propranolol hydrochloride can be considered a persistent and bioaccumulative pharmaceutical in the environment.This drug and its by-products are potentially toxic and have adverse effects,since these compounds have been associated with endocrine-disrupting effects,reproductive deficiencies,embryo abnormalities and pericardial oedema.TiO_(2)–La 0.05%–carboxymethyl-β-cyclodextrin(CMCD)nanoparticles were successfully prepared by a simple two-step method,which consists of sonification and functionalization.The characterization analyses reveal that lanthanum is dispersed on the semiconductor surface,probably forming Ti–O–La bonds,which can induce oxygen vacancies and surface defects that effectively restrain the recombination of photogenerated electron/holes pairs.The efficiency of TiO_(2)–La 0.05%–CMCD samples in degradation of propranolol under UV-light irradiation is higher than that of pristine TiO_(2) within 20 min reaction,probably due to complex formation between theβ-blocker and the oligosaccharide,which allows us to propose a photocatalytic mechanism based on the formation of intermediates and competition of these compounds to the radicals and CMCD cavities.

propranolol and verapamil inhibit mRNA expression of RyR2 and SERCA in L-thyroxin-induced rat ventricular hypertrophy

AIM: To study the alteration in the mRNA level of cardiac ryanodine receptor 2 (RyR2) and sarco-endoplasmic reticulum Ca2+-ATPase (SERCA) in L-thyroxin-induced hypertrophy. METHODS: L-thyroxin (500 g/kg) daily was injected for 10 d. RT-PCR was used to determine mRNA expression. RESULTS: An increase in the relative amount of RyR2 (111 %) and SERCA mRNA (65 %) expression was observed in the hypertrophied rats (RyR2: 77±11; SERCA: 87±10, n=9) compared with the normal rats (RyR2: 36±10; SERCA: 53±10, n=9). propranolol was effective to inhibit the increase in RyR2 (51±7) and SERCA (63±13) mRNA expression in hypertrophied rats, respectively. Verapamil also reduced RyR2 (62±5) and SERCA (75±8) mRNA expression. CONCLUSION: Both RyR2 and SERCA mRNA level in L-thyroxin-induced cardiac hypertrophy was over-expressed and propranolol or verapamil inhibited the alteration.

Efficacy and adverse effects of oral propranolol in infantile hemangioma:a meta-analysis of comparative studies

Background A number of clinical trials evaluated the efficacy and adverse effects of oral propranolol in the treatment of infantile hemangioma(IH),but the treatment has not yet been standardized.This meta-analysis aims to reevaluate the efficacy and adverse effects of oral propranolol in comparative studies and to provide a reliable basis for clinical administration in the therapy for IH.Methods Data were obtained from PubMed,Embase,Cochrane Library,Web of Science,China National Knowledge Infra-structure and Wanfang database,from inception to December 1st,2018.The pooled risk ratios(RR)with 95%confidence intervals(95%CI)were calculated and used to evaluate the effect size.The meta-analysis was performed using the random-effects model due to heterogeneity between the studies.The Cochrane Collaboration 6 aspects of bias,methodological index for non-randomized studies and the Newcastle-Ottawa Scale were used to assess the risk for bias.Sensitivity analysis,publication bias and subgroup analysis were performed.Results Eighteen unique studies involving 2701 unique children were included in the analysis.The response rate was reported in 18 trials,which compared oral propranolol with other treatments.The heterogeneity was statistically significant(Ppropranolol(1 mg/kg/day),1.42(95%CI 1.12-1.80)for medium dose(2 mg/kg/day)and 1.46(95%CI 1.17-1.82)for high dose(3 mg/kg/day),but the high dose had higher adverse events rate than medium dose,with 3.60%and 86.22%,respectively.The effectiveness of propranolol therapy among cases of treatment duration less than 6 months(RR=1.24,95%CI 1.05-1.47)was inferior to that of treatment duration greater than or equal to 6 mon

Carvedilol Versus Other Nonselective Beta Blockers for Variceal Bleeding Prophylaxis and Death: A Network Meta-analysis

