increasing trend from 623/100,000 in 2003 to 1898/100,000 in 2018(Ptrend<0.001),surpassing the eastern region in 2013.Advanced age,male sex,rural area,central region,hypertension,diabetes,depression,low education and income level,retirement or unemployment,excessive physical activity,and unimproved sanitation facilities were significantly associated with stroke.In conclusion,the increasing prevalence of stroke in China was primarily driven by economically underdeveloped regions.It is important to develop targeted prevention programs in underdeveloped regions.Besides traditional risk factors,more attention should be paid to nontraditional risk factors

Objective The multidisciplinary antenatal diagnosis staff bring together practitioners who are involved in the management of the antenatal period at birth.This project was designed following the French experience to institute multidisciplinary consultation meetings for prenatal diagnosis(MCMPD)in Benin.Methods This is a descriptive cross-sectional study examining the knowledge,attitudes,and practices of Beninese practitioners in the field of MCMPD to develop the pilot phase of the project.Results We collected 108 participants from different specialties.Pediatricians and pediatric surgeons were in the majority at 23.9% and 16.5%,respectively.Sixty-seven percent of participants were from the public sector(n=75).One practitioner felt that it was not a good idea to implement these meetings.Almost all staff(96.2%)agreed that this meeting would have a positive impact on reducing neonatal mortality.Omphalocele(58.5%),spina bifida aperta(43.6%),and gastroschisis(34%)were the most commonly diagnosed antenatal conditions in Benin.No neonatal pathology required medical termination of the pregnancy according to 35.6% of the participants.Conclusions The objective of reducing infant mortality due to medical and surgical pathologies is a noble one and deserves to be supported.This innovative project,developed through this study,the first of its kind in the subregion,will contribute inexorably to the achievement of the third Goal Sustainable Development.

Background Family environments can shape children’s personalities and social networks,rendering distinguishing adverse childhood experiences(ACEs)from family and society essential,but related evidence remains limited.Aims This cross-sectional study aimed to investigate the correlations between intrafamilial and social ACEs,their associations with depressive symptoms and cognitive impairment and the(education-moderated)mediating role of social ACEs.Methods Data for this cross-sectional study were from the China Health and Retirement Longitudinal Study.Nine intrafamilial(0,1,2,3,and 4 or more)and three social(0,1,and 2 or more)ACEs were identified.Depressive symptoms were assessed using the 10-item Center for Epidemiological Studies Depression Scale.Global cognition,including episodic memory and mental intactness,was calculated as z scores.Binary and ordered logistic regressions,generalised linear models with Gaussian family and identity link,and mediation analysis were used.Results 13435 participants aged 59.0(51.0–66.0)were included.Compared with participants with no intrafamilial ACEs,those with 1,2,3,and 4 or more intrafamilial ACEs tended to develop more social ACEs,with odds ratios(ORs)of 1.55(95%confidence interval(CI):1.36 to 1.76),2.36(95%CI:2.08 to 2.68),3.46(95%CI:3.02 to 3.96)and 6.10(95%CI:5.30 to 7.02),respectively.Both intrafamilial and social ACEs were associated with depressive symptoms(OR>3 for four or more intrafamilial ACEs and two or more social ACEs)and global cognition(β=−0.26 for four or more intrafamilial ACEs andβ=−0.29 for two or more social ACEs).Social ACEs mediated the associations of intrafamilial ACEs with depressive symptoms and global cognition by 12.3%and 13.1%,respectively.Furthermore,as education levels increased,the impact of intrafamilial ACEs on depressive symptoms was increasingly mediated through social ACEs,while the mediating role of social ACEs between intrafamilial ACEs and cognitive impairment gradually diminished.Conclusions Improving children’s social environments an
