Impact of smoking status and pathologic type on epidermal growth factor receptor mutations in lung cancer

Background epidermal growth factor receptor （EGFR） responsive to the treatment with tyrosine kinase inhibitors. large series of lung carcinomas. mutations in lung carcinomas can make the disease more We aimed to evaluate the prevalence of EGFR mutations in a Methods We examined 1195 consecutive lung cancer patients for EGFR mutations in exons 18, 19, and 21 using direct sequencing of polymerase chain reaction products. A detailed smoking history was obtained. Patients were categorized as never smokers （〈100 lifetime cigarettes）, former smokers （quit 〉1 year ago）, or current smokers （quit 〈1 year ago）. Results There were EGFR mutations in 9 （4.5%） of 201 squamous carcinomas, in 1 （2%） of 50 large cell carcinomas, and in 1 （2.3%） of 44 small cell carcinomas that were investigated. Three hundred and twenty-seven mutations were found in the series of 858 adenocarcinomas （38.1%）. Among 858 lung adenocarcinomas, we detected EGFR mutations in 250 （48.6%） of 514 never smokers, 39 （33.9%） of 115 former smokers, and 38 （16.6%） of 229 current smokers. Significantly fewer EGFR mutations were found in people who smoked for more than 15 pack-years （P=-0.0002） or stopped smoking less than 15 years ago （P=0.033） compared with individuals who never smoked. Conclusions Adenocarcinoma is the most frequent EGFR mutation pathologic type in lung cancer. The likelihood of EGFR mutations in exons 18, 19 and 21 decreases as the number of pack-years increases. Mutations were less common in people who smoked for more than 15 pack-years or who stopped smoking cigarettes less than 15 years ago. These data can assist clinicians in assessing the likelihood of exons 18, 19, or 21 EGFR mutations in Chinese patients with lung cancer when mutational analysis is not feasible.

Efficacy and clinical/molecular predictors of erlotinib monotherapy for Chinese advanced non-small cell lung cancer

Background A retrospective analysis of clinical data were conducted reviewing patients who were given erlotinib at Peking Union Medical College （PUMC） Hospital from May 2005 to December 2009. Relationships between clinical factors, epidermal growth factor receptor （EGFR） mRNA expression, EGFR gene mutations, KRAS gene mutations and clinical outcomes were investigated in Chinese patients with advanced non-small cell lung cancer （NSCLC）. Methods Patients with stage ⅢB/Ⅳ NSCLC who had not previously participated in erlotinib related clinical trials were enrolled into this study. All patients were given oral erlotinib 150 mg per day. Tumor samples of some patients were accessed with mutant-enriched polymerase chain reaction assay （EGFR, KRAS gene mutations） and multiplex branched DNA assay （EGFR mRNA expression）. Results Seventy-nine patients were enrolled in this study, 23 patients had a partial response （PR）, 36 patients had a stable disease （SD）, 20 patients had a PD, with an objective response rate of 29.1%, and a disease control rate of 74.7%. Females （P=0.023）, non-smokers （P=0.013）, patients with a skin rash （P=0.047）, and with highly differentiated tumors （P=0.037） were significantly correlated with the objective response rate. Patients with a lower ECOG PS （P=0.002）, highly differentiated tumors （P=0.014）, non-smokers （P=0.002）, and patients with a skin rash （P 〈0.001） were significantly correlated with the disease control rate. The median progression-free survival was 35 weeks （95% CI： 13-57 weeks） and 1-year survival was 72.3%. Highly-differentiated tumors （P=0.027） and patients with a skin rash （P 〈0.001） were significantly correlated with PFS. Seventeen patients were tested for EGFR/KRAS gene mutations and EGFR mRNA expression. Progression-free survival （PFS） of patients with EGFR exon 19/21 mutations was 66 weeks, longer than patients with wild type EGFR exon 19/21 （P=0.018）. AIM: To demonstrate the relationship between H-ras oncogene and hepatocellular carcinoma (HCC) metastasis. METHODS: Activated H-ras oncogene was transfected into SMMC 7721, a cell line derived from human HCC, by calcium phosphate transfection method. Some metastasis-related parameters were detected in vitro, including adhesion assay, migration assay, expression of collagenase IV(c IV ase) and epidermal growth factor receptor (EGFR). RESULTS: The abilities of H-ras-transfected cell clones in adhesion to laminin (LN) or fibronectin (FN), migration, c IV ase secretion increased markedly, and the expression of EGFR elevated moderately. More importantly, these alterations were consistent positively with the expression of p21, the protein product of H-ras oncogene. CONCLUSION: H-ras oncogene could induce the metastatic phenotype of HCC cell in vitro to raise its metastatic potential.

Objective： To observe the effects of moxibustion pretreatment on the protein expressions of epidermal growth factor receptor （EGFR）, phosphorylation extracellular signal-regulated kinase I/2 （p-ERKI/2） and activated protein-1 （AP-2）, the key factors of extracellular signal-regulated kinase signaling transduction pathway in gastric tissue of rats with stress-induced gastric mucosal damage, and to discuss the mechanisms of moxibustion therapy in promoting the restoration of damaged gastric mucosa. Methods： Thirty Sprague-Dawley （SD） rats were randomly divided into a normal group, a model group, and a moxibustion group using the random digits table, 10 in each group. Except the rats in the normal group, rats in the other two groups were used to make stress-induced gastric mucosal damage model using restraint and cold stress. Before modeling, rats in the moxibustion group were alternately treated with moxibustion a/t Zusanli （ST 36） and Zhongwan （CV 12）, or Pishu （BL 20） and Weishu （BL 22）, once a day, for a total of 8 d. Histolo^cal changes of gastric mucosa were observed under the light microscopy, the expression of gastric tissue p-ERKI/2 was detected by immunohistochemistry assay, and the protein levels of EGFR and AP-I were measured by Western blots. Results： Compared with rats in the normal group, gastric mucosal damage was more serious, and protein expressions of gastric tissue EGFR, p-ERK1/2 and AP-1 increased in the model group （P〈0.01, P〈O.05, P〈0.05）. Compared with rats in the model group, gastric mucosal damage was milder, and protein expressions of gastric tissue EGFR, p-ERK1/2 and AP-1 increased in the moxibustion group （all P〈0.01）. Conclusion： Moxibustion at Zusanli （ST 36）, Zhongwan （CV 12）, Pishu （BL 20） and Weishu （BL 21） could increase EGFR, p-ERK1/2 and AP-1 expression levels in gastric tissue of stress-induced gastric mucosal damage rats, maintain the information transfer function of ERK signaling transduction pathway, and promote restoration of ga
