Effect of L-Arginine on pulmonary artery Smooth Muscle Cell Apoptosis in Rats with Hypoxic pulmonary Vascular Structural Remodeling

This study investigated the effect of L-arginine (L-Arg) on the apoptosis of pulmonary arterysmooth muscle cells (PASMC) in rats with hypoxic pulmonary vascular structural remodeling,and itsmechanisms.Seventeen Wistar rats were randomly divided into a control group (n=5),a hypoxia group(n=7),and a hypoxia+L-Arg group (n=5).The morphologic changes of lung tissues were observed underoptical microscope.Using the terminal deoxynucleotidyl transferase-mediated deoxyuridine triphosphate-biotin nick end labeling assay,the apoptosis of PASMC was examined.Fas expression in PASMC wasexamined using immunohistochemistry.The results showed that the percentage of muscularized artery insmall pulmonary vessels,and the relative medial thickness and relative medial area of the small and medianpulmonary muscularized arteries in the hypoxic group were all significantly increased.pulmonary vascularstructural remodeling developed after hypoxia.Apoptotic smooth muscle cells of the small and median pul-monary arteries in the hypoxia group were significantly less than those in the control group.After 14 d ofhypoxia,Fas expression by smooth muscle cells of median and small pulmonary arteries was significantlyinhibited.L-Arg significantly inhibited hypoxic pulmonary vascular structural remodeling in association withan augmentation of apoptosis of smooth muscle cells as well as Fas expression in PASMC.These resultsshowed that L-Arg could play an important role in attenuating hypoxic pulmonary vascular structural remod-eling by upregulating Fas expression in PASMC,thus promoting the apoptosis of PASMC.

Background Anomalous origin of the left coronary artery from the pulmonary artery （ALCAPA） is a rare congenital heart anomaly. We aimed to illustrate the clinical features and long-term prognosis of patients with ALCAPA.Methods Twenty three patients （13 males and 10 females, ages ranging from 2.5 months to 65 years） identified as ALCAPA in Beijing Anzhen Hospital from April 1984 to June 2009 were divided into two groups, based on the age of onset： group 1 （≤12 months, n=16） and group 2 （〉12 months, n=7）.Results Fifty six point three percent of patients in group 1 had been misdiagnosed as endocardial fibroelastosis （9/16）,18.8% as dilated cardiomyopathy （3/16） and 6.3% as myocardial infarction （1/16）. Patients in group 2 were usually diagnosed as coronary heart disease, myocarditis, or patent ductus arteriosus. Electrocardiography in group 1 revealed abnormal Q waves with T wave inversion in leads I, avL, V4-V6, especially in lead avL （deep and wide Q wave）; but no specific manifestations in group 2. A higher percentage of patients in group 1 had cardiomegaly on chest radiograph （86.7% vs. 33.3%, P=0.031）, while pulmonary artery protrusion was more common in group 2 （26.7% vs. 83.3%,P=0.046）. Lower left ventricular ejection fraction （LVEF） was present in group 1 than in group 2 （（48.5±11.5）% vs.（65.0±6.1）%, P 〈0.001）. Apical ventricular aneurysm （62.5% vs. 0%, P=0.007）, enhanced echogenicity of papillary muscles （87.5% vs. 28.6%, P=0.011） and endocardial thickening （93.8% vs. 14.3%, P 〈0.001） were more frequent in group 1 than in group 2. The ratio of the proximal right coronary artery （RCA） diameter to the aortic root diameter exceeded 0.14 in all cases, more prominent in group 2 （0.26±0.05 vs. 0.33±0.03, P=0.009）. Increased coronary artery collaterals within the interventricular septum were detected in 18 patients （78.3%） by Doppler imaging. Background Increased proliferation of pulmonary vascular cells and muscularisation of pulmonary vessels are frequently observed in human smokers and in animals exposed to cigarette smoke. To elucidate the molecular mechanisms leading to these changes, we studied the in vitro effect of cigarette smoke extract （CSE） on proliferation of pulmonary artery smooth muscle cells （PASMCs） and activation of protein kinase C （PKC）, an important kinase implicated in cell proliferation. Methods PASMCs cultured from 12 normal Wistar rats were studied in the following conditions：（1） PASMCs were exposed to different concentrations of CSE for 24 hours, then MTT colorimetric assay was used for detection of cell proliferation. Cell viability was assessed by trypan blue exclusion. （2） PASMCs were pre-incubated with phorbol 12-myristate 13-acetate （PMA） for 24 hours or Ro31-8220 for 30 minutes before exposure to 5% CSE for 24 hours. Cell proliferation was examined by MTT colorimetric assay, cell cycle analysis and proliferating cell nuclear antigen （PCNA） immunocytochemical staining. （3） PASMCs were exposed to 5% CSE for 24 hours. Then PKC-a mRNA expression was detected by reverse transcription-polymerase chain reaction （RT- PCR） and protein expression by Western blotting, while PKC-α translocation was observed by immunofluorescence staining and confocal microscopy. （4） PASMCs were transfected with specific antisense oligodeoxynucleotides against PKC-a 6 hours before exposure to 5% CSE for 24 hours. PKC-α protein expression and cell proliferation were detected by methods described previously. Results （1） Low concentration of CSE （5%） increased proliferation of PASMCs, whereas high concentrations （20%, 30%） were inhibitory as a result of cytotoxicity. （2） The value of absorbance （Value A）, proliferation index （PI）, S-phase cell fraction （SPF） and average optical density of PCNA staining in PASMCs from 5% CSE exposure group （0.306 ± 0.033, 0.339 ± 0.033, 0.175 ± 0.021, 0.315 ± 0.038, respectively） were significantly increased compa