Background and Aims:We aimed to perform a network meta-analysis(NWM)to examine comparative effectiveness of non-selective beta blockers(NSBBs)on prophylaxis of gastroesophageal variceal bleeding(GVB)and mortality benefit.Methods:MEDLINE(OVID)and EMBASE databases were searched for eligible randomized clinical trials(RCTs)from inception to July 3,2021.Outcomes of interest included primary/secondary prophylaxis of GVB,failure to achieve hepatic venous pressure gradient(HVPG)decremental response,liver-related and all-cause mortality.A Bayesian NWM was performed to derive relative risk(RR)with 95%credible intervals(CrIs).The ranking probability of each NSBB was assessed by surface under cumulative ranking curve(SUCRA).Results:Thirty-three RCTs including 3,188 cirrhosis patients with gastroesophageal varices were included.Compared with placebo,nadolol ranked first for reducing variceal bleeding[RR:0.25,(95%CrI:0.11–0.51);SUCRA:0.898],followed by carvedilol[RR:0.33,(95%CrI:0.11–0.88);SUCRA:0.692]and propranolol[RR:0.52,(95%CrI:0.37–0.75);SUCRA:0.405].Carvedilol was more effective than propranolol in achieving HVPG decremental response[RR:0.43,(95%CrI:0.26–0.69)].Carvedilol ranked first for reducing all-cause mortality[RR:0.32,(95%CrI:0.17–0.57);SUCRA:0.963],followed by nadolol[RR:0.48,(95%CI:0.29–0.77);SUCRA:0.688],and propranolol[RR:0.77,(95%CI:0.58–1.02);SUCRA:0.337].Similar findings were observed for liver-related mortality.Carvedilol ranked the safest.The RR of adverse events was 4.38,(95%CrI:0.33–161.4);SUCRA:0.530,followed by propranolol[RR:7.54,(95%CrI:1.90–47.89);SUCRA:0.360],and nadolol[RR:18.24,(95%CrI:91.51–390.90);SUCRA:0.158].Conclusions:Carvedilol is the preferred NSBB with better survival benefit and lower occurrence of adverse events among patients with gastroesophageal varices.

Increased expression of insulin-like growth factor-binding protein-3 is implicated in erectile dysfunction in two-kidney one-clip hypertensive rats after propranolol treatment

This study aimed to investigate the role of insulin-like growth factor-binding protein-3 （IGFBP-3） in erectUe dysfunction （ED） in two-kidney one-clip （2K-1C） hypertensive rats treated with the β-blocking agent propranolol. Adult male Wistar rats were randomly divided into three groups： a normal control group, a hypertensive control group and a propranolol treatment group （n=9）. After 4 weeks of propranolol treatment, intracavemous pressure （ICP） responses to electrical stimulation of the cavernous nerves were evaluated. The expression of IGFBP-3 and insulin-like growth factor-1 （IGF-1） mRNA and protein in the rat cavernous tissue were detected by quantitative real-time PCR and Western blot, respectively. The concentration of cyclic guanosine monophosphate （cGMP） in the cavernous tissue was determined by enzyme-linked immunosorbent assay （ELISA）. Cavernosal pressure in response to cavernous nerve stimulation was decreased 4 weeks after propranolol treatment （P〈0.01, compared to the hypertensive control group）. IGFBP-3 mRNA and protein expression was increased in the propranolol treatment group compared to the hypertensive control group （P〈O.01）, whereas IGF-1 expression was decreased in the propranolol treatment group compared to the hypertensive control group （P〈0.01）. In addition, cavernous cGMP concentration was decreased in the prepranolol treatment group compared to the hypertensive control group （P〈0.01）. Taken together, these results suggest that the upregulation of IGFBP-3 may play a role in the development of ED in hypertensive rats.

Buccal administration of mucoadhesive blend films saturated with propranolol loaded nanoparticles

The aims of this study were to prepare and characterize hydroxypropyl methylcellulose(HPMC)/polycarbophil(PC) mucoadhesive blend films saturated with propranolol hydrochloride(PNL)-loaded nanoparticles to improve permeability of drugs that undergo firstpass metabolism. An ionic cross-linking method and film casting technique was used to prepare nanoparticles and mucoadhesive blend films, respectively. Increasing concentrations of PNL(70, 80, 90 mg/film) in HPMC/PC blend films containing PNL-loaded nanoparticles(PN-films) and HPMC/PC blend films containing PNL(80 mg/film) without nanoparticles(PPfilms) were prepared to test swelling, mucoadhesiveness, release, permeation and physicochemical properties. Scanning electron microscope(SEM) images showed a partially smooth surface with a wrinkled occurrence and spherically shaped, well-dispersed nanoparticles on the surface of PN-films containing PNL 80 mg/film(PN-films-80). The size of the nanoparticles on the surface of PN-films-80 was around 100 nm, which was similar to the nanoparticle size observed using light scattering technique. The swelling index(SI)of all PN-films and PP-films increased greatly in the first period time(10–20 min) and reached swelling equilibrium at 20 min and 30 min, respectively. For the PN-films, the concentration of PNL influenced the mucoadhesive properties and tended to be higher when the amount of PNL increased. Immediate release of all blend film formulations was found in early time points(10–30 min). After 120 min, the release of PN-films-70 was lower than the other PNfilms. Permeation studies using porcine buccal mucosa showed that inclusion of nanoparticles in the films increased the permeability of PNL compared to PP-films. Therefore, buccal administration of mucoadhesive blend films containing PNL-loaded nanoparticles could be a promising approach for drugs that undergo first-pass metabolism.

