Accurate positioning of the 24-hour ph monitoring catheter:Agreement between manometry and ph step-up method in two patient positions

AIM： TO investigate the agreement between esophageal manometry and ph step-up method in two different patient positions. METHODS： Eighteen subjects were included in the study. First, the distance from the nose to the proximal border of the lower esophageal sphincter （LES） was measured manometrically. Then a different investigator, who was blinded to the results of the first study, measured the same distance using the ph step-up method, with the patient in both upright and supine positions. An assessment of agreement between the two techniques was performed. RESULTS： In the supine position, the measurement of only one subject was outside the range accepted for correct positioning （~〈 3 cm distal or proximal to the LES）. In the upright position, errors in measurement were recognized in five subjects. Bland-Airman plots revealed good agreement between measurements obtained manometrically and by the ph-step up method with the patient in the supine position. CONCLUSION： In the case of nonavailability of manometric detection device, the ph step-up method can facilitate the positioning of the 24 h ph monitoring catheter with the patient in the supine position. Thisshould increase the use of ph-metry in clinical practice for subjects with suspected gastroesophageal reflux disease if our results are supported by further studies.

Ion sensitive field effect transistor(ISFET)devices are highly accurate,convenient,fast and low-cost in the detection of ions and biological macromolecules,such as DNA molecules,antibodies,enzymatic substrates and cellular *** high-throughput cell metabolism detection,we successfully designed a very large-scale biomedical sensing application specific integrated circuit(ASIC)with a 640×640 ISFET *** circuit design is highly integrated by compressing the size of a pixel to 7.4×7.4μm^(2)and arranging the layout of even and odd columns in an interdigital pattern to maximize the utilization of *** chip can operate at a speed of 2.083M pixels/s and the dynamic process of the fluid flow on the surface of the array was monitored through ion *** ph sensitivity is 33±4 mV/ph and the drift rate is 0.06 mV/min after 5 h,indicating the stability and robustness of the ***,the chip was applied to monitor ph changes in CaSki cells metabolism,with ph shifting from 8.04 to 7.40 on *** platform has the potential for continuous and parallel monitoring of cell metabolism in single-cell culture arrays.

THE RELATIONSHIP BETWEEN THE SPONTANEOUS NOCTURNALEPISODES OF ALKALINIZATION AND AUTONOMIC NERVOUS FUNCTION ON FD PATIENTS

Objective To study the relationshlp between the spontaneous nocturnal eplsodes of alkaiinization and the autonomic nerve system function and vagal function-Methods 24-hour intragastric ph was measured and autonomic and vagal function was measured with the time domain analyses of heart rate variability in 2o patients with functional dyspepsia but without diseases of the cardiovascular system- Results 13 of 2o had the nocturnal episodes of alkalinization. The to tal 24-hour SDNN and MSSD were normal in 2o subjects with FD. There was no signiflcant difference (P>O. O5) in the comparison of the total SDNN and rMSSD of the 2 groups with alkalinization and without alkalinization. The 2 groups both had higher PNN5os in the nocturnal time, and there was no significant difference (P>O. O5). Conclusion The results suggest that the total autonpmlc nerve function and vagal function of patients with FD are normal vagal activities of the 2 groups are both increased in the nocturnal period. The effect of bile reflux on the intragastric ph

Oblective To observe the effect of bile reflux on the intragastric ph. Methods We used ambulatory intragastric ph monitoring along with simultaneous Bilitec 2000 to measure the ph and absorbance of the mixed gastric juice diluted with bile with increasing bilirubin concentration in vitro and the yellow and clear gastric juice aspirated under endoscopy. In vivo,we studied 20 functional dispepsia patients for 24 h with simultaneous intragastric ph and bilirubin monitoring. Results In vitro dilutions,the absorbance value reached to 0. 900 while the concentration was only 20% ,but when the ph of the solution was more than 4,it′s concentration was already up to 60％. The mean absorbance of the yellow gastric juice(0. 348± 0. 131) was significantly higher than that of the clear juice (0. 008 ± 0. 003)(P＜0.05). The ph of 80％ yellow gastric juice was less than 4, the percentage was not significantly different from the clear samples (81％) (P＞0. 05). The ph showed no correlation with the absorbance in the yellow and clear groungs. Eight of the 12 patients with the spontaneous nocturnal episcodes of alkalinization had simultaneous increase of absorbance,4 of the 8 subjects with no aikalinization had increase of absorbance. Gonclusion In some concentration,bile reflux affect little on the intragastric ph.