BACKGROUND Endometrial stromal sarcoma(ESS)is a rare malignant mesenchymal tumor.Early in the disease,the findings on magnetic resonance imaging are similar to those of leiomyoma.When the lesion involves both vascular and cardiac tissue,it might be misdiagnosed as intravenous leiomyomatosis,which is not common in the clinic.CASE SUMMARY We present the case of a 34-year-old female patient with tumor embolus,which extended from the right iliac vein and ovarian vein to the inferior vena cava(IVC),and then to the right atrium and right ventricle,and finally protruded into the pulmonary artery.The patient had undergone a hystero-myomectomy 7 years previously.Based on the findings of the imaging examinations,the diagnosis of intravenous leiomyomatosis was considered preoperatively.The patient then underwent complete resection of the endovascular and intracardiac tumor embolus.The postoperative pathology results confirmed metastatic ESS with endovascular and intracardiac involvement.The patient was discharged from hospital in good condition,and there was no sign of recurrence 5 mo after the operation.CONCLUSION Extending from the iliac vein and ovarian vein to the IVC,this metastatic ESS invaded both vascular and cardiac tissues.For patients with ESS involving vascular and cardiac tissues,pathological examinations are essential for the differential diagnosis,such as intravenous leiomyomatosis.In addition,due to the high recurrence rate of ESS,long-term and close follow-up evaluation is necessary.

BACKGROUND Primary intimal sarcoma of the pulmonary artery is a rare malignant tumor originating from the pulmonary artery,which has a low incidence rate and is easily misdiagnosed as pulmonary embolism.There is no standard protocol for the treatment of primary intimal sarcoma of the pulmonary artery.CASE SUMMARY This study reports a patient with primary intimal sarcoma of the pulmonary artery who was admitted to our hospital in 2017.The clinical characteristics,diagnosis,treatment and outcome of the patient were retrospectively analyzed.The patient was a Chinese Han male aged 44 years.He had three consecutive episodes of syncope,and was thus admitted to a local hospital.Computed tomography pulmonary angiography showed multiple lesions with abnormal densities in the pulmonary trunk,left pulmonary artery,mediastinum and pericardium,which were consistent with recurrence after tumor resection.He underwent surgery,and was pathologically diagnosed with intimal sarcoma of the pulmonary artery.He relapsed 3 mo after surgery,and apatinib was administered.His condition was stable after 4 mo,with tolerable and controllable adverse reactions.He subsequently died 19 mo after surgery.CONCLUSION Primary intimal sarcoma of the pulmonary artery has no specific clinical or imaging manifestations.The diagnosis of this disease depends on histopathology and immunohistochemistry,and has a poor clinical prognosis.Surgical treatment is currently a favorable option for primary intimal sarcoma of the pulmonary artery,and targeted therapy may provide new insights for the development of effective treatment methods.