Topical propranolol Hydrochloride Gel for Superficial Infantile Hemangiomas

The clinical efficacy and safety of topical propranolol hydrochloride gel in the treatment of superficial infantile hemangiomas (IHs) were assessed. Fifty-one cases of IHs from Oct. 2010 to Sept. 2011 were subjected to the topical propranolol hydrochloride gel intervention in Fuzhou General Hospital of Nanjing Military Commands, China. Changes in size, texture, color, peak systolic velocity of the hemangiomas, resistance index and adverse effects were observed. The results were evaluated by using Achauer system, and responses of IHs to pranpronolol were considered scaleⅠ(poor) in 4 patients (17.24%), scaleⅡ(moderate) in 18 patients (24.14%), scale Ⅲ (good) in 22 patients (44.83%) and scale Ⅳ (excellent) in 7 patients (13.79%). The response of superficial hemangiomas was significantly better than other hemangiomas (P0.05). Our study indicates that topical application of 3% propranolol hydrochloride gel is effective and safe in treating IHs.

Background:Systemic treatment with propranolol is proven to be effective for patients with hemangiomas with less side-effect.We used a propranolol gel for topical use on hemangiomas.Methods:In this retrospective study,we analyzed 148 patients who had been treated topically with propranolol gel for 12 weeks.We analyzed the data of patients and clinically gave each hemangioma a"hemangioma score"to determine the treatment success.Results:In 147 of the 148 patients,strong signs of resolution under treatment included lightening,paling,and less vascularization.The hemangioma score showed a signifi cant decrease during the treatment.Relevant serum levels of propranolol were not found.Adverse effects were rare and not related to propranolol.Conclusion:Topical treatment with propranolol gel is suitable for specific hemangiomas in addition to cryotherapy and systemic treatment with propranolol.

Effect of periocular injection of celecoxib and propranolol on ocular level of vascular endothelial growth factor in a diabetic mouse model

AIM： To investigate the effects of periocular injection of propranolol and celecoxib on ocular levels of vascular endothelial growth factor （VEGF） in a diabetic mouse model. METHODS： Forty 4-6wk BALB-C male mice weighing 20-25 g were used. The study groups included： nondiabetic control （group 1）, diabetic control （group 2）, diabetic propranolol （group 3）, and diabetic celecoxib （group 4）. After induction of type 1 diabetes by streptozotocin, propranolol （10 μg） and celecoxib （200 μg dissolved in carboxymethylcellulose 0.5%） were injected periocularly. The ocular level of VEGF was measured in all the study groups using enzyme-linked immuno sorbent assay （ELISA） method. RESULTS： Ocular VEGF level was significantly increased （1.25 fold） in the diabetic control group when compared to the non-diabetic group one week after induction with streptozotocin （P=0.002）. Both periocular propranolol and celecoxib significantly reduced ocular VEGF levels （P=0.047 and P〈0.001, respectively）. The effect was more pronounced with celecoxib, CONCLUSION： The periocular administration of propranolol and celecoxib can significantly reduce ocular VEGF levels in a diabetic mouse model.

Effects of propranolol on β-Adrenergic Receptor ofExperimental Acute Myocardial Infarction in Rats

By using receptor autoradiography to observe the distribution and density of receptors, the effects of propranolol, a β-blocker, on β-adrenergic receptor of experimental acute myocardial infarction (AMI) were studied. One week after ligation of proximate left anterior descend (LAD) coronary artery, [3H] DHA binding sites were markedly decreased in both infarctregion and non-infarct region. After treatment of propranolol (100μg/kg), the [3H] DHA binding sites were obviously increased in the infarct region, and they were further decreased in the non-infarct region. The ratio of [3H] DHA binding sites of the infarct region to non-infarct region was from 0. 24 at LAD ligation to 0. 87 after propranolol treatment, which was close to 0. 97 of control group (sham operation). The results indicated that the propranolol acted directly on myocardial β-adrenergic through the receptor regulation of the balance of β-receptors between the infarct region and non-infarct region, and improvement of the myocardial consonation and contraction synergism, thereby protecting the heart affected by AMI.
