Focal choroidal excavation complicated with choroidal neovascularization in young and middle aged patients

AIM: To investigate the clinical and optical coherence tomography(OCT) features of focal choroidal excavation(FCE) complicated with choroidal neovascularization(CNV) in young and middle aged patients. METHODS: We performed a retrospective review of 26 patients with FCE accompanied by CNV. All patients underwent a complete ophthalmic examination. We analyzed the clinical characteristics of patients, focusing on the spectral-domain OCT features. All patients received intravitreal injection of anti-vascular endothelial growth factor(anti-VEGF) agents. And we assessed the changes of central retinal thickness and best-corrected visual acuity(BCVA) after anti-VEGF therapy. RESULTS: The mean age of 26 patients was 35.5±7.3 y(range, 21-48 y). Of the 26 FCE lesions, 11 were located subfoveal, 6 were parafoveal, and 9 were extrafoveal. The mean FCE depth was 129.8±50.3 μm, and the mean width was 901.3±306.0 μm. The FCE depth was correlated positively with the width, but not correlated with age or refractive error. CNV was located within the excavation(19 eyes) or adjacent to the excavation(7 eyes). After anti-VEGF therapy, the central retinal thickness was significantly reduced and the BCVA was significantly improved. In the absorption process of subretinal fluid, we found that the fluid in the excavations needed to be absorbed at the last. A small amount of residual fluid could still be seen in a few deep excavations even after a longterm follow-up.CONCLUSION: FCE may be an important reason to cause CNV. Especially in young patients with idiopathic CNV, we should pay attention to the use of OCT to check the presence of FCE. Anti-VEGF therapy is generally effective for CNV associated with FCE.

In Vivo Monitoring of neovascularization in Tumour Angiogenesis by Photoacoustic Tomography

Photoacoustic tomography （PAT） is presented to in vivo monitor neovascularization in tumour angiogenesis with high resolution and high contrast images in a rat. With a circular scan system, the photoacoustic signal, generated by laser pulses at a wavelength of 532nm from a Q-switched Nd：YAG laser is captured by a hydrophone with a diameter of 1 mm and a sensitivity of 850nV/Pa. The vascular structure around the rat tumour is imaged clearly, with optimal contrast, because blood has strong absorption near this wavelength. Serial noninvasive photoacoustic images of neovascularization in tumour angiogenesis are also obtained consecutively from a growing tumour implanted under the skin of a rat over a period of two weeks. This work demonstrates that PAT can potentially provide a powerful tool for tumour angiogenesis detection in cancer research. It will bring us closer to clinical applications for tumour diagnosis and treatment monitoring.

Inhibitory effect of maspinon neovascularization in diabetic retinopathy

BACKGROUND Diabetic retinopathy(DR)is a serious and potentially blinding complication of diabetes mellitus.Retinal neovascularization is one of the main pathological features of proliferative DR,and inhibiting retinal neovascularization is a research focus.AIM The aim was to evaluate the effect of intravitreal injection of recombinant human maspin on neovascularization in DR.METHODS An oxygen-induced retinopathy(OIR)mouse model was used to simulate neovascularization in DR.New born C57BL/6J mice were randomly divided to a normal control group,a maspin injection OIR group,and an OIR group.The mice in the maspin injection OIR group were injected with recombinant human maspin in the bilateral vitreous cavity on postnatal day P12,and those in the OIR group were injected with sterile phosphate buffered saline.The protein expression of vascular endothelial growth factor(VEGF)and hypoxia-inducible factor 1-alpha(HIF-1α)in the retina was measured by western blotting,and the mRNA expression of VEGF and HIF-1αwas measured by real-time polymerase chain reaction.The vascular cell nuclei that broke through the inner limiting membrane(ILM)were counted in haematoxylin-eosin stained retinal sections.RESULTS It was found that the number of vascular cell nuclei breaking through the ILM was 31.8±8.75 in the OIR group,which was significantly more than that in the normal control group(Pneovascularization in DR by modulating the HIF-1α/VEGF pathway,which provides a potential and effective strategy for the treatment of DR.

Burn blister fluids in the neovascularization stageof burn wound healing: A comparison betweensuperficial and deep partial-thickness burn wounds

Burn wound healing is a complex and dynamic process that involves the interaction between different cell types and mediators. neovascularization is an imperative stage of wound healing and consists of not only angiogenesis but also adult vasculogenesis. A superficial partial-thickness burn (SPTB) heals within 2 weeks without scarring. A deep partial-thickness burn (DPTB), conversely, requires 2 weeks or longer to heal and requires an aggressive treatment to prevent hypertrophic scarring. Burn blisters on the skin are a hallmark of not only SPTB but also DPTB;however, the effect of burn blister fluids on the neovascularization in these types of burns has not been fully explored. To verify this effect, the role of different burn fluids and the angiogenic factors that modulate this process are currently under investigation.