Introduction:Early intervention in patients with congenitally disconnected pulmonary artery improves long-term outcome.Case presentation:We present 3 cases of isolated disconnected pulmonary artery in the absence of associated structural heart disease during a period of 3 years.Conclusion:Transcatheter stenting of the feeding ductus arteriosus re-established pulmonary artery flow and growth.Successful surgical repair was achieved,and normal perfusion and complete function of the ipsilateral lung were regained.

Summary： This study iflvestigated the potential role of ERK1/2-cyclinE1 signaling pathway in rat pulmonary artery smooth muscle cells （rPASMCs） proliferation and pulmonary vascular remodeling induced by cigarette smoke exposure. A total of 24 male Wistar rats were randomly divided into 4 groups： control group （C group）, S-1M, S-3M and S-6M groups （animals in the groups were exposed to smoke for 1, 3, and 6 months, respectively）. HE staining and anti-u-smooth muscle actin antibody staining were performed to observe the degree of pulmonary vascular remodeling. Imrnunohistochemis- try and Western blotting were performed to evaluate ERK1/2 and cyclinE1 expression in pulmonary vessels. Primary cultured rat pulmonary artery smooth muscle cells （rPASMCs） were exposed to ciga- rette smoke extract （CSE）. ERK inhibitor （PD98059） and cyclinE1 siRNA were used to verify the role of ERK1/2 and cyclinE 1 in CSE-induced rPASMCs proliferation. Cell proliferation was assessed by cell counting and 5-bromo-2-deoxyuridine （BrdU） incorporation. Our results showed that abnormal pulmo- nary vascular remodeling was found in cigarette smoked rats. Compared to C group, activated ERK1/2 and cyclinE1 expression was significantly increased in smoke-exposure groups. This up-regulated ex- pression was positively correlated with the severity of pulmonary vascular remodeling, and there was positive correlation between the expression of ERK1/2 and cyclinE1. PD98059 and cyclinE1 siRNA in- hibited the proliferation of rPASMCs. The expression of cyclinE1 could be down-regulated by PD98059. Our data demonstrated that increased expression of ERK1/2 and cyclinE1 might be involved in the pathogenesis of abnormal rPASMCs proliferation and rat pulmonary vascular remodelling induced by cigarette smoke exposure.

In order to investigate the effect of nuclear factor kappa B (NF κB) on vascular endothelial growth factor (VEGF) mRNA expression of human pulmonary artery smooth muscle cells (HPASMCs) in hypoxia, the cultured HPASMCs in vitro were stimulated with pyrrolidine dithiocarbamate (PDTC), an inhibitor of NF κB. The NF κB p65 nuclei positive expression was detected by immunocytochemical technique. The IκBα protein expression was measured by Western blot. RT PCR was used to detect the VEGF mRNA expression of HPASMCs. The results showed that no significant change was observed in the NF κB p65 nuclei positive expression of cultured HPASMCs during 6 h-24 h in normoxia, but the levels of NF κB p65 nuclei positive expression of cultured HPASMCs were significantly increased in hypoxia groups as compared with those in all normoxia groups ( P <0.05). The IκBα protein expression of cultured HPASMCs showed no significant change during 6 h-24 h in normoxia, but significantly decreased in hypoxia as comapred with that in normoxia groups ( P <0.05). PDTC (1 to 100 μmol/L) could inhibit the VEGF mRNA expression of HPASMCs in a concentration dependent manner in hypoxia. In conclusion, NF κB can be partly translocation activated from cytoplasm into nuclei in the cultured HPASMCs under hypoxia. The inhibition of NF κB activation can decrease the VEGF mRNA expression. It is suggested that the activation of NF κB is involved in the VEGF mRNA expression of HPASMCs under hypoxia.
