Hoarseness of voice is a common symptom of many laryngeal and extralaryngeal conditions. It is very crucial to identify the culprit behind it, keeping in mind all the various differentials. We re-port a case of a 35-year-old woman presenting with complaint of hoarseness of voice. Upon car-diac evaluation, the patient was diagnosed with bicuspid aortic valve, with severe aortic stenosis and dilation of ascending aorta. We suspected that the plausible reason for hoarseness of voice was compression of recurrent laryngeal nerve due to dilation of aorta, i.e., Ortner’s syndrome. But to our surprise, CT of aorta and neck revealed extensive elongated cervical vagus nerve schwannoma. Such type of association is unique and new to voluminous medical literature.

Plexiform schwannoma is a benign tumour arising from the sheath of myelinated nerve fibres that may occur in any part of the body. It is very rare in the head and neck region and poses diagnostic challenges. Here we present two unusual cases. The first is about a fifteen-year-old boy with a large plexiform schwannoma in the right retro maxillary region, extending up to but not invading the orbit. Initial cytology reported it as a pleomorphic adenoma, but histological findings after excision confirmed it to be a benign plexiform schwannoma. A zygomatic osteotomy was required to excise the tumour in its entirety. The second reported case is about solitary lesion on the tongue of a 46-year-old Chinese man, clinically mimicking a mucocoele. This case report highlights the diagnostic challenges posed by plexiform schwannoma, and the role of imaging and cytopathology, as well as the importance of definitive histopathological analysis. It also describes the surgical management of these lesions, and provides a review of the literature, in particular of cases occurring in the head and neck region.

Most common site of schwannoma in larynx is aryepiglottic fold and false cords. Epiglottis involvement is rare. The ultimate diagnosis is based on the histologic demonstration of characteristic spindle cells with typical nuclear palisading and positive immunohistochemistry for S-100 protein, but negative for a-smooth muscle actin, CD34, and vimentin. Effective treatment is complete excision.

Objective Intracranial schwannomas are frequently located in the VIII cranial nerve, followed by the V and VII cranial nerves, but are quite rare in the olfactory cranial nerves. Here, we report a case of an olfactory schwannoma and review the literature to improve the diagnosis and treatment of olfactory schwannoma. Methods We report a case of olfactory schwannoma in a 51-year-old Chinese man who experienced dizziness and no other *** Magnetic resonance imaging(MRI) showed a neoplastic mass located on the anterior cranial base to the right of the midline and near the cribriform plate and sphenoidal plane. The lesion travelled through the cribriform plate into the nasal cavity. This mass was initially thought to be an olfactory groove meningioma. We performed a craniotomy for surgical excision, and the tumor was completely resected, and the skull base was reconstructed at the same time. There were no complications during surgery, and the patient recovered well. The histopathological diagnosis was a *** Olfactory schwannomas are extremely rare and similar to olfactory ensheating cell tumors, and the immunohistochemical staining of leukocyte antigen 7(Leu7/CD57) can be used to identify them. Although the standard treatment of olfactory schwannoma remains unclear, in all reports, most patients can have excellent prognosis after an effective surgical resection.

Introduction Plexiform schwannoma accounts for approximately 2%-5% of *** often afflicts young adults,affecting males and females at a similar rate,and is most commonly located in the head,neck,trunk,or upper *** schwannoma occasionally occur in the foot,but are rarely found in the lower limbs[1].The tumor usually presents as a solitary,slow-growing,asymptomatic nodule,and the maximum diameter of plexiform schwannomas is less than 2 cm[2].Trauma-related plexiform schwannoma is rare and needs to be differentiated in ***,we report an unusual case of 28-year-old women with a reddish-brown plaque of her right thigh accompanying tenderness,the patient had been injured in the same location where the plaque later developed,and the histopathological results were consistent with plexiform schwannoma.

AIM： To investigate the role of vascular endothelial growth factor （VEGF/and its receptors VEGFR-1 and 2 in the growth and differentiation of gastrointestinal strornal tumors （GISTs）. METHODS： Thirty-three GISTs, 15 leiomyomas and 6 schwannomas were examined by immunohistochemistry in this study. RESULTS： VEGF protein was expressed in the cytoplasm of tumor cells, and VEGFRol and 2 were expressed both in the cytoplasm and on the membrane of all tumors. Irnrnunohistochernical staining revealed that 26 GISTs （78.8%）, 9 leiornyornas （60.0%） and 3 schwannornas （50.0%/were positive for VEGF; 24 GISTs （72.7%/, 12 leiornyornas （80.0%） and 4 schwannornas （66.7%） were positive for VEGFR-1; 30 GISTs （90.9%/, 5 leiornyornas （33.3%/and 4 schwannornas （66.7%） were positive for VEGFR-2. VEGFR-2 expression was statistically different between GISTs and leiomyomas （P 〈 0.0001）. However, there was no correlation between the expression of VEGF pathway componenets and the clinical risk categories. CONCLUSION： Our results suggest that the VEGF pathway may play an important role in the differentiation of GISTs, leiomyomas and schwannomas.

AIM： Gastrointestinal stromal tumors （GISTs） are rare. GISTs differ from other mesenchymal tumors of the gastrointestinal tract （e.g. leiornyomas and schwannomas）. The purpose of this study was to investigate the role of Ets-1 in the growth and differentiation of GISTs. METHODS： Twenty-eight GISTs, nine leiomyomas and six schwannomas were examined by immunohistochemical staining method for Ets-1 in this study. Specimens were selected from surgical pathology archival tissues at Nagasaki University Hospital. RESULTS： Ets-1 protein was expressed in the cytoplasm of cells in all of these tumors. Immunohistochemical staining revealed that 27 GISTs （96.4%）, six leiomyomas （66.7%）, and five schwannomas （83.3%） were positive for Ets-1. Ets-1 expression was statistically different between GISTs and leiomyomas （P〈0.005）. However, there was no correlation between Ets-1 expression and clinical risk categories. CONCLUSION： Ets-1 plays an important role in the growth and differentiation of GISTs, leiomyomas and schwannomas.