Inhibiting the effect of ^(90)Sr-^(90)Y ophthalmic applicators on rat corneal neovascularization induced by sutures

AIM：To investigate a practical technique used to inhibit corneal angiogenesis with a ^90Sr-^90Y ophthalmic applicator.METHODS：A ^90Sr-^90Y ophthalmic applicator was detected with a radioactive nuclide application treatment healthy protection standard.The applicator used was produced through medical dosimetry research;it had a concave applicator add measured the applicator temperature,serviceable humidity range,applicator appearance status,applicator radiation homogeneity,radioautography,and radiological safety of the original applicator surface.A vessel model was established using newborn rats,with sutures around the corneal limbus.Corneal neovascularization（CNV） were observed with a slit lamp.The new vessel length and response area were measured.RESULTS：Low-dose radiation can inhibit CNV after corneal sutures.The absorbed dose of the applicator（0.046 Gy/s） was safe for the treatment of it.The lengths of new vessels and the areas of new vessels were lower than the new born vessel rat group（P〈0.01）.CONCLUSION：The optimal radiation dose emitting from the applicator can be safe and potentially used in humans.

Assessment of neovascularization within carotid plaques in patients with ischemic stroke

AIM:To assess neovascularization within human ca-rotid atherosclerotic soft plaques in patients with isch-emic stroke.METHODS:Eighty-one patients with ischemic stroke and 95 patients without stroke who had soft athero-sclerotic plaques in the internal carotid artery were studied.The thickest soft plaque in each patient was examined using contrast-enhanced ultrasound.Time-intensity curves were collected from 5 s to 3 min after contrast injection.The neovascularization within the plaques in the internal carotid artery was evaluated using the ACQ software built into the scanner by 2 of the experienced investigators who were blinded to the clinical history of the patients.RESULTS:Ischemic stroke was present in 7 of 33 patients(21%) with grade Ⅰ plaque,in 14 of 51 pa-tients(28%) with grade Ⅱ plaque,in 26 of 43 patients(61%) with grade Ⅲ plaque,and in 34 of 49 patients(69%) with grade Ⅳ plaque(P neovascularization is a marker for unstable plaque.

Implants made of magnesium(Mg)are increasingly employed in patients to achieve osteosynthesis while degrading in situ.Since Mg implants and Mg^(2+)have been suggested to possess anti-inflammatory properties,the clinically observed soft tissue inflammation around Mg implants is enigmatic.Here,using a rat soft tissue model and a 1-28 d observation period,we determined the temporo-spatial cell distribution and behavior in relation to sequential changes of pure Mg implant surface properties and Mg^(2+)release.Compared to nondegradable titanium(Ti)implants,Mg degradation exacerbated initial inflammation.Release of Mg degradation products at the tissue-implant interface,culminating at 3 d,actively initiated chemotaxis and upregulated mRNA and protein immunomodulatory markers,particularly inducible nitric oxide synthase and toll-like receptor-4 up to 6 d,yet without a cytotoxic effect.Increased vascularization was demonstrated morphologically,preceded by high expression of vascular endothelial growth factor.The transition to appropriate tissue repair coincided with implant surface enrichment of Ca and P and reduced peri-implant Mg^(2+)concentration.Mg implants revealed a thinner fibrous encapsulation compared with Ti.The detailed understanding of the relationship between Mg material properties and the spatial and time-resolved cellular processes provides a basis for the interpretation of clinical observations and future tailoring of Mg implants.

Microarray expression profile and functional analysis of circular RNAs in choroidal neovascularization

Choroidal neovascularization(CNV) is a leading cause of visual loss in age-related macular degeneration(AMD). However, the molecular mechanism for CNV progression is still unclear. This study aimed to identify CNV-related circular RNAs(circRNAs), a novel class of non-coding RNAs with diverse functions. A total of 117 circRNAs were differentially expressed in the murine CNV model by microarrays. Gene ontology(GO)enrichment and Kyoto Encyclopedia of Genes and Genomes(KEGG) analysis were performed to identify the functions of selected circRNAs. The host genes of these circRNAs were predicted to be targeted to neurogenesis(ontology: biological process), proteinaceous extracellular matrix(ECM)(ontology: cellular component), and binding(ontology: molecular function). Differentially expressed circRNAs-mediated regulatory networks were enriched in ECM receptor interaction. Most of the dysregulated circRNAs could potentially bind to five different mi RNAs by Target Scan and mi Randa. Specifically, circ_15752 was identified in this circRNAs pool which may facilitate vascular endothelial cell proliferation, migration, and tube formation, suggesting a critical role in endothelial angiogenesis. Our work suggests that dysregulated circRNAs may be involved in CNV pathogenesis and serve as potential biomarkers for CNV.
